Multidisciplinary introduction of peritoneal dialysis enabling ICU liberation and home-based care in Ebstein's anomaly with refractory ascites and CKD: a case report.

Patients with refractory hepatic ascites and chronic kidney disease (CKD) secondary to right-sided heart failure pose unique therapeutic challenges when renal replacement therapy (RRT) is required in the intensive care unit (ICU). Conventional hemodialysis often worsens hypotension and venous congestion in these settings. Peritoneal dialysis (PD), by contrast, offers slow and continuous ultrafiltration that preserves hemodynamic stability, yet its use during vasopressor-dependent shock in the ICU is rarely described. We report the case of a 50-year-old woman with Ebstein's anomaly-related righ

Totally thoracoscopic beating-heart tricuspid valve repair 30 years after Ebstein's anomaly surgery: a case report and literature review.

BACKGROUND: Ebstein's anomaly is a rare congenital malformation of the tricuspid valve that frequently progresses to severe tricuspid regurgitation and right-sided heart failure. Although early surgical repair can provide durable clinical improvement, late recurrent tricuspid regurgitation is not uncommon and may require technically challenging reoperation. In this setting, minimally invasive thoracoscopic beating-heart repair may offer an alternative to redo sternotomy, particularly for high-risk patients. CASE PRESENTATION: A 50-year-old woman with congenital tricuspid valve disease had pre

Primary Tricuspid Regurgitation in ACHD: Diagnosis and Treatment.

Primary tricuspid valve regurgitation includes congenital and acquired disorders, especially Ebstein anomaly and tricuspid valve dysplasia. Severe tricuspid regurgitation causes progressive right atrial and ventricular dilation, arrhythmias, exercise intolerance, impaired left ventricular filling, and eventual biventricular dysfunction. Some patients remain minimally symptomatic for decades despite progressive disease. Poor prognostic factors include severe cardiomegaly, ventricular dysfunction, cyanosis, and arrhythmias. Surgical indications include symptoms, declining exercise capacity, cyan

Ascending aortic dilation and aortic regurgitation - a neglected problem among single-ventricle patients after Fontan palliation.

BACKGROUND: The Fontan procedure revolutionized the management of univentricular heart physiology but is associated with long-term complications. Although pulmonary vascular disease, atrioventricular valve regurgitation, and ventricular dysfunction are well-recognized risks, aortic dilation and aortic regurgitation (AR) are emerging concerns. This study aimed to assess the prevalence and risk factors for aortic dilation and AR in adult post-Fontan patients. METHODS: We retrospectively analyzed 101 single-ventricle patients who underwent Fontan palliation and were hospitalized at NIKARD betwee

Hammock mitral valve in a 12-day-old female: pathophysiology, management and legal issues.

INTRODUCTION: Hammock mitral valve (HMV), or mitral arcade (MA), is an extremely rare congenital mitral anomaly characterized by absent fibrous chordae and hypertrophic papillary muscles directly inserting into the leaflets. This rare valve structure can trigger severe mitral regurgitation and overwhelm a newborn's heart and lungs within days. Because symptoms often resemble those of more common respiratory or cardiac issues, early diagnosis is difficult - but vital. Recognizing the anomaly in time and referring the infant to a specialized center can make the difference between life and death.

CCTA of Pediatric Congenital Right Heart Obstructive Lesions: A Pictorial Review.

Pediatric congenital right heart obstructive lesions encompass a spectrum of diseases that obstruct blood flow from the right atrium to the pulmonary artery. Right ventricular inflow obstructions include tricuspid valve abnormalities, such as Ebstein anomaly, tricuspid valve dysplasia, and tricuspid atresia. Right ventricular outflow obstructions include pulmonary valve stenosis, pulmonary atresia, and tetralogy of Fallot. Cardiac computed tomography angiography (CCTA) is a valuable tool for the diagnosis, treatment planning, and follow-up of these lesions. In this pictorial review, we highlig

Severe Congenital Pulmonary Valve Stenosis Diagnosed in Adulthood: A Case Report.

Pulmonary valve stenosis is a congenital heart disease that is usually diagnosed during childhood, while presentation in adulthood is uncommon and may lead to delayed diagnosis. We report the case of a 21-year-old patient with no significant medical history who presented with a six-month history of exertional dyspnea and constrictive chest pain. Electrocardiography showed right axis deviation with signs of right ventricular hypertrophy, and chest radiography revealed dilatation of the left pulmonary artery. Transthoracic echocardiography demonstrated severe valvular pulmonary stenosis with a p

Distribution, recognition and clinical implications of syndromic conditions in adults with congenital heart disease.

OBJECTIVE: Syndromic conditions are frequently associated with congenital heart disease (CHD). Their distribution, recognition and prognostic impact in adults with CHD (ACHD) remain poorly characterised due to limited assessment beyond childhood. METHODS: We retrospectively analysed 9826 patients with ACHD from a tertiary ACHD centre. Syndromic conditions were identified from clinical records and categorised as chromosomal, single-gene or other syndromes. Clinical characteristics, comorbidities and CHD complexity were compared between syndromic and non-syndromic patients. Multivariable Cox re

Cor triatriatum sinister resembling Lucas type C1a in a cat.

A six-year-old neutered male cat was presented for acute respiratory distress. Transthoracic echocardiography identified a left atrial membrane consistent with cor triatriatum sinister (CTS) and turbulent flow across a single fenestration. Additionally, it was suspected that the distal chamber may receive at least one pulmonary vein. Because respiratory signs improved with corticosteroids but not with a short diuretic trial, and because echocardiography suggested that the distal chamber might receive at least one pulmonary vein, cardiac computed tomography angiography was performed to further

Integrated pregnancy and perinatal management of complex fetal congenital heart disease: a preliminary retrospective study.

BACKGROUND: Congenital heart disease (CHD) is the most prevalent congenital abnormality worldwide and is the primary contributor to neonatal mortality. Obstetricians in China continue to face uncertainties about optimal delivery timing, appropriate delivery mode, perinatal interventions, and postnatal surgical outcomes for fetuses with complex CHD. OBJECTIVE: To investigate integrated pregnancy and perinatal management strategies for fetuses diagnosed with complex CHD by prenatal ultrasound in China. METHODS: A retrospective analysis was performed on 123 fetuses with complex CHD diagnosed by

Isochronal Versus Electrogram-Based Conduction Velocity Estimation to Predict Inducible Monomorphic Ventricular Tachycardia in Repaired Tetralogy of Fallot.

BACKGROUND: Isthmus conduction velocity (CV) predicts ventricular tachycardia risk in tetralogy of Fallot and traditionally is calculated using electrogram-based methods (electrogram CV index [EGM-CVi]). Isochronal activation mapping approximates CV in adults with ischemic and nonischemic cardiomyopathy but has never been tested in congenital heart disease populations. Our objectives were to (1) investigate the diagnostic performance of isochronal CV index (ISO-CVi) in predicting inducible ventricular tachycardia and (2) compare it to traditional EGM-CVi in a population of patients with repair

Primary biventricular repair in neonates with borderline left ventricle.

BACKGROUND: Borderline left ventricle (LV) represents a spectrum of defects, from mild to severe left heart hypoplasia, often with other cardiac anomalies. Decision-making regarding primary biventricular (BiV) repair is challenging. We sought to identify risk factors associated with adverse outcome after neonatal primary BiV repair for borderline LV. METHODS: A retrospective review was undertaken of neonatal primary BiV repair for borderline LV between 2001 and 2022. The primary outcome was composite of death, transplant, conversion to single-ventricle circulation, pulmonary vascular resistan

Early-Diastolic Right Ventricle-Pulmonary Artery Pressure Gradient by Doppler Echocardiography to Guide Cardiac Magnetic Resonance Assessment of Pulmonary Regurgitation after Right Ventricular Outflow Tract Reconstruction.

BACKGROUND: Pulmonary regurgitation (PR) after right ventricular outflow tract (RVOT) reconstruction is a major driver of right ventricular (RV) remodeling. Cardiac magnetic resonance (CMR) is the reference standard for quantifying PR but is not routinely feasible in many pediatric follow-up settings. We evaluated whether an echocardiographic early-diastolic RV-pulmonary artery (PA) pressure gradient (Echo-EaDPG), derived from early-diastolic peak PR velocity (ΔP = 4v2), can serve as a practical complementary hemodynamic marker to guide CMR assessment. METHODS: We retrospectively analyzed pat

Minimally invasive treatment strategy for severe congenital pulmonary valve insufficiency and pulmonary artery aneurysmal dilatation: a case report.

Pulmonary valve insufficiency most commonly occurs following various precordial interventions, including repair of tetralogy of Fallot, balloon dilatation for pulmonary stenosis, and surgical or percutaneous treatment for pulmonary atresia with intact ventricular septum. Isolated congenital pulmonary valve insufficiency is rare, and insufficiency secondary to myxomatous degeneration of the pulmonary valve is even rarer. We report the case of a 16-year-old female with severe pulmonary valve insufficiency and aneurysmal dilatation of the pulmonary artery caused by myxomatous degeneration of the
