Triple Vector-Borne Co-infection During Pregnancy in the United Arab Emirates: A Case Report at 21 Weeks' Gestation.

Pregnancy is a unique physiological state characterised by altered immune responses that may predispose women to severe manifestations of infection. Vector-borne diseases such as malaria and dengue pose significant risks to pregnant women and their foetuses, including miscarriage, preterm labour, placental abruption, and intrauterine growth restriction (IUGR). Co-infection with multiple vector-borne pathogens is extremely rare but can exacerbate maternal morbidity and complicate foetal outcomes. This case report aims to describe the clinical presentation, diagnosis, management, and maternal an

Plasmodium vivax Malaria in Afghanistan: A Scoping Review of Epidemiology, Relapse, Treatment and Elimination Challenges.

BACKGROUND: Plasmodium vivax predominates in Afghanistan. Recurrent disease and the safety requirements of anti-relapse treatment complicate elimination, while routine case counts provide limited information about completed radical cure. OBJECTIVE: To synthesize Afghanistan-specific evidence on P. vivax epidemiology, recurrence, treatment, G6PD safety, diagnosis, resistance and elimination, and translate it into measurable service priorities. METHODS: This JBI- and PRISMA-ScR-guided nested scoping review reassessed 287 publications and 45 reports from a parent evidence map using a vivax-focu

Severe Plasmodium vivax Malaria Complicated by Secondary Hemophagocytic Lymphohistiocytosis, Disseminated Intravascular Coagulation, and Acute Pancreatitis: A Case Report.

Plasmodium vivax (P. vivax) malaria, once regarded as a comparatively benign infection, is increasingly recognized as a cause of severe, potentially life-threatening multisystem disease. Concurrent secondary hemophagocytic lymphohistiocytosis (HLH), disseminated intravascular coagulation (DIC), and acute pancreatitis complicating P. vivax infection is exceptionally rare. A previously healthy 19-year-old Bangladeshi woman presented with a 5-day history of high-grade fever with rigors, generalized weakness, and upper abdominal pain. Laboratory evaluation demonstrated pancytopenia, markedly eleva

Pediatric Mixed Plasmodium vivax-P. falciparum Infection with Disparate Parasitemias: Diagnostic and Surveillance Challenges.

Background and Clinical Significance: Malaria remains a significant public health issue in Latin America, where Plasmodium vivax predominates but P. falciparum continues to circulate. Mixed-species infections are uncommon and can pose diagnostic challenges, particularly when parasite densities differ markedly, increasing the risk of underdetecting P. falciparum with conventional methods. Case report: We report a 9-year-old boy from an endemic area with a six-day febrile syndrome. Thick smear and peripheral blood film microscopy, complemented by rapid diagnostic tests for pan-Plasmodium and HRP

Malaria-Associated Acute Kidney Injury: A Key Driver of Mortality in Endemic Regions.

BACKGROUND: Malaria remains a major global health challenge, particularly in endemic regions where severe disease contributes substantially to morbidity and mortality. Among its systemic complications, malaria-associated acute kidney injury (MAKI) is increasingly recognized as a significant yet underappreciated contributor to poor clinical outcomes. Despite advances in antimalarial therapy, MAKI continues to be associated with high mortality and growing evidence suggests a link to long-term renal sequelae, including chronic kidney disease (CKD). This review synthesizes current evidence on the

Stage-aware transcriptomics reveals selective haplotype persistence in short-term ex vivo cultured Plasmodium vivax.

Plasmodium vivax (Pv) infections are developmentally asynchronous and often polyclonal, complicating interpretation of bulk parasite transcriptomes. Here, we analyzed paired in vivo and short-term ex vivo transcriptomes from Ethiopian clinical isolates using stage deconvolution and PvMSP1 haplotyping. Ex vivo maturation modestly increased inferred schizont representation while largely preserving the proportion of trophozoites and gametocytes. After adjustment for parasite stage composition, in vivo and ex vivo transcriptomes remained globally similar, with no genes significantly differentially

Low prevalence of sub-microscopic malaria among rural communities of an endemic area in Kapit Division, Malaysian Borneo.

BACKGROUND: Malaria remains a major public health concern, especially with zoonotic Plasmodium knowlesi presenting specific challenges in Southeast Asia. In Malaysia, Sarawak ranks second in P. knowlesi cases after Sabah. Rural populations in endemic areas experience increased exposure to forest and forest-edge zones where vector mosquitoes breed, thereby raising infection risk and complicating routine surveillance. These groups may carry asymptomatic or submicroscopic infections that act as hidden reservoirs, potentially hindering elimination efforts. This study aimed to assess the prevalence

Computational Screening of Djiboutian Medicinal Plants Reveals Potential Dual Inhibitors Against Plasmodium falciparum and Plasmodium vivax.

OBJECTIVES: Malaria remains a major global health burden, particularly in endemic regions such as Djibouti, where Plasmodium falciparum and Plasmodium vivax co-circulate, complicating disease control strategies. Increasing resistance to current antimalarial drugs reduces treatment effectiveness and highlights the urgent need for new, safe, and affordable therapeutic agents. This study aimed to identify potential inhibitors from Djiboutian medicinal plants using an integrated in silico approach targeting key proteins from both parasite species. METHODS: A library of 222 phytoconstituents was s

Malaria in Menoreh Hills, Indonesia: an eco-epidemiological perspective on persistence of malaria transmission.

Persistent malaria transmission remains a considerable challenge in Indonesia despite national elimination efforts, with the Menoreh Hills representing a residual transmission hotspot characterized by ecological and socio-behavioral complexity. This highland region, spanning the boundaries of the Kulon Progo, Purworejo, and Magelang districts presents a complex eco-epidemiological setting in which malaria transmission persists despite substantial reductions in incidence. Spatial analyses consistently reveal micro-foci and cross-district clusters, underscoring the structural and ecological inte

Comparable severity of Plasmodium vivax and Plasmodium falciparum malaria in hospitalized patients in the Peruvian Amazon.

BACKGROUND: Plasmodium vivax malaria has long been considered less severe than P. falciparum infection. However, there is growing evidence that P. vivax can lead to severe and life-threatening disease, although its burden is likely to be underestimated, particularly in low-transmission settings. This study aimed to contextualize the application of World Health Organization (WHO) severe malaria criteria in a low-transmission setting by describing the clinical and parasitological profile of hospitalized malaria patients in the Peruvian Amazon. METHODS: We conducted a retrospective hospital-based

Presumed Relapse of Plasmodium vivax Malaria Presenting With Thrombocytopenia in a Non-endemic Area: A Case Report.

Malaria remains a significant global health problem, particularly in tropical regions. Although Plasmodium vivax infection is often considered benign, it possesses the unique ability to cause relapse through dormant liver stages known as hypnozoites. Delayed diagnosis may occur in non-endemic or low-incidence settings because malaria can mimic more common febrile illnesses such as dengue infection. We report a case of presumed relapse of P. vivax malaria in a 54-year-old female presenting in a non-endemic area with cyclical fever, chills, sweating, headache, nausea, and vomiting. The patient h

Clinical and Hematological Profile of Rare Manifestations Associated with Malaria: A Retrospective Observational Study.

BACKGROUND: Malaria continues to be a significant global health challenge, caused predominantly by Plasmodium falciparum and Plasmodium vivax. While its classical manifestations are well described, rare and atypical organ-specific complications are increasingly recognized and may mimic other tropical diseases, delaying diagnosis and management. OBJECTIVE: This study aimed to identify the spectrum of rare clinical and hematological manifestations of malaria and describe patient outcomes following appropriate therapy. METHODS: This retrospective case series analyzed twelve adult patients with

Mixed Plasmodium vivax and falciparum Malaria in a Young Infant: The Critical Role of Peripheral Smears in an Endemic Region.

Malaria remains a major global public health challenge, particularly in endemic regions and among travelers. Mixed-species malarial co-infections in these areas are frequently underdiagnosed or underestimated due to their lower incidence, especially in vulnerable pediatric populations. Such co-infections often result in severe complications and delayed targeted therapy. Here, we report the case of an 18-month-old male infant who presented with a low-grade fever, abdominal pain, severe anemia, and hepatosplenomegaly. Given the regional endemicity and season, Plasmodium vivax infection was initi

Chronic Malaria Is Associated With Trauma-related Splenic Rupture Requiring Splenectomy.

Splenic rupture is a recognized complication of acute Plasmodium falciparum and Plasmodium vivax malaria, but the risk of splenic rupture in chronic asymptomatic infections is unknown. In Timika, Papua, Indonesia, we determined the proportion of PCR-detectable asymptomatic peripheral parasitemia in patients undergoing trauma-related splenectomy (2015-21) and found it was more than twice the proportion compared to a 2013 household survey of the general population (87.9% [29/33] vs 38.6% [697/1807]; P < .0001). Our findings suggest asymptomatic parasitemia with either P. falciparum or P. vivax i

Community awareness and health providers' perspectives on zoonotic Plasmodium knowlesi malaria in Thailand: A mixed-methods assessment.

BACKGROUND: Plasmodium knowlesi poses an emerging challenge for malaria control in Southeast Asia due to its zoonotic nature, diagnostic complexity and lack of species-specific control strategies. These factors complicate both prevention and case management efforts. Understanding both community awareness and healthcare provider perspectives is critical for informing targeted responses. This study aimed to assess awareness of P. knowlesi malaria among at-risk community members in southern Thailand and to explore contextual challenges through qualitative insights from healthcare providers, inclu

Malaria-associated splenic haemorrhage requiring emergency splenectomy: a rare complication managed in a resource-constrained setting.

Falciparum malaria and Vivax malaria are notorious for the development of multiple complications, including acute kidney injury, acute liver injury, severe hemolysis, bone marrow suppression, severe acidosis, hypoglycemia and CNS complications. We hereby present two cases of rare complications associated with Malaria, which were managed in a resource-constrained setting in Eastern Africa. Two individuals working in the malaria endemic region of South Sudan were detected with Mixed Plasmodium falciparum-Plasmodium vivax when they reported to the hospital with acute febrile illness for two days.
