Percutaneous Treatment of Type A Aortic Dissection Using Atrial Septal Defect Occlusion Device.

Endovascular techniques are commonly employed for type B aortic dissections and are rarely reported for type A dissections. We present the case of a 78-year-old female diagnosed with a type A aortic dissection, with coronary arteries and supra-aortic vessels perfused from the true lumen and no significant aortic valve dysfunction. Given her recent cardiovascular surgery, the anticipated prolonged recovery, and multiple comorbidities, a percutaneous approach was preferred, with classical surgery available as stand by. The patient underwent endovascular treatment using two self-expandable, doubl

Immediate Reversal of Hypoxemia With PFO Closure in Carcinoid Heart Disease and Platypnea-Orthodeoxia Syndrome.

BACKGROUND: Intracardiac shunting via a patent foramen ovale (PFO) is an under-recognized cause of refractory hypoxemia. CASE SUMMARY: A 48-year-old woman with metastatic gastric neuroendocrine tumor involving the tricuspid valve presented with recurrent shortness of breath and severe hypoxemia refractory to supplemental oxygen. Computed tomography angiography ruled out pulmonary embolism. Transthoracic echocardiography with a bubble study demonstrated an interatrial communication suggesting a PFO. Right heart catheterization showed bidirectional shunting. Intracardiac echocardiography confir

Atrial Septal Defect Surgical Closure Following Trametinib Utilization in Noonan Syndrome-Associated Hypertrophic Cardiomyopathy.

BACKGROUND: Noonan syndrome-associated hypertrophic cardiomyopathy (NS-HCM) is often lethal when presenting severely in infancy, especially when associated with structural heart disease. Trametinib, an MEK inhibitor that attenuates abnormal signaling in the RAS/MAPK pathway, has been shown to improve NS-HCM outcomes. Surgery for associated heart defects in hypertrophic cardiomyopathy remains worrisome for increased risk of complications. FIRST-IN-HUMAN/EARLY REPORTS SUMMARY: An infant presented with a large secundum atrial septal defect complicating NS-HCM. The patient was treated with tramet

Long-term efficacy of the treat-to-close strategy for patients with atrial septal defect-pulmonary artery hypertension and characteristics of indicated populations.

OBJECTIVE: "Treat-to-close" strategy in the treatment of patients with Atrial Septal Defects (ASD) combined with severe Pulmonary Hypertension (PAH). This study explored the long-term survival of patients undergoing the "treat-to-close" strategy and the characteristics of the applicable population. METHODS: A total of 141 ASD-PAH patients were collected. Patients were classified as "Occlusion Group" (OG) and "Conservative Treatment Group" (CTG) depending on the suitability of ASD occlusion after drug treatment. The baseline characteristics and long-term follow-up outcomes of the two groups we

Rare Right Atrial Thrombus Deferred Atrial Septal Defect Closure and Implantable Cardioverter-Defibrillator Placement.

A 47-year-old man with nonischemic cardiomyopathy and atrial septal defect (ASD) was scheduled for percutaneous ASD closure and to be followed by implantable cardioverter-defibrillator placement. Preprocedural transesophageal echocardiography unexpectedly revealed a large right atrial thrombus, leading to cancellation of both procedures. The patient was managed conservatively with warfarin. This case illustrates the rarity of right atrial thrombus in patients without atrial arrhythmia or indwelling devices and emphasizes the critical role of preprocedural imaging in preventing potential emboli

Inadvertent Left Ventricular Implantation of a Leadless Pacemaker Managed by Percutaneous Extraction.

BACKGROUND: Although leadless pacemakers minimize lead- and pocket-related complications, their implantation requires a precise technique to avoid rare but serious malposition events. CASE SUMMARY: We present an 84-year-old woman with inadvertent left ventricular (LV) implantation of an Aveir leadless pacemaker. The device was successfully extracted using a carefully executed double-snare technique, followed by new device implantation in the right ventricle. DISCUSSION: There is limited literature guiding the management of inadvertent LV leadless pacemaker implantation, particularly regardin

Hybrid Transcatheter and Endoscopic Retrieval of a Pulmonary Artery-Embolized ASD Occluder in Hemodynamic Compromise.

BACKGROUND: Embolization of atrial septal defect (ASD) occluders is uncommon but potentially life-threatening, often requiring urgent intervention. Transcatheter retrieval is typically attempted first, but success depends on achieving slenderizing of the device-which may be limited by anatomy or device position, frequently necessitating surgical removal. CASE SUMMARY: A 25-year-old woman presented with hemodynamic compromise after embolization of an ASD occluder into the main pulmonary artery. A tailored hybrid strategy was undertaken: controlled snare-guided mobilization of the device into t

Antithrombotic Therapy in Percutaneous Atrial Structural Interventions.

Percutaneous left atrial appendage occlusion (LAAO), patent foramen ovale (PFO) closure, and atrial septal defect (ASD) closure rely on temporary antithrombotic therapy to prevent device-related thrombus during endothelialization, yet optimal regimens remain uncertain and vary widely across clinical practice. This review synthesizes contemporary evidence on postprocedural antithrombotic strategies, comparing efficacy and safety data and identifying key gaps in knowledge. After LAAO, therapeutic approaches range from short-term anticoagulation with vitamin K antagonists or direct oral anticoagu

Unrecognized Atrial Septal Defect Presenting with Postoperative Occipital Infarction.

A 49-year-old woman developed acute homonymous hemianopia three days after elective surgery. Neuroimaging confirmed an embolic right occipital infarction. Transthoracic echocardiography demonstrated early bubble passage, and transesophageal imaging identified a 4.6 mm ostium secundum atrial septal defect. The defect was successfully closed percutaneously with complete shunt resolution. This case emphasizes the value of targeted multimodality imaging in uncovering occult interatrial shunts and highlights presumed paradoxical embolism as a potential mechanism of postoperative or cryptogenic stro

Mitral transcatheter edge-to-edge repair with inverse leaflet grasping in the setting of prior atrial septal defect closure: a case report.

BACKGROUND: According to the 2025 European Society of Cardiology (ESC)/European Association for Cardio-Thoracic Surgery guidelines, transcatheter edge-to-edge repair has a Class IIa recommendation in patients with primary mitral regurgitation at high surgical risk following Heart Team evaluation. Procedural complexity may be increased by prior septal closure devices and challenging leaflet anatomy. We describe a modified technical approach that enabled successful treatment in this setting. CASE SUMMARY: A 75-year-old woman with a history of atrial septal defect closure presented with advanced

Feasibility of atrial septal defect type II occluders determined by computed tomography virtual cardioscope.

OBJECTIVES: This study aims to assess the suitability of large ASD type II occluders by evaluating the locations of the ASD and its extensions, as well as the adequacy of the rim, through the utilization of CT virtual cardioscopy. BACKGROUND: Atrial septal defects (ASDs) represent the most frequent congenital heart abnormalities observed in adults and the second most prevalent in infants. There has been a notable paradigm shift over the past four decades which percutaneous transcatheter closure as the preferred treatment modality over traditional surgical method. METHODS: We conducted a retr

Unresolved questions in atrial septal defect pathophysiology: Shunt dynamics, exercise physiology, and pulmonary vascular disease risk.

Despite advancements in the diagnosis and management of atrial septal defect (ASD), key hemodynamic aspects remain poorly understood. This review explores limitations of the pulmonary-to-systemic flow ratio (Qp:Qs), the impact of exercise on shunt volume, and predictors of pulmonary arterial hypertension. We highlight that Qp:Qs may not reflect true pulmonary blood flow, particularly during exertion, and that exercise-induced augmentation of shunt flow could predispose to right heart overload. Future studies integrating advanced imaging and hemodynamic measures are essential to refine risk str

The inferior vena cava collapsibility index as a non-invasive marker of early haemodynamic changes following atrial septal defect closure.

INTRODUCTION: Atrial septal defect is a common CHD, and transcatheter closure is now the gold standard treatment. In an atrial septal defect, left-to-right shunting increases pulmonary blood flow. This study aimed to evaluate early changes in pulmonary artery, pulmonary vein, and inferior vena cava flow parameters following transcatheter closure of an atrial septal defect. MATERIALS AND METHODS: This retrospective study included 31 paediatric patients with haemodynamically significant atrial septal defect (Qp/Qs ≥ 1.5) who underwent transcatheter closure between January 2023 and June 2024. Transcatheter closure of atrial septal defect device-related aortic erosion in a pediatric patient.

A 5-year-old female developed aortic erosion without pericardial effusion 2 years after atrial septal defect (ASD) device closure with an Amplatzer Septal Occluder device. A defect between the right atrium and aorta was found, caused by systolic impingement of the device on the aorta. Transcatheter closure of perforation using an Amplatzer Duct Occluder II device was successfully done, avoiding open heart surgery. At the 3-year follow-up, the patient remains asymptomatic with no residual shunt and no other complications.

Interatrial Conduction Block in Pediatric Patients with Ostium Secundum Atrial Septal Defect.

Background: Atrial arrhythmias represent a frequent long-term complication in patients with atrial septal defects (ASDs). Interatrial block (IAB), reflecting delayed or impaired conduction across Bachmann's bundle, has been proposed as an electrophysiological substrate predisposing to atrial arrhythmogenesis. However, evidence regarding its prevalence and clinical correlates in pediatric patients with ASD remains limited. The present study aimed to characterize interatrial conduction patterns and assess the occurrence of IAB in children with large secundum ASD undergoing percutaneous closure.
