Outcomes of interstage home monitoring and unanticipated readmissions in infants with a functional univentricular heart.

OBJECTIVE: This study aimed to evaluate the results of interstage home monitoring program and its impact on outcomes after initial univentricular palliation. METHODS: Patients with univentricular hearts who survived the initial palliation and were discharged from the hospital between 2013 and 2022 were included. The interstage outcomes in patients who received home monitoring program were evaluated, and survival was compared with those who did not. RESULTS: Among 207 patients included, initial palliation consisted of 117 Norwood procedures, 58 aortopulmonary shunts, 28 ductal stents, and 7 p

Case Report: Angiography-guided transvenous single-chamber pacemaker implantation in mirror-image dextrocardia with congenitally corrected transposition of the great arteries.

BACKGROUND: Mirror-image dextrocardia with situs inversus is uncommon. Transvenous permanent pacemaker implantation is technically demanding when it coexists with congenitally corrected transposition of the great arteries (ccTGA), because venous access, atrioventricular valve crossing, and fluoroscopic lead orientation may differ from usual anatomy. CASE PRESENTATION: A 61-year-old man was admitted with recurrent dizziness and palpitations for 5 years; his symptoms had worsened during the preceding 2 weeks. Preoperative 12-lead electrocardiography showed atrial fibrillation with a slow ventri

Effect of preoperative oral iron supplementation on postoperative outcomes in children with acyanotic congenital heart disease undergoing cardiac surgery: a pragmatic randomised clinical trial.

BACKGROUND: Studies assessing the effect of preoperative iron supplementation in paediatric cardiac surgery are limited and yield conflicting data. OBJECTIVES: The study aimed to evaluate the effect of preoperative oral iron supplementation on allogeneic blood transfusion after cardiac surgery for acyanotic CHD. METHOD: This was a prospective, open-label, outcome assessor-blinded, randomised clinical trial performed in a large tertiary care centre in India. Children (haemoglobin <13 gm/dl) with acyanotic CHD were recruited for the study. Children in the intervention arm received colloidal ir

Neurological manifestations and vasculopathy in PLCG2-associated antibody deficiency and immune dysregulation (APLAID): a case series.

BACKGROUND: Autosomal dominant pathogenic variants in PLCG2 are associated with autoinflammation and PLCG2-associated antibody deficiency and immune dysregulation (APLAID), characterized by immunological abnormalities and multi-system inflammation. Descriptions of associated neurological manifestations and vascular anomalies are limited. We describe three patients with APLAID, vasculopathies, neuroinflammation, and cerebral vascular tortuosity. These cases highlight underrecognized and clinically important manifestations of APLAID. CASE PRESENTATIONS: All three patients had de novo variants i

The Society of Thoracic Surgeons (2026) Expert Consensus Document on Management of Borderline Left Heart Excluding Unbalanced Atrioventricular Septal Defect and Strategies for Biventricular Conversion.

BACKGROUND: Strategies to recruit the left ventricle in borderline left heart (BLH), whether utilizing primary or staged recruitment strategy, are challenged by lack of consensus regarding optimal substrate, timing, and technique. METHODS: A panel of experienced congenital cardiac surgeons, cardiologists and imaging experts were convened to develop key questions related to BLH management, excluding unbalanced atrioventricular septal defect, from neonatal period through 1 year, using the PICO (Patients/Population, Intervention, Comparison/Control, Outcome) framework. Literature searches were p

CCDC69 Regulates Adipocyte Differentiation and Cellular Morphology in SGBS Cells-A Human Preadipocyte Cell Line.

CCDC69 was previously identified as a gene upregulated during adipogenic differentiation of human Simpson-Golabi-Behmel syndrome (SGBS) preadipocytes but not mouse 3T3-L1 preadipocytes. Here, the role of CCDC69 in adipogenesis was investigated using SGBS cells. CCDC69 expression during adipogenic differentiation was assessed by quantitative reverse transcription PCR and Western blotting. CCDC69 function was examined by adenovirus-mediated overexpression and knockdown, followed by analyses of lipid accumulation, adipocyte marker expression, cell morphology, and cytoskeletal organization. CCDC69

Sex-Specific Adaptations in Placental Mitochondrial Biogenesis and Function in babies Born with Critical Congenital Heart Disease.

Congenital heart disease (CHD) is the leading cause of birth defects; most critical (c)CHD newborns will develop neurodevelopmental disorders. Abnormal placental development may be involved, as genetics explains only 30% of sporadic CHD cases. Growing evidence suggests that placental dysfunction may result in heart and brain defects, as it shares a common early development. We hypothesized that newborns with critical CHD exhibit sex-specific alterations in placental mitochondrial biogenesis and function which may affect the fetal growth trajectory. Term placenta and paired umbilical cord (UC)

Cardiovascular Magnetic Resonance Predictors of Residual Pulmonary Regurgitation and Biventricular Recovery After Pulsta Valve Implantation.

Current recommendations for pulmonary valve replacement are based primarily on cardiovascular magnetic resonance (CMR)-derived right ventricular (RV) volume thresholds established in repaired tetralogy of Fallot. However, patients undergoing self-expanding percutaneous pulmonary valve implantation (PPVI) often have mixed pulmonary regurgitation (PR) and pulmonary stenosis with heterogeneous right ventricular outflow tract (RVOT) anatomy. We investigated CMR predictors of residual PR, residual RV enlargement (RVE), and residual biventricular imbalance (BVI) after Pulsta PPVI. We retrospectively

A New HOPE for Hypoplastic Left Heart Syndrome: Technique for Primary En-Bloc Heart and Aorta Transplant with Hypothermic Oxygenated Recovery of a DCD Heart.

BACKGROUND: Primary heart transplantation for hypoplastic left heart syndrome (HLHS) has been limited by the scarcity of neonatal donors. Although hypothermic oxygenated machine perfusion (HOPE) enables donation after circulatory death (DCD) heart transplantation, previous cannulation techniques sacrificed the donor aortic arch, precluding the reconstruction required for HLHS. METHODS: A neonatal-sized porcine DCD heart was recovered with the entire thoracic aorta preserved en-bloc using a novel split-cannulation strategy compatible with HOPE. Following 304 minutes of ex situ preservation, th

Septal dysfunction and Abnormal Systolic Stretch Are Associated with Right Ventricular Dysfunction in Repaired Tetralogy of Fallot.

AIMS: Right ventricular (RV) dysfunction is a major determinant of outcomes after Tetralogy of Fallot repair (rToF). Although right bundle branch block is present in most patients and electro-mechanical dyssynchrony (EMD) has been associated with RV dysfunction, only a subset of patients ultimately develops RV dysfunction. We hypothesized that septal dysfunction is a key feature associated with the development of EMD-induced RV dysfunction in rToF. METHODS: We retrospectively studied 126 pediatric rToF patients (median age 14.7 years) with contemporaneous ECG, echocardiography, and cardiac ma

Latent profiles and network analysis of emotional behaviors in preschool children after congenital heart disease surgery.

BACKGROUND: Emotional and behavioral problems are prominent and highly heterogeneous among preschool children after congenital heart disease surgery, but their latent subgroup characteristics and core symptoms remain unclear. The associations between the family psychological environment (e.g., parenting stress, family hardiness) and children's emotional and behavioral outcomes also need further clarification. OBJECTIVE: To identify heterogeneous subgroups and central symptoms of emotional and behavioral problems in preschool children after congenital heart disease surgery, and to examine the

Left-sided surgical approach for tricuspid valve replacement in congenitally corrected transposition of the great arteries with dextrocardia and situs inversus totalis: A case report.

BACKGROUND: Congenitally corrected transposition of the great arteries (ccTGA) is a rare congenital cardiac anomaly characterized by atrioventricular and ventriculoarterial discordance, resulting in a physiologically corrected circulation. When ccTGA is combined with dextrocardia and situs inversus totalis, surgical management becomes particularly challenging because of the mirror-image anatomy and altered spatial orientation. CASE PRESENTATION: A 61-year-old man presented with progressive dyspnea and was diagnosed with ccTGA accompanied by dextrocardia and situs inversus totalis. Transthorac

Left Ventricular Assist Device Implantation in Dextrocardia: Lessons From a Complex Redo Case.

Dextrocardia with situs solitus poses significant surgical challenges for left ventricular assist device (LVAD) implantation, particularly in patients with prior sternotomy and complex valvular disease. We report on a 60 year old man with dextrocardia, advanced heart failure, prior mechanical mitral valve replacement, and bicuspid aortic valve disease who underwent successful Corheart 6 LVAD implantation during redo sternotomy with concomitant bioprosthetic aortic and mitral valve replacement. Intraoperative findings included extensive pannus/thrombus on the mechanical mitral prosthesis (causi
