Systematic analysis of the atrioventricular canal in 129 heart specimens identified anatomic and microarchitectural advantages in Down syndrome.

OBJECTIVE: To systematically analyze the macroscopic and microscopic anatomic characteristics of the common atrioventricular valve in patients with Down syndrome (DS) and those without DS. Such findings could explain the difference in outcomes between these 2 groups. METHODS: We reviewed the clinical records and examined archived heart specimens with unrepaired common atrioventricular canal (AVC) defect. The common atrioventricular valve (CAVV) annulus and leaflets were measured for dimensions and area. The histology of leaflets at age <1 month, 3 months, and 6 months were reviewed. Leaflet n

Mitral and tricuspid valve repair in an operated case of complete atrioventricular septal defect: A surgical challenge case report!

Atrioventricular canal defects constitute about 3%-5% of total congenital heart defects, with a prevalence of 0.3-0.4/1000 live births. Mitral valve defects are associated with atrioventricular canal (septal) defects in most cases. Our case is of a 37-year-old male who underwent double patch closure of atrial and ventricular septal defects at the age of 24 years, now presenting with NYHA-III symptoms of severe mitral regurgitation and severe tricuspid regurgitation. The mitral valve was of atrio-ventricular canal type. We repaired both mitral and tricuspid valves using soft rings. Because of a

Absence of the Left Circumflex Artery in Congenital Heart Disease With Vasospastic Myocardial Infarction.

BACKGROUND: Congenital absence of the left circumflex artery (LCx) is extremely rare, and we have limited understanding of its mechanisms, diagnosis challenges, and clinical implications. CASE SUMMARY: A 36-year-old woman with a history of atrioventricular septal defect repair presented with recurrent chest pain. High-sensitivity cardiac troponin T was 0.441 ng/mL (reference range: <0.014 ng/mL), and the electrocardiography showed ST-segment depression in V1-V4. Coronary angiography revealed congenital absence of the LCx, with perfusion from the right coronary artery (RCA). Severe stenosis in

Thirty years of left atrioventricular valve reoperations following atrioventricular septal defect repair: Impact of valvar stenosis.

BACKGROUND: The most frequent reason for reoperation after an index atrioventricular septal defect (AVSD) repair is left atrioventricular valve (LAVV) dysfunction. LAVV stenosis (LAVVS), although less common than regurgitation, may be a more problematic lesion to repair. We sought to identify baseline LAVV anatomic characteristics and outcomes associated with both LAVV reoperation overall and LAVVS in particular. METHODS: A retrospective review was conducted of patients who underwent a biventricular index AVSD repair between January 1995 and August 2024. All AVSD subtypes were included. Univa

Utility of Intraoperative 3D TEE to Unmask a Hidden Intracardiac Fistula.

A 70-year-old woman with a previously repaired atrioventricular septal defect at 8 years of age was found to have severe mitral valve regurgitation in the setting of a mitral valve cleft identified on transesophageal echocardiogram (TEE) and computed tomography imaging. She underwent surgical repair and was found to have an intracardiac shunt on intraoperative TEE that was initially obscured by the severe mitral regurgitant jet. This case highlights the utility of intraoperative 3-dimensional TEE which helped to identify and localize the intracardiac shunt and allowed for appropriate repair of

Atrioventricular Inflow and Subsequent Ventricular Growth After Staged Ventricular Recruitment for Unbalanced Atrioventricular Canal Defect.

BACKGROUND: In patients with unbalanced atrioventricular canal (uAVC) defect, the hypoplastic ventricle grows variably after staged ventricular recruitment, which involves atrial septation without ventricular septal defect closure. We hypothesized that the atrioventricular inflow size into the hypoplastic ventricle after recruitment correlates with ventricular growth. METHODS: This single-institution retrospective review included 53 patients who underwent staged ventricular recruitment for uAVC between 2011 and 2024. Atrioventricular inflow jet width into the hypoplastic ventricle was assesse

A Rare Case of Dextrocardia With Atrioventricular Septal Defect and Situs Inversus in an 8-Month-Old Infant Complicated by Severe Acute Malnutrition.

The key clinical message in this case report highlights the rare coexistence of situs inversus totalis, dextrocardia, atrioventricular septal defect, and severe acute malnutrition in an Ethiopian infant. It underscores the need for heightened awareness, timely diagnosis, and integrated management strategies for such complex congenital and nutritional disorders and emphasizes the need for early surgical referral in resource-limited settings, as delayed intervention worsens outcomes.

Rare Presentation of Partial Atrioventricular Septal Defect in the Elderly: A Case Report.

Partial atrioventricular septal defect (AVSD) is a congenital heart anomaly that typically manifests in childhood and rarely presents in elderly patients. However, this anomaly can lead to hemodynamically significant cardiopulmonary complications when left untreated. In this report, we present a case of a 68-year-old male patient who presented with symptoms of heart failure and atrial flutter and was found to have partial AVSD incidentally discovered by echocardiography. He was stabilized with diuretics and beta-blockers, and anticoagulation therapy was initiated, given his risk for thromboemb

Unilateral absence of the pulmonary artery in a paediatric cohort.

We present six paediatric patients with unilateral absence of the pulmonary artery in pursuance of rising the clinician's recognition. Unilateral absence of the pulmonary artery was misdiagnosed in one patient by echocardiography. One patient had corrective surgery of a complete atrioventricular septal defect, and another had a right lobectomy. The remaining patients did not undergo any intervention and were asymptomatic at the end of the study period. Although there is a lack of consensus regarding optimal surgical approach, we opted for a more conservative strategy with good results.
