Liposomal bupivacaine versus ropivacaine thoracic paravertebral block for thoracoscopic atrial septal defect repair: analgesic efficacy and cardiac biomarker outcomes in a single-center, assessor-blinded, randomized trial.

BACKGROUND: Thoracoscopic atrial septal defect (ASD) repair, although minimally invasive, can cause substantial postoperative pain. Thoracic paravertebral block (TPVB) provides effective chest wall analgesia, and liposomal bupivacaine (LB), a long-acting local anesthetic with sustained release, may prolong analgesic duration. We investigated whether ultrasound-guided TPVB using long-acting LB improves postoperative analgesia without compromising cardiovascular safety compared with ropivacaine (RP). METHODS: This randomized, assessor-blinded trial enrolled 94 patients undergoing thoracoscopic

Aggregatibacter aphrophilus bacteraemia with suspected infective endocarditis in a patient with bioprosthetic aortic valve replacement and atrial septal defect repair.

Introduction. Despite the established pathogenicity of Aggregatibacter aphrophilus, few clinical cases have been described in published literature. A. aphrophilus is a rare cause of prosthetic valve endocarditis and diagnosis is challenging based on nonspecific symptoms on presentation. High clinical suspicion for infective endocarditis is warranted in patients with cardiac prosthetic material and positive blood cultures. Poor dentition and dental procedures, along with other risk factors, are associated with A. aphrophilus bacteraemia. We describe a case of A. aphrophilus bacteraemia in which

Transcatheter Closure of Apical Septal Rupture With Third Cavity Formation After Surgical Repair.

BACKGROUND: Apical septal rupture following surgical closure of a ventricular septal defect is a rare but life-threatening complication. Reoperation in such cases may be high risk, making transcatheter closure a viable and less invasive alternative. CASE SUMMARY: We report an 8-month-old male infant who presented with cardiac failure with Ross class-III and severe respiratory distress. Clinical examination and imaging revealed an apical septal tear with formation of a third cavity extending toward the right ventricle and compromising right ventricular and left ventricular function. The patien

A Two-vessel Circumflex Left Aortic Arch: An Extremely Rare Arch Configuration.

We present the case of a 6-month-old male infant with a ventricular septal defect, in whom cardiac computed tomography revealed a rare association of a circumflex left aortic arch with bilateral brachiocephalic trunks. Recognition of this unusual arch configuration is crucial as it has significant implications for surgical planning.

VSD Device Removal Due to Intermittent Complete Heart Block.

BACKGROUND: Percutaneous closure of perimembranous ventricular septal defects (pmVSDs) has markedly increased with the development of newer, softer occlusion devices. CASE SUMMARY: We report a 12-year-old girl who underwent percutaneous pmVSD closure. One week later, she was admitted for paroxysmal complete heart block (CHB). She was started on intravenous corticosteroids. A percutaneous retrieval of the device was performed successfully. Sinus rhythm was maintained throughout the 8-month follow-up period. DISCUSSION: Paroxysmal CHB is a rare but serious complication following percutaneous p

Mathematical simulations of paediatric haemodynamics in isolated ventricular septal defect.

Computer modelling of the cardiovascular system has the potential to revolutionize personalized medical care. This is especially promising for congenital heart defects, such as ventricular septal defect (VSD), a hole between the two ventricles of the heart. However, relatively few studies have built computer models for VSD, nor have they considered how natural adaptation to the cardiovascular system with age might interact with the presence of a small-, medium- or large-sized VSD. Here, we combine a lumped-parameter model of the cardiovascular system with two key modelling components: a size-d

An Extremely Rare Association of the Giant Right Atrium with Uncorrected Tetralogy of Fallot in an Adolescent Male Patient.

Uncorrected tetralogy of Fallot (TOF) can be associated with widespread abnormalities, but the giant right atrium (RA) is an extremely rare one. The giant RA can cause additional problems causing atrial arrythmias, thromboembolism, and compression of adjacent structures. Tricuspid regurgitation is common with annular dilatation. In symptomatic cases, reduction atrioplasty is recommended. Sternotomy should be careful to avoid damage to the giant RA. In asymptomatic cases, anticoagulation should be carried out. We report a rare case of uncorrected TOF associated with the giant RA and got success

Paradoxical Embolism in Young Patients With Acute Coronary Syndrome Without Coronary Atherosclerosis.

Acute coronary syndrome (ACS) in young patients (<50 years) accounts for 2%-10% of all ACS presentations. We describe a case series of young patients who presented with ACS in the absence of coronary atherosclerosis or other identifiable causes such as spontaneous coronary artery dissection and Takotsubo syndrome. Further diagnostic evaluation, often initiated after cryptogenic stroke or a recurrent systemic embolic event, revealed the presence of significant right-to-left shunting because of an atrial septal defect or patent foramen ovale. After exclusion of other etiologies for thromboemboli

A novel microdeletion syndrome involving 17q23.3 region in a child with short stature and multiple congenital anomalies: A Case Report.

Microdeletion syndromes are rare chromosomal abnormalities causing diverse clinical features due to gene dosage imbalances. Deletions in chromosome 17q23.3 have been linked to neurodevelopmental and growth disorders, although the genotype-phenotype correlations remain unclear. An eight-month-old male baby presented with short stature, low-set ears, cardiac anomalies, and multiple congenital malformations. He was born via C-section due to intrauterine growth restriction and exhibited craniofacial dysmorphism, lymphatic vessel dysplasia, micropenis, undescended testes, atrial septal defect, and

Single-stage surgical repair of congenital aortopulmonary window associated with interrupted aortic arch in two toddlers: a case report.

Congenital aortopulmonary window (APW) associated with interrupted aortic arch (IAA) is a rare and severe form of congenital heart disease. The optimal surgical timing and strategy remain challenging, particularly beyond infancy. We report two toddlers diagnosed with APW and IAA who underwent one-stage surgical repair. At the time of surgery, the children were 17 and 22 months old, with body weights of 6.6 kg and 8.0 kg, respectively. Both patients survived and experienced no perioperative complications. During postoperative follow-up of 12 and 19 months, respectively, the children demonstrate

Complex presentation of neonatal arthrogryposis multiplex congenita: insights into a multifactorial disorder.

This case report details a neonate diagnosed with arthrogryposis multiplex congenita (AMC), specifically the amyoplasia subtype, presenting with bilateral talipes equinovarus, elbow contractures, ulnar deviation of the wrists and clinodactyly. Additional clinical findings included mild developmental dysplasia of the hips and a small atrial septal defect. Genetic analysis revealed heterozygous variants of uncertain significance in the SYNE1 and CHD4 genes, both of which were inherited from asymptomatic parents. While SYNE1 is associated with autosomal recessive AMC type 3 and CHD4 with Sifrim-H

[Parinaud syndrome : a diplopia unlike any other].

Sudden-onset diplopia warrants an emergency management in ophthalmology. A binocular, vertical and persistent diplopia, should raise suspicion of a central lesion, particularly within the brainstem. We reporte the case of a 53-year-old man, who presented with acute vertical binocular diplopia. Ophthalmological examination revealed a bilateral elevation limitation, characteristic of Parinaud's syndrome. Brain magnetic resonance imaging identified a right-sided ischemic midbrain infarction, and further cardiovascular work-up demonstrated a patent foramen ovale causing a right-to-left shunt.

Selective Hemostasis in Repaired Tetralogy of Fallot With an Absent Left Pulmonary Artery: The Bleeding Lifeline.

BACKGROUND: Hemoptysis after repair of tetralogy of Fallot is uncommon and should prompt evaluation for residual pulmonary arterial lesions, infection, and systemic-to-pulmonary collaterals. CASE SUMMARY: A 30-year-old woman with repaired tetralogy-type anatomy and an absent left pulmonary artery presented 5 months after intracardiac repair with hemoptysis. Postoperative computed tomographic angiography showed an absent/nonvisualized left pulmonary artery, a small focal hemorrhagic opacity in the left lower lung, and systemic collateral supply from the descending thoracic aorta and left infer

Right-to-left shunting following left atrial decompression in three dogs.

Three dogs with advanced myxomatous mitral valve disease underwent left atrial decompression (LAD) via transseptal puncture and balloon atrial septostomy as a palliative treatment for refractory left-sided congestive heart failure. Variable degrees of pulmonary hypertension were present at the time of LAD. Each dog initially exhibited predominantly left-to-right shunting across the iatrogenic atrial septal defect but subsequently developed clinically relevant right-to-left shunting in the postoperative period. Progressive hypoxaemia, haemodynamic instability and clinical deterioration occurred

Holt-Oram syndrome presenting with failure to thrive due to a hemodynamically significant patent ductus arteriosus: the importance of early cardiac evaluation in infants with radial ray abnormalities.

BACKGROUND: Holt-Oram syndrome (HOS) is a rare autosomal dominant disorder caused by pathogenic variants in the TBX5 gene and characterized by radial ray malformations associated with congenital heart defects and/or cardiac conduction abnormalities. Although atrial and ventricular septal defects are the most common cardiac manifestations, other congenital cardiac lesions may occur. Early recognition of cardiac involvement is essential to prevent complications and improve outcomes. CASE PRESENTATION: We report the case of an 8-month-old female infant presenting with failure to thrive, feeding

Diagnostic and Surgical Challenges in Atypical Right Coronary Sinus-Right Atrial Rupture With Concurrent VSD.

BACKGROUND: Sinus of Valsalva aneurysm is a rare anomaly that, when ruptured, causes high-flow shunts and rapid-onset heart failure. We report a rare anatomical variant of right coronary sinus aneurysm rupturing into the right atrium with a concurrent perimembranous ventricular septal defect (pmVSD). CASE SUMMARY: A 16-year-old male with known pmVSD presented with progressive NYHA functional class III dyspnea for 15 days. Volume overload was initially misattributed to ventricular septal defect progression. However, transthoracic echocardiography demonstrated a restrictive pmVSD (3.4 mm openin
