Case Report: Low cardiac output syndrome with multisystem complications following total repair of tetralogy of fallot.

BACKGROUND: Tetralogy of Fallot (ToF) repair in infants may be complicated by low cardiac output syndrome (LCOS), which can precipitate rare multisystem complications. Junctional ectopic tachycardia (JET), atrial thrombosis, and cortical blindness are underrecognized sequelae requiring multidisciplinary management. CASE PRESENTATION: We report a 23-month-old male with ToF who developed LCOS post-surgery, followed by refractory JET, left atrial thrombus, embolic occipital infarction, and transient cortical blindness. LCOS was defined by elevated lactate, low ScvO₂, hypotension, and oliguria. J

Atrial Flutter Radiofrequency Ablation After Secondary Surgery in Adult Congenital Heart Disease.

BACKGROUND: Patients with adult congenital heart disease have a high risk of atrial flutter (AFL) because of multiple operations, and it is often difficult to treat them with medication alone. CASE SUMMARY: An 18-year-old female patient with repaired tetralogy of Fallot (rTOF) who developed drug-refractory AFL 2 years after a second surgery involving pulmonary valve replacement and removal of a previous right ventricular outflow tract patch presented to out hospital. Catheter ablation was successfully performed for typical cavotricuspid isthmus-dependent AFL without complications, and sinus r

Influence of Technical Performance Score on Outcomes After Tetralogy of Fallot Repair in a Low-Middle-Income Country.

ObjectivesTetralogy of Fallot (TOF) repair is associated with low mortality in high-performance centers, but outcomes are worse in low- and middle-income countries (LMICs). Prior studies have not demonstrated a strong link between Technical Performance Score (TPS) and mortality. The aim of this was to evaluate risk factors for mortality and complications after TOF repair in a high-volume LMIC heart center with a focus on surgical performance.MethodsWe retrospectively reviewed children undergoing TOF repair between 2015 and 2022. Patients over two years of age or with complex defects beyond atr

Strategies for preventing arrhythmic sudden death in adult congenital heart disease.

Arrhythmic sudden cardiac death (SCD) remains a major cause of late mortality in adults with congenital heart disease (ACHD), second only to heart failure. Risk prediction is challenging due to the heterogeneity of underlying defects and surgical histories. This review summarizes contemporary evidence on arrhythmic SCD prevention in ACHD, focusing on lesion-specific risk stratification, device-based therapies, and emerging technologies. Traditional markers-ventricular dysfunction, QRS prolongation, and prior arrhythmias-show limited predictive value. Recent models such as PREVENTION-ACHD and t

Hybrid rule-based and on-premises LLM pipeline for extracting CMR and CPET metrics from free-text reports in repaired tetralogy of Fallot.

BACKGROUND: Patients with repaired tetralogy of Fallot (rTOF) require lifelong surveillance with cardiovascular magnetic resonance (CMR) and cardiopulmonary exercise testing (CPET). However, results are frequently stored as unstructured free-text reports, hindering large-scale analysis and research. OBJECTIVES: This study aimed to develop and evaluate a privacy-preserving hybrid natural language processing pipeline combining regular expressions (regex) and an on-premises large language model (LLM) to accurately extract key CMR and CPET metrics from legacy free-text reports in patients with rT

Transcatheter pulmonary valve implantation in patients with repaired tetralogy of Fallot: International evidence and the current status in Japan.

In repaired tetralogy of Fallot (TOF), chronic pulmonary regurgitation drives right ventricular (RV) dilation, ventricular arrhythmias, and heart failure. Surgical pulmonary valve replacement improves outcomes but requires redo sternotomy. Transcatheter pulmonary valve implantation (TPVI) has emerged as a less invasive alternative. This review summarizes contemporary evidence and the Japanese context for TPVI. International registries report high procedural success and favorable mid-term outcomes with both balloon-expandable and self-expanding platforms. In Japan, two platforms are approved-SA

Evaluation and application of electrocardiographic age model for children.

ECG-age, derived from ECG signals using deep neural networks (DNNs), correlates with health status but has been predominantly studied in adults, neglecting the unique development trajectories of pediatric hearts. This study evaluates and proposes a pediatric-specific ECG-age model for clinical application. Validated on 58,672 ECGs from healthy children, three approaches were assessed: a pre-trained adult model, the same model retrained on pediatric data, and feature-enhanced models which was used to enhance the interpretability of the model. Results demonstrate limited applicability of adult-d

Ultrasound-guided guidewire maneuver to resolve peritoneal drainage catheter obstruction in an infant: A case report.

Peritoneal drainage failure is a rare but important complication in pediatric patients, and identifying its underlying cause is crucial for safe and timely management. We report a 7-month-old female infant with trisomy 13 and tetralogy of Fallot who developed respiratory failure secondary to ascites and underwent peritoneal drainage under ultrasound guidance. When drainage subsequently failed, ultrasound revealed a membranous soft-tissue structure-suspected omentum-protruding through a catheter side hole. Conventional maneuvers, including saline flushing and positional changes, were ineffectiv

Strain Analysis from Transverse CMR Cine Imaging in Congenital Heart Disease: Feasibility, Reproducibility, and Comparison to Global Longitudinal Strain.

Background: Global longitudinal strain (GLS), derived from long-axis cine images (LAX), is a sensitive marker for myocardial dysfunction and a strong predictor for clinical events and future ventricular deterioration. In patients with complex congenital heart disease (CHD) transverse-oriented cine imaging is part of the standard cardiac magnetic resonance (CMR) protocol. We aimed to study the feasibility and reproducibility of strain measurements derived from transverse-oriented cine images (Transverse Strain (TrS)) and compare them with standard GLS. Methods: We retrospectively analyzed CMR c

Long-term Oral Management for 2q37 Deletion Syndrome Patient.

2q37 deletion syndrome is an extremely rare chromosomal disorder, with approximately 150 cases reported worldwide. Affected individuals are known to have multisystem involvement, though dental findings associated with this syndrome remain poorly documented. This report describes the long-term dental management and oral characteristics of a pediatric patient with 2q37 deletion syndrome. The patient was an 11-year 2-monthold boy who presented with the chief complaints of dental caries and maxillary anterior crowding. Similarly to in previous reports regarding this syndrome, a clinical examinatio

Development and multicentre validation of an artificial intelligence electrocardiogram model for ventricular remodeling in repaired tetralogy of Fallot.

AIMS: Periodic cardiac MRI (CMR) is recommended to identify adverse ventricular remodelling in repaired tetralogy of Fallot (TOF), but access to CMR is uneven, and compliance is poor. We developed a 12-lead electrocardiogram (ECG) artificial intelligence (AI) biomarker to identify CMR-quantified adverse biventricular remodelling in repaired TOF. METHODS AND RESULTS: Six (1 train/5 external test) North American retrospective cohorts with paired ECG and CMR were included. The main outcome was a composite of ≥2 TOF-specific CMR abnormalities: right ventricular (RV) end-diastolic volume ≥ 160 mL/

Effect of the Implantation Angle on the Durability of a Bio-Prosthetic Pulmonary Valve in the Tetralogy of Fallot.

BACKGROUND AND OBJECTIVES: Tetralogy of Fallot (TOF) patients require pulmonary valve replacement (PVR) after total correction to prevent the late complications associated with pulmonary valve dysfunction. This study aimed to evaluate factors affecting the durability of bio-prosthetic pulmonary valves, with an emphasis on the angle of implantation as well as age, size or materials of prosthetic valve. METHODS: Of the 50 consecutive patients with TOF who underwent redo-PVR due to prosthetic valvular failure (PVF) between January 2001 and May 2020, 36 had good images for measuring implantation

Late Gadolinium Enhancement in Repaired Tetralogy of Fallot May Overestimate Right Ventricular Myocardial Fibrosis.

Cardiac magnetic resonance (CMR) with late gadolinium enhancement (LGE) imaging shows myocardial fibrosis and is associated with outcome in repaired Tetralogy of Fallot (rToF). However, patients with rToF have surgical material implanted, also appearing bright in LGE images. Therefore, right ventricular myocardial fibrosis could be overestimated or falsely diagnosed, which may affect prognostic value and serial examinations. The aim of this study was to assess right ventricular LGE in patients with rToF and to compare this with placement of surgical material. This retrospective cohort study in

Kingella kingae Endocarditis and Septic Pulmonary Emboli in a 12-Year-Old Male With Repaired Tetralogy of Fallot and a Bioprosthetic Pulmonary Valve.

Kingella kingae, typically known for causing osteoarticular infections in young children, is an emerging cause of infective endocarditis (IE) in older children with congenital heart disease. This report describes a 12-year-old with repaired tetralogy of Fallot and a bioprosthetic pulmonary valve who developed culture-negative IE due to K. kingae, confirmed by microbial cell-free DNA testing and valve tissue PCR. His course was complicated by septic pulmonary emboli, requiring valve replacement. The case highlights the role of molecular diagnostics in managing culture-negative IE and the need f
