Efficacy and safety of transseptal puncture-assisted atrial fibrillation ablation in patients with atrial septal occluder devices: an updated systematic review and meta-analysis.

BACKGROUND: Transseptal puncture (TSP) is required for most catheter ablation (CA) procedures for atrial fibrillation (AF) because it enables left atrial access. In patients with atrial septal occluder (ASO) devices, device-related changes in septal anatomy may affect procedural planning, feasibility, and safety. We performed an updated systematic review and meta-analysis to evaluate reported outcomes of TSP-assisted CA in patients with ASO. METHODS: We searched PubMed, Embase, and the Cochrane Library from inception to April 28, 2026, including studies evaluating TSP-assisted CA in patients

Modified Warden technique for repair of partial anomalous pulmonary venous drainage. A case report.

Partial anomalous pulmonary venous drainage is an uncommon condition representing 0.6% of all congenital malformations that may be associated with an atrial septal defect. In the present report, we describe a modification of the Warden technique for surgical repair of partial anomalous pulmonary venous drainage with atrial septal defect by using a superior vena cava flap and a right atrial appendage flap to create a new tension-free cavoatrial continuity (neo right superior vena cava), along with redirection of the anomalous pulmonary veins to the left atrium through the atrial septal defect u

Transseptal Mitral Valve-in-Valve Implantation Via a Residual Iatrogenic ASD.

BACKGROUND: Degenerated surgical mitral bioprostheses may require reintervention, and transcatheter mitral valve-in-valve implantation has emerged as a less invasive alternative. Residual iatrogenic atrial septal defects (ASDs) may complicate hemodynamics, but they can also be incorporated into the procedural strategy. CASE SUMMARY: A 58-year-old woman with a prior mitral bioprosthesis presented with advanced heart failure, prosthetic stenosis, pulmonary hypertension, and a residual iatrogenic ASD with left-to-right shunting. The ASD was used as the transseptal access route owing to favorable

A single center experience with the use of Occlunix septal occluder for closure of atrial septal defects.

The Occlunix® septal occluder (OSO) is a new generation double disk occluder device for catheter closure of the secundum atrial septal defect (ASD). Fifty-six consecutive patients who underwent ASD device closure with the OSO between February 2022 and April 2024 were evaluated in this single-center observational study. All procedures were done under transesophageal echocardiography guidance. The OSO was successfully deployed in all patients (technical success 100%). Fifty-five patients had complete closure of the intervened ASD after device closure (procedural success 98.2%). The sole patient

Incidental Discovery of an 8.2-cm Pulmonary Artery Aneurysm During Routine Ablation Evaluation.

BACKGROUND: Pulmonary artery aneurysms (PAAs) are most commonly identified in the setting of pulmonary hypertension or congenital heart disease. Large aneurysms with normal pulmonary pressures are less frequently encountered and create uncertainty regarding natural history and management. CASE SUMMARY: We report a 73-year-old woman with prior mitral valve repair and atrial septal defect closure who underwent elective atrial tachycardia ablation, during which intraprocedural transesophageal echocardiography incidentally revealed an 8.2-cm main PAA. She was asymptomatic, and right- and left-hea

Atrial Flutter as a Late Presentation of Scimitar Vein in an Adult With Prior ASD Repair.

Adult patients with unexplained atrial arrhythmias and right heart dilation should prompt consideration of congenital anomalies such as scimitar vein, even decades after atrial septal defect repair. Early recognition using multi-modality imaging may prevent delayed diagnosis and progressive cardiopulmonary complications.

Pathophysiological Significance of Variants of the HAND1 Gene Promoter in Congenital Atrial Septal Defects: A Study in 632 Chinese Subjects.

BACKGROUND: Atrial septal defect (ASD) is a common congenital heart disease (CHD) and genetic variation in the HAND1 gene is associated with cardiac development. The variants in the promoter region of the HAND1 gene are unknown. METHODS: We performed Sanger sequencing of DNA from 632 subjects (320 ASD patients and 312 healthy controls). The identified variants were also subjected to further cellular functional validation, electrophoretic mobility shift analysis (EMSA), and JASPAR database analysis. RESULTS: A total of 12 variants were identified in the promoter region of HAND1 gene, seven of

Congenital Pericardial Agenesis: An Innocent Finding or Clinically Significant Condition? A Case Series and Literature Review.

Congenital pericardial agenesis (CPA) is a rare anomaly that is often considered a benign incidental finding but may present with nonspecific symptoms and mimic structural heart disease. Its clinical relevance remains incompletely defined, particularly regarding the distinction between a harmless anatomical variant and a clinically significant condition. We present a retrospective two-center case series of four patients with imaging-confirmed CPA, combined with a narrative review of the literature aiming to evaluate the clinical spectrum, diagnostic challenges, and management implications of C

Jagged-1 mutation is associated with congenital heart defects: a case report.

BACKGROUND: Congenital heart defects (CHD) refer to a wide range of heart structural abnormalities that exist at birth, ranging from malformations with a good prognosis to more complicated and severe lesions. Congenital heart defects can result from mutations in genes encoding cell signaling transducers such as Jagged-1 (JAG1). Mutations in JAG1 are linked to rare autosomal-dominant disorders, often resulting from haploinsufficiency in the Notch signaling pathway, which affects organ development. Reporting novel mutations and associated phenotypes emphasizes the importance of tailored approach

Progressive Enlargement of a Coronary Artery Communication With an Atrial Septal Defect Closure Device.

A 70-year-old man underwent coronary angiography 8 years after transcatheter atrial septal defect closure. Although no coronary stenosis was observed, a progressively enlarged coronary artery communication originating from the atrial branch of the left circumflex artery and extending into the device was identified. Serial imaging demonstrated progressive enlargement and elongation of this vascular structure over time. Computed tomography showed contrast opacification within the device and left-to-right leakage, suggesting incomplete neoendothelialization and peridevice leak. These findings may

Significant Left Atrial Appendage Leak After LAA Exclusion System Occluded by ASD Closure Device.

BACKGROUND: Incomplete left atrial appendage (LAA) closure ("leak") is a challenge of LAA occlusive techniques and carries thromboembolic risk. Closure of significant leaks after surgical AtriClip is rare and less described. CASE SUMMARY: An 81-year-old man with atrial fibrillation on apixaban, prior LAA AtriClip (2023), and multiple recent falls underwent transesophageal echocardiography that showed a residual severe central LAA ostium. Percutaneous ostia closure was performed using an atrial septal defect (ASD) plug via transseptal puncture approach. Residual flow ceased in the LAA, and ant

Partial Anomalous Pulmonary Venous Return in Lung Cancer Surgery: Diagnostic Challenges and Surgical Considerations in Two Cases.

Partial anomalous pulmonary venous return (PAPVR) is a rare congenital anomaly in which one or more pulmonary veins drain into the systemic venous system instead of the left atrium, in the absence of associated cardiac defects such as atrial septal defect. When only a single anomalous pulmonary vein is involved, the resulting shunt volume is typically small, allowing asymptomatic progression into advanced age. Consequently, such cases are often incidentally diagnosed or remain undetected. We report herein two cases of PAPVR that required pulmonary resection for primary or metastatic lung cance

Total Knee Arthroplasty in the Presence of Congenital Heart Disease: An Under-Recognized Risk.

BACKGROUND: Adults with congenital heart disease (CHD) increasingly present for noncardiac surgery. Total knee arthroplasty (TKA) induces significant hemodynamic and thromboembolic stress and, in patients with intracardiac shunts such as patent foramen ovale (PFO) or atrial septal defect (ASD), may precipitate paradoxical embolism and other under-recognized complications. METHODS: A systematic review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Right Atrial Non-Valvular Papillary Fibroelastoma Causing Paradoxical Embolic Stroke Through an Ostium Secundum Atrial Septal Defect: A Case Report.

Papillary fibroelastoma (PFE) is a rare benign cardiac tumor, usually left-sided and valvular. Right-sided, non-valvular PFEs are uncommon, and systemic embolization from these lesions is particularly rare, typically requiring an interatrial shunt. We report a surgically confirmed right-atrial, non-valvular PFE presenting with paradoxical embolic stroke through an ostium secundum atrial septal defect (ASD). A 72-year-old woman with recurrent ischemic cerebrovascular events was admitted with an acute posterior circulation stroke. Transthoracic echocardiography was nondiagnostic. Transesophageal
