Reliability of malaria rapid diagnostic tests (RDTs) in imported malaria: implications for diagnostic preparedness in a climate-sensitive mediterranean region.

BACKGROUND: Climate change is increasing the environmental suitability of the Mediterranean basin for malaria transmission, raising concerns about the potential re-emergence of the disease in areas where it has long been eradicated. As imported cases represent the potential source of local transmission in receptive areas, the rapid and accurate identification of imported malaria cases is a major public health priority. This study evaluated the diagnostic performance of a point-of-care Rapid Diagnostic Test (RDT) compared with an automated Real-Time PCR assay in a clinical setting in Sicily, It

A critical role of the histone lysine demethylase JmjC1 in regulating severe malaria-associated virulence genes in Plasmodium falciparum.

Cerebral malaria involves the expression of a particular var subgroup, termed upsA, in Plasmodium falciparum, which encodes a subset of PfEMP1 variants that avidly bind to human receptors on endothelial cells, resulting in obstruction of microvasculature in the brain and severe pathogenesis. Despite the clinical importance of the upsA var genes, the mechanism of their low-frequency activation in asymptomatic or uncomplicated malaria cases remains poorly understood. Here, we identify a JmjC-domain-containing histone demethylase PfJmjC1 as a vital transcriptional silencer of upsA var genes, whic

Performance of a Novel Plasmodium falciparum Rapid Diagnostic Test in Areas of Widespread hrp2/3 Gene Deletion.

BACKGROUND: Rapid diagnostic tests (RDTs) have improved malaria diagnosis; however, the emergence of hrp2/3 gene deletions threatens the reliability of HRP2-based RDTs. This study evaluated the diagnostic performance of a novel RDT detecting both HRP2 and PfLDH in a single test line. METHOD: A cross-sectional study was conducted at two health centers in Ethiopia, recruiting 1004 study participants. Blood samples were tested using the novel and comparator RDTs, using microscopy and nested polymerase chain reaction (PCR) as the reference standards. Plasmodium falciparum hrp2 and hrp3 genotyping

Unlocking the Enigmas of Plasmodium Aspartyl Protease in Antimalarial Therapy: Flap Region Dynamics and Enzyme Repression of Non-Food Vacuole Using Molecular Dynamics Simulations.

INTRODUCTION: Plasmepsin X (PMX) is a crucial aspartyl protease that prevents malaria parasite invasion and egresses into erythrocytes, making it a potential multiple-stage therapeutic target. METHODS: Recent research has led to the identification of UCB7362, an orally active PMX inhibitor with experimental antimalarial efficiency. In this report, we investigate the binding mechanism of UCB736 to the nonfood vacuole PMX. Most importantly, the detailed molecular description of the binding mechanism of UCB7362 to PMX sites that are not fully explored in the literature. We also examined the infl

Artemisinin Partial Resistance Mutations in Zanzibar and Tanzania Suggest Regional Spread and African Origins, 2023.

Artemisinin partial resistance, driven by Plasmodium falciparum K13 mutations, threatens malaria control. Zanzibar is vulnerable to artemisinin partial resistance spread but lacks recent molecular surveillance. We sequenced samples in Zanzibar and mainland Tanzania collected from 2022 to 2024. K13 mutations were found in 2 of 1440 participants (P441L, A675V) in Zanzibar and 6 of 3762 (R561H, P441L) in mainland Tanzania. Based on whole genome sequencing data, K13 mutations appear to be of African origin with regional spread. Frequent parasite importation appears to maintain partner drug mutatio

A novel bi-triazole (4RJ) with potent dual-stage antiplasmodial and transmission-blocking activity against Plasmodium falciparum and Plasmodium vivax.

Malaria is an infection caused by protozoa of the genus Plasmodium, responsible for more than 282 million cases annually. Despite the availability of antimalarial drugs, controlling the disease remains a challenge due to resistance and the lack of compounds effective at various stages of the parasite's life cycle. Bi-triazole derivatives are emerging as a growing class in the development of new antimalarials. The compound 4RJ, for example, has demonstrated activity against the asexual stages of resistant Plasmodium falciparum strains in previous studies. However, its effects on field isolates

Postdeployment Plasmodium Infections among South Korean Soldiers Receiving Mefloquine Chemoprophylaxis in South Sudan.

Postdeployment Plasmodium infection continues to occur despite recommended chemoprophylaxis among military personnel and long-term travelers deployed to malaria-endemic regions. We evaluated factors associated with postdeployment Plasmodium infection among South Korean soldiers receiving weekly mefloquine chemoprophylaxis during prolonged deployment to South Sudan. We conducted a retrospective study of 42 soldiers from a military unit who completed a standardized 40-week deployment to South Sudan between 2020 and 2023. All personnel were prescribed weekly mefloquine throughout deployment and 4

Drug resistance reversal: an overview of real world success stories.

This perspective provides an overview of what we know from real-world data on reversal of drug resistance evolution. An important question for those who study drug resistance is whether it is possible to reverse the evolution of resistance, for example by halting the use of specific drugs for a patient or even a community or country. Evidence for successful reversal of resistance levels is spread out in many different subfields of infectious disease. I use examples from P. falciparum malaria, several bacterial species including E. coli, M. tuberculosis, and S. aureus, and the viral pathogen HI

G-quadruplex-binding ligand RHPS4 reduces Plasmodium falciparum mitochondrial DNA level providing a novel antimalarial strategy.

Malaria caused by Plasmodium falciparum remains a major health threat, killing over 600,000 people annually. The spread of resistance to all major antimalarials, including artemisinins, highlights the urgent need for new drugs with distinct mechanisms of action. Here, we show that the G-quadruplex ligand RHPS4, an acridine derivative, displays strong antiplasmodial activity against both drug-sensitive and drug-resistant P. falciparum strains and clinical isolates. RHPS4 primarily targets the trophozoite stage and induces major mitochondrial alterations, including reduction of mitochondrial DNA

Monthly persistence and reappearance of reported malaria occurrence due to Plasmodium falciparum and Plasmodium vivax in the Peruvian Amazon, 2009-2024: a longitudinal geospatial study.

Malaria elimination requires distinguishing continued reporting from return after zero-reporting periods. In this longitudinal ecological study, we compared monthly persistence and reappearance of reported Plasmodium falciparum and Plasmodium vivax malaria in the Peruvian Amazon from 2009 to 2024. Reported surveillance episodes, which were not assumed to represent unique individuals, were aggregated by month, species, and 350 harmonized geographic units while retaining zero counts. A dynamic hurdle model combined a Bernoulli occurrence component, a zero-truncated negative binomial positive-cou

Innate immune sensing of Plasmodium sporozoite cell traversal drives protective T cell responses.

Attenuated Plasmodium sporozoites (SPZs) are being tested as a potential malaria vaccine as they elicit robust adaptive immune responses that protect against parasite challenge. Yet how they engage the innate immune system remains poorly understood. Here we isolated pure GFP+ Plasmodium berghei and Plasmodium falciparum SPZs, and sorted them onto mouse bone marrow-derived macrophages or human peripheral blood mononuclear cells to assess innate immune activation using bulk and single-cell RNA sequencing. SPZs induced a distinctive activation pattern in vitro resembling cell wounding responses,

Inhibition of Plasmodium DNA double strand repair by the combined action of 17-AAG and ML216.

The RecQ helicase PfBlm from the human malaria parasite Plasmodium falciparum plays a critical role in maintaining genome integrity through its function in homologous recombination mediated DNA repair. Given its essential role in resolving recombination intermediates, PfBlm has emerged as a potential antimalarial drug target. Chemical inhibition of PfBlm helicase activity using the small-molecule inhibitor ML216 sensitize the parasites towards DNA damage inducing agents. Here, we investigated whether inhibition of the molecular chaperone PfHsp90 enhances the antimalarial efficacy of ML216. We

Isoleucine absence from adult human hemoglobin is mirrored in mosquito proteomes and limits malaria parasite growth.

Amino acid composition within a proteome is conventionally viewed as a property of individual protein function. However, specific nutrient scarcity has been shown to reshape which amino acids are encoded in the genome of microbes. Mosquitoes have fed on blood for 200 million years, relying on blood amino acids for reproduction. We wondered whether this ancient dependency has left an imprint on the mosquito proteome. Here we show a proteome-wide shift to lower isoleucine usage in blood-feeding mosquitoes, mirroring the known isoleucine deficiency of adult human hemoglobin. In comparing amino ac

Targeting a malaria merozoite surface protein with mRNA vaccine generates multifunctional antibodies.

INTRODUCTION: Malaria is a leading health problem with high disease burden and mortality rates worldwide. Currently approved vaccines target the sporozoite form of Plasmodium falciparum that initially infects the liver, but only provide modest protection against malaria in young children. There is an urgent need to develop next-generation malaria vaccines that target multiple parasite developmental stages for greater efficacy. Antibodies to merozoites, which are involved in blood-stage replication, and are associated with clinical illness, have multiple functional activities and can protect ag

A comparison of indoor and outdoor malaria vectors in a longitudinal cohort study in Webuye, Western Kenya.

Background Malaria remains a major public health challenge in Kenya, with ~ 75% of the population is at risk. Despite the decline in malaria prevalence in children associated with sustained coverage of Insecticide Treated Nets in Kenya, this gain has not been uniform and there is a great deal of heterogeneity in transmission including areas with much higher transmission than the national average. This calls for a closer look at vector control to understand where it is failing. The main malaria vectors in Webuye are Anopheles gambiae s.s, An. arabiensis and An. funestus which are all known to b

Experimental Evolution of Reduced Susceptibility to Artemether-Lumefantrine in Plasmodium berghei Following Serial Drug Pressure.

Malaria remains a major global health problem, with uneven progress and the greatest disease burden clustered in sub-Saharan Africa. Antimalarial drug resistance continues to undermine malaria control efforts, with widespread resistance reported in Plasmodium falciparum, including reduced susceptibility to artemisinin-based combination therapies (ACTs). However, despite increasing concerns over the long-term effectiveness of ACTs, the processes through which resistance to artemether-lumefantrine (AL) partner drugs evolves and contributes to resistance against individual drug components remain

PfATG18 links V-ATPase assembly to endocytic membrane dynamics in malaria parasites.

Malaria parasites replicate inside red blood cells, degrading hemoglobin within a specialized digestive vacuole. Efficient hemoglobin processing is essential for parasite survival and influences antimalarial drug susceptibility. The vacuole constantly fuses with incoming hemoglobin-filled vesicles, yet the mechanisms that balance cargo influx with membrane homeostasis remain unclear. Here, using conditional reverse genetics, quantitative live-cell imaging, and 3D electron microscopy, we characterize the autophagy-related protein 18 of Plasmodium falciparum (PfATG18) as a key regulator of vacuo
