Biodegradable stents and occluders in CHD: current evidence, biomechanical constraints, and translational challenges.

Congenital heart disease (CHD) often requires device implantation during childhood, when somatic growth and changing haemodynamics place particular demands on permanent implants. Metallic stents and nitinol occluders are effective, but their lifelong persistence can complicate repeat interventions, limit imaging surveillance, and interfere with growth-related remodelling. This review examines the rationale, current evidence, biomechanical constraints, and translational challenges of biodegradable stents and occluders in CHD. These devices aim to provide temporary mechanical support followed by

Development and validation of a prediction model for cryptogenic ischemic stroke risk in migraine patients with right-to-left shunt: A 10-year study.

AimLong-term risk stratification for cryptogenic ischemic stroke (CIS) remains uncertain in patients with migraine and contrast-enhanced transcranial Doppler ultrasound-confirmed right-to-left shunting (RLS). Therefore, we aimed to develop and externally validate a simple baseline score for identifying individuals at low risk.MethodsThis 10-year study included 978 and 237 patients in the development and external validation cohorts, respectively. Internal validation involved a five-fold cross-validation strategy. Multivariable Cox proportional hazards modeling was used to develop the Migraine R

Risk factors and outcomes of early postoperative complications following congenital heart surgery: a retrospective study at a tertiary care hospital in Nairobi, Kenya.

INTRODUCTION: Congenital Heart Disease (CHD) remains a major health challenge globally, with surgical interventions being the primary treatment modality for many defects. While advancements in cardiac surgery have improved survival rates, early postoperative complications continue to pose risks, particularly in settings with limited resources like Kenya. The study aimed to assess the rate of any early postoperative complications and identify factors associated with it in children after cardiac surgery at a tertiary care hospital in Nairobi, Kenya from 2019 to 2024. METHODS: This was an single

Heterotaxy Syndrome Combining Univentricular Heart, Polysplenia and Truncated Pancreas: A Case Report.

BACKGROUND AND OBJECTIVES: Univentricular heart is a complex congenital heart disease with rare survival into adulthood. It could be part of a heterotaxy syndrome when, apart from the cardiac malformations, there are associated abnormal arrangements of extra-cardiac organs like the lungs, spleen and pancreas. The syndrome is said to be rarer than the univentricular heart and to have a worse clinical outcome. METHODS: After clinical assessment, the patient was investigated with Chest X-ray, Electrocardiography, Echocardiography, and contrast-enhanced CT scan of the chest and abdomen to make th

Polymeric and metallic occluders for interventional therapy of cardiovascular diseases.

Interventional therapy has, as a minimally invasive approach, been widely adopted for treating cardiovascular diseases, particularly structural heart disease, where occluders play a pivotal role in transcatheter closure. This paper comprehensively reviews the evolutionary trajectory and emerging frontiers of cardiovascular occluders, beginning with their interventional applications in congenital heart defects (atrial septal defect, ventricular septal defect, patent foramen ovale, and patent ductus arteriosus) and left atrial appendage closure. The focus then shifts to occluder materials and de

Pathogenic Variants in NKX2-5 Associated With Familial Atrial Septal Defect and Conduction System Disease.

BACKGROUND: Most atrial septal defects (ASDs) occur sporadically, but familial cases can occur and carry a 10% risk of transmission to offspring. CASE SUMMARY: We describe a 3-generation family with autosomal dominant secundum ASD and conduction system disease related to a novel NKX2-5 variant (c.300del; p.Asp101Thrfs∗75). The index patient, a 58-year-old woman with prior ASD repair and mitral valve replacement, presented with chronotropic incompetence, atrial fibrillation, left bundle branch block, and prolonged pauses, leading to pacemaker implantation. Her mother had an ASD, atrial fibrill

A Standardized Workflow for Multimodal Assessment of Right-to-Left Shunt-Related Ischemic Brain Injury.

Right-to-left shunt (RLS) has been associated with cryptogenic stroke, migraine, and other neurological conditions in which ischemic brain injury may be clinically relevant. In routine practice, however, assessment is often fragmented. Shunt presence may be documented, but shunt burden, provoked versus non-provoked patterns, anatomical complexity, and MRI-defined brain injury are not always integrated into a reproducible workflow. Here, we describe a standardized protocol for multimodal assessment of RLS-related ischemic brain injury in a clinically selected cohort. The workflow includes patie

'Double-dunk' implantation of valveless ring-reinforced PTFE conduits for neonatal truncus arteriosus repair.

This case describes a neonatal truncus arteriosus repair using an 8 mm ring-reinforced, valved polytetrafluoroethylene conduit for right ventricular outflow tract reconstruction via the 'double-dunk' technique. The diagnosis was established prenatally in a patient with familial history of the disease. Anatomically, both pulmonary arteries arose adjacent to each other from the posterolateral common trunk; there was a mildly regurgitant quadricuspid truncal valve and an accidental finding of an intramural course of both coronary arteries.  Clinically, the neonate presented with mild cyanosis and

Modeling patient variants of Cnot1 and Cdc42bpb results in distinct forms of congenital diaphragmatic hernia in mice.

Congenital diaphragmatic hernia (CDH) is a severe congenital anomaly characterized by impairment of both diaphragm and lung development in utero. CDH presents as a spectrum of forms and severities, with diaphragm defects arising in the dorsal/posterior region typically correlating with more severe pulmonary disease and higher risk of mortality than those appearing in ventral/anterior regions. The genetic etiology underlying CDH is complex, with many implicated genes showing variable expressivity and incomplete penetrance in both human patients and mouse models. Here we present in vivo validati

Detection of Patent Foramen Ovale: Transcranial Doppler and Echocardiography Are Not Equivalent to Define a Large Shunt.

BACKGROUND: The decision to close patent foramen ovale (PFO) depends on the size of PFO, the coexistence of interatrial septum aneurysm, and the importance of right-to-left shunt assessed by the number of bubbles on transcranial Doppler (TCD) and echocardiography after saline or glucose injection. The interventional threshold for the number of bubbles is considered to be the same for trans-thoracic and trans-esophageal echocardiography (TTE and TEE) and TCD. Our objective was to study whether the same thresholds can be used for TCD and echocardiography. PATIENTS AND METHODS: In this comparati

Complex Patent Foramen Ovale: Anatomic Variants, Technical Considerations, and Outcomes.

Patent foramen ovale (PFO) closure is a common intervention aimed at reducing recurrent stroke in patients with cryptogenic embolism. Complex PFO anatomies, characterized by features such as long tunnels, large defect sizes, atrial septal aneurysms, multifenestrated septa, prominent Eustachian valves or Chiari networks, and rigid or thickened septal tissue, pose significant challenges for successful device deployment and closure. Advances in imaging technologies, including transesophageal echocardiography, intracardiac echocardiography, and 3-dimensional imaging, have significantly enhanced th

Transcatheter Closure of Apical Septal Rupture With Third Cavity Formation After Surgical Repair.

BACKGROUND: Apical septal rupture following surgical closure of a ventricular septal defect is a rare but life-threatening complication. Reoperation in such cases may be high risk, making transcatheter closure a viable and less invasive alternative. CASE SUMMARY: We report an 8-month-old male infant who presented with cardiac failure with Ross class-III and severe respiratory distress. Clinical examination and imaging revealed an apical septal tear with formation of a third cavity extending toward the right ventricle and compromising right ventricular and left ventricular function. The patien

VSD Device Removal Due to Intermittent Complete Heart Block.

BACKGROUND: Percutaneous closure of perimembranous ventricular septal defects (pmVSDs) has markedly increased with the development of newer, softer occlusion devices. CASE SUMMARY: We report a 12-year-old girl who underwent percutaneous pmVSD closure. One week later, she was admitted for paroxysmal complete heart block (CHB). She was started on intravenous corticosteroids. A percutaneous retrieval of the device was performed successfully. Sinus rhythm was maintained throughout the 8-month follow-up period. DISCUSSION: Paroxysmal CHB is a rare but serious complication following percutaneous p

Mathematical simulations of paediatric haemodynamics in isolated ventricular septal defect.

Computer modelling of the cardiovascular system has the potential to revolutionize personalized medical care. This is especially promising for congenital heart defects, such as ventricular septal defect (VSD), a hole between the two ventricles of the heart. However, relatively few studies have built computer models for VSD, nor have they considered how natural adaptation to the cardiovascular system with age might interact with the presence of a small-, medium- or large-sized VSD. Here, we combine a lumped-parameter model of the cardiovascular system with two key modelling components: a size-d

Single-stage surgical repair of congenital aortopulmonary window associated with interrupted aortic arch in two toddlers: a case report.

Congenital aortopulmonary window (APW) associated with interrupted aortic arch (IAA) is a rare and severe form of congenital heart disease. The optimal surgical timing and strategy remain challenging, particularly beyond infancy. We report two toddlers diagnosed with APW and IAA who underwent one-stage surgical repair. At the time of surgery, the children were 17 and 22 months old, with body weights of 6.6 kg and 8.0 kg, respectively. Both patients survived and experienced no perioperative complications. During postoperative follow-up of 12 and 19 months, respectively, the children demonstrate

[Parinaud syndrome : a diplopia unlike any other].

Sudden-onset diplopia warrants an emergency management in ophthalmology. A binocular, vertical and persistent diplopia, should raise suspicion of a central lesion, particularly within the brainstem. We reporte the case of a 53-year-old man, who presented with acute vertical binocular diplopia. Ophthalmological examination revealed a bilateral elevation limitation, characteristic of Parinaud's syndrome. Brain magnetic resonance imaging identified a right-sided ischemic midbrain infarction, and further cardiovascular work-up demonstrated a patent foramen ovale causing a right-to-left shunt.
