Congenital aortopulmonary window (APW) associated with interrupted aortic arch (IAA) is a rare and severe form of congenital heart disease. The optimal surgical timing and strategy remain challenging, particularly beyond infancy. We report two toddlers diagnosed with APW and IAA who underwent one-stage surgical repair. At the time of surgery, the children were 17 and 22 months old, with body weights of 6.6 kg and 8.0 kg, respectively. Both patients survived and experienced no perioperative complications. During postoperative follow-up of 12 and 19 months, respectively, the children demonstrate

BACKGROUND: Aortopulmonary window is an uncommon CHD that often coexists with additional cardiovascular anomalies. Criss-cross heart, defined by rotation of the ventricular mass with crossed atrioventricular inflow streams, is exceedingly rare. The coexistence of aortopulmonary window and criss-cross heart is exceptional and markedly increases diagnostic and therapeutic complexity.
CASE SUMMARY: We report a full-term neonate with prenatal suspicion of aortopulmonary window. Postnatal echocardiography and CT demonstrated situs solitus, concordant atrioventricular and ventriculoarterial connect

We present a rare fetal case of aortopulmonary window (APW) accompanied by severe growth restriction and a complex chromosomal abnormality involving two pathogenic 15q copy number variants encompassing the IGF1R gene. Fetal echocardiography at 28 weeks confirmed the APW and a perimembranous ventricular septal defect. This co-occurrence suggests a more complex relationship between 15q aberrations and APW and warrants further study. Moreover, this case expands the phenotypic spectrum of 15q imbalances and underscores the need for integrated prenatal evaluation combining advanced cardiac imaging

OVERVIEW: Interrupted aortic arch in combination with an aortopulmonary window is an uncommon congenital cardiovascular defect. The anatomic separation between the aortic arch and descending aorta, combined with the communication between the ascending aorta and pulmonary trunk, leads us to a unique case where we can utilize the patient's anatomic defect towards an efficient perfusion strategy.
DESCRIPTION: The patient was a 19-day-old neonate who was initially discharged home after birth with no suspicion of congenital heart disease. The patient was later diagnosed with a type A aortic interr

Complex congenital heart diseases, such as common arterial trunk (CAT) and aortopulmonary window (APW) with interrupted aortic arch (IAA), present challenges in low-resource settings where technical limitations restrict surgical care. We describe three cases referred from Cameroon and treated at the Jakaya Kikwete Cardiac Institute (JKCI) in Tanzania through authentic South-South cooperation with the purpose of describing clinical, anatomical, and surgical aspects of these patients, and assessing this collaborative model. The patients, aged 7 months to 3 years, included two CATs and one APW wi

Berry syndrome is a rare and complex congenital cardiac disease consisting of a constellation of anomalies including aortopulmonary window, anomalous origin of the right pulmonary artery from the ascending aorta, and interrupted aortic arch. Surgical treatment is typically performed in the first week or two after birth in patients with low-risk factors. Bilateral pulmonary artery banding as a stage to complete repair has been offered to neonates with prohibitive early surgical risks. We present a patient with Berry syndrome who underwent initial palliation with bilateral pulmonary artery bandi

We report the case of a boy diagnosed at age 9 with a large aortopulmonary septal defect and Eisenmenger syndrome, highlighting the challenges of late diagnosis and management of fixed pulmonary hypertension. Over the course of six years (2018-2024), the patient underwent several follow-ups, including invasive hemodynamic assessments and echocardiography, revealing progressive pulmonary vascular disease. Despite the administration of medical therapy with sildenafil, bosentan, and antiplatelets, the patient's condition remained unfavorable due to fixed pulmonary vascular obstructive disease. Th

Aortopulmonary window with Tetralogy of Fallot is a rare cardiac defect with very few reported cases in literature. We report an incidentally detected type 1 aortopulmonary window during routine intraoperative transesophageal echocardiography, which helped to tailor the anesthetic and surgical plan for better post-operative outcome.

Pulmonary atresia is a rare and complex congenital cardiac condition accompanied by an aortopulmonary window (APW) and double-outlet right ventricle (DORV). Here, we describe a case of a newborn with DORV, ventricular septal defect (VSD), pulmonary atresia, and APW. The patient underwent successful postnatal evaluation and echocardiography, revealing significant anomalies, including a large AP window providing pulmonary blood flow. The coexistence of DORV with pulmonary atresia and APW emphasizes the complexity of managing such congenital heart diseases, necessitating a multidisciplinary appro

The aortopulmonary window is a rare congenital heart defect. Isolated aortopulmonary window, without other associated anomalies, accounts for up to 25% of all cases. Surgical closure has long been, and remains, the gold standard in many cardiovascular centres. However, percutaneous closure has emerged as a viable alternative using various types of occluder devices, selected based on the morphology, size, and rims of the defect to minimise complications such as valvular interference or coronary ostial obstruction. We report the case of an infant with an isolated aortopulmonary window successful

We report a case of a term neonate presenting with anomalous origin of the right coronary artery from the pulmonary trunk, an aortopulmonary window, interrupted aortic arch type A, and an atrial septal defect within the oval fossa. The infant, admitted in cardiogenic shock, was stabilized with fluid resuscitation, intubation, and inotropic support. Comprehensive imaging revealed the complex cardiac anomalies. Surgical intervention successfully addressed all the lesions. The patient showed excellent postoperative recovery, with no complications at 6 months. Our experience underscores the critic

Aortopulmonary window (APW) is a rare congenital heart defect that may progress to pulmonary hypertension if untreated. Presented here is the case of a 25-year-old woman with type 1 APW diagnosed in childhood who presented with chest pain, headache, dyspnoea, and transient loss of consciousness. Imaging confirmed severe pulmonary hypertension (mean pulmonary artery pressure 50 mmHg) and a positive vasoreactivity test, allowing for percutaneous closure. A 14 mm Amplatzer Septal Occluder was implanted under transoesophageal echocardiographic and fluoroscopic guidance. Postoperative complications

INTRODUCTION: Aortopulmonary window (APW) is a rare congenital heart defect that causes left-to-right shunting and pulmonary overcirculation. Appropriate intervention prevents long-term complications.
CASE PRESENTATION: We report a 3-year-old Iranian girl (weight ~ 14 kg, height ~ 95 cm) presenting with exertional dyspnea, systolic murmur, and bounding pulses. Echocardiography showed a 3 mm Type I APW with mild pulmonary hypertension. Pre-procedure catheterization revealed a mean pulmonary artery pressure (PAP) of 26 mmHg and a Qp/Qs ratio of 1.8. The defect was closed percutaneously using a

IntroductionAortopulmonary window (APW) is an extremely rare congenital anomaly of the heart where there is a direct communication between the ascending aorta and main pulmonary artery. Surgical repair of APW on cardiopulmonary bypass (CPB) is considered the standard of surgical care. In this study, we selected patients with a type I APW to undergo direct surgical ligation to determine if this technique is safe and can produce equivalent results to standard open-heart repair.MethodTwelve infants with simple or isolated type I APW underwent direct surgical ligation by a single surgeon at Childr

BACKGROUND: Prenatal diagnosis of Berry syndrome, a rare combination of cardiac anomalies including aortopulmonary window (APW), aortic origin of the right pulmonary artery (RPA), interrupted aortic arch (IAA), hypoplastic aortic arch, or coarctation of the aorta (COA), poses a significant challenge. Due to the rarity of the disease, and the limited case reports available to features the complex malformation of Berry syndrome postpartum, this article introduces an innovative approach to visually showcase this unusual disease. The proposed method provides a comprehensive display of the structur

Background: Aortopulmonary window (APW) is a rare anomaly with variable morphology and associated cardiac anomalies. We evaluated impact of patient and operative factors on mid-term outcomes following APW repair. Methods: Twenty-nine patients underwent surgical APW repair at our institution from 1996 to 2022. Eight (28%) had simple APW, accompanied by only atrial septal defect or patent ductus arteriosus; 21 (72%) had complex APW with additional cardiovascular lesions, including nine with interrupted aortic arch. Median operative age was 19 days (range 2 days-1.5 years) via single-patch (n = 1

We report a case of a residual aortopulmonary window where a new Konar-MF occluder device was used to close the defect. This device has a low profile and conforms to the anatomy of aortopulmonary window very nicely without unnecessary protrusion on either side. This report highlights the advantage of Konar-MF occluder device for closure of such defects.
