Congenital pulmonary valve stenosis is a common cause of right ventricular (RV) outflow tract obstruction. Advanced atrioventricular (AV) block in congenital heart disease is usually associated with abnormalities of AV septation or conduction system malposition. An association between isolated pulmonary valve stenosis and advanced AV block has not previously been reported. A 16-year-old male presented with exertional dyspnea and recurrent presyncope. Electrocardiography (ECG) revealed 2:1 AV block. Transthoracic echocardiography demonstrated severe valvular pulmonary stenosis with a peak gradi

Pulmonary valve stenosis is a congenital heart disease that is usually diagnosed during childhood, while presentation in adulthood is uncommon and may lead to delayed diagnosis. We report the case of a 21-year-old patient with no significant medical history who presented with a six-month history of exertional dyspnea and constrictive chest pain. Electrocardiography showed right axis deviation with signs of right ventricular hypertrophy, and chest radiography revealed dilatation of the left pulmonary artery. Transthoracic echocardiography demonstrated severe valvular pulmonary stenosis with a p

BACKGROUND: The incidence of arrhythmias after transcatheter pulmonary valve replacement (TPVR) beyond the early postprocedural period and their relationship to valve type (balloon-expandable [BEPVR] vs self-expanding [SEPVR] pulmonary valve replacement) have not been well defined.
OBJECTIVES: The goal of this study was to assess the incidence, types, and timing of arrhythmias observed after BEPVR and SEPVR.
METHODS: We retrospectively reviewed patients undergoing TPVR at our institution from 2012 to 2022 with rhythm monitoring beyond 14 days' postprocedure. The primary outcome was a composi

A 28-year-old woman presented with heart failure symptoms secondary to severe pulmonary valve regurgitation in the setting of congenital pulmonary valve stenosis that required balloon valvuloplasty. Computed tomography angiography fit analysis demonstrated a severely dilated main pulmonary artery that did not meet the minimum oversizing criteria for placement of a 25-mm Harmony transcatheter pulmonary valve. A hybrid transcatheter pulmonary valve placement approach took place, and a post main pulmonary artery plication right ventricle rotational angiogram was performed. The patient underwent a

The use of augmented reality (AR) for pre-procedural planning has increased over the past decade, yet its real-time application during complex cardiac interventions remains novel. This case highlights the integration of AR technology during a challenging transcatheter pulmonary valve replacement (TPVR) using a self-expanding system in a pediatric patient. A 12-year-old male with a history of congenital pulmonary valve stenosis presented with progressive right ventricular dilation and exercise intolerance. Dynamic CTA was processed via ARTICOR® software (Artiness, Milano, Italy) and visualized

Congenital heart disease may increase the incidence of necrotizing enterocolitis, especially in low-birth-weight infants. We report a case of a newborn with pulmonary valve stenosis who developed neonatal necrotizing enterocolitis IIIB. The infant underwent initial cardiac surgery followed by a laparotomy. After cardiac surgery, a right atrial thrombus was found by cardiac ultrasound. Enterococcus faecium was identified using metagenomic next-generation sequencing of ascitic fluid. The infant received targeted antibiotic therapy and anticoagulant treatment and was then discharged.

Use of the Harmony valve within a failing surgical bioprosthesis has not been described to date, especially with respect to compression of the valve and valve housing when Harmony is used in smaller, non-compliant landing zones. Described is a case of a 45-year-old male with a history of congenital pulmonary valve stenosis who underwent surgical pulmonary valve placement with a 25-mm Biocor Epic bioprosthesis and developed pulmonary regurgitation. The patient underwent cardiac catheterization with high-pressure balloon valvuloplasty with intentional fracturing and resultant compression of the

We report the case of a 9-year-old male with severe congenital pulmonary valve stenosis referred to our centre for percutaneous valvotomy. On admission, trans-thoracic echocardiogram confirmed a unicuspid pulmonary valve with a peak/mean pulmonary valve gradient of 91/53 mmHg and a pulmonary annulus of 13.8 mm (-0.8 Z Score). It also showed an enlarged RV (RV/LV ratio 0,9). During cardiac catheterisation, an additional atrial septal defect (secundum) with significant left to right shunt (Qp/Qs > 2) was diagnosed, which was not amenable to percutaneous closure. The patient was referred for surg

INTRODUCTION/OBJECTIVES: Doppler echocardiographic estimation of transpulmonary pressure gradient (PG) is widely used to determine severity of pulmonary valve stenosis and indication for transcatheter intervention. The objective of this study was to describe agreement between Doppler echocardiographic methods of transpulmonary PG estimation and direct peak-to-peak pressure gradient at catheterization (PGcath) in dogs. We hypothesized that with reference to PGcath, mean echocardiographic PG (PGecho-mean) would have less bias than peak modal instantaneous echocardiographic PG (PGecho-peak).

A 79-year-old woman, previously treated for congenital pulmonary valve stenosis through surgical and transcatheter procedures, presented with worsening dyspnea. This led to the identification of valve thrombosis in her bioprosthetic pulmonary valve. This report delves into the clinical importance of obstructive valve thrombosis, its diagnostic assessment, and potential therapeutic strategies. This is the first documented case of valve-in-valve thrombosis involving the transcatheter pulmonary valve.

Lesions of the semilunar valve and the aortic arch can occur either in isolation or as part of well-described clinical syndromes. The polygenic cause of calcific aortic valve disease will be discussed including the key role of NOTCH1 mutations. In addition, the complex trait of bicuspid aortic valve disease will be outlined, both in sporadic/familial cases and in the context of associated syndromes, such as Alagille, Williams, and Kabuki syndromes. Aortic arch abnormalities particularly coarctation of the aorta and interrupted aortic arch, including their association with syndromes such as Tur

INTRODUCTION: Congenital heart diseases (CHDs) constitute the most prevalent congenital pathology, and they are a consequence of structural and functional abnormalities during fetal development. The etiology of CHD involves the interaction of genetic and environmental factors. Fetal cardiac surgery aims at preventing natural pathways of CHD in utero, mitigating progression to more complex abnormalities. The goal of this review was to demonstrate the benefits and risks of fetal interventions in the two most prevalent CHDs, pulmonary stenosis and pulmonary atresia with an intact ventricular sept

Aneurysms of the pulmonary artery are uncommon vascular pathologies that are associated with congenital structural cardiac anomalies, pulmonary hypertension, vasculitis, neoplasm, iatrogenic, and infection. PAAs are commonly asymptomatic and accidentally diagnosed, however, if symptomatic, clinical features are generally non-specific and depend on the etiology of PAA. CT pulmonary angiography remains the gold standard imaging modality and other diagnostic imaging tests include transthoracic echocardiography and right heart catheterization. Definitive treatment of PAA is surgery, however, conse

Smith Magenis syndrome (SMS) is a rare neurobehavioral disorder caused by 17p11.2 microdeletion encompassing Retinoic Acid-Induced 1 (RAI1) gene (90% of cases) or by RAI1 point mutation (10% of cases). The neuropsychological phenotype of individuals with 17p11.2 deletion and in those with RAI1 variants mostly overlaps. However, cardiac defects have been described only in patients with a deletion so far. Here, we present the first case of a patient affected by SMS caused by RAI1 variant in whom a severe congenital pulmonary valve stenosis was diagnosed at birth, requiring trans catheter dilatat

Right ventricular outflow tract (RVOT) anomalies comprise a wide spectrum of congenital heart disease, typically characterized by obstruction to flow from the right ventricle to pulmonary arteries. This review highlights important considerations surrounding management strategy as well as clinical outcomes for the neonate with RVOT anomaly, including: pulmonary atresia with intact ventricular septum, congenital pulmonary valve stenosis, tetralogy of Fallot, and Ebstein anomaly with anatomic or physiologic RVOT obstruction.

Congenital pulmonary valve stenosis (PVS) is a common congenital heart defect. In the infancy of cardiac surgery, open surgical valvotomy or closed surgical transventricular pulmonary valvotomy (Brock procedure) were the mainstays of therapy. We report the longest-known published follow-up of two women who as young children underwent pulmonary valvotomy for PVS and subsequent uncomplicated open pulmonary valve replacement over 60 years later.

INTRODUCTION: Balloon Valvuloplasty is the first-line treatment for congenital valve pulmonary stenosis (PS) in children and adults.
METHODS: We reported immediate results of balloon valvuloplasty in congenital pulmonary stenosis from 2014 to 2019.
RESULTS: BPV was performed in 40 patients, the diagnosis was based on cardiac ultrasound data showing valvular dysplasia in 92% of cases, the main associated anomaly is FOP in 22.5% of cases, immediate success (defined by a peak-to-peak gradient after the DPPC of less than 50mmHg) was found in 88% of cases, allowing a significant reduction in mea
