Reaching consensus as to how knowledge of development underscores our understanding of deficient ventricular septation.

Some of us recently discussed the problems existing in describing the channels that permit interventricular shunting. We offered suggestions for improvement, particularly when assessing the channel that is found when both arterial trunks arise from the morphologically right ventricle. Our proposals engendered significant debate, with several criticisms appearing in an editorial commentary. The commentator now accepts that not all of the criticisms were justified. In an attempt to seek further consensus, we have now joined with additional colleagues so as to clarify the aspects of our initial w

Utility of inhaled nitric oxide for pulmonary hypertension in cyanotic congenital heart disease: a cohort study with propensity score matching.

Pulmonary hypertension can lead to hemodynamic instability and worsen the outcome after the repair of cyanotic congenital heart disease with decreased pulmonary blood flow. However, the safety and effectiveness of targeted therapy, such as inhaled nitric oxide, remain controversial. This retrospective cohort study included patients who underwent corrective repair for tetralogy of Fallot, double outlet right ventricle, or pulmonary atresia with ventricular septal defect with hypoplastic pulmonary vasculature at Fuwai Hospital between 2014 and 2021. Patients were divided into a regular treatment

Genetic Variants Associated With Congenital Heart Disease: A Meta-Analysis of Ethnicity and Subtype-Specific Susceptibility.

BACKGROUND: Congenital heart disease (CHD) is the most common heterogeneous birth defect, with prevalence varying across populations. A comprehensive meta-analysis could refine the genetic risk estimates and enhance our understanding of CHD susceptibility. METHODS: We conducted a meta-analysis of 175 case-control studies investigating 107 genetic variants across 72 gene regions. Pooled odds ratios were calculated using 6 genetic models, with subgroup analyses by ethnicity and CHD subtype. Gene Ontology and network analyses elucidated the functional significance of implicated genes. RESULTS:

Classification of Fetal Congenital Heart Disease by Prenatal Ultrasound and Its Diagnostic Value.

PURPOSE: To evaluate diagnostic value of prenatal ultrasound in classifying fetal congenital heart disease (CHD) and to verify its diagnostic accuracy by comparing its results with postnatal pathological findings. METHODS: This retrospective study analyzed 372 pregnant women with fetal CHD, detected by ultrasound, who underwent interventional prenatal diagnosis. Prenatal CHD cases were classified at three levels. Pathological and anatomical examinations of CHD fetuses were performed in 305 pregnant women who terminated pregnancies; results were compared to those of prenatal ultrasonography.

Conotruncal Anomalies of the Fetus.

Approximately 0.8 % of all children are born with heart defects, with the prenatal incidence naturally being even higher. Among all congenital heart defects (CHD), conotruncal anomalies are the most common critical heart defects - after ventricular and atrial septal defects. Conotruncal anomalies are anomalies of the infundibulum (conus arteriosus) and great vessels (truncus arteriosus). These anomalies predominantly affect the right ventricular outflow tract and account for about 25-30 % of all CHD cases. Based on their incidence and clinical relevance, this article describes perimembranous v

Double-Outlet Right Ventricular Malformation in a Two-Year-Old Aberdeen Angus Cow.

A 2-year-old Aberdeen Angus cow was presented with lethargy and decreased appetite at the VA-MD College of Veterinary Medicine Large Animal Teaching Hospital. Initial clinical examination revealed cyanosis, tachycardia, polycythemia, and a significant increase in lactate levels. The heifer experienced spontaneous death while hospitalized, prompting a postmortem examination. Gross evaluation demonstrated that the aorta arose entirely from the right ventricle, while the main pulmonary artery maintained its normal position, consistent with a diagnosis of double-outlet right ventricle. Additional

Optimising pre-operative assessment in double-outlet right ventricle: the complementary role of echocardiography and CT angiography.

BACKGROUND: Double-outlet right ventricle is a complex congenital cardiac anomaly in which both great arteries arise predominantly from the right ventricle. Accurate anatomical evaluation is critical for surgical planning. While transthoracic echocardiography is commonly used for intracardiac assessment, its limitations in visualising extracardiac structures highlight the need for additional imaging modalities like CT angiography. OBJECTIVE: To compare the diagnostic accuracy and clinical utility of transthoracic echocardiography and CT angiography in the pre-operative evaluation and surgical

Outcomes of Fontan patients undergoing combined heart-liver transplantation in pediatric hospitals across the United States.

BACKGROUND: With increasing numbers of patients surviving Fontan palliation, there is a rise in Fontan-associated liver disease and transplantation strategies inclusive of combined heart-liver transplantation (CHLT). Therefore, we reviewed a combined dataset assessing outcomes of pediatric patients undergoing CHLT. METHODS: Patients undergoing Fontan CHLT between 2010 and 2024 at pediatric hospitals were queried from a merged dataset of the Pediatric Health Information System and United Network for Organ Sharing. Matching (2:1) was completed with Fontan heart-only transplant recipients. RESU

Long-Term Surgical Outcomes in Double Outlet Right Ventricle Based on Detailed Anatomical Sub-Typology.

OBJECTIVES: Long-term surgical outcomes for double outlet right ventricle (DORV) continue to pose challenges for most paediatric cardiac centres. This study aimed to investigate the mid- to long-term outcomes of a large cohort of DORV patients. METHODS: DORV patients who underwent surgery guided by a refined anatomical sub-typology in our centre between August 2001 and December 2023 were retrospectively reviewed. The primary end-point was cardiac mortality and the secondary end-point was unplanned surgical reintervention. RESULTS: A total of 1135 patients were included in the study. Among th

Identifying Pathogenic Variants in Vietnamese Children with Functional Single Ventricle Based on Whole-Exome Sequencing.

Background: Functional single ventricle (FSV) comprises a heterogeneous group of congenital heart diseases (CHDs) with severe and complex abnormalities. The multifactorial etiology of the disease poses challenges in identifying specific pathogenic factors and planning effective interventions and preventive treatments for patients. Methods: Whole-exome sequencing (WES) was performed to identify variants in relevant genes in 29 FSV patients from different families. Results: In total, 95 heterozygous variants across 48 CHD-associated genes were identified, including 85 missense, four small indel,

Young-onset hepatocellular carcinoma following double outlet right ventricle surgery without liver congestion: a case report with genetic analysis.

AIM: Congenital heart disease (CHD) can lead to liver dysfunction and HCC, typically through congestion, hypoxia, or low cardiac output. However, HCC development without these factors is not well understood. METHODS: We report a 16-year-old girl with a history of double outlet right ventricle surgery in infancy, without Fontan procedure. Imaging and histology confirmed HCC without liver fibrosis, inflammation, or congestion. Whole exome sequencing was performed on tumor and non-tumor liver tissue. Genes associated with both CHD and HCC were identified from public databases, and protein expres

Right Ventricular Myocardial Strain and Proteomic Analysis of Fetuses With Left Ventricular Hypoplasia: A Cross-Sectional Study.

BACKGROUND AND AIMS: Decreased right ventricular (RV) function in fetuses with left ventricular hypoplasia (LVH) increases the risk of adverse outcome. This study aimed to quantitatively evaluate RV myocardial function prenatally in LVH, and explore the relevant mechanism from protein expression. METHODS: 81 singleton fetuses diagnosed with LVH and 81 normal controls were retrospectively included. We used RV global longitudinal strain (RVGLS) derived from two-dimensional speckle tracking imaging to evaluate RV myocardial function and "XGboost" algorithm to select effective factors affecting R

Multimodality Imaging of Double-Outlet Right Ventricle, Superior-Inferior Ventricles, and Malposed Great Arteries.

BACKGROUND: The hallmark of superior-inferior ventricles is that the interventricular septum is horizontal to the diaphragm, with the ventricles lying one above each other. This is an unusual ventricular arrangement and can be associated with other defects, which makes it difficult to discern the intracardiac relationships. CASE SUMMARY: A 2-kg premature baby received the diagnosis of double-outlet right ventricle, superior-inferior ventricles, a small subarterial and an additional large muscular ventricular septal defect (VSD), malposed great arteries, and coarctation of the aorta. A 3-dimen
