Transcatheter Treatment of Large Aortopulmonary Window: A Case Report.

INTRODUCTION: Aortopulmonary window (APW), a large aortopulmonary septal defect (APSD), is a serious and rare defect within congenital heart diseases. CASE REPORT: In this study, we reported an APW case with severe pulmonary arterial hypertension. This patient was successfully treated by transcatheter closure with a muscular ventricular septal defect (VSD) occluder. CONCLUSION: We had a successful experience with transcatheter closure of a large APW using a muscular VSD occluder. There was no residual shunt or complications during the 6-month follow-up.

The value of transthoracic echocardiography in the diagnosis of anomalous origin of the right pulmonary artery from the ascending aorta: A single center experience from China.

BACKGROUND: Anomalous origin of pulmonary artery (AOPA) from the ascending aorta is a rare but serious congenital cardiac malformation, which frequently involves the right pulmonary artery (RPA). METHODS: We retrospectively analyzed the echocardiographic characteristics of 9 cases with anomalous origin of the right pulmonary artery from the ascending aorta (AORPA) studied from 2007 to 2014 in our institution. The results were compared with the findings at surgery. RESULTS: The corrective surgery was performed in all 9 cases with AORPA. The diagnosis by transthoracic echocardiography (TTE) of

Transcatheter closure of late-onset residual aortopulmonary septal defect using a muscular ventricular septal occluder.

Late-onset residual shunt after surgical repair of aortopulmonary septal defect (APSD) is a rare event complicating the management strategy. Surgical reoperation was the treatment of choice traditionally, while associated with increased risk and suffering. We report a case of successful treatment of this type of residual shunt using a transcatheter closure technique with an infrequently used muscular ventricular septal device.

Outcomes of aortopulmonary window repair in children: 33 years of experience.

BACKGROUND: The purpose of this study was to assess the outcomes of children undergoing repair of aortopulmonary window (APW). METHODS: We conducted a retrospective review of all children (n=43) who underwent surgical repair of APW between 1980 and 2013. RESULTS: Median age at surgery was 40 days (range, 13 to 125). Simple APW was present in 15 of 43 patients (35%), and 28 of 43 patients (65%) patients had concomitant cardiovascular anomalies. The aorta was repaired by direct suturing in 36 patients (84%) patients and patching in 7 patients (16%). The main pulmonary artery was repaired by di

Aortopulmonary window and the interrupted aortic arch: midterm results with use of the single-patch technique.

BACKGROUND: An aortopulmonary window (APW) associated with an interrupted aortic arch (IAA) can be associated with significant rates of perioperative mortality and recurrent arch obstruction. We assessed the outcomes associated with the use of a single pericardial patch technique for primary repair. METHODS: Between 2002 and 2011, 9 neonates and 2 infants with APW and IAA underwent single-stage repair, under a hypothermic (28°C) continuous cardiopulmonary bypass with antegrade selective cerebral perfusion. A single autologous pericardial patch (glutaraldehyde-fixed) was used both to augment t

Aortopulmonary window: a rare congenital heart disease.

We report the case of a 3,5 month-old infant presented at cardiology consultation of our institution for investigation of a heart murmur associated with failure to thrive and respiratory distress. Echocardiogram showed dilatation of left heart cavities, patent foramen ovale, small muscular ventricular septal defect and pulmonary hypertension. Angio-CT was performed on a 64-slice CT to better characterize the congenital heart disease. Aortopulmonary window was diagnosed and surgically corrected. Patient evolution was good with discharge 10 days after surgery. The authors review the literature a

Absent aortic valve associated with double outlet right ventricle and aortopulmonary window: physiologic implications of a rare malformation in both the fetus and neonate.

Absence of the aortic valve is a rare congenital heart defect that is detectable in the prenatal period. In this condition, functional aortic valve leaflets are absent; in their place are rudimentary, immobile noncoapting plate-like structures at the level of the annulus resulting in severe aortic regurgitation. We report the fetal diagnosis and subsequent postnatal course of an infant with the novel association of absent aortic valve, double outlet right ventricle, and aortopulmonary window. This unique combination of defects resulted in an unusual pattern of circular shunting that produced e

Diagnosis, treatment and outcomes of patients with aortopulmonary window.

BACKGROUND: Aortopulmonary window (APW) is a communication between the ascending aorta and the pulmonary artery in the presence of two separate semilunar valves and is the rarest of septal defects. AIMS: To present our experience with the diagnosis and outcome of APW cases. STUDY DESIGN: Retrospective cohort study. METHODS: Between June 2003 and October 2011, thirteen patients were diagnosed with APW. Clinical features of patients, findings of echocardiographic and angiographic examination, results of surgical intervention and follow-up were reviewed retrospectively. RESULTS: Eleven childr

Anatomical repair of aortopulmonary window with anomalous origin of the right coronary artery from the pulmonary artery.

Aortopulmonary window (APW) with an anomalous origin of a coronary artery is extremely rare. We report surgical management of a four-week-old infant with the association of a distal type of APW and an anomalous origin of the right coronary artery (RCA) from the pulmonary artery. Complete anatomical correction comprising division of the great arteries and transferring the RCA as an autologous flap to the aortic defect was successfully performed.

Iatrogenic aortopulmonary window after balloon dilation of left pulmonary artery stenosis following arterial switch operation.

Branch pulmonary artery stenosis may occur in 4%-28% of patients after an arterial switch operation. Balloon dilation can be attempted with variable results, while stenting is a more definitive option when balloon dilation fails. We report the case of a young boy who underwent balloon dilation of a stenosed left pulmonary artery 9 years after an arterial switch operation and was noted to have an aortopulmonary window about a year later. This was treated with covered stent implantation, which dealt both with the aortopulmonary window and the residual stenosis. The diagnostic process with cardia

Transposition of great arteries with aortopulmonary window: our surgical experience.

BACKGROUND: Transposition of the great arteries with aortopulmonary window is a rare congenital cardiac anomaly. An arterial switch operation with repair of the aortopulmonary window is the preferred operation in this subset. As the tissue between the great arteries is missing, it is considered to be a complex operation. The purpose of this study is to present our experience of a simple yet highly effective surgical technique for the management of this rare complex cardiac defect. METHODS: We detail our experience of the surgery of this complex defect in 4 patients. Standard technique of an a

Transpulmonary correction of an isolated aortopulmonary window in an adult.

We describe the operative management of a 65-year-old patient with a congenital type I aortopulmonary window. At surgery, heavy calcifications on the aortic side of the defect, and close proximity with the left coronary ostium, prevented patch repair from the transaortic access. Patch closure through a combined transpulmonary approach was therefore required. This case illustrates unique features of a late-presenting aortopulmonary window.

Holt-Oram syndrome with aortopulmonary window--a rare association.

A 4-month-old male infant presented with recurrent cough for 2 months. He had a shortened right upper limb with absent right thumb and continuous murmur in the left parasternal area. The X-ray showed an absent radius and the first metacarpal and phalyngeal bones on the right side. Echocardiogram revealed aortopulmonary window and small secundum atrial septal defect. Aortopulmonary window was successfully treated by device closure. Holt-Oram syndrome with aortopulmonary window is an extremely rare association.

BACKGROUND: Aortopulmonary window is a rare cardiac anomaly. We report our experience with this rare lesion and highlight the criteria for treating this defect without the use of cardiopulmonary bypass. METHODS: From May 2007 to April 2012, 10 patients, aged 1 to 10 months and weighing 3.4 to 5.5 kg, were operated on by a single surgeon using both off-pump and standard techniques. All patients underwent preoperative and postoperative echocardiographic assessment, and were followed up with clinical examinations and echocardiography. RESULTS: There was no operative death and all patients were
