Holt-Oram syndrome presenting with failure to thrive due to a hemodynamically significant patent ductus arteriosus: the importance of early cardiac evaluation in infants with radial ray abnormalities.

BACKGROUND: Holt-Oram syndrome (HOS) is a rare autosomal dominant disorder caused by pathogenic variants in the TBX5 gene and characterized by radial ray malformations associated with congenital heart defects and/or cardiac conduction abnormalities. Although atrial and ventricular septal defects are the most common cardiac manifestations, other congenital cardiac lesions may occur. Early recognition of cardiac involvement is essential to prevent complications and improve outcomes. CASE PRESENTATION: We report the case of an 8-month-old female infant presenting with failure to thrive, feeding

Assisted Reproductive Technology and Risk of Congenital Heart Defects in the Offspring: A Systematic Review and Meta-Analysis.

BACKGROUND: To date, >10 million children have been conceived via assisted reproductive technology (ART) globally. However, there are some concerns about potential adverse outcomes associated with ART-conceived pregnancies, including the risk of congenital heart defects (CHDs). This meta-analysis aimed to evaluate the association between ART and the risk of major CHDs as well as their specific subtypes. METHODS: Major electronic databases were screened for studies comparing the incidence of CHD following ART-conceived pregnancies versus spontaneous conception. The primary end point was study-

Multimodality imaging and surgical management of a rare combination: type 2A bridging bronchus, pulmonary artery sling, and atrial septal defect, a case report.

We report the case of a 15-year-old female presenting with exertional syncope and a history of refractory respiratory symptoms. Comprehensive preoperative multimodality imaging-including echocardiography, multi-detector computed tomography (MDCT) with three-dimensional reconstruction, and bronchoscopy-revealed a rare complex of congenital anomalies: a type 2 A bridging bronchus (BB), a pulmonary artery sling (PAS), and an atrial septal defect (ASD). Crucially, imaging confirmed the absence of significant tracheal stenosis, thereby obviating the need for tracheoplasty. The patient successfully

Infantile 22q11.2 deletion syndrome with secondary monogenic variants following inconclusive whole exome sequencing: a case report.

BACKGROUND: Chromosome 22q11.2 deletion syndrome (22q11.2DS), historically known as DiGeorge syndrome, is a common microdeletion disorder characterized by a wide range of multisystem manifestations. While Whole Exome Sequencing (WES) is increasingly used as a first-tier diagnostic tool for complex infant presentations, it has limited sensitivity for detecting copy-number variants such as microdeletions. Parental consanguinity adds further diagnostic complexity by increasing the likelihood of co-occurring single-gene variants. CASE PRESENTATION: We present the case of a 12-month-old female bor

Flow versus resistance: hemodynamic phenotyping of pulmonary hypertension in infants with shunts and concurrent lung disease.

Neonatal pulmonary hypertension (PH) can be categorized into three distinct physiologies: resistance-driven precapillary pulmonary arterial hypertension (PAH), flow-driven precapillary PH, and postcapillary PH. We present eight patients, divided into three groups: premature infants with BPD, infants with chronic respiratory failure secondary to CDH or genetic syndrome, and an infant with a multilevel shunt. In each case, echocardiography was an important, non-invasive tool for screening risk of PAH. However, many of the common echocardiographic parameters used to assess PAH are confounded by v

Comparison of Sizing Methods to Optimize Device Selection for Percutaneous Closure of Atrial Septal Defects.

BACKGROUND AND AIMS: Accurate sizing of atrial septal defects (ASDs) is crucial for appropriate device selection during percutaneous transcatheter closure. Despite the availability of multiple imaging and sizing techniques, no consensus exists on the optimal method for determining device size. This study aimed to compare preprocedural ASD measurements obtained using different techniques with the final device waist diameter and to identify the measurement method demonstrating the most favorable agreement and reproducibility. METHODS: This retrospective study included 149 pediatric patients (64

Metachronous multiple insulinomas in oculo-facio-cardio-dental syndrome harboring a novel BCOR deletion: a case report with review of literature.

Oculo-facio-cardio-dental (OFCD) syndrome is a rare X-linked dominant disorder caused by pathogenic variants in the BCOR gene and is characterized by distinctive craniofacial, ocular, cardiac, and dental abnormalities. Although OFCD syndrome is a well-defined developmental disorder, its association with endocrine tumors has not been clearly established. We report a female patient with genetically confirmed OFCD syndrome harboring a novel heterozygous deletion in BCOR who developed metachronous multiple insulinomas. The patient presented with recurrent hypoglycemic episodes, and pancreatic neur

Large left ventricular apical aneurysm in a child with Noonan syndrome and hypertrophic cardiomyopathy.

Noonan syndrome (NS) is a multisystem disorder with frequent heart defects, including hypertrophic cardiomyopathy (HCM). Left ventricular (LV) apical aneurysm is an extremely rare presentation in pediatric HCM. A 4-year-old child with NS, PTPN-11 mutation and HCM presented with progressive mid-cavity LV obstruction leading to a large LV apical aneurysm. Magnetic resonance imaging confirmed a large, wide-neck LV apical aneurysm. Surgery combined septal myectomy and patch repair of the aneurysm, resulting in uneventful recovery and significant gradient reduction. While multimodal imaging assists

[Thrombus formation in the locking wire of a Nit Occlud PFO device. Advantage of the “reverse reconfiguration” of the left atrial disc].

We describe the case of a 42-year-old female patient who suffered a cryptogenic ischemic stroke associated with a patent foramen ovale (PFO). During occlusion treatment with PFO devices, a thrombus was identified in the delivery system distal to the left atrial disc. Based on the reverse reconfiguration design, in which the left disc folds around its center, the device was recaptured, absorbing the thrombus protected by the folded disc, preventing embolization.

A Conduction-Sparing Atrial Patch Placement Technique During Atrioventricular Septal Defect Repair.

BackgroundSurgical repair of atrioventricular septal defect (AVSD) carries a persistent risk of postoperative complete heart block due to the close proximity of the atrioventricular conduction system to the inferior atrial septum during atrial patch placement. We evaluated a modified atrial patch placement technique designed to minimize conduction system injury during AVSD repair.MethodsAll patients undergoing AVSD repair between January 2019 and December 2024 were retrospectively reviewed. Patients were stratified according to the atrial patch placement technique. We evaluated a standard appr

Imaging the Adult With Secundum Atrial Septal Defect: A Practical Echocardiographic Approach From Diagnosis to Device Closure.

BACKGROUND: Secundum atrial septal defect (ASD) is one of the most common congenital heart defects encountered in adults. Echocardiography plays a central role in diagnosis, assessment of haemodynamic significance, anatomical characterization, transcatheter closure planning, and follow-up. However, the frequently non-circular geometry of secundum ASD can make accurate anatomical assessment challenging, particularly with single-plane measurements. PURPOSE: This narrative review provides a practical echocardiographic approach to adult secundum ASD, emphasizing the complementary roles of transth

Preliminary study of individualized therapy for ventricular septal defect with severe pulmonary arterial hypertension.

BACKGROUND: Pulmonary arterial hypertension (PAH) is a serious complication of congenital heart disease (CHD) with a left-to-right shunt, particularly ventricular septal defect (VSD). In patients presenting with severe PAH, therapeutic decision-making remains challenging because precise and individualized assessment criteria are lacking. In current clinical practice, treatment strategies are often based mainly on isolated hemodynamic parameters obtained by right heart catheterization (RHC), which may not fully reflect disease severity or reversibility. OBJECTIVES: This study aimed to explore

Cyanotic Heart Disease.

Cyanotic congenital heart defects are malformations of the heart that affect its structure and function. The etiology of most of these defects is multifactorial, but some common causes include maternal conditions, genetic defects, and genetic mutations. Six of the most common cyanotic congenital heart defects include Tetralogy of Fallot, transposition of the great arteries, persistent truncus arteriosus, total anomalous pulmonary venous return, tricuspid atresia, and Ebstein's anomaly. These defects share several common features, however, each is distinguished by key cardiac abnormalities and
