Two young athletes with Tetralogy of Fallot-stress echocardiography in therapeutic decision-making: a case report.

BACKGROUND: The surveillance of athletes with congenital heart disease remains challenging despite recent recommendations. Stress echocardiography as a diagnostic tool is not yet part of the routine follow-up but has emerged as an innovative approach to assess cardiac reserve and exercise capacity. It can unmask cardiac exercise pathophysiology and hence inform decision making for repeat interventions in athletes with complex congenital heart disease. CASE SUMMARY: This case report describes two athletes with Tetralogy of Fallot who, despite similar conditions, exhibit different cardiovascula

Age-dependent Dynamics of the Electrocardiographic Parameters in Cardiovascular Disease-Free Children.

BACKGROUND: Normative pediatric electrocardiographic (ECG) parameters are standardized, but lack temporal resolution for neonates and infants. These values are clinically important, as they support the diagnosis, risk stratification, and management of cardiovascular diseases (CVD). METHODS: Five ECG parameters (heart rate (HR), QRS, PR, QT, QTc intervals) were retrospectively analyzed from 7,346 recordings from 6,967 patients at a large pediatric hospital. Patients were only included if their ECG was adjudicated as normal by a pediatric cardiologist. Patients were assigned to 45 age groups: n

Incidence, risk factors, and outcome of early post-operative arrhythmia after open-heart surgery in the paediatric cardiac intensive care unit: a retrospective study.

BACKGROUND: Early post-operative arrhythmia is a frequent complication after paediatric cardiac surgery. Although usually transient, it contributes to considerable morbidity and may increase mortality. This study aimed to determine the incidence, predictors, and outcomes of early post-operative arrhythmia following paediatric open-heart surgery. METHODOLOGY: A single-centre retrospective cohort study was conducted on paediatric patients who underwent open-heart surgery for congenital or acquired heart diseases between January 2022 and December 2024. Pre-operative, intraoperative, and post-ope

Long-Term Arrhythmic Complications in Patients with Repaired Tetralogy of Fallot: A Single-Center Experience from 1960 to the Present.

Advances in surgical techniques have substantially improved survival in patients with repaired tetralogy of Fallot (ToF). However, residual lesions following repair may lead to long-term complications, including arrhythmias and the need for device therapy. We conducted a retrospective single-center cohort study including patients who underwent surgical repair of ToF between 1960 and the present. Patients were categorized into three treatment eras (<1980, 1980-2000, >2000). Data on surgical strategy, mortality, arrhythmias, and cardiac implantable electronic device (CIED) implantation were coll

Osteopathic Manipulative Treatment in 564 Children with Congenital Heart Disease: A Project Report.

BACKGROUND: Congenital heart diseases are the most common congenital malformations, affecting 4 to 9 per 1000 children, with increasing global prevalence. As surgical mortality rates decline, the focus has shifted toward improving the quality of life and perioperative outcomes for pediatric patients. Multidisciplinary rehabilitation, including osteopathic care, is increasingly incorporated into recovery programs. Osteopathic manipulative treatment combines manual techniques with lifestyle guidance to alleviate postoperative pain and promote recovery. This project report describes the impact of

Tetralogy of Fallot: Genetic, Epigenetic and Clinical Insights into a Multifactorial Congenital Heart Disease.

Tetralogy of Fallot (TOF) is the most common cyanotic congenital heart disease, classically characterized by right ventricular outflow tract obstruction, ventricular septal defect, overriding aorta, and right ventricular hypertrophy. Recent advances in molecular and genomic research indicate that TOF is part of a phenotypic continuum encompassing Trilogy, Tetralogy, and Pentalogy of Fallot, in which the variability of anatomical presentation reflects shared genetic and epigenetic mechanisms with highly variable penetrance and expressivity. Variants in NOTCH1, FLT4, KDR, GATA6, and TBX1 highlig

In silico Analysis of CHD4 Mutations Reveals Domain-Specific Impacts on Cardiovascular Disorders Among Patients With Rare Diseases.

Chromodomain-helicase-DNA-binding protein 4 (CHD4) is a critical ATP-dependent chromatin remodeler that plays fundamental roles in transcriptional repression, DNA damage repair, and lineage specification, making it indispensable for cardiovascular development and function. Pathogenic CHD4 mutations are linked to syndromic and nonsyndromic conditions, often presenting with severe cardiac and vascular anomalies. However, most of these mutations are unique and nonrecurrent, complicating variant classification. In this study, we establish a connection between recent advances in CHD4 structure and

The use of human decellularized amniotic membrane as pulmonary valve leaflets in right ventricular outflow tract reconstruction - an in vivo proof of concept study.

INTRODUCTION: Despite fundamental improvements in surgical treatment of Congenital Heart Defects, there are still challenges related to premature failure of the material used for such corrections, thus resulting in repeated operations during a patient's life. This is particularly the case for complex defects with Right Ventricular Outflow Tract (RVOT) obstruction, such as in Tetralogy of Fallot/Pulmonary Atresia, whereby the pulmonary valve reconstruction remains problematic due to short-term durability of the currently used replacement solutions. We set out to test, for the first time, the su

Dynamics of matrix metalloproteinases and procollagen type-1 N-terminal propeptide following right ventricular outflow tract reconstruction: the protective mechanism of restrictive physiology.

OBJECTIVE: Right ventricular outflow tract reconstruction frequently leads to long-term pulmonary failure and ventricular dilation. This study aims to investigate the dynamics of matrix metalloproteinase-2, matrix metalloproteinase-9, and procollagen type-1 N-terminal propeptide in predicting postoperative remodelling and to evaluate the "protective" potential of restrictive right ventricular physiology. METHODS: This prospective comparative cohort study included 20 patients undergoing right ventricular outflow tract reconstruction (Research Group) and 20 patients undergoing atrial or ventric

Surgical repair of Tetralogy of Fallot - Long-term outcomes into the second decade after transannular patch repair and annulus-sparing patch repair.

OBJECTIVES: Different techniques are used to perform repair of Tetralogy of Fallot (TOF), depending on the development of the pulmonary valve and the degree of the right ventricular outflow tract obstruction. METHODS: All patients who underwent TOF repair with either an annulus-sparing patch or a transannular patch (TAP) between January 1995 and April 2021 were included in this retrospective chart review. Mortality was cross-checked with the national health insurance. Survival and competing-risks analyses were conducted to report on long-term outcomes. RESULTS: One-hundred-fifty patients (50

Right ventricular outflow tract stenting for obstructive lesions in adult congenital heart disease.

Currently, the use of stents in the right ventricular outflow tract (RVOT) has been described as alternative for palliation in neonates with Tetralogy of Fallot (ToF) and critical infundibular stenosis, who are not candidates for total correction in a single stage. However, there is limited evidence of the benefit of this intervention in other clinical scenarios, and just a few reports in adults. A retrospective study was conducted on 23 patients with significant RVOT obstruction who underwent RVOT stent placement and were not considered candidates for surgery. Patients 17 years old at the tim

Agreement between single and multiple ROI strategies for hepatic T1 and ECV mapping in tetralogy of Fallot and Fontan circulation.

PURPOSE: Native T1 and extracellular volume (ECV) mapping have emerged as promising tools for noninvasive assessment of hepatic congestion and fibrosis; however, optimal strategies for region-of-interest (ROI) selection remain uncertain. The aim was to evaluate the level of agreement between two methods for quantifying hepatic native T1 and ECV: single wide ROI versus multiple small ROIs, in patients with repaired tetralogy of Fallot (TOF) and Fontan circulation. METHODS: This agreement study prospectively enrolled outpatients with repaired TOF or Fontan circulation between 2022 and 2025. All

Visualisation of cardiac magnetic resonance-based anatomic models in virtual reality to evaluate candidacy for transcatheter pulmonary valve replacement in surgically palliated right ventricular outflow tracts.

Cardiovascular MRI with magnetic resonance angiography supports clinical indication for transcatheter pulmonary valve replacement. We aimed to: (1) assess the feasibility of generating virtual right ventricular outflow tract models directly from magnetic resonance angiography datasets and (2) compare transcatheter pulmonary valve replacement candidacy based on visualisation of anatomic and valve models in virtual reality to outcomes and dispositions suggested by industry fit analysis derived from cardiac CT. Patients with native or surgically palliated right ventricular outflow tracts consider

Surgical repair of a child with tetralogy of Fallot with absent left pulmonary artery: a case report.

INTRODUCTION AND IMPORTANCE: Tetralogy of Fallot (TOF) with unilateral absence of a pulmonary artery is a rare anomaly with challenging surgical management. Key risks include postoperative pulmonary hypertension due to a reduced pulmonary vascular bed. This report presents a case of TOF with absent left pulmonary artery (ALPA) that resulted in a fatal postoperative complication, highlighting a potentially underreported mechanism. CASE PRESENTATION: A 5-year-old female with Tetralogy of Fallot with absent left pulmonary artery (TOF-ALPA) underwent successful surgical repair, including ventricu

15O-water PET perfusion in complex congenital heart disease.

AIMS: Positron emission tomography (PET) perfusion with 15O-water is the reference standard for myocardial blood flow (MBF) quantification. Its strengths are low radiation dose, optimal tracer kinetics, and a short imaging protocol. However, the use of 15O-water or PET perfusion in patients with various complex congenital heart diseases has been rarely studied. We report the initial experiences of 15O-water PET perfusion in these patients. METHODS AND RESULTS: All 15O-water scans from patients with complex congenital heart disease at Helsinki University Hospital were retrospectively assessed

Utility of three-dimensional echocardiography for evaluating right ventricular size and function and ventricular myocardial deformation in repaired tetralogy of fallot.

PURPOSE: Right ventricular (RV) dysfunction remains a major long-term complication in patients with repaired Tetralogy of Fallot (rTOF). While cardiac magnetic resonance (CMR) is the gold standard for right ventricular (RV) assessment, it is limited by accessibility and cost. Three-dimensional echocardiography (3DE), with its strain imaging capabilities, offers a promising alternative for the serial evaluation of right ventricular (RV) size and function. This study aims to compare RV volumetric and myocardial deformation parameters obtained by 2D and 3D echocardiography (2DE, 3DE) with RV func
