An Extremely Rare Association of the Giant Right Atrium with Uncorrected Tetralogy of Fallot in an Adolescent Male Patient.

Uncorrected tetralogy of Fallot (TOF) can be associated with widespread abnormalities, but the giant right atrium (RA) is an extremely rare one. The giant RA can cause additional problems causing atrial arrythmias, thromboembolism, and compression of adjacent structures. Tricuspid regurgitation is common with annular dilatation. In symptomatic cases, reduction atrioplasty is recommended. Sternotomy should be careful to avoid damage to the giant RA. In asymptomatic cases, anticoagulation should be carried out. We report a rare case of uncorrected TOF associated with the giant RA and got success

Selective Hemostasis in Repaired Tetralogy of Fallot With an Absent Left Pulmonary Artery: The Bleeding Lifeline.

BACKGROUND: Hemoptysis after repair of tetralogy of Fallot is uncommon and should prompt evaluation for residual pulmonary arterial lesions, infection, and systemic-to-pulmonary collaterals. CASE SUMMARY: A 30-year-old woman with repaired tetralogy-type anatomy and an absent left pulmonary artery presented 5 months after intracardiac repair with hemoptysis. Postoperative computed tomographic angiography showed an absent/nonvisualized left pulmonary artery, a small focal hemorrhagic opacity in the left lower lung, and systemic collateral supply from the descending thoracic aorta and left infer

Giant Aneurysm of a Classic Blalock-Taussig Shunt in Adult Tetralogy of Fallot.

BACKGROUND: Giant aneurysmal degeneration of a classic Blalock-Taussig (BT) shunt is a rare, life-threatening late complication of tetralogy of Fallot (TOF) palliation. CASE: A 47-year-old woman with unrepaired TOF palliated in childhood presented with exertional dyspnea and a right thoracic mass. Echocardiography revealed severe right ventricular outflow tract obstruction, a large ventricular septal defect, and a pulsatile extracardiac structure. Computed tomography and cardiac magnetic resonance imaging confirmed a giant (72 × 66 mm) true aneurysm of a right BT shunt. She underwent successf

Assisted Reproductive Technology and Risk of Congenital Heart Defects in the Offspring: A Systematic Review and Meta-Analysis.

BACKGROUND: To date, >10 million children have been conceived via assisted reproductive technology (ART) globally. However, there are some concerns about potential adverse outcomes associated with ART-conceived pregnancies, including the risk of congenital heart defects (CHDs). This meta-analysis aimed to evaluate the association between ART and the risk of major CHDs as well as their specific subtypes. METHODS: Major electronic databases were screened for studies comparing the incidence of CHD following ART-conceived pregnancies versus spontaneous conception. The primary end point was study-

Genotype-phenotype variability in TRMT10A deficiency: divergent developmental outcomes associated with biallelic loss-of-function variants.

Loss-of-function variants in TRMT10A are known to cause a rare autosomal recessive disorder primarily characterized by microcephaly, short stature, and impaired glucose metabolism. However, the extent of phenotypic variability and the factors influencing clinical heterogeneity remain poorly understood. Here, we describe two individuals harboring distinct biallelic loss-of-function variants in TRMT10A who exhibited divergent developmental phenotypes. The first patient, a 34-year-old man with a homozygous early frameshift variant, presented with preserved cognitive function and previously unrepo

Pulmonary Valve Growth After Valve-Sparing Right Ventricle Outflow Tract Stenting in Tetralogy of Fallot: A Single-Centre Retrospective Cohort Study.

Right ventricular outflow tract (RVOT) stenting is an established initial palliation for neonates and infants with Tetralogy of Fallot (ToF). However, data on valve-sparing RVOT stenting and its impact on pulmonary valve (PV) growth are limited. Our aim was to evaluate pulmonary valve annular growth following valve-sparing RVOT stenting and to examine how this growth related to the type of definitive surgical repair performed. This retrospective single-centre study included 96 consecutive patients with palliated ToF over a 16-year period. Valve-sparing RVOT stenting was achieved in 46 patients

Uncorrected Tetralogy of Fallot Complicated by Brain Abscess and Postoperative Intracranial Haemorrhage: A Case Report.

Cerebral abscess is a rare but potentially fatal complication of cyanotic congenital heart disease, in which chronic hypoxia, polycythaemia and right-to-left shunting allow bacteraemia to bypass pulmonary filtration. A woman in her 20s with uncorrected tetralogy of Fallot presented with severe headache, right-sided weakness and seizures. Examination showed hemiparesis, hypoaesthesia and hyperreflexia; blood tests revealed secondary polycythaemia and leucocytosis. No dental, sinus, otological or respiratory source was identified. Unenhanced computed tomography demonstrated a left parietal space

A Comparative Evaluation of ChatGPT and Google Search Output for Caregiver Education in Tetralogy of Fallot.

INTRODUCTION: Caregivers of children with tetralogy of Fallot (TOF) frequently seek medical information online to better understand their child's diagnosis, treatment, and long-term care. Recently, generative artificial intelligence (AI) tools, including Google AI Overview and ChatGPT, have emerged as widely accessible sources of health information. However, the quality, readability, and accuracy of these platforms for caregiver education in congenital heart disease remain poorly characterized. METHODS: Forty frequently asked caregiver questions regarding TOF were identified using Google's "P

Unique coexistence of tetralogy of Fallot and congenital heart block linked to maternal autoantibodies in a Down syndrome fetus: a case report.

We report a rare fetal presentation of tetralogy of Fallot and complete atrioventricular block linked to maternal anti-Ro antibodies. However, trisomy 21 was confirmed postnatally. Despite pacemaker implantation, the neonate died on day 55 due to pulmonary complications. This complex case highlights diagnostic and management challenges involving tetralogy of Fallot, Down syndrome, and complete heart block associated with maternal autoantibodies.

End-Diastolic Forward Flow Through the Pulmonic Valve in Carcinoid Heart Disease.

BACKGROUND: End-diastolic forward flow (EDFF) through the pulmonic valve is classically described after repaired tetralogy of Fallot and is usually attributed to restrictive right ventricular (RV) physiology. Its significance in carcinoid pulmonic valve disease is less established. CASE SUMMARY: We describe 6 patients with carcinoid heart disease and pulmonic valve involvement referred for operative management. Perioperative echocardiography demonstrated late-diastolic antegrade flow through the pulmonic valve after atrial activation and before ventricular systole, consistent with EDFF during

Decoding Long Noncoding RNAs in Cardiac Development and Congenital Heart Disease: Recent Advances and Clinical Perspectives.

Long noncoding RNAs (lncRNAs) have emerged as crucial regulators in cardiac development and congenital heart disease. With advances in high-throughput sequencing, single-cell omics, and epigenetic profiling, accumulating evidence suggests that lncRNAs participate in transcriptional, posttranscriptional, and structural remodeling processes underlying cardiac morphogenesis. This in-depth review summarizes recent progress in elucidating the roles of lncRNAs in heart development and congenital heart disease pathogenesis. Specifically, we discuss how lncRNAs modulate key developmental signaling pat

Survival and Associated Factors for Mortality Among Infants with Tetralogy of Fallot in a Middle-Income Country.

Tetralogy of Fallot (TOF) is the most common cyanotic heart disease, yet population-based survival data from low- and middle-income countries remain limited. This study evaluated survival up to 15 years of age and identified factors associated with mortality in Malaysia. This population-based cohort included all children with TOF registered in the Paediatric Cardiology Clinical Information System who were born between 2006 and 2022. Clinical characteristics and outcomes were analysed. Kaplan-Meier analysis estimated survival probabilities at 1, 5, 10, and 15 years, while Cox proportional hazar

The grey zone before pulmonary valve replacement in repaired tetralogy of Fallot: a paediatric perspective on the 2024 American Heart Association scientific statement.

The 2024 American Heart Association scientific statement broadened risk assessment in repaired tetralogy of Fallot beyond right ventricular volume, bringing together ejection fraction, exercise capacity, pressure load, and a validated risk score. One group of patients remains difficult: those whose right ventricular volumes are borderline and who meet no threshold for intervention. We describe this borderline range as a grey zone, with an indexed end-diastolic volume of approximately 140 to 160 mL/m2 and an indexed end-systolic volume of approximately 70 to 80 mL/m2. Within this range, no sing

Cyanotic Heart Disease.

Cyanotic congenital heart defects are malformations of the heart that affect its structure and function. The etiology of most of these defects is multifactorial, but some common causes include maternal conditions, genetic defects, and genetic mutations. Six of the most common cyanotic congenital heart defects include Tetralogy of Fallot, transposition of the great arteries, persistent truncus arteriosus, total anomalous pulmonary venous return, tricuspid atresia, and Ebstein's anomaly. These defects share several common features, however, each is distinguished by key cardiac abnormalities and

Genomic Yield of Long-Read Sequencing in Congenital Heart Disease.

BACKGROUND: Congenital heart disease (CHD) is the most common birth defect. Ninety percent of isolated cases remain genotype-elusive despite short-read genome sequencing (GS). Our goal was to assess genomic yield with long-read GS compared with short-read GS in CHD. METHODS: We performed Illumina short-read GS on 1101 CHD probands. In a subset of 46 genotype-elusive probands, we performed additional PacBio long-read GS as well as parental sequencing of 7 complete trios. We compared variant calls genome-wide, including across dark and CHD genes in samples with paired short-read and long-read G

Machine Learning for Predicting 12-Month Survival after Liver Transplantation: Temporal Validation, Calibration, and Decision-Analytic Evaluation.

Accurate prediction of posttransplant survival is critical for optimizing liver allocation under organ scarcity. Although the Model for End-Stage Liver Disease (MELD) is widely used for prioritization, it was not designed to estimate posttransplant outcomes, limiting its utility for individualized risk stratification. We developed and temporally validated machine-learning models to predict 12-month survival after liver transplantation using routinely collected donor-recipient variables from a real-world cohort. Sequential temporal validation was used to approximate prospective deployment. Perf
