BACKGROUND: Pregestational diabetes (PGD) increases congenital heart defect (CHD) risk over 5-fold. Maternal exercise enhances eNOS (endothelial nitric oxide synthase) activity, benefiting embryos, though its causal role remains unclear. This study investigated the role of eNOS in maternal exercise-mediated protection of fetal heart development in a PGD mouse model.
METHODS: PGD was induced in eNOS+/- or wild-type female mice via streptozotocin before breeding with wild-type or eNOS+/- males. Pregnant females had access to a running wheel for voluntary exercise or remained sedentary. Fetuses

BACKGROUND: Manually modified microvascular plugs (MVPs) to pulmonary flow restrictors (PFRs) are innovative devices for managing pulmonary overcirculation percutaneously, though achieving optimal restriction while maintaining device stability can be challenging.
CASE PRESENTATION: We describe a full-term newborn with a double outlet right ventricle and a large malaligned subaortic ventricular septal defect, complicated by pulmonary overcirculation. At 24 days of life (2.4 kg, 50 cm), bilateral MVP-based PFRs were implanted, with an MVP-7Q placed in the right pulmonary artery (4.1 mm diameter

OBJECTIVES: The impact of conotruncal anomalies (CTAs), including tetralogy of Fallot, truncus arteriosus, ventriculo-arterial discordance, double-outlet right ventricle and interrupted aortic arch type B, on long-term outcomes remains poorly described in the Fontan cohort. We sought to review the outcomes of Fontan patients with CTAs in Australia and New Zealand.
METHODS: We reviewed the data from 1835 patients who underwent a Fontan operation between 1975 and 2023 from the Australia and New Zealand Fontan Registry.
RESULTS: CTAs occurred in 895 patients (49%), including D-TGA (Transpositio

Double-outlet right ventricle (DORV) with a doubly committed juxtaarterial ventricular septal defect (VSD) presents significant surgical challenges. We report a neonate with DORV, a hypoplastic aortic arch, and a patent ductus arteriosus. Virtual reality (VR) imaging provided detailed visualization, confirming the VSD's position beneath both great arteries. Intraventricular rerouting was unfeasible given the patient's small systemic structures and tricuspid valve chordae. The patient underwent successful VSD closure through a transarterial approach, arterial switch operation, and arch repair.

Double-outlet right ventricle (DORV) is a highly variable heart disease. Biventricular repair of noncommitted ventricular septal defect (VSD) forms of DORV is often overlooked in favor of palliative surgery such as Fontan. We present the case of a 7-year-old patient, with the diagnosis of noncommitted VSD DORV, who underwent biventricular correction, with good short-term result. The management of complex DORV with noncommitted VSD is evolving. Previously oriented toward Fontan palliation, it now benefits from surgical advances allowing biventricular repair, reflecting surgical advances preserv

BACKGROUND: Aortic root replacement (ARR) for patients with repaired conotruncal defects is challenging. We report outcomes of ARR in this complex patient population.
METHODS: After querying 2710 ARR cases at 2 academic centers between 2004 and 2021, 18 patients (0.7%) with previously repaired conotruncal defects were identified. These included 10 patients with tetralogy of Fallot, 4 with transposition, 2 with double-outlet right ventricle, 1 with truncus arteriosus, and 1 with single ventricle with pulmonary atresia. Surgical history, baseline characteristics, hospital courses, and outcomes

BackgroundAspirin has the potential of offering coronary arterial protection from thromboembolism for patients following the arterial switch operation (ASO) but is used inconsistently and the benefits are unknown. We sought to discern trends and outcomes associated with the use of aspirin following the ASO.MethodsPatients undergoing the ASO were identified from the Pediatric Health Information System and divided into two groups: those who received aspirin post-ASO versus the nonaspirin group to evaluate trends, variations, and outcomes.ResultsA total of 6,350 ASO cases were identified (44 cent

Pulmonary atresia is a rare and complex congenital cardiac condition accompanied by an aortopulmonary window (APW) and double-outlet right ventricle (DORV). Here, we describe a case of a newborn with DORV, ventricular septal defect (VSD), pulmonary atresia, and APW. The patient underwent successful postnatal evaluation and echocardiography, revealing significant anomalies, including a large AP window providing pulmonary blood flow. The coexistence of DORV with pulmonary atresia and APW emphasizes the complexity of managing such congenital heart diseases, necessitating a multidisciplinary appro

BACKGROUND: Criss-cross heart (CCH) is a rare congenital malformation in which the atrioventricular inflow vectors are approximately perpendicular. CCH is associated with other defects including malposition of the great arteries, supero-inferior ventricles, and ventricular septal defects (VSD). A recent study in a mouse model demonstrates that CCH and associated malformations can be the result of a growth arrest of the outflow tract. In order to confront this hypothesis, we studied 16 cases of criss-cross heart with detailed anatomical description and clinical outcomes. RESULTS: All patients w

BACKGROUND: Double-inlet right ventricle is an extremely rare and complex cardiac malformation characterized by the opening of both atria into the right ventricle and is usually associated with other major cardiac malformations. In this case report, we have discussed an isolated case of double-inlet right ventricle without other malformations and conducted a literature review to compare our case, which is the third in the medical literature, with previously published papers on double-inlet right ventricle.

Objective The aim of the study was to identify the various spectrum of single ventricle (SV) physiology and to correlate the anatomical variants. Materials and Methods All congenital heart disease cases with computed tomography/magnetic resonance imaging during the period from 2008 to 2023 were retrospectively assessed. Among these, we identified those corresponding to the definition of SV ( n = 160). In each case, we described the situs, looping, atrioventricular (AV)/ventriculoarterial (VA) connections, and associated anomalies. Descriptive statistics of each parameter were obtained and c

BACKGROUND: Occasionally, coarctation of the aorta (CoA) may go unnoticed and present in adulthood with left ventricular dysfunction. Variations in anatomy of the neck vessels can add to the complexity in management.
CASE SUMMARY: Our patient underwent surgical repair of a double-outlet right ventricle during which CoA was overlooked. She presented later with peripartum "cardiomyopathy" when CoA was diagnosed on 2-dimensional echocardiography. During transcatheter intervention, an aberrant right subclavian artery (ARSA) was seen arising before CoA opposite the origin of the left subclavian ar

Beyond its well-established role in cancer, KRAS is also crucial for embryogenesis, as its absence leads to embryonic lethality. However, the precise mechanisms underlying the developmental functions of KRAS, as well as the respective roles of its two splicing isoforms, KRAS4A and KRAS4B, remain incompletely characterized. To address these issues, we generated Kras4A knock-out (Kras4A-/-) and Kras4B-/- mouse models using CRISPR/Cas9 technology, and compared their phenotypes to those of a Kras-/- model, in which both isoforms are simultaneously inactivated. We observed that Kras-/- and Kras4B-/

PURPOSE: Pregnancy in women with adult congenital heart disease (ACHD), whether in its natural history or after surgical correction, represents a unique pathophysiological model that requires careful, multidisciplinary management to ensure favorable maternal, fetal and neonatal outcomes. Investigating the impact of congenital cardiac conditions on maternal and feto-neonatal health, the effect of pregnancy-related cardiovascular changes on maternal cardiac health, and the outcomes for offspring born from ACHD mothers is highly relevant, due to the increasing number of ACHD women reaching adulth

Total anomalous pulmonary venous connection is a rare congenital heart defect that can be diagnosed prenatally and might be very important for perinatal management. In addition to other cardiac abnormalities (levocardia, situs inversus, small left ventricle, double outlet right ventricle, parallel great vessels, and hypoplastic aortic arch), total anomalous pulmonary venous connection of a subdiaphragmatic type was diagnosed during a prenatal echocardiography examination in the second half of pregnancy. Fetal echocardiography monitoring showed no signs of congestive heart failure. The neonate

BACKGROUND: Automated technologies have become increasingly vital in fetal cardiac assessment. Fetal heart quantification (HQ), which utilizes two-dimensional speckle tracking, provides quantitative measures of morphology and function. This study aims to identify the progression of many fetal cardiac function parameters during pregnancy in normal pregnant women and to determine which parameters are relevant to fetal well-being and worth considering.
METHODS: We conducted a prospective observational study involving fetuses of 26 normal pregnant women to evaluate 27 fetal HQ parameters (includi
