BACKGROUND: Post-myocardial infarction ventricular septal defect (PMIVSD) remains a lethal complication despite modern reperfusion. In patients with preserved or mildly reduced ejection fraction, higher shear across residual jets may increase the risk of device instability and hemolysis after transcatheter closure.
CASE SUMMARY: A 48-year-old man presented with PMIVSD 15 days after chest pain. Echocardiography showed a left ventricular ejection fraction of 50% and a 13-mm apicoseptal defect. Surgery was advised but was declined. Percutaneous closure with an 18-mm muscular ventricular septal d

Left ventricular outflow tract (LVOT) stenting is a palliative strategy for late-presenting d-transposition of the great arteries (d-TGA) with ventricular septal defect (VSD) and severe LVOT obstruction (LVOTO), particularly in settings with limited surgical resources. We reviewed six patients who underwent LVOT stenting at our center. Median age at intervention was 6 months (Q1-Q3: 25 days-18 months) and median weight was 4.0 kg (Q1-Q3: 3.3-5.1 kg). Median stent length was 23 mm (Q1-Q3, 15-29 mm). Median pre-procedural arterial oxygen saturation was 53.5% (Q1-Q3: 51-65%), increasing to median

Background: Pulmonary atresia (PA) with ventricular septal defect (VSD) and major aortopulmonary collateral arteries (MAPCAs), a life-threatening congenital heart defect (CHD), is frequently associated with abnormal pulmonary blood flow and vascular remodeling, causing hypoxia and heart failure. Segmental pulmonary hypertension (PH), a distinct PH type, may exist in some patients. Pulmonary vasodilators have been considered for treatment; however, evidence of their efficacy and safety remains lacking. Methods: A systematic review was conducted using PubMed, MEDLINE, The Cochrane Library, and I

OBJECTIVES: Patients with congenitally corrected transposition of the great arteries (ccTGA) may undergo physiologic repair, leaving the right ventricle systemic, or anatomic repair, correcting the double discordance. We compared both strategies in an international study using propensity-score matching.
METHODS: Patients from 12 European centres, aged <18 years, who underwent either physiologic or anatomic repair between 1990 and 2010 were included (n = 266). Patients were matched by optimal pair matching on ventricular septal defect, subpulmonary left ventricular outflow tract obstruction, ≥

BACKGROUND: Transthoracic minimally invasive device closure (TDC) via the transthoracic approach can be used to treat ventricular septal defect (VSD). However, studies investigating transthoracic closure in pediatric patients with perimembranous ventricular septal defects (PmVSDs) located close to the aortic valve and without preoperative aortic regurgitation (AR) are still lacking. The aim of this study was to compare the efficacy and safety of TDC and conventional surgical repair (CSR) for PmVSDs measuring ≤2 mm from the aortic valve in pediatric patients without preexisting AR.
METHODS: A

OBJECTIVES: Ventricular septal defect (VSD) is the most common congenital heart defect. Much remains unclear about the long-term prognosis for VSD patients. This study aimed to investigate the all-cause and cardiovascular mortality in patients with VSD compared with controls without congenital heart disease in Sweden.
METHODS: Swedish National Patient Register was used to identify all VSD patients born between 1970 and 2017. Mortality data were collected from the Swedish National Cause of Death register. Patients with associated complex congenital heart disease or non-congenital VSD were excl

Ventricular septal defect (VSD) occluders commonly rely on permanent nitinol frameworks, which may contribute to long-term mechanical mismatch and late complications. Here, we developed a tissue-compliant composite membrane by embedding a 3D-printed poly(vinyl alcohol) (PVA) grid within a shape-memory poly(glycerol dodecanedioate) (PGD) matrix. Grid spacing was varied from 0.1 to 0.5 mm to tune reinforcement density. FTIR indicated that PVA was incorporated mainly through physical interlocking rather than new covalent bonding. The composite preserved near-body-temperature shape recovery. In wa

BACKGROUND: Pediatric pulmonary hypertension comprises a variety of etiologies across different age groups, requiring a systematic approach to diagnosis in children.
CASE SUMMARY: A 7-year-old boy with nonverbal autism, oral aversion, and resolved muscular ventricular septal defect presented with heart failure and was found to have severe pulmonary hypertension. The initial diagnosis and treatment course was altered after a misleading cause, and he was ultimately found to have undetectable vitamin C levels. After repletion, he was weaned off pulmonary hypertension therapy and received nutriti

BACKGROUND: Exercise is beneficial in adult congenital heart disease, but patients with prosthetic materials, including valve-in-valve right ventricle-to-pulmonary artery (RV-PA) conduits, require tailored advice regarding high-impact sports.
CASE SUMMARY: A 19-year-old man with repaired pulmonary atresia, ventricular septal defect, and valve-in-valve RV-PA conduit placement developed exertional dyspnea after blunt chest trauma during competitive ice hockey. Imaging revealed significant prosthetic conduit stenosis due to stent compression. He underwent urgent surgical conduit replacement, wit

Late device migration following transcatheter perimembranous ventricular septal defect (VSD) closure is exceedingly uncommon, as most cases of embolization occur within the first hours to weeks after implantation. In the present case, migration was observed 18 months post-procedure, representing an unusually late presentation. Notably, surgical retrieval demonstrated complete circumferential endothelialization of the device, confirming true long-term tissue incorporation before subsequent erosion and detachment. Furthermore, serial early and mid-term imaging consistently showed a stable device

BACKGROUND: Given the established and reproducible benefit of 3D-printed models in complex congenital heart disease (CHD) surgical planning, the focus has shifted from validating their utility to refining anatomical fidelity through enhanced imaging integration. The purpose of this study is to evaluate the perceived clinical utility of multimodality fusion 3D-printed cardiac models and to determine whether inclusion of valve structures confers incremental benefit over conventional single-modality 3D models in enhancing anatomical understanding and preoperative surgical planning among pediatric

BACKGROUND: Complex CHDs may impair organ development. One proposed mechanism is an altered relationship between blood flow, oxygen delivery, and subsequent organ growth. In this study, we examined whether fetal lung, intracranial, liver, and kidney volumes differ among fetuses with transposition of the great arteries with an intact ventricular septum, transposition of the great arteries with a ventricular septal defect, and healthy controls.
METHODS: Eleven fetuses with transposition of the great arteries (6 with a ventricular septal defect and 5 with an intact ventricular septum) and 22 hea

Aortic arch repair in neonates with transposition of the great arteries is associated with prolonged myocardial ischaemia during a single-stage arterial switch and arch repair. Using the myocardial and cerebral perfusion technique described herein, the heart can remain beating in sinus rhythm during the aortic arch repair, reducing the duration of cardioplegic heart arrest and myocardial ischaemia.

Congenitally corrected transposition of great arteries (cc-TGA) is often accompanied by associated lesions with a wide spectrum of presentations. Traditionally, ventricular septal defects (VSDs) in such patients have been managed surgically with either physiologic repair or, more recently, there is a paradigm shift towards anatomical repair, such as the double switch operation. Percutaneous VSD device closure in cc-TGA physiology is extremely rare, with only isolated case reports in the literature. The present case report highlights a 15-year-old female, previously diagnosed with cc-TGA with m
