First- and Second-Trimester Cardiovascular Anomalies in Trisomy 21 Fetuses: Anatomy, Embryology, Genetics and Imaging.

Background: Trisomy 21 (T21) is strongly associated with congenital heart disease, particularly atrioventricular septal defect (AVSD), ventricular septal defect (VSD), atrial septal defect (ASD) and selected conotruncal and arch anomalies. First- and second-trimester ultrasound, Doppler and fetal cardiac MRI enable increasingly early and detailed characterization of these lesions, while advances in molecular cardiogenesis have linked specific phenotypes to dosage-sensitive genes on chromosome 21. Methods: This narrative review synthesizes contemporary evidence on structural and functional card

Association of postoperative haemodynamic parameters with progression of left atrioventricular regurgitation in infants: leveraging high-fidelity data: a hypothesis-generating study.

BACKGROUND: The success of an atrioventricular septal defect repair depends largely on the function of the left atrioventricular valve. Given the exposure of this valve to systemic pressures, postoperative haemodynamic management has been of interest, though practices are variable. METHODS: A single-centre, retrospective study of infants less than 1 year of age admitted to the paediatric cardiac ICU following atrioventricular septal defect repair between January 2022 and May 2025 was conducted. Utilising high-fidelity physiologic data, the primary aim of this study was to characterise haemody

Repeated epicardial lead dislodgement in a child with trisomy 21 and complete atrioventricular block: A case report.

UNLABELLED: Lead dislodgement is a rare complication of epicardial leads, which are generally chosen for pacemaker implantation in pediatric patients. Here, we report the case of a 12-year-old girl with trisomy 21 and a complete atrioventricular septal defect. The patient required epicardial pacemaker implantation; however, the epicardial left ventricular lead unexpectedly dislodged twice. This report discusses the potential causes and outlines our management strategy. LEARNING OBJECTIVE: Age, body size, growth-related changes, lifestyle, and durability complicate pacemaker implantation in ch

Adult congenital heart disease: a review of the moderately complex lesions.

The population of adults living with congenital heart disease (CHD) has expanded substantially over the last 40 years due to advances in diagnostic, transcatheter, and surgical techniques. Consequently, clinicians, regardless of their specialty, will encounter patients presenting with sequelae of both simple and complex CHD more frequently. Familiarisation with the pathophysiology and basic management of these conditions, including the role of adult congenital heart disease (ACHD) centres, is, therefore, vital. The moderately complex congenital heart defects that may present in adulthood inclu

Outcome of primary mitral valve surgery in pediatric patients: Results from a bi-centric study.

OBJECTIVES: Due to heterogenous mitral valve(MV) disease expression, primary MV surgery in children might be challenging, entailing suboptimal results. METHODS: In this bi-centric collaboration all children undergoing primary MV surgery from 1987 to 2023 were included. Patients with atrioventricular septal defect and univentricular physiology were excluded. Study end-points were survival and need for MV reoperation and/or replacement, including multivariable risk factor analysis. RESULTS: Overall 109 patients underwent 158 procedures for predominantly mitral regurgitation(MR)(n = 82, 74.5%)

Transitional atrioventricular septal defect: diagnostic variability and its clinical significance.

INTRODUCTION: Discrepancies between preoperative echocardiographic diagnosis and intraoperative surgical findings for incomplete atrioventricular septal defects are frequently encountered. We assess the frequency of diagnostic discordance and its impact, focusing on subtype-specific predictors of the postoperative course. METHODS: We reviewed 172 patients post repair of incomplete atrioventricular septal defects at a single institution between November 2000-February 2025, including transitional atrioventricular septal defects (n = 74), partial atrioventricular septal defects (n = 71), and pri

Complete atrioventricular septal defect in Thoroughbred foals: Echocardiographic and postmortem findings.

Complete atrioventricular septal defect (AVSD) is rarely reported in horses. We retrospectively reviewed three Thoroughbred foals (2017-2023) with necropsy-confirmed complete AVSD and summarized their clinical presentation, transthoracic echocardiographic findings, and postmortem anatomy. All foals showed early signs of congenital cardiac disease including weakness, failure to thrive and cardiac murmurs. Echocardiography demonstrated a common atrioventricular junction with a single atrioventricular valve and combined atrial and ventricular septal defects; all were classified as Rastelli type C

Risk factors and long-term outcomes of complete atrioventricular septal defect repair in children.

BACKGROUND: The prognostic determinants following primary biventricular repair for complete atrioventricular septal defect are complex. We describe contemporary surgical outcomes from a large cohort. METHODS: We retrospectively enrolled 342 patients who underwent primary biventricular complete atrioventricular septal defect repair between January 2010 and April 2025. Univariable and multivariable Cox regression were used to determine predictors. Reoperation and mortality were described using cumulative incidence functions estimated by Fine-Gray competing-risk models. RESULTS: Median follow-u

Infective endocarditis of the right atrioventricular valve and transvenous pacing lead in a patient with Down syndrome.

An 11-year-old with Down syndrome, autism, prior atrioventricular septal defect repair, and a transvenous pacemaker presented with prolonged fever and raised inflammatory markers. Eight years prior, she had a group A streptococcal sepsis with infection of an epicardial pacemaker. Echocardiography demonstrated a large vegetation on the right atrioventricular valve, with encasement of the pacemaker lead. Following treatment with intravenous antibiotics, she underwent a right atrioventricular valve repair, vegetation excision, and epicardial pacemaker placement. Post-operative recovering was unev

Inpatient hospitalizations in adults with congenital heart disease and down syndrome: A national perspective.

BACKGROUND: Adults with Down syndrome (DS) have a greater likelihood of being diagnosed with congenital heart disease (CHD) and may face unique cardiovascular, respiratory, and immunologic health burdens during hospitalization. While CHD survival has improved in recent decades, national data on hospitalization outcomes of patients diagnosed with DS remain limited. OBJECTIVES: The present study aimed to evaluate trends in hospital admissions, inpatient management, and perioperative outcomes stratified by the presence of DS among CHD patients. METHODS: This was a retrospective cohort of the 20

Genetic Variation and Association with Post-Operative Outcomes for Neonates and Infants in the Cardiac Intensive Care Unit.

INTRODUCTION: Congenital heart defects (CHD) occur in 1% of live births, with an estimated at least 33% of affected infants having genetic defects. It has become standard to screen children with CHD for genetic findings that could aid clinical decision-making, yet modern testing yields large volumes of information without understanding clinical utility. Genome-wide studies have identified associations between large copy number variants and neurocognitive outcomes in patients with CHD. Still, little research exists on the prognostic power of screening tests such as microarrays. The goal of this

Lung-Protective Effect of Alveolar Recruitment Maneuvers in Children with Right Vertical Infra-Axillary Thoracotomy for Repair of Congenital Heart Disease.

BACKGROUND/OBJECTIVES: Right vertical infra-axillary thoracotomy to repair ventricular septal defects (VSDs) and atrial septal defects (ASDs) is less invasive than conventional surgical repair via median sternotomy. However, right vertical infra-axillary thoracotomy (RVIAT) may result in unilateral lung injury, a serious postoperative complication requiring extracorporeal circulation and unilateral lung collapse. The aim of this study was to evaluate whether repeated lung recruitment provides enhanced respiratory compliance and lung oxygenation in children who have undergone right vertical inf

Brain Abscess Revealing an Undiagnosed Atrioventricular Septal Defect With a Common Atrium in a Young Adult.

Brain abscess is an uncommon but life-threatening intracranial infection that most often arises from contiguous spread but may also occur through hematogenous dissemination in patients with congenital heart disease. This is usually associated with defects that permit right-to-left shunting of septic emboli. Its occurrence in association with atrioventricular septal defect (AVSD) is rarely reported, particularly as the initial presentation in adulthood. We report an 18-year-old male who presented with a generalized tonic-clonic seizure preceded by a 4-day history of frontal headache. Examinatio
