Single Ventricle: Radiology Primer for Preprocedural Evaluation.

Single ventricle is an uncommon congenital abnormality characterized by a single anatomic or functional ventricle that cannot sustain two separate serial circulations. Imaging plays an important role in classifying a single ventricle, quantitating ventricular and valvular function, detecting associated cardiovascular anomalies, and identifying critical factors for management. Echocardiography is the first-line imaging modality. MRI and CT are useful when echocardiography is indeterminate or suboptimal, especially for evaluation of vascular anatomy. Morphologically, two atria can connect to a s

Ablation of Intra-Atrial Re-Entrant Tachycardia in an Adult With Double-Inlet Ventricle Using Pulsed-Field Ablation.

BACKGROUND: Supraventricular tachycardia is common in patients with adult congenital heart disease and carries a high morbidity. Ablation of these arrhythmias is an effective treatment strategy, but difficulty accessing the cardiac chambers, variations in electroanatomical circuits, and concerns of damaging the surrounding structures can pose procedural challenges. CASE SUMMARY: A 45-year-old man with complex congenital heart disease and intra-atrial re-entrant tachycardia initially underwent ablation with radiofrequency energy but experienced clinical recurrence. He presented for a second pr

An isolated double-inlet right ventricular malformation: a case report and review of the literature.

BACKGROUND: Double-inlet right ventricle is an extremely rare and complex cardiac malformation characterized by the opening of both atria into the right ventricle and is usually associated with other major cardiac malformations. In this case report, we have discussed an isolated case of double-inlet right ventricle without other malformations and conducted a literature review to compare our case, which is the third in the medical literature, with previously published papers on double-inlet right ventricle. CASE PRESENTATION: A 2-month-old Syrian male infant was admitted to the cardiology depa

A Retrospective Study on the Imaging Spectrum of Functional Single Ventricle and Its Associations.

Objective  The aim of the study was to identify the various spectrum of single ventricle (SV) physiology and to correlate the anatomical variants. Materials and Methods  All congenital heart disease cases with computed tomography/magnetic resonance imaging during the period from 2008 to 2023 were retrospectively assessed. Among these, we identified those corresponding to the definition of SV ( n  = 160). In each case, we described the situs, looping, atrioventricular (AV)/ventriculoarterial (VA) connections, and associated anomalies. Descriptive statistics of each parameter were obtained and c

Contemporary outcomes and health care costs associated with single-ventricle heart failure admissions in adults in the United States.

OBJECTIVE: Advances in surgical and medical therapies have enabled patients with single-ventricle (SV) physiology to survive into adulthood, leading to a growing population of adults with SV congenital heart disease (SV-ACHD). This study aims to characterize contemporary SV-ACHD health care utilization and outcomes in comparison to acquired heart failure (HF). METHODS: The National Inpatient Sample was queried for the years 2016-2021 to identify all nonelective hospital admissions for HF or cardiogenic shock in patients aged 18 years or older. Patients with a diagnosis of common ventricle, do

Staged Ventricular Septation of the Double-Inlet Ventricle: How-I-Do-It.

Ventricular septation of the double-inlet ventricle is a largely abandoned operation due to poor historical outcomes. However, there has been renewed interest in septation as an alternative to Fontan palliation given its long-term sequelae. As one of the few centers to revisit septation in the early 1990s, our institution has long-term data on a series of patients with a double-inlet ventricle who underwent biventricular repair. This manuscript is a summary of our approach to staged septation of the double-inlet ventricle, with a focus on patient selection criteria, surgical techniques, periop

A Case of Double Inlet Left Ventricle in a 63-Year-Old Female Patient.

This case report describes the medical history and presentation of an elderly patient who was born with single ventricle physiology, an anomaly that is both unique and complex. Patients with single ventricle cardiac anomalies may be susceptible to life-threatening complications. However, advances in medical treatment and understanding have allowed for clinicians to develop surgical and medical interventions to treat patients with univentricular cardiac defects. This case is unique in the sense that the patient has been able to demonstrate remarkable adaptability to this condition and have a su

Ventricular Septation of the Double-Inlet Ventricle: Over Three Decades of Follow-Up.

There is renewed interest in septation of the double-inlet ventricle as an alternative to Fontan palliation. We examined our septation experience with over 30 years of follow-up. We retrospectively reviewed patients with double-inlet ventricle from 1990 to 2011. Patients with two adequate atrioventricular valves, a volume-overloaded ventricle, and no significant subaortic obstruction were septation candidates. Of 98 double-inlet ventricle patients, 9 (9.2%) underwent attempted septation via a one-stage (n = 2, 22.2%) or two-stage (n = 7, 77.8%) approach. Ages at primary septation were 7.5 and

Twin atrioventricular nodes and accessory pathways in congenital heart diseases with abnormal atrioventricular connections: Association with the developmental hierarchy of cardiac morphology.

BACKGROUND: Twin atrioventricular (AV) nodes (TWAVNs) are common in heterotaxy syndrome. OBJECTIVE: The purpose of this study was to investigate the presence and implications of TWAVNs and accessory pathways in congenital heart diseases (CHDs) with abnormal AV connections. METHODS: A retrospective study of a 1980-2022 cohort with sufficient electrocardiographic (ECG) data for review was conducted. RESULTS: We enrolled 136 patients with heterotaxy syndrome, 70 with congenitally corrected transposition of the great arteries (ccTGA) (4 with Ebstein anomaly), 47 with double-inlet ventricle (DIV

Ventricular septation for double inlet ventricle: Avoiding conduction injury.

OBJECTIVES: Although conduction location can be reliably predicted in double inlet ventricle, ventricular septation continues to carry a significant risk of complete heart block. This study describes our experience using intraoperative conduction mapping during ventricular septation. METHODS: Patients undergoing ventricular septation from 2017 to 2023 were identified. Conduction mapping was performed on the open, decompressed, beating heart to identify the His bundle. Mapped His bundle locations were compared with those predicted from magnetic resonance imaging diagnoses by a senior pediatric

Staged Ventricular Septation in Double-Inlet Ventricle - A Strategy to Avoid Fontan?

Single-stage ventricular septation for double-inlet left or right ventricle (DILV or DIRV) has historically been associated with poor outcomes. We hypothesize that staged ventricular septation may demonstrate favorable clinical outcomes to be an alternative to Fontan palliation. This single-center retrospective study reviewed patients with DILV or DIRV who underwent staged ventricular septation between 2015-2021. The strategy involves pulmonary artery banding or Norwood procedure during infancy (stage 1), followed by partial ventricular septation to anchor the septum, while maintaining systemi

Concomitant Single-Stage Unifocalization and Cavopulmonary Anastomosis (Glenn Shunt) in an Adolescent Patient With Univentricular Physiology and Major Aortopulmonary Collateral Arteries: A Technically Challenging Case.

Long-segment pulmonary atresia (PA), non-confluent branch pulmonary arteries, ventricular septal defect, tricuspid valve atresia (type 1A), and single ventricle physiology is a relatively rare and extremely heterogeneous form of congenital heart disease. This subset of patients having pulmonary atresia, ventricular septal defect, and major aortopulmonary collateral arteries (MAPCAs) have to undergo multiple unifocalization staging operations before a complete repair is attempted. Most of the patients were deemed inoperable. We report a rare case of a concomitant single-stage unifocalization an

Risk factors associated with the development of double-inlet ventricle congenital heart disease.

BACKGROUND: Congenital heart disease (CHD) is the most common birth defect group and a significant contributor to neonatal and infant death. CHD with single ventricle anatomy, including hypoplastic left heart syndrome (HLHS), tricuspid atresia (TA), and various double-inlet ventricle (DIV) malformations, is the most complex with the highest mortality. Prenatal risk factors associated with HLHS have been studied, but such data for DIV are lacking. METHODS: We analyzed DIV cases and nonmalformed controls in the National Birth Defects Prevention Study, a case-control, multicenter population-base

Anatomy of the Functionally Univentricular Heart.

The world of pediatric cardiac surgery and cardiac surgery as a whole lost one of the great pioneers with the passing, at the beginning of 2018, of Francis Fontan. Hence to add to the recognition of his achievements, the European Congenital Heart Surgeons Association (ECHSA) has established a lecture to be given in his memory at their annual meetings. It was a significant honor and privilege to be invited to present the initial lecture. In this report, we describe the essence of the presentation. Many patients are now palliated by construction of the Fontan circulation. Very few of those put f

Late proximal pulmonary artery occlusion in a child with a single chamber after a right-sided Blalock-Taussig shunt.

The paper presents the management of a child born with pulmonary valve atresia, a single (double-inlet) ventricle, right ventricular hypoplasia, and perimembranous septal defect. The first stage of treatment consisted in a Blalock-Taussig shunt. Control angiography performed 1 year after surgery confirmed that the anastomosis was correct, and there was no narrowing at the connection. The first stage of treatment was complicated by the occlusion of the left pulmonary artery, as diagnosed during cardiac catheterization before the planned bidirectional Glenn anastomosis. A decision was made to pe

INTRODUCTION AND OBJECTIVES: The association of univentricular heart with double inlet and Ebstein's "like" anomaly of the common atrioventricular valve is extremely rare. METHODS: Two hearts with this association are described with the segmental sequential system which determine the atrial situs, the types of atrioventricular and ventriculoarterial connections and associated anomalies. RESULTS: Both hearts had atrial situs solitus, and a univentricular heart with common atrioventricular valve, a foramen primum and double outlet ventricle with normal crossed great arteries.
