A right pulmonary artery to left atrium fistula represents a rare cause of cyanosis in childhood. It may be diagnosed late due to the presence of minimal symptoms. We report the case of a 3-year-old child who presented with dyspnea and cyanosis. Transthoracic echocardiography revealed an anomalous fistulous connection between the right pulmonary artery and a large aneurysm communicating with the roof of the left atrium. The patient underwent successful transcatheter occlusion using an atrial septal defect occluder device. The patient had an uneventful recovery on follow-up with resolution of c

BACKGROUND: Congenital heart defects (CHD) is a leading cause of perinatal morbidity and mortality. Cardiac axis assessment on the four-chamber view has been proposed as a simple adjunct marker for detecting underlying cardiac anomalies during second-trimester anomaly scans.
AIM: The aim of the study was to evaluate the association between cardiac axis deviation and congenital cardiac defects and assess its diagnostic performance as a screening parameter.
MATERIALS AND METHODS: This prospective observational study included 810 pregnant women undergoing routine anomaly scans at 18-24-week ges

This report describes an infant who experienced two distinct episodes of postoperative pulmonary hemorrhage after a Rastelli-type procedure, each driven by a different mechanism requiring tailored management. The initial right-sided pulmonary hemorrhage was attributed to excessive flow through major aortopulmonary collateral arteries (MAPCAs). Although hemostasis could not be achieved by coil embolization alone, selective temporary occlusion using balloon catheter limited the bleeding to a more segmental distribution. Despite airway obstruction leading to cardiac arrest, bleeding did not worse

Platypnea-orthodeoxia syndrome (POS) is a rare condition characterized by positional dyspnea and hypoxemia that may cause secondary erythrocytosis through chronic hypoxic stimulation. We describe a 71-year-old woman with an 80-pack-year smoking history who presented with persistent erythrocytosis and progressive positional dyspnea. Workup revealed an atrial septal defect with bidirectional shunting and normal erythropoietin, consistent with secondary erythrocytosis from suspected POS rather than polycythemia vera. In patients with unexplained erythrocytosis and positional dyspnea, evaluation f

INTRODUCTION: The superior sinus venosus atrial septal defect is situated immediately inferior to the junction of the superior caval vein (SCV) and the right atrium. It is usually associated with partial anomalous pulmonary venous drainage. Surgical correction used to be the only treatment option, but the transcatheter approach using a covered stent has recently evolved substantially.
PATIENTS AND METHODS: This is a single-centre retrospective analysis of all patients who underwent transcatheter correction of sinus venosus atrial septal defect in our institution between January 2024 and Janua

CLINICAL CONDITION: A 32-year-old man presented with a secundum atrial septal defect (ASD) complicated by pulmonary arterial hypertension (PAH) with elevated pulmonary vascular resistance (PVR), creating a major operability dilemma.
KEY QUESTIONS: What is the diagnostic work-up in suspected pulmonary hypertension? Should genetic testing be performed in congenital heart disease-associated PAH? How should ASD patients with elevated PVR be managed? Should closure be pursued despite persistent PVR elevation? What is the surgical strategy in this setting?
OUTCOME: PAH-targeted therapy improved sy

Purpose: To evaluate the prevalence and cardiac CT features of inferior left atrial diverticula (ILAD) communicating with the right atrium (RA) or inferior vena cava (IVC), a novel type of interatrial communication. Materials and Methods: This retrospective study included 11,512 consecutive patients who underwent cardiac CT. CT features and prevalence of ILAD communicating with the RA or IVC were analyzed. Shunts were defined as anatomical defects between the two structures with or without visible contrast flow. In a subset of the patients we compared interatrial septal aneurysm (n = 20) and I

BACKGROUND: Functional tricuspid regurgitation (TR) is frequently present in adults with secundum atrial septal defect (ASD) due to chronic right-sided volume overload and right heart remodeling.
AIMS: The aim of this study is to evaluate changes in TR severity, tricuspid annular and right ventricular (RV) reverse remodeling 12 months after transcatheter ASD closure, including RV systolic function assessed by conventional parameters and RV free wall longitudinal strain (RVFWLS). We also evaluated baseline echocardiographic predictors of persistent ≥ moderate TR at follow-up.
METHODS: We pros

Myxoma is a common primary benign cardiac tumor that typically occurs in middle age. It is usually single and frequently found in the left atrium, particularly attached to the interatrial septum. An atrial septal defect (ASD) is a congenital defect, and myxomas often involve the interatrial septum. Therefore, the coexistence of these two conditions, especially when myxomas are multiple, is rare. We present a middle-aged man with a secundum-type ASD and two myxomas located in the right atrium and left ventricle, which were discovered incidentally. This implies that, in patients with one or mult

Primary-unloading of the left ventricle in Acute Myocardial Infarction (AMI) may reduce infarct size and resultant heart-failure occurrence. Existing methods, such as percutaneous axial flow pumps, pose cost and complication challenges. Hemodynamic parameters from AMI patients in published cohorts were applied in a real-time cardiovascular computer-model simulating AMI. Hemodynamic changes were tracked after induction of 2.5-12.5 mm Iatrogenic Atrial-Septal Defect (iASD). Subsequently, in-vivo evaluation in swine was performed by inducing AMI using percutaneous-balloon occlusion of the left co

Increased radiation exposure during congenital heart disease procedures may have a detrimental effect on patients, especially in the pediatric population. Advances in imaging systems and adherence to the ALARA (As Low As Reasonably Achievable) principle are paramount in minimizing radiation exposure. This is a single center, retrospective study designed to understand the degree of radiation reduction achievable with a novel ultra-low dose fluoroscopy protocol (ULDFP). Patients were included in the study if they underwent any one of the following procedures between 12/1/2021 and 12/31/2023: atr

In transcatheter closure of patent foramen ovale (PFO) associated with cryptogenic stroke (CS), a small coexisting atrial septal defect (ASD) can serve as a potential source of residual shunting if not adequately addressed. We conducted a retrospective study of 14 patients with PFO and coexisting ASD who underwent transcatheter closure between December 2019 and May 2025. The first five patients treated with Amplatzer™ PFO Occluder demonstrated residual right-to-left shunting graded ≥ 3 at one-year follow-up. Following the introduction of the GORE® CARDIOFORM Septal Occluder (GSO) and GORE® CAR

BACKGROUND: Patients with coarctation of the aorta (CoA) and an interrupted aortic arch (IAA) may present with small mitral valves (MVs) and a reduced left ventricular (LV) volume. Biventricular repair (BVR) in these patients is dependent on adequate size of the left cardiac structures.
PURPOSE: This study evaluated the impact of the hemodynamic characteristics of atrial septal defects (ASDs) on MV growth following surgical repair.
METHODS: We retrospectively reviewed the data of patients diagnosed with CoA or IAA between 2007 and 2024. The z score for MV size measured 6 months postoperative
