Background/Objectives: Noonan syndrome (NS) is characterized by marked phenotypic variability. Although the classical phenotype includes short stature, craniofacial dysmorphism, skeletal abnormalities, and congenital heart defects, some patients present with subtle or atypical manifestations, making clinical recognition challenging. Methods: This retrospective case series included six pediatric patients with molecularly confirmed NS managed at the Department of Endocrinology and Metabolic Diseases, Polish Mother's Memorial Hospital-Research Institute, Lodz, Poland. Medical records were reviewe

BACKGROUND: Most atrial septal defects (ASDs) occur sporadically, but familial cases can occur and carry a 10% risk of transmission to offspring.
CASE SUMMARY: We describe a 3-generation family with autosomal dominant secundum ASD and conduction system disease related to a novel NKX2-5 variant (c.300del; p.Asp101Thrfs∗75). The index patient, a 58-year-old woman with prior ASD repair and mitral valve replacement, presented with chronotropic incompetence, atrial fibrillation, left bundle branch block, and prolonged pauses, leading to pacemaker implantation. Her mother had an ASD, atrial fibrill

This case describes a neonatal truncus arteriosus repair using an 8 mm ring-reinforced, valved polytetrafluoroethylene conduit for right ventricular outflow tract reconstruction via the 'double-dunk' technique. The diagnosis was established prenatally in a patient with familial history of the disease. Anatomically, both pulmonary arteries arose adjacent to each other from the posterolateral common trunk; there was a mildly regurgitant quadricuspid truncal valve and an accidental finding of an intramural course of both coronary arteries. Clinically, the neonate presented with mild cyanosis and

Right ventricle-to-pulmonary artery reconstruction is essential in congenital heart surgery. In Japan, cryopreserved homograft availability is limited, necessitating alternative materials. This study systematically elucidated the status of right ventricle-to-pulmonary artery conduit use and surgeon priorities in Japan. In 2024, a web-based nationwide survey was conducted by the Japanese Society of Paediatric Cardiology and Cardiac Surgery, targeting 126 institutions. We analysed data from 2021 to 2023 regarding primary diagnoses, conduit types, and selection criteria. Responses were obtained f

OBJECTIVE: Durable and biocompatible pulmonary valved conduits remain a challenge in congenital cardiac surgery. The GORE PV1 device is a fully synthetic expanded polytetrafluoroethylene-based conduit designed to improve durability and reduce calcification. This study characterizes the gross and histopathologic findings in explanted PV1 conduits.
METHODS: Six GORE PV1 conduits explanted from pediatric patients (median age: 13.5 years) were evaluated. All patients were part of a previously reported cohort. Clinical data, imaging, and operative notes were reviewed. Gross and histologic examinat

PURPOSE: Tetralogy of Fallot with absent pulmonary valve (TOF-APV) can manifest neonatally with severe respiratory insufficiency secondary to tracheobronchomalacia (TBM). Typical treatment of patients with TOF-APV with significant respiratory compromise is directed at the underlying heart defect rather than the airway. Complete repair often does not ameliorate airway issues and subsequently requires a tracheostomy or ventilator.
DESCRIPTION: Given this lack of effective treatment, a bioresorbable, patient-specific, 3-dimensionally printed splint was developed that provides external support to

OBJECTIVES: To describe the mitral valve (MV) anomalies found in patients with transposition of the great arteries (TGA), the surgical procedures performed, and the fate of the abnormal MV.
METHODS: From 1990 to 2020, 52 patients out of 1590 TGA {S, D, D} patients (3.3%) undergoing biventricular repair were identified with abnormal MV. Anomalies were a mitral cleft (n = 46 [88%], ejection/outflow tract in 40 and as atrioventricular septal defect [AVSD]-type in 6) and/or anomalies of the subvalvular apparatus (60%). A ventricular septal defect was present in 88.5% and pulmonary stenosis in 17.

BACKGROUND: Various types of UVB radiation source (290-320 nm) are used in treating psoriasis and their therapeutic mechanism has been attributed to immunosuppressive properties. Recently, a new UVB source generated by a 308-nm excimer laser has been introduced for the treatment of psoriasis.
OBJECTIVE: In this study we investigated the immunohistochemical evaluation of T cells and the expression of various apoptosis-related molecules in the psoriatic hyperproliferative skin before and after treatment with 308-nm monochromatic excimer light (MEL).

INTRODUCTION: Hammock mitral valve (HMV), or mitral arcade (MA), is an extremely rare congenital mitral anomaly characterized by absent fibrous chordae and hypertrophic papillary muscles directly inserting into the leaflets. This rare valve structure can trigger severe mitral regurgitation and overwhelm a newborn's heart and lungs within days. Because symptoms often resemble those of more common respiratory or cardiac issues, early diagnosis is difficult - but vital. Recognizing the anomaly in time and referring the infant to a specialized center can make the difference between life and death.

OBJECTIVES: Fresh, non-cryopreserved and non-seeded, decellularized pulmonary homografts (DPHs) were translated in 2005 into clinical practice. The aim of this current study is to summarize our 20-year clinical experience with DPH for pulmonary valve replacement (PVR) in congenital heart disease.
METHODS: Prospective follow-up of all DPH implanted within 2 institutions. Indication for PVR according to current clinical guidelines was the key inclusion criterion without age limits; patients with active endocarditis were not included.
RESULTS: From January 2005 to August 2025, 310 patients (188

BACKGROUND: While long-term survival for tetralogy of Fallot continues to improve, residual hemodynamic abnormalities are common and increase risk for cardiovascular events during pregnancy.
CASE SUMMARY: A young woman with tetralogy of Fallot with pulmonary atresia (TOF/PA) having undergone biventricular repair presented with heart failure symptoms and near-systemic right ventricular (RV) pressure in the setting of an unplanned pregnancy. Given her desire to continue the pregnancy coupled with the significant maternal and neonatal risk, percutaneous intervention was performed, improving RV p

OBJECTIVE: Syndromic conditions are frequently associated with congenital heart disease (CHD). Their distribution, recognition and prognostic impact in adults with CHD (ACHD) remain poorly characterised due to limited assessment beyond childhood.
METHODS: We retrospectively analysed 9826 patients with ACHD from a tertiary ACHD centre. Syndromic conditions were identified from clinical records and categorised as chromosomal, single-gene or other syndromes. Clinical characteristics, comorbidities and CHD complexity were compared between syndromic and non-syndromic patients. Multivariable Cox re

A six-year-old neutered male cat was presented for acute respiratory distress. Transthoracic echocardiography identified a left atrial membrane consistent with cor triatriatum sinister (CTS) and turbulent flow across a single fenestration. Additionally, it was suspected that the distal chamber may receive at least one pulmonary vein. Because respiratory signs improved with corticosteroids but not with a short diuretic trial, and because echocardiography suggested that the distal chamber might receive at least one pulmonary vein, cardiac computed tomography angiography was performed to further

BACKGROUND: Congenital heart disease (CHD) is the most prevalent congenital abnormality worldwide and is the primary contributor to neonatal mortality. Obstetricians in China continue to face uncertainties about optimal delivery timing, appropriate delivery mode, perinatal interventions, and postnatal surgical outcomes for fetuses with complex CHD.
OBJECTIVE: To investigate integrated pregnancy and perinatal management strategies for fetuses diagnosed with complex CHD by prenatal ultrasound in China.
METHODS: A retrospective analysis was performed on 123 fetuses with complex CHD diagnosed by

BACKGROUND: Isthmus conduction velocity (CV) predicts ventricular tachycardia risk in tetralogy of Fallot and traditionally is calculated using electrogram-based methods (electrogram CV index [EGM-CVi]). Isochronal activation mapping approximates CV in adults with ischemic and nonischemic cardiomyopathy but has never been tested in congenital heart disease populations. Our objectives were to (1) investigate the diagnostic performance of isochronal CV index (ISO-CVi) in predicting inducible ventricular tachycardia and (2) compare it to traditional EGM-CVi in a population of patients with repair

BACKGROUND: Borderline left ventricle (LV) represents a spectrum of defects, from mild to severe left heart hypoplasia, often with other cardiac anomalies. Decision-making regarding primary biventricular (BiV) repair is challenging. We sought to identify risk factors associated with adverse outcome after neonatal primary BiV repair for borderline LV.
METHODS: A retrospective review was undertaken of neonatal primary BiV repair for borderline LV between 2001 and 2022. The primary outcome was composite of death, transplant, conversion to single-ventricle circulation, pulmonary vascular resistan

BACKGROUND: Pulmonary regurgitation (PR) after right ventricular outflow tract (RVOT) reconstruction is a major driver of right ventricular (RV) remodeling. Cardiac magnetic resonance (CMR) is the reference standard for quantifying PR but is not routinely feasible in many pediatric follow-up settings. We evaluated whether an echocardiographic early-diastolic RV-pulmonary artery (PA) pressure gradient (Echo-EaDPG), derived from early-diastolic peak PR velocity (ΔP = 4v2), can serve as a practical complementary hemodynamic marker to guide CMR assessment.
METHODS: We retrospectively analyzed pat

BACKGROUND: Transcatheter pulmonary valve replacement (TPVR) requires selecting the optimal device type, size, and position. Variability in anatomical shape may affect procedural planning.
AIM: This study aimed to evaluate interobserver agreement (IOA) in TPVR device selection using 3D patient-specific anatomies and explore a framework for determining agreement.
METHODS: Six experienced observers from three regions of the Americas independently assessed valve type (self-expanding [S-E], balloon-expandable [B-E], surgical approach [SA]), size, and position for 60 3D reconstructed anatomies (p
