Haploinsufficiency of Endothelial Nitric Oxide Synthase Mitigates Beneficial Effects of Maternal Exercise on Fetal Heart Development During Pregestational Diabetes.

BACKGROUND: Pregestational diabetes (PGD) increases congenital heart defect (CHD) risk over 5-fold. Maternal exercise enhances eNOS (endothelial nitric oxide synthase) activity, benefiting embryos, though its causal role remains unclear. This study investigated the role of eNOS in maternal exercise-mediated protection of fetal heart development in a PGD mouse model. METHODS: PGD was induced in eNOS+/- or wild-type female mice via streptozotocin before breeding with wild-type or eNOS+/- males. Pregnant females had access to a running wheel for voluntary exercise or remained sedentary. Fetuses

Balloon dilation of the para-device space to alleviate severe flow restriction in an oversized pulmonary flow restrictor: a case report.

BACKGROUND: Manually modified microvascular plugs (MVPs) to pulmonary flow restrictors (PFRs) are innovative devices for managing pulmonary overcirculation percutaneously, though achieving optimal restriction while maintaining device stability can be challenging. CASE PRESENTATION: We describe a full-term newborn with a double outlet right ventricle and a large malaligned subaortic ventricular septal defect, complicated by pulmonary overcirculation. At 24 days of life (2.4 kg, 50 cm), bilateral MVP-based PFRs were implanted, with an MVP-7Q placed in the right pulmonary artery (4.1 mm diameter

Outcomes of Fontan operation in patients with conotruncal anomalies from Australia and New Zealand Fontan Registry: a review of 1835 patients†.

OBJECTIVES: The impact of conotruncal anomalies (CTAs), including tetralogy of Fallot, truncus arteriosus, ventriculo-arterial discordance, double-outlet right ventricle and interrupted aortic arch type B, on long-term outcomes remains poorly described in the Fontan cohort. We sought to review the outcomes of Fontan patients with CTAs in Australia and New Zealand. METHODS: We reviewed the data from 1835 patients who underwent a Fontan operation between 1975 and 2023 from the Australia and New Zealand Fontan Registry. RESULTS: CTAs occurred in 895 patients (49%), including D-TGA (Transpositio

Surgical Repair of Double-Outlet Right Ventricle With Doubly Committed Juxtaarterial Ventricular Septal Defect: The Role of Virtual Reality.

Double-outlet right ventricle (DORV) with a doubly committed juxtaarterial ventricular septal defect (VSD) presents significant surgical challenges. We report a neonate with DORV, a hypoplastic aortic arch, and a patent ductus arteriosus. Virtual reality (VR) imaging provided detailed visualization, confirming the VSD's position beneath both great arteries. Intraventricular rerouting was unfeasible given the patient's small systemic structures and tricuspid valve chordae. The patient underwent successful VSD closure through a transarterial approach, arterial switch operation, and arch repair.

Conduit Size, Branch Pulmonary Artery Size, and Reoperation in Patients With Truncus Arteriosus.

BackgroundOptimal right ventricle-to-pulmonary artery (RV-PA) conduit size for patients with truncus arteriosus is controversial. We aimed to determine the relationship between branch PA size and the need for conduit reoperation following repair of truncus arteriosus.MethodsWe performed a single-center chart review of patients who underwent truncus arteriosus repair from January 2009 to December 2023. Branch PAs were measured in systole, at the narrowest point if focal stenosis was present. For branch PA diameter analyses, the smaller diameter PA was used. Univariate Cox proportional hazards r

Postoperative Care of a Paediatric Patient With Common Arterial Trunk Associated With Ventricular Septal Defect and Pulmonary Arterial Hypertension: A Rare Case Report.

Common truncus arteriosus is a rare congenital cardiac malformation. This case was associated with a ventricular septal defect (VSD) and also combined with pulmonary hypertension, which, together with the young age of the child and the fragility of the tissues, demanded a high degree of refinement in surgical operation, making the procedure complex and extremely risky. This case reports the postoperative care experience of a child with a common arterial trunk (CAT) combined with a VSD and pulmonary hypertension. Throughout the postoperative nursing process, blood pressure fluctuations should b

Clarifying the cardiovascular morphology and associated abnormalities in patients with tricuspid Atresia using multidetector computed tomography angiography.

The present study sought to assess the intracardiac morphology and associated cardiovascular anomalies in patients with tricuspid atresia using multidetector CT angiography. CT angiography datasets of all patients diagnosed with tricuspid atresia at our institution between November 2014 and December 2021 were retrospectively reviewed. The type of tricuspid atresia and associated cardiovascular structural anomalies detected on CT angiography were evaluated. Tricuspid atresia was identified in 404 patients (276 males [68.3%]; mean age at diagnosis: 4.54 ± 6.61 years [range: 9 months-40 years]).

A Novel Surgical Approach for Double-Outlet Right Ventricle With Noncommitted Ventricular Septal Defect.

Double-outlet right ventricle (DORV) is a highly variable heart disease. Biventricular repair of noncommitted ventricular septal defect (VSD) forms of DORV is often overlooked in favor of palliative surgery such as Fontan. We present the case of a 7-year-old patient, with the diagnosis of noncommitted VSD DORV, who underwent biventricular correction, with good short-term result. The management of complex DORV with noncommitted VSD is evolving. Previously oriented toward Fontan palliation, it now benefits from surgical advances allowing biventricular repair, reflecting surgical advances preserv

Primary repair of common arterial trunk: A systematic meta-analysis of short- and long-term outcomes.

OBJECTIVES: Primary repair of the common arterial trunk is still burdened by high mortality rates. Because of the low incidence and complexity of common arterial trunk, evidence is limited to reports with small sample sizes, and issues such as the ideal surgical timing to address the primary repair are still debated. We performed a systematic review and meta-analysis to estimate the pooled mortality, morbidity, and reoperation rates after common arterial trunk primary repair. METHODS: PubMed, Google Scholar, Ovid-MEDLINE, and Ovid-EMBASE databases were searched to identify studies on common a

Anomalous systemic venous drainage in truncus arteriosus - An unusual association.

Truncus arteriosus (TA) is a major cyanotic heart malformation infrequently associated with aortic arch interruption and isolation of the branch pulmonary artery. Anomalous superior vena caval drainage to the left atrium (LA) is hitherto unreported in TA. In a 45-day-old infant diagnosed with TA and cyanosis, anomalous drainage of the right superior vena cava into the LA was not identified preoperatively or intraoperatively. It was detected postoperatively due to persistent hypoxemia. The anomaly posed unique challenges in the subsequent management. This case report highlights the unusual asso

Born to be Bad: Combination of Aorticopulmonary Window With Double-Outlet Right Ventricle-Pulmonary Atresia.

Pulmonary atresia is a rare and complex congenital cardiac condition accompanied by an aortopulmonary window (APW) and double-outlet right ventricle (DORV). Here, we describe a case of a newborn with DORV, ventricular septal defect (VSD), pulmonary atresia, and APW. The patient underwent successful postnatal evaluation and echocardiography, revealing significant anomalies, including a large AP window providing pulmonary blood flow. The coexistence of DORV with pulmonary atresia and APW emphasizes the complexity of managing such congenital heart diseases, necessitating a multidisciplinary appro

Criss cross heart: an outflow tract defect?

BACKGROUND: Criss-cross heart (CCH) is a rare congenital malformation in which the atrioventricular inflow vectors are approximately perpendicular. CCH is associated with other defects including malposition of the great arteries, supero-inferior ventricles, and ventricular septal defects (VSD). A recent study in a mouse model demonstrates that CCH and associated malformations can be the result of a growth arrest of the outflow tract. In order to confront this hypothesis, we studied 16 cases of criss-cross heart with detailed anatomical description and clinical outcomes. RESULTS: All patients w

Double-Outlet Left Ventricle With Leftward Anterior Aorta in Infants.

INTRODUCTION: Among the conotruncal abnormalities, double-outlet left ventricle (DOLV) has been considered an embryologic conundrum, and no more than 200 cases have been described since 1975. DOLV refers to anomalous ventriculoarterial connections with the great arteries arising entirely or predominantly from the left ventricle. It represents a heterogenous group of heart defects considering embryology, morphology, and hemodynamics. DOLV can be detected prenatally or postnatally by transthoracic echocardiography. METHODS: This study presents a case series of 3 newborns with DOLV and a leftwar
