Perioperative Challenges of Unrepaired Tetralogy of Fallot in a Child With Severe Secondary Erythrocytosis: Lessons for the Modern Era.

In the United States, secondary erythrocytosis in children with unrepaired Tetralogy of Fallot (ToF) is rare yet may present in children from underresourced environments. We report our experience caring for a 5-year-old male with unrepaired ToF from a low-resourced country presenting with a hematocrit of 76.2%, requiring unique perioperative considerations. This work aligns with the unifying mission among congenital heart providers to improve access to high-quality surgery globally through collaboration.

A complementary proteomics and cardiac magnetic resonance approach to characterize right ventricular remodeling after tetralogy of Fallot repair.

BACKGROUND: Patients with repaired tetralogy of Fallot (TOF) can develop right ventricular (RV) dilation and failure. We sought to utilize an untargeted serum proteomics approach to characterize the proteome signature associated with RV dilation in repaired TOF to improve the understanding of mechanistic drivers of RV remodeling. METHODS: Patients with repaired TOF undergoing cardiac magnetic resonance imaging were recruited. Groups were based on indexed RV end-diastolic volume (RVEDVi). Serum proteomics identified differentially abundant proteins (p value < 0.05), and pathway enrichment iden

An Unexpected Cause of STEMI in Partially Repaired Tetralogy of Fallot.

BACKGROUND: Coronary embolism (CE) is an uncommon but clinically relevant nonatherosclerotic cause of ST-elevation myocardial infarction (STEMI). Adult patients with congenital heart disease may be particularly vulnerable when residual intracardiac shunts, atrial enlargement, and atrial arrhythmias coexist. CASE SUMMARY: A 77-year-old man with partially repaired tetralogy of Fallot, residual ventricular septal defect with bidirectional shunting, and new-onset atrial fibrillation presented with inferior STEMI. Coronary angiography revealed distal right coronary artery occlusion with a large th

Cardiovascular Magnetic Resonance Predictors of Residual Pulmonary Regurgitation and Biventricular Recovery After Pulsta Valve Implantation.

Current recommendations for pulmonary valve replacement are based primarily on cardiovascular magnetic resonance (CMR)-derived right ventricular (RV) volume thresholds established in repaired tetralogy of Fallot. However, patients undergoing self-expanding percutaneous pulmonary valve implantation (PPVI) often have mixed pulmonary regurgitation (PR) and pulmonary stenosis with heterogeneous right ventricular outflow tract (RVOT) anatomy. We investigated CMR predictors of residual PR, residual RV enlargement (RVE), and residual biventricular imbalance (BVI) after Pulsta PPVI. We retrospectively

Septal dysfunction and Abnormal Systolic Stretch Are Associated with Right Ventricular Dysfunction in Repaired Tetralogy of Fallot.

AIMS: Right ventricular (RV) dysfunction is a major determinant of outcomes after Tetralogy of Fallot repair (rToF). Although right bundle branch block is present in most patients and electro-mechanical dyssynchrony (EMD) has been associated with RV dysfunction, only a subset of patients ultimately develops RV dysfunction. We hypothesized that septal dysfunction is a key feature associated with the development of EMD-induced RV dysfunction in rToF. METHODS: We retrospectively studied 126 pediatric rToF patients (median age 14.7 years) with contemporaneous ECG, echocardiography, and cardiac ma

Left ventricular dysfunction and massive intracavitary thrombus in unrepaired tetralogy of Fallot with endomyocardial fibroelastosis: a case report.

Left ventricular dysfunction and thrombosis in unrepaired tetralogy of Fallot are rarely reported. We report the case of a 9-year-old boy with unrepaired tetralogy of Fallot, severe biventricular dysfunction, large left ventricular thrombus, and diffuse subendocardial fibroelastosis. After surgical repair, recovery of ventricular function and thrombus resolution were observed. This case highlights the interplay between cyanosis, myocardial remodelling, and hypercoagulability and emphasises the role of multimodality imaging.

Long-term outcomes in adults with repaired tetralogy of Fallot: a single-centre retrospective cohort.

BACKGROUND: Adults with repaired tetralogy of Fallot represent a growing clinical population at risk for long-term cardiovascular complications. Characterising their postoperative course and late outcomes is essential to guide surveillance and management. OBJECTIVES: To describe the clinical characteristics and long-term outcomes of adult patients (≥18 years) with repaired tetralogy of Fallot who received care at a quaternary-care institution between January 2000 and December 2024. METHODS: We performed a retrospective cohort study. Adult patients with a diagnosis of repaired tetralogy of Fa

Comparative Outcomes of Left Anterior Thoracotomy Versus Resternotomy for Pulmonic Valve Replacement After Tetralogy of Fallot Repair.

OBJECTIVE: Redo pulmonary valve replacement (PVR) in patients with prior tetralogy of Fallot (TOF) repair is surgically challenging. This study aimed to compare the clinical outcomes of the left anterior minithoracotomy (LAT) approach with the traditional resternotomy approach for PVR. METHODS: A retrospective analysis was conducted on 38 TOF patients who underwent redo PVR, categorized into LAT (n = 20) and resternotomy (n = 18) groups. Patients requiring concomitant valvular surgeries or repair of residual intracardiac shunts were excluded. We compared demographics, operative details, and p

4D Flow Acquisition Parameters as Technical Correlates of Hemodynamic Measurements: An Analysis of the multicenter Tetralogy of Fallot Registry of 4D flow Energetics and Kinetics (TREK).

BACKGROUND: Advanced 4D flow cardiovascular magnetic resonance (CMR) biomarkers such as kinetic energy (KE), vorticity (ω), and viscous energy loss (VEL) are increasingly used to characterize abnormal flow in repaired tetralogy of Fallot (rTOF), but their sensitivity to acquisition parameters including magnet vendor, sequence and signal quality has not been defined. We sought to evaluate the real-world variability in 4D flow sequences and technical correlates to 4D flow measurements of the aorta (Ao) and right ventricle, leveraging the multicenter Tetralogy of Fallot 4D flow Registry of Energe

An Extremely Rare Association of the Giant Right Atrium with Uncorrected Tetralogy of Fallot in an Adolescent Male Patient.

Uncorrected tetralogy of Fallot (TOF) can be associated with widespread abnormalities, but the giant right atrium (RA) is an extremely rare one. The giant RA can cause additional problems causing atrial arrythmias, thromboembolism, and compression of adjacent structures. Tricuspid regurgitation is common with annular dilatation. In symptomatic cases, reduction atrioplasty is recommended. Sternotomy should be careful to avoid damage to the giant RA. In asymptomatic cases, anticoagulation should be carried out. We report a rare case of uncorrected TOF associated with the giant RA and got success

Assisted Reproductive Technology and Risk of Congenital Heart Defects in the Offspring: A Systematic Review and Meta-Analysis.

BACKGROUND: To date, >10 million children have been conceived via assisted reproductive technology (ART) globally. However, there are some concerns about potential adverse outcomes associated with ART-conceived pregnancies, including the risk of congenital heart defects (CHDs). This meta-analysis aimed to evaluate the association between ART and the risk of major CHDs as well as their specific subtypes. METHODS: Major electronic databases were screened for studies comparing the incidence of CHD following ART-conceived pregnancies versus spontaneous conception. The primary end point was study-

Genotype-phenotype variability in TRMT10A deficiency: divergent developmental outcomes associated with biallelic loss-of-function variants.

Loss-of-function variants in TRMT10A are known to cause a rare autosomal recessive disorder primarily characterized by microcephaly, short stature, and impaired glucose metabolism. However, the extent of phenotypic variability and the factors influencing clinical heterogeneity remain poorly understood. Here, we describe two individuals harboring distinct biallelic loss-of-function variants in TRMT10A who exhibited divergent developmental phenotypes. The first patient, a 34-year-old man with a homozygous early frameshift variant, presented with preserved cognitive function and previously unrepo

Pulmonary Valve Growth After Valve-Sparing Right Ventricle Outflow Tract Stenting in Tetralogy of Fallot: A Single-Centre Retrospective Cohort Study.

Right ventricular outflow tract (RVOT) stenting is an established initial palliation for neonates and infants with Tetralogy of Fallot (ToF). However, data on valve-sparing RVOT stenting and its impact on pulmonary valve (PV) growth are limited. Our aim was to evaluate pulmonary valve annular growth following valve-sparing RVOT stenting and to examine how this growth related to the type of definitive surgical repair performed. This retrospective single-centre study included 96 consecutive patients with palliated ToF over a 16-year period. Valve-sparing RVOT stenting was achieved in 46 patients

Uncorrected Tetralogy of Fallot Complicated by Brain Abscess and Postoperative Intracranial Haemorrhage: A Case Report.

Cerebral abscess is a rare but potentially fatal complication of cyanotic congenital heart disease, in which chronic hypoxia, polycythaemia and right-to-left shunting allow bacteraemia to bypass pulmonary filtration. A woman in her 20s with uncorrected tetralogy of Fallot presented with severe headache, right-sided weakness and seizures. Examination showed hemiparesis, hypoaesthesia and hyperreflexia; blood tests revealed secondary polycythaemia and leucocytosis. No dental, sinus, otological or respiratory source was identified. Unenhanced computed tomography demonstrated a left parietal space

A Comparative Evaluation of ChatGPT and Google Search Output for Caregiver Education in Tetralogy of Fallot.

INTRODUCTION: Caregivers of children with tetralogy of Fallot (TOF) frequently seek medical information online to better understand their child's diagnosis, treatment, and long-term care. Recently, generative artificial intelligence (AI) tools, including Google AI Overview and ChatGPT, have emerged as widely accessible sources of health information. However, the quality, readability, and accuracy of these platforms for caregiver education in congenital heart disease remain poorly characterized. METHODS: Forty frequently asked caregiver questions regarding TOF were identified using Google's "P
