OBJECTIVE: Durable and biocompatible pulmonary valved conduits remain a challenge in congenital cardiac surgery. The GORE PV1 device is a fully synthetic expanded polytetrafluoroethylene-based conduit designed to improve durability and reduce calcification. This study characterizes the gross and histopathologic findings in explanted PV1 conduits.
METHODS: Six GORE PV1 conduits explanted from pediatric patients (median age: 13.5 years) were evaluated. All patients were part of a previously reported cohort. Clinical data, imaging, and operative notes were reviewed. Gross and histologic examinat

BACKGROUND: Ebstein anomaly is a rare congenital malformation of the tricuspid valve, accounting for less than 1% of all congenital heart diseases. Ebsteinoid anomaly is an uncommon morphological variant characterized by abnormal tricuspid valve leaflet morphology without the classical apical displacement of the septal leaflet seen in Ebstein anomaly. The coexistence of an Ebsteinoid anomaly, giant right atrial aneurysm (RAA), and atrial septal defect (ASD) is exceptionally rare.
CASE PRESENTATION: A 15-year-old girl presented with progressive exertional dyspnoea (New York Heart Association [

PURPOSE: Tetralogy of Fallot with absent pulmonary valve (TOF-APV) can manifest neonatally with severe respiratory insufficiency secondary to tracheobronchomalacia (TBM). Typical treatment of patients with TOF-APV with significant respiratory compromise is directed at the underlying heart defect rather than the airway. Complete repair often does not ameliorate airway issues and subsequently requires a tracheostomy or ventilator.
DESCRIPTION: Given this lack of effective treatment, a bioresorbable, patient-specific, 3-dimensionally printed splint was developed that provides external support to

OBJECTIVES: To describe the mitral valve (MV) anomalies found in patients with transposition of the great arteries (TGA), the surgical procedures performed, and the fate of the abnormal MV.
METHODS: From 1990 to 2020, 52 patients out of 1590 TGA {S, D, D} patients (3.3%) undergoing biventricular repair were identified with abnormal MV. Anomalies were a mitral cleft (n = 46 [88%], ejection/outflow tract in 40 and as atrioventricular septal defect [AVSD]-type in 6) and/or anomalies of the subvalvular apparatus (60%). A ventricular septal defect was present in 88.5% and pulmonary stenosis in 17.

BACKGROUND: Various types of UVB radiation source (290-320 nm) are used in treating psoriasis and their therapeutic mechanism has been attributed to immunosuppressive properties. Recently, a new UVB source generated by a 308-nm excimer laser has been introduced for the treatment of psoriasis.
OBJECTIVE: In this study we investigated the immunohistochemical evaluation of T cells and the expression of various apoptosis-related molecules in the psoriatic hyperproliferative skin before and after treatment with 308-nm monochromatic excimer light (MEL).
METHODS: Ten patients (three women and seven

Glenn shunt dysfunction in single‑ventricle palliation is multifactorial. Although aorto‑pulmonary collaterals are usually considered, systemic veno‑venous collaterals and congenital portosystemic shunts (CPSS) may be underrecognized causes (1,7). Our patient, a 30‑month‑old child with mitral valve atresia, severe hypoplastic left ventricle, large ventricular septal defect (VSD), pulmonary valve atresia, and ductus arteriosus-dependent pulmonary blood flow, who was undergone some palliation stages including patent ductus arteriosus (PDA) stenting / modified Blalock-Taussig (BT) shunt and a rig

BACKGROUND: Venous access exhaustion poses a significant challenge for the implantation of intracardiac devices. Epicardial alternatives are often used in these cases, despite their long-term deleterious effects. This case presents a transhepatic approach for left bundle branch pacing in a patient with Ebstein anomaly and a prior Glenn procedure.
METHODS: Following pre-procedural evaluation with triphasic CT, ultrasound-guided hepatic vein puncture was performed. Guidewires were advanced into the right heart chambers, allowing passage of a delivery sheath for physiologic pacing. Atrial lead p

RATIONALE: Atrial septal defect (ASD) is a common congenital heart defect that can lead to left-to-right shunting, volume overload, and subsequent right atrial dilation - conditions that predispose to atrial arrhythmias such as atrial fibrillation (AF). The coexistence of Ebstein anomaly further increases the risk of early-onset arrhythmias and thromboembolic events. Stroke in young patients with such congenital abnormalities remains a rare but serious complication.
PATIENT CONCERNS: An 18-year-old female presented with intermittent palpitations and exertional chest discomfort. She had a past

Congenitally corrected transposition of the great arteries (ccTGA) is an uncommon congenital heart defect characterized by atrioventricular and ventriculo-arterial discordance resulting from abnormal leftward looping of the primitive cardiac tube. Although this double discordance produces a physiologically corrected circulation, the morphologic right ventricle functions as the systemic ventricle, and the tricuspid valve serves as the systemic atrioventricular valve, predisposing affected individuals to late complications. Prenatal diagnosis of ccTGA is often challenging, particularly in isolat

CASE SUMMARY: A 14-year-old boy was admitted after resuscitated ventricular fibrillation. He had tetralogy of Fallot associated with Ebstein's anomaly, surgically repaired at age 7 without tricuspid intervention. His ECG demonstrates overlapping features of both conditions: high-amplitude right bundle branch block and right ventricular hypertrophy consistent with repaired tetralogy of Fallot, along with broad "Himalayan" P waves, first-degree atrioventricular block, and fragmented QRS complexes reflecting Ebstein-related conduction abnormalities. This electrocardiographic coexistence illustrat

BACKGROUND: Holt-Oram syndrome, also known as atrio-digital syndrome, is a rare autosomal dominant genetic disorder primarily characterized by upper limb malformations and congenital heart defects, associated with mutations in the TBX5 gene. Diagnosis is typically based on clinical presentation, and the prognosis for affected individuals is closely related to the severity of cardiac involvement. Our study aims to highlight some key features of Holt-Oram syndrome to improve the level of clinical diagnosis.
METHODS: We collected 11 patients with clinical features strongly suggestive of Holt-Ora

BACKGROUND: Ebstein's anomaly is a rare congenital heart disease characterized by apical displacement of the tricuspid valve and a small functional right ventricle. Surgical repair, particularly cone reconstruction, may be complicated by atrioventricular conduction disturbances requiring permanent pacing.
CASE SUMMARY: A 47-year-old woman with Ebstein's anomaly underwent cone repair and developed complete heart block postoperatively. Because of her distorted right-sided anatomy and concerns regarding long-term dys-synchrony with right ventricular endocardial pacing, left bundle branch area pa

We report a 14-year-old female with a history of surgically corrected Ebstein anomaly, including a bidirectional Glenn shunt and atrial septostomy. Following a bioprosthetic tricuspid valve replacement, the patient developed complete atrioventricular block and became pacemaker-dependent. A unique transvenous approach was utilized: the ventricular lead was advanced from the left axillary vein through the left subclavian vein, superior vena cava, right pulmonary artery, and main pulmonary artery into the right ventricle, resulting in successful left bundle branch pacing. This case illustrates an

Congenital heart diseases (CHDs) are characterized by profound metabolic remodeling of mitochondrial pathways. However, data regarding mitochondrial respiration, oxidative phosphorylation (OXPHOS), and fatty acid oxidation (FAO) in patients with Ebstein anomaly (EA) are currently unavailable. This study evaluated 14 EA patients and 18 healthy volunteers. In accordance with the 2020 ESC guidelines, patients were stratified into two cohorts: EA-0 (patients currently without an indication for intervention) and EA-1 (patients meeting Class Ia or IIb indications for surgical intervention). Platelet

INTRODUCTION: The da Silva Cone repair (CR) has demonstrated to be an effective intervention for Ebstein's anomaly (EA). Perioperative management can be complicated by arrhythmias, but previous studies showed improvement with preoperative catheter and intraoperative surgical ablation. The purpose of this study was to describe the evolving diagnostic protocol and perioperative findings at our center.
METHODS: All patients referred to UPMC Children's Hospital of Pittsburgh from February 2016 to December 2024 with EA who underwent CR were reviewed. An electrophysiology study (EPS) was performed

Heparin-induced thrombocytopenia (HIT) type 2 is a major complication of heparin therapy and associated with considerable morbidity and mortality. Correct diagnosis, especially in the intensive care setting where thrombocytopenia occurs frequently, is of paramount importance. We report a 44-year-old female patient undergoing surgical replacement of the tricuspid valve due to Ebstein anomaly. Peri-operatively, the patient was treated with unfractionated heparin (UFH). Sixteen days after treatment initiation, computed tomography incidentally revealed multiple splenic infarctions consistent with

OBJECTIVE: This descriptive case series aimed to delineate the prenatal sonographic spectrum of Ebstein anomaly (EA) across all trimesters, analyze common diagnostic pitfalls, and propose systematic imaging clues for first-trimester detection.
METHODS: In this retrospective study, 40 fetuses with EA were included. A standardized echocardiographic protocol integrating transvaginal imaging for the first trimester (n = 7) and multiplanar analysis for the second and third trimesters (n = 33) was applied. Key morphological and functional parameters were documented.
RESULTS: The sonographic spectr
