PURPOSE: Pulmonary hypertension (PH) is a progressive complication in adults with atrial septal defect (ASD), significantly impacting prognosis. Although right heart catheterization (RHC) is the gold standard for diagnosis, it is invasive. This study aimed to evaluate the diagnostic accuracy of various echocardiographic parameters compared to RHC for detecting PH specifically in adult patients with uncorrected secundum ASD.
METHODS: This cross-sectional diagnostic study utilized retrospective data from the COngenital HeARt Disease in adult and Pulmonary Hypertension (COHARD-PH) registry at Sa

Background Primary cardiac tumors are uncommon, with an incidence rate ranging from 0.001‰ to 0.3‰. Among cardiac tumors, 75% are benign and 25% are malignant. Approximately 50% of benign cardiac tumors are myxomas, while 75% of malignant cardiac tumors are sarcomas. Compared with myxomas, malignant cardiac tumors have a poorer prognosis. Cardiac myxosarcomas often have an insidious onset in young and middle-aged patients, with an overall survival period of 6 to 12 months. Similar to soft tissue tumors, cardiac sarcomas also consist of a range of histological subtypes. Among these, cardiac myx

The presence of a large ostium secundum atrial septal defect (OS-ASD) results in right atrial and right ventricular volume overload followed by pulmonary hypertension. In this context, a Crochetage electrocardiographic pattern can be observed in some cases, indicating a particular clinical and cardiological evolution. We present a case of an 8-year-old boy with OS-ASD and pulmonary hypertension, whose electro-vectorcardiogram reveal the typical characteristics first described in 1958.

BACKGROUND: Hypoattenuated leaflet thickening (HALT) after transcatheter tricuspid valve replacement (TTVR) is increasingly recognized, but its clinical consequences remain incompletely characterized.
CASE SUMMARY: A 63-year-old woman developed severe platypnea-orthodeoxia 5 days after TTVR with the Evoque system. Cardiac computed tomography confirmed HALT of all 3 leaflets. HALT-induced functional stenosis elevated right atrial pressure, reversing a preexisting left-to-right shunt across a secundum atrial septal defect. Low-dose thrombolysis improved oxygen saturation from 69% to 90%, but wa

OBJECTIVE: Salvaging an embolized balloon-expandable valve (Edwards Sapien 3 Ultra) from the left atrium during transcatheter mitral valve replacement.
KEY STEPS: 1) Secure the embolized valve in the left atrium. 2) Use multiple snares to crimp and snare the outflow of the valve into a more conical shape to facilitate valve retrieval. 3) Align the valve in the inferior vena cava to avoid obstruction. 4) Proceed with planned transcatheter mitral valve replacement. 5) Consider closure of the iatrogenic atrial septal defect (ASD).
POTENTIAL PITFALLS: Failure to retrieve the valve from the left

BACKGROUND: Atrial Septal Defects (ASDs) are a common type of congenital heart defect (CHD) in children. Accurate pre-procedural assessment is crucial for successful transcatheter closure. This study aims to compare ASD size and surrounding rims measurements from transthoracic echocardiography (TTE) and transesophageal echocardiography (TEE) in pediatric patients undergoing transcatheter closure of ASDs, assess agreement between TTE and TEE measurements, and evaluate TTE’s potential as a first-line imaging modality for ASD assessment. METHODS: This prospective cohort study was conducted on 50

Pulmonary arterial hypertension (PAH) can cause significant dilatation of the pulmonary arteries, which may externally compress the left main coronary artery (LMCA). We present 2 such cases successfully treated with percutaneous coronary intervention using drug-eluting stents. Case A was a 29-year-old woman with idiopathic group 1 PAH with severe LMCA stenosis due to main pulmonary artery compression, diagnosed during routine cardiac catheterization. Case B was a 23-year-old woman with PAH secondary to secundum atrial septal defect who presented with non-ST-segment elevation myocardial infarct

Atrial septal defects represent one of the most common congenital heart diseases in humans as well as in dogs and cats. However, reports in wildlife animals are limited. Additionally, ensuring treatment compliance and regular follow-ups in these species are often challenging. The aim of the present clinical brief is to describe the first successful long-term medical management of a nonrestrictive atrial septal defect in a crab-eating macaque (Macaca fascicularis). A seven-year-old castrated male crab-eating macaque was diagnosed with a severe left-to-right shunting, 7.5 mm atrial septal defect

Background: Cardiac erosion after transcatheter closure of secundum atrial septal defect (ASD) is a rare (0.1-0.3%) but potentially life-threatening complication. Available evidence remains limited to isolated case reports and small case series. Methods: A case-based review was conducted in accordance with CABARET recommendations. PubMed, Scopus, Web of Science, and the Cochrane Central Register of Controlled Trials (CENTRAL) were searched from inception through January 2026. Adult cases with anatomically confirmed aortic or aorto-atrial erosion after transcatheter closure of a secundum ASD we

Atrial septal defect (ASD) affects 1:11.3 children in some US states; however, the antecedents of these trends are yet to be identified. A total of 1882 ASD rates (ASDRs) for 2003-2020 were sourced from the National Birth Defects Prevention Network reports. A total of 406,893 ASDs are reported. Substance (cigarettes, binge alcohol, cannabis, cannabinoids, analgesics, cocaine) exposure data were taken from the National Survey of Drug Use and Health. Income and ethnicity data were derived from the US Census. Adjustment was performed by mixed effects, survey and generalized additive regression. C

OBJECTIVES: To describe the use of transcatheter electrosurgical septal tendon transection to consolidate a fenestrated atrial septal defect into a single defect suitable for single-device closure.
KEY STEPS: 1) Cross the anterior and posterior fenestrations with separate guiding catheters under transesophageal echocardiography guidance. 2) Establish through-and-through access under fluoroscopy by advancing a guidewire into the left atrium and capturing it with a snare. The guidewire in use is preprepared by removing insulation over a short segment and shaping this into a controlled "flying V

Atrial septal defect (ASD) is typically managed with percutaneous transcatheter closure via femoral venous access. In patients with interrupted inferior vena cava (IVC), however, conventional access is not feasible. Although the transhepatic approach has been described, it is less frequently utilized. We report 2 patients with secundum ASD and interrupted IVC who underwent successful device closure through transhepatic venous access. Complete closure was obtained in both cases. These cases highlight the feasibility of transhepatic access for ASD closure when femoral access is not possible. Ult

BACKGROUND: Atrial septal defects (ASDs) represent 10% of congenital heart disease. Transcatheter closure is the standard of care for anatomically suitable defects, but pediatric long-term outcomes remain underreported. This study examined the prevalence, outcomes, and risk factors for complications following ASD device closure in children.
METHODS: We retrospectively reviewed children <18 years undergoing transcatheter ASD closure at a single tertiary center (1989-2016). Demographic, procedural, and follow-up data were analyzed. Kaplan-Meier methods assessed freedom from major complications

Although aortic valve regurgitation develops after device closure of the atrial septal defect, its mechanism remains unclear. We observed a paediatric patient who developed significant aortic regurgitation one year after the procedure. Although there was no contact between the aorta and the device immediately after deployment, they gradually came into close proximity. Multiple factors appeared to have contributed to this phenomenon, including a relatively large diameter of both the device and the aortic sinus compared to body size, and a rapid increase in the left atrial volume. Coupled with l
