Anomalous systemic venous drainage in truncus arteriosus - An unusual association.

Truncus arteriosus (TA) is a major cyanotic heart malformation infrequently associated with aortic arch interruption and isolation of the branch pulmonary artery. Anomalous superior vena caval drainage to the left atrium (LA) is hitherto unreported in TA. In a 45-day-old infant diagnosed with TA and cyanosis, anomalous drainage of the right superior vena cava into the LA was not identified preoperatively or intraoperatively. It was detected postoperatively due to persistent hypoxemia. The anomaly posed unique challenges in the subsequent management. This case report highlights the unusual asso

Common trunk branching from the renal artery to the diaphragm, adrenal gland, and testis: a case report with embryological hypotheses by observation.

The vascular systems of the kidneys and gonads are highly susceptible to variations because of the degeneration of the mesonephros and mesonephric artery and the progressive ascending development of the metanephros. In this report, we present a rare case of a common arterial trunk originating from the left renal artery and supplying the left diaphragm, adrenal gland, and testis. Anatomical dissection of a 95-year-old male was conducted during the 2023 academic year at Tokai University Medical School. The left renal artery had a common trunk that bifurcated to superior and inferior branches, fo

Identification of novel genomic regions associated with yield-related traits in cowpea (Vigna unguiculata [L.] Walp) landraces.

BACKGROUND: Hypersplenism is commonly seen in patients with non-cirrhotic portal hypertension (NCPH). While a splenectomy alone can effectively relieve the hypersplenism, it does not address the underlying portal hypertension. The present study was undertaken to analyze the impact of shunt and non-shunt operations on the resolution of hypersplenism in patients with NCPH. The relationship of symptomatic hypersplenism, severe hypersplenism and number of peripheral cell line defects to the severity of portal hypertension and outcome was also assessed. METHODS: A retrospective analysis of NCPH pa

Surgical Repair of Common Arterial Trunk With Ventriculoarterial Septal Defect and Dual Orifice Truncal Valve.

We present a case of a rare example of a ventriculo-arterial septal defect found in a patient with a common arterial trunk, with balanced aortic and pulmonary components, but with separate valvar orifices within the common truncal valve. We managed the lesion using a two-patch approach. Performing a palliative procedure to relieve the elevated right ventricular pressure aided in the preservation of the pulmonary component of the common valve. We validated the success of the technique using postoperative computerized tomography and four-dimensional flow magnetic resonance imaging.

Molecular Pathways and Animal Models of Truncus Arteriosus.

In normal cardiovascular development in birds and mammals, the outflow tract of the heart is divided into two distinct channels to separate the oxygenated systemic blood flow from the deoxygenated pulmonary circulation. When the process of outflow tract septation fails, a single common outflow vessel persists resulting in a serious clinical condition known as persistent truncus arteriosus or common arterial trunk. In this chapter, we will review molecular pathways and the cells that are known to play a role in the formation and development of the outflow tract and how genetic manipulation of t

Congenital heart defects differ following left versus right avian cardiac neural crest ablation.

The cardiac neural crest is critical for the normal development of the heart, as its surgical ablation in the chick recapitulates common human congenital heart defects such as 'Common Arterial Trunk' and 'Double Outlet Right Ventricle' (DORV). While left-right asymmetry is known to be important for heart development, little is known about potential asymmetric differences between right and left cardiac neural folds with respect to heart development. Here, through surgical ablation of either left or right cardiac neural crest, we find that right ablation results in more varied and more severe he

An ectopic thyrolingual trunk arising from the common carotid artery: a rare variant.

The common carotid artery (CCA) typically bifurcates into the external and internal carotid arteries (ECA and ICA). In the head and neck area, the ECA gives off a few anterior branches from proximal to distal: the superior thyroid artery (STA), the lingual artery (LA), and the facial artery (FA). Occasionally, these branches can fuse into trunks, with the linguofacial trunk being the most common. During a computed tomography angiography (CTA) of a 67-year-old patient, a common arterial trunk, 11.3 mm proximal (prior) to the CCA bifurcation was recorded. The trunk was formed by the STA and the

Rare association of aortic atresia with balanced superior-inferior ventricles: case report of successful neonatal biventricular repair and review of the literature.

Aortic atresia is a rare finding and has not been previously described with superior-inferior ventricles. Presented here is a case of a heart with these concomitant findings and review of reported cases of aortic atresia in the absence of hypoplastic left heart syndrome. The aim of this report is to help highlight associated findings and the clinical approach taken. Also highlighted is the importance of not mistaking aortic atresia for common arterial trunk.

Unilateral Variation of the Left Ascending Pharyngeal Artery: A Case Report.

During routine cadaveric dissection of a 77-year-old male, a series of unique arterial variations were observed within the left anterior cervical region. Firstly, the ascending pharyngeal artery (APA) was observed branching from the common carotid artery (CCA), between the internal (ICA) and external carotid (ECA) arteries. The APA continued superiorly without branching, as is typical for APA. Secondly, a common arterial trunk was observed emerging from the posterior cervical segment of the left ICA. As the common trunk continued superiorly, it bifurcated into two distinct branches. The first

Clinical Presentation and Therapy of Truncus Arteriosus.

Truncus arteriosus (TA, also known as common arterial trunk) consists of only one great artery ("the truncus") with a semilunar valve (truncus valve) arising from the heart and an additional ventricular septal defect and (Fig. 50.1). This great artery is positioned above the ventricular septal defect and gives rise to the coronary arteries, the pulmonary arteries, and the aortic arch. Historically, TA has been classified by Collet and Edwards in three types, where in type I there was a common pulmonary artery truncus, in type II the left and right PA arise separately but close to each other, i

Diagnosis, Management and Outcome of Truncus Arteriosus Communis Diagnosed during Fetal Life-Cohort Study and Systematic Literature Review.

Background/Objectives: Truncus arteriosus communis (TAC) is a rare congenital heart defect characterized by a single arterial trunk that supplies systemic, pulmonary, and coronary circulations. This defect, constituting approximately 1-4% of congenital heart diseases, poses significant challenges in prenatal diagnosis, management, and postnatal outcomes. Methods: A retrospective analysis was conducted at the local tertiary referral center on cases of TAC diagnosed prenatally between 2019 and 2024. Additionally, a systematic literature review was performed to evaluate the accuracy of prenatal d

Common arterial trunk with pulmonary atresia.

We report two congenitally malformed hearts found at autopsy to have common arterial trunk and pulmonary atresia. Both exhibited usual atrial arrangement, along with concordant atrioventricular connections. In one case, the common arterial trunk arose predominantly from the right ventricle, while the other had a balanced commitment. In both, the atretic pulmonary trunk arose from the left posterolateral aspect of the common trunk. Confluent right and left pulmonary arteries, which were hypoplastic but patent, were present. On the inner aspect of the common trunk, there was a dimple immediately
