Septal leaflet augmentation during Da Silva cone repair: Valvar function and anatomic features compared to nonpatch repair.

OBJECTIVES: To evaluate outcomes of Da Silva cone repair (CR) using autologous pericardial patch for septal leaflet augmentation in Ebstein anomaly (EA) and identify tricuspid valve (TV) morphology associated with patch use. METHODS: This single-site retrospective study included patients undergoing CR for EA between January 2016 and November 2024. Patients with a prior Starnes procedure or TV surgery were excluded. Patients were categorized into patch and nonpatch groups. The primary outcome was TV function (stenosis or regurgitation) at the latest follow-up, with a secondary composite outcom

Outcomes of Heart Transplantation in Single-Ventricle Physiology: A Retrospective Single-Center Experience with Emphasis on Surgical Complexity.

Background: Patients with single-ventricle physiology represent a high-risk group for heart transplantation. Due to complex anatomical and physiological challenges, including multiple prior sternotomies, pulmonary artery abnormalities, and systemic consequences of altered circulation, they represent both a surgical and a clinical challenge. We aimed to analyze perioperative challenges, as well as early and long-term complications, in this specific group of patients. Methods: We performed a retrospective data analysis of a high-volume heart transplant center, focusing on patients with single-ve

One Anatomy, Multiple Valve Choices: Interobserver Agreement in Selecting Transcatheter Pulmonary Valve.

BACKGROUND: Transcatheter pulmonary valve replacement (TPVR) requires selecting the optimal device type, size, and position. Variability in anatomical shape may affect procedural planning. AIM: This study aimed to evaluate interobserver agreement (IOA) in TPVR device selection using 3D patient-specific anatomies and explore a framework for determining agreement. METHODS: Six experienced observers from three regions of the Americas independently assessed valve type (self-expanding [S-E], balloon-expandable [B-E], surgical approach [SA]), size, and position for 60 3D reconstructed anatomies (p

Planimetry of Aortic Valve Area Using CTA: Cutoff Derivation for Stenotic Bicuspid and Tricuspid Valves.

BACKGROUND: Computed tomography based planimetric assessment of the anatomic aortic valve area (aAVACTA) in aortic stenosis is routinely performed. Unlike transthoracic echocardiography (TTE)-based effective AVA by TTE, it lacks clearly defined severity cutoff values, limiting clinical utility. METHODS: In this retrospective single-center analysis with computed tomography angiography data from 2013 to 2025, cutoffs were determined from 1294 TTE-based conclusive severe or nonsevere patients by congruence of maximum velocity, mean pressure gradient, and effective AVA by TTE. In separate receive

Regional mechanical and microstructural variations in ascending thoracic aortic aneurysms influenced by tricuspid and bicuspid aortic valves.

Ascending thoracic aortic aneurysms (ATAAs) exhibit regional biomechanical heterogeneity linked to microstructural changes and comorbidities. This study investigated microstructure and mechanical property variations across six regions of the ATAA wall from 10 patients after repair surgery. Planar biaxial tensile tests and two-photon microscopy were performed to determine material parameters for a microstructure-motivated constitutive model. These parameters describe the behavior of the aortic wall across a wide range of stretch values and ratios. Our results revealed significant inter- and int

Rare regulatory mutations disrupt mesenchymal molecular programs driving endocardial cushion formation in bicuspid aortic valve.

Bicuspid aortic valve, a prevalent congenital malformation, predisposes individuals to severe complications. Although the condition exhibits substantial heritability, known protein-coding and common regulatory mutations explain a minority of cases. To assess the contribution of rare regulatory variants, here we integrate high-resolution three-dimensional genome organization profiling with matched whole-genome sequencing from eight individuals with bicuspid aortic valves and eight with standard tricuspid aortic valves. In bicuspid aortic valve patients, mutation-driven chromatin rewiring affect

Phloroglucinol Exposure During Early Pregnancy and Risk of Major Congenital Malformations: A Nationwide Cohort Study in France.

BACKGROUND AND AIMS: Phloroglucinol is a spasmolytic drug widely prescribed in France for gastrointestinal or pelvic pain, and frequently used during pregnancy despite limited evidence regarding its safety. We aimed to evaluate the association between first-trimester phloroglucinol exposure and the risk of major congenital malformations (MCMs) in a nationwide cohort. METHODS: Using data from the French EPI-MERES cohort based on the national health data system (SNDS), we identified all pregnancies ending after 22 weeks between 2010 and 2022, excluding women with phloroglucinol use in the year

Ebstein Anomaly: Imaging, Pathophysiology, and Therapeutics.

Ebstein anomaly is a rare congenital malformation defined by failed delamination and apical displacement of the tricuspid valve, which produces an atrialized segment of the right ventricle and variable right ventricular dysfunction. Its clinical expression is wide, ranging from severe neonatal cyanosis and heart failure, which carry high perinatal mortality, to incidental detection in adulthood. Because anatomy and physiology vary widely among patients, imaging is central to diagnosis, risk stratification, and treatment planning. Transthoracic echocardiography remains the first-line study and

Cardiomegaly With an Absent Tricuspid Septal Leaflet: A Cadaveric Case Report.

The heart functions as a coordinated four-chambered pump, with four valves ensuring unidirectional blood flow. The tricuspid valve, located between the right atrium and right ventricle, consists of anterior, posterior, and septal leaflets that coapt during systole to prevent retrograde flow. Valvular disorders may result in stenosis or regurgitation, thereby increasing cardiac workload and predisposing to chamber dilation and heart failure. Congenital abnormalities, including partial or complete leaflet absence, are rare but can significantly disrupt normal hemodynamics. Reports of absent tric

Cone reconstruction for severe tricuspid valve dysplasia in a dog.

Tricuspid valve dysplasia (TVD) is a congenital cardiac malformation in dogs characterized by abnormal tricuspid valve morphology, leaflet tethering, impaired leaflet coaptation, and right atrial and ventricular remodelling. This report describes cone reconstruction for severe TVD in a dog. A three-year-and-10-month-old female Labrador retriever presented with a grade IV/VI systolic murmur and exercise intolerance. Echocardiography revealed severe tricuspid regurgitation, a right atrial-to-left atrial ratio of 1.57, malformed and tethered tricuspid valve leaflets, impaired leaflet coaptation,

A Small-Diameter Tricuspid Valve Prosthesis for the Correction of Congenital Heart Defects Based on an Adult Donor Mitral Valve Homograft.

BACKGROUND: Tricuspid valve replacement is the preferred method when valve repair is not feasible. Stented xenobioprostheses, which limit the growth of the fibrous ring due to their rigid titanium frame and often have a short service life, are particularly problematic in children. An alternative approach is the implantation of a mitral valve homograft in the tricuspid position, which is more resistant to calcification. However, the limited availability of small-diameter homografts from adult donors restricts their use. To address this, a new technique for downsizing an adult mitral homograft w

Bailout valve-in-valve therapy during transcatheter aortic valve replacement in bicuspid and tricuspid aortic stenosis: an observational single-centre study in China.

BACKGROUND: Bailout valve-in-valve (ViV) transcatheter aortic valve replacement (TAVR) is a critical rescue strategy for procedural failure, yet evidence regarding its outcomes in bicuspid aortic valve (BAV) anatomy remains limited. METHODS: This retrospective, single-centre study analysed 1597 patients (48.3% BAV) undergoing TAVR. Patients were stratified by the requirement for bailout ViV, which was conducted for significant residual aortic regurgitation (AR) or valve embolisation. Predictors were identified via multivariate logistic regression. Early- and mid-term survival outcomes were co

Can the Stand-Alone Cervical Interfacet Fusion Achieve Spinal Stability? A Comprehensive Finite Element Analysis.

BackgroundDegenerative pathologies of the cervical spine often involve facet and disc degeneration, leading to biomechanical instability of the cervical spine. A promising surgical solution is a cervical interfacet fusion system (CIFS), typically combined with an anterior disc cage to promote bone fusion and restore stability. However, the necessity of the anterior disc cage remains controversial because it increases surgical time, cost, and risk. The aim of this study was to evaluate the biomechanical performance of CIFS alone compared with its combination with an anterior disc cage using a C

Outcome analysis of bioprosthetic valve replacement in patients aged ≥ 50 years with Ebstein's anomaly.

OBJECTIVE: Cone reconstruction has become the preferred surgical technique for Ebstein's anomaly (EA). However, some patients aged ≥ 50 years are unsuitable for repair due to severe leaflet dysplasia, marked annular dilation, or impaired right ventricular function. This study evaluates the clinical outcomes of domestic bioprosthetic valve replacement in patients aged ≥ 50 years with EA. METHODS: A retrospective analysis was conducted on 15 consecutive patients aged ≥ 50 years with EA who underwent initial bioprosthetic tricuspid valve replacement at the Department of Cardiovascular Surgery, A

Arrhythmia Occurrence After Balloon-Expandable and Self-Expanding Transcatheter Pulmonary Valve Replacement.

BACKGROUND: The incidence of arrhythmias after transcatheter pulmonary valve replacement (TPVR) beyond the early postprocedural period and their relationship to valve type (balloon-expandable [BEPVR] vs self-expanding [SEPVR] pulmonary valve replacement) have not been well defined. OBJECTIVES: The goal of this study was to assess the incidence, types, and timing of arrhythmias observed after BEPVR and SEPVR. METHODS: We retrospectively reviewed patients undergoing TPVR at our institution from 2012 to 2022 with rhythm monitoring beyond 14 days' postprocedure. The primary outcome was a composi
