Sustained remission of post-Fontan protein-losing enteropathy achieved by combination therapy with high-dose spironolactone and the somatostatin analogue octreotide: a case report.

BACKGROUND: Protein-losing enteropathy (PLE) is a serious complication affecting approximately 5–12% of patients following the Fontan procedure for congenital heart disease and is associated with high mortality. Its pathophysiology is multifactorial, involving elevated central venous pressure, abnormal intestinal lymphatics, and altered mesenteric vascular resistance. No gold-standard therapy has been established, and current management approaches include hemodynamic optimization, nutritional therapy, and medications such as somatostatin analogues and spironolactone. Though spironolactone is f

Eccentricity Index Is Associated With Pulmonary Arterial Hypertension in Children After Repair of Complete Atrioventricular Septal Defects.

INTRODUCTION: Eccentricity Index is an echocardiographic predictor of disease severity in children with pulmonary arterial hypertension (PAH). Its utility after atrioventricular septal defect (AVSD) repair remains unclear due to surgical manipulation of the ventricular septum. We evaluated end-systolic (EIs) and maximal (EIm) as predictors of hemodynamics post-AVSD repair. METHODS: Cohort 1 was 89 children, 5-24 months after complete AVSD repair, and similarly aged controls. Subgroups of those with (+) and without (-) PAH were based on other echocardiographic markers of PAH. Cohort 2 was 20 c

Diagnostic Yield of Exome Sequencing in Patients With Congenital Heart Disease From Southern Africa.

BACKGROUND: Congenital heart disease (CHD) is a leading cause of pediatric morbidity and mortality worldwide. The genetics of CHD in African populations is not well understood, although it has been shown in other settings that a genetic diagnosis can have implications for patient management and risk stratification. In this study, we aimed to identify pathogenic and likely pathogenic (P/LP) variants in a cohort of patients with CHD from Southern Africa. METHODS: Exome sequencing was used to screen 356 patients with diverse cardiac phenotypes from South Africa and Namibia. RESULTS: A P/LP vari

Balanced Atrio Ventricular Canal Defect: Distilling the Data.

Complete atrioventricular canal defect (CAVC) is the seventh most common congenital heart defect, affecting 1 in 1712 live births. Embryologically CAVC defect arises from failure of normal fusion of the endocardial cushions resulting in a spectrum of abnormalities that involve the atrial septum, atrioventricular (AV) valve, subvalvar apparatus, and the ventricular septum. This article will discuss historic timelines, evolution of management strategies, and outcomes following current surgical approaches in well-balanced CAVC. Recent advances in genetics and imaging techniques will also be discu

Managing Feeding Refusal as an Unusual Presentation of Severe Atrioventricular Valve Regurgitation in a PostFontan Patient with Down Syndrome.

BACKGROUND: Feeding refusal is an uncommon and often overlooked manifestation of hemodynamic deterioration in patients with Fontan physiology. CASE PRESENTATION: We report a child with trisomy 21 and unbalanced atrioventricular septal defect who presented several years after Fontan completion with progressive feeding refusal and ascites. Comprehensive evaluation revealed severe atrioventricular valve regurgitation without Fontan pathway obstruction. Medical optimization resulted in clinical improvement, and surgical valve repair was planned. CONCLUSION: Feeding refusal may represent an early

Improvement of acute hemolysis after mechanical valve replacement by prosthesis rotation without redo replacement.

UNLABELLED: A1-year-old female with a partial atrioventricular septal defect and a borderline left ventricular size underwent a left-sided atrioventricular valve plasty at four months. Due to persistent regurgitation, a reoperation was performed at one year for left-sided atrioventricular valve replacement. The valve was implanted in an anti-anatomical position and the side of the posterior wall translocated toward the left atrium because of a narrow annulus. Postoperative course was initially good, but lactate dehydrogenase levels significantly rose to 2262 U/L by postoperative day 14. Echoca

A Systematic Review of Modeling Platforms for Atrioventricular Valves in Atrioventricular Septal Defects.

Given the high rate of re-intervention in patients with atrioventricular septal defects (AVSD), there is a continuing need to accurately model the complex valve anatomy and surgical repair strategies. This systematic review examined the current scientific landscape for modeling platforms designed to study abnormal congenital heart valves that are used or could be used for AVSD investigation. Of 1,050 sources screened, 47 were included (14 in silico; 26 in vitro; 7 in vivo). In silico models studying valve pathologies have advanced in recent years, but few targeted AVSD valves. In vitro platfor

Acute Severe Left Atrioventricular Valve Regurgitation Secondary to Spontaneous Rupture of Chordae Tendinae in a Patient with Down Syndrome.

We present a unique case of spontaneous chordal rupture in a young infant with Down syndrome and unrepaired transitional atrioventricular septal defect. This resulted in abrupt onset of severe left atrioventricular valve regurgitation, cardiogenic shock, and multiorgan dysfunction. Our case highlights the need for a high index of suspicion for a ruptured chord in a young infant presenting with acute onset of pulmonary edema.
