BACKGROUND: Platypnoea-orthodeoxia syndrome (POS) is a rare but underdiagnosed cause of positional dyspnoea and hypoxaemia. It results from right-to-left interatrial shunting through a patent foramen ovale (PFO) or atrial septal defect (ASD), often precipitated by anatomical or functional distortions.
OBJECTIVES: We systematically characterised the clinical spectrum, anatomical correlates and procedural outcomes associated with percutaneous closure in patients diagnosed with intracardiac POS.
METHODS: A systematic search of PubMed, Embase and MEDLINE databases was conducted. Case reports, ca

BACKGROUND: Atrial septal defect (ASD) device closure generally provides excellent long-term outcomes; nevertheless, atrial arrhythmias such as atrial fibrillation and macro-reentrant atrial tachycardias may still appear years later. Although these are well-recognized late sequelae, focal atrial tachycardia (FAT), particularly from the perinodal region after ASD device closure, is exceptionally uncommon.
CASE SUMMARY: A 44-year-old woman presented 10 years after percutaneous ASD closure with recurrent palpitations. Electrocardiography demonstrated a narrow-complex regular tachycardia. Electro

The current research aimed to analyze the influencing factors for congenital heart disease (CHD) in children based on a case-control study. From January 2020 to January 2022, 60 children with CHD under 6 years of age admitted to the Affiliated Hospital of Southwest Medical University were selected as the research group. Another 60 children matched for age and sex who underwent physical examinations in our hospital without CHD were selected as the control group. The disease classification of children with CHD was determined, and the related influencing factors were analyzed. Among 60 children w

Recent advances in plasma metabolomics have highlighted the potential role of right ventricular-centered metabolic profiling in pulmonary arterial hypertension associated with atrial septal defect. By commenting on a trans-cardiac metabolomic gradient approach, this letter discusses how right ventricular metabolic signatures may reflect adaptive and maladaptive mechanisms beyond hemodynamic burden alone. We contextualize these findings within emerging evidence from lipidomics, endothelial biomarkers, and systemic disease models, emphasizing the translational value of integrating metabolomics i

INTRODUCTION: Post-repair atrial septal anatomical variations may increase transseptal puncture difficulty. While the novel AccuSafe guidewire shows promise as a procedural tool, its utility in such complex cases requires further evidence-based validation. METHODS AND RESULTS: We report a case of a 73-year-old female who developed atrial fibrillation (AF) following surgical repair of an atrial septal defect (ASD). An initial ablation attempt at another institution had failed. The patient presented with recurrent palpitations, concurrent heart failure, and anxiety. At our center, the AccuSafe t

BACKGROUND: Transseptal puncture is essential for transcatheter edge-to-edge repair (TEER) but is challenging in patients with previously implanted septal closure devices.
CASE SUMMARY: A 79-year-old woman with prior transcatheter closure of both a patent foramen ovale and an atrial septal defect developed severe degenerative mitral regurgitation (MR). Transseptal access was considered difficult due to the 2 closure devices. However, chronic MR caused enlargement of the left atrium and interatrial septum, creating a narrow but feasible puncture zone. Using the VersaCross system, transseptal p

BACKGROUND: Iatrogenic atrial septal defects (iASDs) after mitral transcatheter edge-to-edge repair (M-TEER) are generally benign. However, advanced atrial remodeling can trigger catastrophic mechanical failure via rapid morphologic expansion.
CASE SUMMARY: An 84-year-old woman with atrial fibrillation and diastolic dysfunction underwent M-TEER. Despite a stable intraprocedural iASD, she developed cardiogenic shock within 3 hours. Three-dimensional transesophageal echocardiography documented hyperacute doubling of the defect area. The mechanism was direct tricuspid regurgitation (TR)-jet impi

BACKGROUND: Inferior sinus venosus (SV) atrial septal defect (ASD) is a rare interatrial communication, found near the inferior vena cava (IVC)-right atrial (RA) junction, that may be overlooked on transesophageal echocardiography (TEE).
CASE SUMMARY: A 52-year-old woman with childhood secundum ASD closure presented with progressive right heart enlargement. Transthoracic echocardiography showed marked right-sided dilation without a detectable shunt. TEE confirmed an intact fossa ovalis patch but did not adequately visualize the IVC-RA junction. Electrocardiogram-gated cardiac computed tomogra

(1) Background: The management of epilepsy during pregnancy requires balancing effective seizure control against potential teratogenic effects of antiseizure medications (ASMs). Data on the safety of lacosamide (LCM), a third-generation ASM, during pregnancy and breastfeeding are limited. (2) Methods: To evaluate the safety and efficacy of LCM during pregnancy and breastfeeding, we report a single-centre case series and provide a comprehensive narrative review of the literature. (3) Results: In total, 22 cases of maternal exposure to LCM throughout pregnancy (1 monotherapy, 21 polytherapy) wer

INTRODUCTION: Percutaneous atrial septal defects (ASD) closure with fluoroscopy guidance is the standard procedure. However, fluoroscopy poses stochastic and deterministic risks for small infants and children. Zero fluoroscopy ASD closure is an alternative, yet its feasibility and safety compared to fluoroscopy remain unclear. Therefore, this study compares outcomes using standardized fluoroscopy and zero fluoroscopy methods for transcatheter ASD closure.
METHODS: Four databases (PubMed, ProQuest, Google Scholar, Wiley) were used to search literature published before July 2023. The main resul

Atrial septal aneurysms (ASAs) are usually asymptomatic and discovered incidentally during routine cardiac imaging. While their exact prevalence is unclear, ASAs can be associated with structural heart defects (such as patent foramen ovale or atrial septal defect) and carry risks of systemic embolization, arrhythmias, and potentially migraine headaches. The diversity of current diagnostic criteria highlights the need for a standardized approach in research and clinical practice. Imaging modalities such as echocardiography, cardiac magnetic resonance imaging, and computed tomography are used fo

Transcatheter structural heart interventions, including aortic, mitral and tricuspid valve replacement or repair, and patent foramen ovale, atrial septal defect, and left atrial appendage closure, have dramatically expanded over the past two decades, providing substantial improvements in both clinical outcomes and quality of life. These interventions are performed in a high-risk patient population, which is at risk for both thrombotic and bleeding complications. The introduction of prosthetic devices into the arterial or venous circulation under heterogeneous hemodynamic conditions inevitably

This report describes a rare case of a girl with constrictive pericarditis (CP), coexisting with atrial septal defect (ASD) and cardiac diverticulum. Her echocardiographic features changed in accordance with the hemodynamic alterations as treatment progressed. During a routine echocardiography before surgery, a 9-year-old girl with ASD was unexpectedly diagnosed with CP. This explains why her symptoms and signs were inconsistent with isolated ASD, including lower limb edema and hypoalbuminemia caused by impaired diastolic ventricular function. Contrast-enhanced computed tomography confirmed th

Percutaneous device closure is an attractive alternative to surgical closure of secundum atrial septal defects (ASDs) for those with suitable anatomy and size. Early surgery (< 2 years of age) allows for a more feasible "mini sternotomy" technique, while device closure is more commonly performed at an older age. Thus, the ability to predict future device-closure candidacy before 2 years of age is needed. Secundum ASD closures after 3 years of age from 2012 to 2023 at a single center were reviewed (n = 54). "Early" echocardiograms (obtained at 12-24 months of age) and "pre-intervention" echocar

INTRODUCTION: Adult atrial septal defect (ASD) is no longer regarded as a benign congenital lesion, as long-term follow-up studies have demonstrated substantial risks of atrial arrhythmias, heart failure (HF), pulmonary hypertension (PH), and excess mortality. Optimal treatment selection in adults therefore requires risk stratification beyond defect size and shunt magnitude alone. Importantly, adult ASD management increasingly demands a physiology-driven and patient-centered approach rather than a purely anatomical paradigm.
AREAS COVERED: This narrative expert review summarizes contemporary

Background/Objectives: Accurate quantification of left-to-right shunt volume is central to clinical decision-making in patients with atrial septal defect (ASD). Conventional echocardiographic Qp:Qs estimation is widely used but limited by operator dependency, Doppler alignment sensitivity, and the quadratic amplification of diameter measurement errors in flow calculations. These factors contribute to clinically relevant variability, particularly in paediatric populations with small vessel dimensions. Simulation-based haemodynamic modelling offers an alternative approach by integrating structur
