Left ventricular quantification in repaired tetralogy of Fallot: two-dimensional echocardiography versus cardiovascular magnetic resonance.

Left ventricular (LV) dysfunction is a known risk factor for adverse outcomes in patients with repaired tetralogy of Fallot (rTOF). While cardiovascular magnetic resonance (CMR) is the reference standard for LV assessment, two-dimensional echocardiography (2DE) remains the most accessible tool for surveillance. The accuracy of commonly used 2DE methods, Simpson's biplane (SiBP) and 5/6 area-length (AL), in the setting of right ventricular (RV) dilation and altered interventricular septum configuration remains uncertain. We conducted a retrospective analysis of pediatric patients with rTOF seen

The Incidence and Spectrum of Congenital Heart Defects in Patients with Anorectal Malformations: A 10-Year Retrospective Cohort Study.

Congenital heart defects (CHD) and anorectal malformations (ARM) frequently co-occur, but contemporary large-scale data are limited. This study aimed to define CHD incidence, spectrum, and risk factors in ARM patients. We conducted a retrospective cohort study of infants with ARM at a tertiary pediatric center (2013–2022). Demographic, clinical, and echocardiographic data were analyzed. Primary outcome was CHD incidence; secondary outcomes included CHD types and associations with ARM complexity and VACTERL features. Among 314 ARM patients, 92 had CHD (incidence 29.3%, 95% CI: 24.3–34.7%). Right ventricular function after repair of tetralogy of Fallot: an echocardiographic and clinical evaluation.

BACKGROUND: Right ventricular dysfunction is a major determinant of long-term outcomes after tetralogy of Fallot repair, particularly in the presence of chronic pulmonary regurgitation. Strain analysis may detect early right ventricular dysfunction more sensitively than conventional echocardiographic parameters. METHODS: This retrospective single-centre study included 63 patients after tetralogy of Fallot repair who underwent postoperative echocardiographic evaluation during mid-term follow-up. Right ventricular function was assessed using tricuspid annular plane systolic excursion, right ven

Quantitative radiomics and morphometry analysis of brain sub-regions reveals neurodevelopmental alterations in children with Tetralogy of Fallot.

BACKGROUND: Tetralogy of Fallot (TOF) is one of the most common forms of cyanotic congenital heart disease. Although surgical advances have markedly improved survival rates, children with TOF remain at increased risk for neurodevelopmental impairments. However, the underlying neural mechanisms contributing to these deficits remain unclear, and reliable early biomarkers for neurodevelopmental risk are still lacking. METHODS: This study enrolled 60 children with TOF and 61 healthy controls who underwent high-resolution three-dimensional T1-weighted structural MRI (sMRI) and neurodevelopmental a

Real-time deep learning interpretation of echocardiographic video for automated detection of anatomical features associated with tetralogy of fallot in pediatric patients : a feasibility study.

Tetralogy of Fallot (TOF) is a complex congenital heart defect requiring precise diagnosis. Echocardiography is essential for detection, but a global shortage of pediatric cardiologists can hinder timely diagnosis. Our study aims to develop an Artificial intelligence (AI) model using deep learning to automate TOF diagnosis from echocardiograms, serving as a screening tool in regions lacking specialists. We trained an AI model with echocardiograms from 174 pediatric patients at Seoul Asan Medical Center (2018–2023). Utilizing Detectron2 and Mask R-CNN, we processed, labeled, and trained the mod

An extremely rare tetralogy of Fallot with absent pulmonary valve and unilateral absence of the pulmonary artery: a rare report of De Bucket Syndrome.

BACKGROUND: Only a limited number of studies have reported on TOF with absent pulmonary valve (APV). Similarly, while cases of TOF with absent pulmonary artery (PA) have been documented, case reports describing TOF with both APV and absent PA are extremely rare. CASE PRESENTATION: The present study investiged the case of a 1-year-old girl born at term with no initial clinical or physical signs of cyanosis. A subtle additional heart murmur detected during routine examination prompted referral to a cardiologist. Subsequent echocardiography and computed tomography (CT) angiography confirmed TOF

Surgical outcomes of repair for tetralogy of Fallot with a previous right ventricular outflow tract stent.

BACKGROUND: The optimal management of neonates with tetralogy of Fallot (TOF) and unfavorable anatomy remains debated. Early primary repair carries risks, while palliative options such as right ventricular outflow tract (RVOT) stenting have emerged as effective alternatives to conventional shunts. However, outcomes of corrective surgery following RVOT stenting remain underexplored. PATIENTS AND METHODS: A retrospective study was done on 11 patients with TOF who underwent RVOT stenting followed by surgical repair between April 2021 and March 2025, from a total of 619 TOF surgeries. Data on dem

End-diastolic forward flow (EDFF) after tetralogy of Fallot repair: Evolution of its role in management and prognosis.

OBJECTIVE: In the era of personalized medicine and multimodality imaging, renewed attention is directed toward the restrictive right ventricular (RV) physiology, including its historic hallmark, the end-diastolic forward flow (EDFF), in Tetralogy of Fallot (ToF) repair. Initially perceived as a marker of favorable restriction, EDFF has been repositioned as a multifactorial hemodynamic phenomenon rather than a specific indicator of myocardial stiffness, based on recent imaging and mechanistic studies. METHODS: This review synthesizes the evolution of perception, the diagnostic assessment, and

De Novo Complex Genomic Rearrangement Spanning 2q31.1 in a Proband With Congenital Malformations: Genotype-Phenotype Correlation and Development of a CGR Detection Pipeline.

The 2q31 region is commonly associated with pathogenic alleles of the HOXD cluster leading to various clinical phenotypes related to skeletal development. We present a proband with tetralogy of Fallot and multiple congenital anomalies. Genomic variant screening including an in-house CGR detection pipeline pairing genome sequencing (GS) structural variant calls with read-depth data revealed a de novo complex genomic rearrangement (CGR) spanning 2.7 Mb across 2q31 characterized by a series of duplications and triplications including the HOXD gene cluster. The genomic structure was assembled by a

An Innovative Hybrid Process for Realistic Three-Dimensional Heart Modeling and Surgical Simulation of Tetralogy of Fallot.

Background: Realistic three-dimensional (3D) heart models are essential for understanding complex congenital defects and improving surgical training. However, conventional rigid models lack the tactile and anatomical fidelity required for procedures such as Tetralogy of Fallot (TOF) repair. Objectives: To develop a reproducible, flexible, and anatomically accurate TOF heart model using a computer aided design (CAD)-driven hybrid workflow, validated against magnetic resonance imaging (MRI) data and assessed through hands-on surgical training. Methods: Anonymized MRI datasets were segmented and

Unique Systemic Vasodilatation of Post-fontan Physiology Compared with Biventricular Physiology: How Different is it on the CI-SVRI Plot?

Fontan circulation is characterized by elevated central venous pressure (CVP) and reduced preload, resulting in low perfusion pressure (PP). Maintaining adequate PP and low CVP may prevent multi-organ failure in post-Fontan patients. Vasodilatation, characterized by low systemic vascular resistance index (SVRI) and high cardiac index (CI), is a poor prognostic predictor. CI–SVRI plots have been proposed to visualize Fontan hemodynamics; however, direct comparisons with biventricular physiology are limited. We compared catheterization data between post-Fontan and repaired tetralogy of Fallot (T

Albumin-Bilirubin Score and Subclinical Right Ventricular Dysfunction After Repair of Tetralogy of Fallot.

BACKGROUND: Right ventricular (RV) dysfunction is a key determinant of long-term morbidity in patients following surgical repair of tetralogy of Fallot (TOF). Identifying practical biomarkers for early detection of right-sided heart failure remains clinically relevant, particularly in pediatric populations with subclinical disease. The albumin-bilirubin (ALBI) score, a simple and objective marker of hepatic function, has emerged as a potential surrogate of systemic congestion in heart failure. This study aimed to evaluate whether the ALBI score reflects right ventricular dysfunction in pediatr

MorphiNet: A Graph Subdivision Network for Adaptive Bi-Ventricle Surface Reconstruction.

Cardiac Magnetic Resonance (CMR) imaging is widely used to personalize heart models for cardiac digital twin analysis because of its ability to visualize soft tissues and capture dynamic functions. However, CMR images have an anisotropic nature, characterized by large inter-slice distances and misalignments from cardiac motion. These limitations result in data loss and measurement inaccuracies, hindering the capture of detailed anatomical structures. In this work, we introduce MorphiNet, a novel network that reproduces heart anatomy learned from high-resolution Computed Tomography (CT) images,
