BACKGROUND: Apical septal rupture following surgical closure of a ventricular septal defect is a rare but life-threatening complication. Reoperation in such cases may be high risk, making transcatheter closure a viable and less invasive alternative.
CASE SUMMARY: We report an 8-month-old male infant who presented with cardiac failure with Ross class-III and severe respiratory distress. Clinical examination and imaging revealed an apical septal tear with formation of a third cavity extending toward the right ventricle and compromising right ventricular and left ventricular function. The patien

We present the case of a 6-month-old male infant with a ventricular septal defect, in whom cardiac computed tomography revealed a rare association of a circumflex left aortic arch with bilateral brachiocephalic trunks. Recognition of this unusual arch configuration is crucial as it has significant implications for surgical planning.

BACKGROUND: Percutaneous closure of perimembranous ventricular septal defects (pmVSDs) has markedly increased with the development of newer, softer occlusion devices.
CASE SUMMARY: We report a 12-year-old girl who underwent percutaneous pmVSD closure. One week later, she was admitted for paroxysmal complete heart block (CHB). She was started on intravenous corticosteroids. A percutaneous retrieval of the device was performed successfully. Sinus rhythm was maintained throughout the 8-month follow-up period.
DISCUSSION: Paroxysmal CHB is a rare but serious complication following percutaneous p

Computer modelling of the cardiovascular system has the potential to revolutionize personalized medical care. This is especially promising for congenital heart defects, such as ventricular septal defect (VSD), a hole between the two ventricles of the heart. However, relatively few studies have built computer models for VSD, nor have they considered how natural adaptation to the cardiovascular system with age might interact with the presence of a small-, medium- or large-sized VSD. Here, we combine a lumped-parameter model of the cardiovascular system with two key modelling components: a size-d

To identify factors that influence the severity of clinical signs associated with ventricular septal defect (VSD) in calves, the clinical and pathological findings of 143 confirmed cases in animals under 12 months of age were retrospectively reviewed. Observed clinical signs included emaciation (92.3%), reduced vitality (83.9%), respiratory signs (77.6%), anorexia (74.1%), exercise intolerance (69.9%), cyanosis or pallor (50.3%), standing with assistance (32.9%), collapse (14.7%), death prior to euthanasia (11.9%), and signs of congestion (7.7%). Cardiac murmurs were detected in all cases and

BACKGROUND: Sinus of Valsalva aneurysm is a rare anomaly that, when ruptured, causes high-flow shunts and rapid-onset heart failure. We report a rare anatomical variant of right coronary sinus aneurysm rupturing into the right atrium with a concurrent perimembranous ventricular septal defect (pmVSD).
CASE SUMMARY: A 16-year-old male with known pmVSD presented with progressive NYHA functional class III dyspnea for 15 days. Volume overload was initially misattributed to ventricular septal defect progression. However, transthoracic echocardiography demonstrated a restrictive pmVSD (3.4 mm openin

BACKGROUND: Giant aneurysmal degeneration of a classic Blalock-Taussig (BT) shunt is a rare, life-threatening late complication of tetralogy of Fallot (TOF) palliation.
CASE: A 47-year-old woman with unrepaired TOF palliated in childhood presented with exertional dyspnea and a right thoracic mass. Echocardiography revealed severe right ventricular outflow tract obstruction, a large ventricular septal defect, and a pulsatile extracardiac structure. Computed tomography and cardiac magnetic resonance imaging confirmed a giant (72 × 66 mm) true aneurysm of a right BT shunt. She underwent successf

INTRODUCTION AND IMPORTANCE: Right-sided infective endocarditis is an uncommon condition, especially in patients with surgically repaired congenital heart disease. A residual shunt after ventricular septal defect closure can act as a substrate for late-onset infection, but such cases are rarely reported. High-velocity jet flow from a residual shunt may contribute to endothelial injury and subsequent infection. Furthermore, differentiating a late pseudoaneurysm at a prior cannulation site from an infectious aortic aneurysm is essential for guiding the appropriate treatment strategy.

BACKGROUND: To date, >10 million children have been conceived via assisted reproductive technology (ART) globally. However, there are some concerns about potential adverse outcomes associated with ART-conceived pregnancies, including the risk of congenital heart defects (CHDs). This meta-analysis aimed to evaluate the association between ART and the risk of major CHDs as well as their specific subtypes.
METHODS: Major electronic databases were screened for studies comparing the incidence of CHD following ART-conceived pregnancies versus spontaneous conception. The primary end point was study-

A 4-year-old boy with dextrocardia, congenitally corrected transposition of the great arteries, pulmonary valve stenosis, and ventricular septal defect demonstrated progressive hypoxemia and mild-to-moderate mitral and tricuspid regurgitation. Cardiac magnetic resonance imaging revealed insufficient right ventricular volume for Senning-Rastelli type anatomical repair. Given the elevated surgical risk associated with mitral regurgitation and inadequate ventricular volume, an initial palliative approach was undertaken at 2 years of age, consisting of systemic-to-pulmonary shunt placement and mit

BACKGROUND: Electrosurgical facilitation can aid difficult left bundle branch area pacing lead advancement, but standardized delivery patterns, reassessment logic, and follow-up lead performance remain insufficiently defined.
METHODS: We retrospectively analyzed 15 consecutive patients undergoing standardized spark-pattern cut-mode electrosurgical bailout after conventional mechanical left bundle branch area pacing lead advancement failed. The generator was operated in unipolar cut-mode at 20 W; spark-pattern delivery denoted brief sliding contact to the exposed cathode segment followed by im

OBJECTIVE: Partial heart transplantation (PHT) offers a growth-accommodating alternative to mechanical valve replacement in congenital heart disease. While experience with semilunar valve PHT is expanding, application to atrioventricular valves remains extremely limited. We report the first mitral valve PHT at our institution, focusing on technical considerations and lessons learned.
CASE SUMMARY: A 12-year-old patient with prior ventricular septal defect closure and supra-annular implantation of a 21-mm mechanical mitral valve due to congenital stenosis developed prosthetic stenosis (mean gr

BACKGROUND: Ventricular tachycardia can occur decades after ventricular septal defect repair, but surface electrocardiogram (ECG) and noninvasive imaging cannot prove patch-dependent reentry.
CASE SUMMARY: A 57-year-old man with ventricular septal defect patch repair in infancy and no adult congenital heart disease follow-up presented with chest pain, dyspnea, and near-syncope. ECG showed sustained monomorphic ventricular tachycardia at 209 beats/min. Transthoracic echocardiography showed left ventricular ejection fraction 30%, right ventricular dysfunction, moderate-to-severe tricuspid regur

Endovascular Coarctoplasty (CoAP) with stenting is the treatment of choice in Coarctation of Aorta (CoA). Literature on the harms or benefits of CoAP in Eisenmenger's syndrome (ES) is scarce. There is the hypothetical possibility of an increase in right-to-left intracardiac shunting once the coarctation is relieved. We report a 17-year-old male adolescent who presented with secondary hypertension, cyanosis, and lower limb claudication. He was diagnosed as a case of ventricular septal defect (VSD), patent ductus arteriosus (PDA), and CoA with associated ES. He underwent successful balloon CoAP

Long noncoding RNAs (lncRNAs) have emerged as crucial regulators in cardiac development and congenital heart disease. With advances in high-throughput sequencing, single-cell omics, and epigenetic profiling, accumulating evidence suggests that lncRNAs participate in transcriptional, posttranscriptional, and structural remodeling processes underlying cardiac morphogenesis. This in-depth review summarizes recent progress in elucidating the roles of lncRNAs in heart development and congenital heart disease pathogenesis. Specifically, we discuss how lncRNAs modulate key developmental signaling pat

BackgroundCongenital heart diseases (CHD) are the most common birth defect. Previous studies found sex disparities in outcomes after cardiac surgery and in access to treatment, but little data is available for low- and middle-income countries (LMIC).MethodsData on CHD surgical admissions in children <18 years of age from 70 sites in 25 LMIC for cases between 2011 and 2020 from the International Quality Improvement Collaborative for Congenital Heart Disease (IQIC) were analyzed to explore whether risk-adjusted in-hospital mortality and major infections differed by sex overall and for six common

BackgroundSustained total all-region (STAR) perfusion is an emerging technique that provides continuous perfusion to the brain, lower body, and heart with mild hypothermia. We hypothesized that using STAR perfusion would result in improved postoperative outcomes.MethodsThe 4 cohorts evaluated (total n = 105) included 1) STAR perfusion for aortic arch reconstruction (Arch w/ STAR), 2) STAR perfusion for aortic arch reconstruction with subsequent cardiac arrest for ventricular septal defect/atrial septal defect (VSD/ASD) (Arch/ASD/VSD w/ STAR), 3) hypothermic circulatory arrest (HCA) with cerebr

BACKGROUND: Pulmonary arterial hypertension (PAH) is a serious complication of congenital heart disease (CHD) with a left-to-right shunt, particularly ventricular septal defect (VSD). In patients presenting with severe PAH, therapeutic decision-making remains challenging because precise and individualized assessment criteria are lacking. In current clinical practice, treatment strategies are often based mainly on isolated hemodynamic parameters obtained by right heart catheterization (RHC), which may not fully reflect disease severity or reversibility.
OBJECTIVES: This study aimed to explore
