Quantification of Tricuspid Valve Regurgitation With 2D and 4D-Flow MRI: Comparison of Different Methods in Adult Patients With Ebstein's Anomaly.

BACKGROUND: Ebstein's anomaly (EA) is a congenital heart disorder involving tricuspid valve dysplasia and right heart abnormalities resulting in severe tricuspid regurgitation (TR). Multiple techniques assess regurgitation severity, but their correlation with EA severity markers remains unclear. PURPOSE: To compare MRI techniques for quantifying TR and regurgitant volume (RVol) in adults with EA. STUDY TYPE: Retrospective. SUBJECTS: Thirty-three adult patients with EA (15 males/18 females). FIELD STRENGTH/SEQUENCE: 3T or 1.5-Tesla, 2D-phase contrast (2D-PC gradient echo) and 4D-flow (gradi

Lyell´s syndrome associated with the use of topical agents: case report.

UNLABELLED: Pseudomonas aeruginosa AG1 (PaeAG1) is a Costa Rican strain that was isolated in 2010 in a major Hospital. This strain has resistance to multiple antibiotics such as β-lactams (including carbapenems), aminoglycosides, and fluoroquinolones. PaeAG1 is considered critical (Priority 1) due to its resistance to carbapenems, and it was the first report of a P. aeruginosa isolate carrying both VIM-2 and IMP-18 genes encoding for metallo-β-lactamases (MBL) enzymes (both with carbapenemase activity). Owing to these traits, we have studied this model for 10 years using diverse approaches inc

Neighborhood Economic Deprivation, Hispanic Ethnic Enclaves, and Congenital Anomalies in Texas.

BACKGROUND: Congenital anomalies are common, costly, and potentially life-threatening. The impacts of structural determinants of health on risk for these conditions have scarcely been studied. We evaluated associations between residence in economically disadvantaged neighborhoods or Hispanic/Latino enclaves and congenital anomalies. METHODS: We performed a registry linkage study of 7.25 million births and 332,935 cases with anomalies in Texas, 1999-2018. We defined exposure groups using maternal residential address at delivery or termination and tract-level Yost socioeconomic index and Hispan

Acute Severe Mitral Regurgitation Following Blunt Chest Trauma and a Febrile Illness in a Young Adult With Degenerative Mitral Valve Disease: A Rare Case Report.

Acute severe mitral regurgitation (MR) in young adults is rare and is usually associated with infective endocarditis, trauma, congenital abnormalities, or connective tissue disorders. However, acute presentation of degenerative mitral valve disease is uncommon in young adults. A 23-year-old man developed acute pulmonary edema following blunt precordial trauma during a football match, with a brief febrile illness occurring several days later. Transthoracic echocardiography demonstrated acute severe eccentric MR due to flail of the P2 scallop resulting from the rupture of the chordae tendineae.

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

The pulmonary valve in tetralogy of Fallot: insights from a necroscopy series.

BACKGROUND: Tetralogy of Fallot is the most common cyanotic congenital malformation of the heart. The right ventricular outflow tract is of great interest in this setting, but most of the focus on this feature has been on the size of the so-called pulmonary valvar "annulus". We aimed to characterise other aspects of the morphology of the pulmonary root in heart specimens with tetralogy of Fallot. METHODS: We reviewed archived hearts with tetralogy of Fallot from four registries. The pulmonary root was examined with specific attention to the number of sinuses, the number of leaflets, presence

The Living Ross Procedure: A New Paradigm in Partial Heart Transplantation.

In the Ross procedure, homograft stenosis and regurgitation have limited the longevity of the right ventricle-to-pulmonary artery (RV-PA) conduit. Partial heart transplantation (PHT) facilitates conduit growth throughout the life of the child. We report the application of PHT of the RV-PA conduit during a Ross procedure, "living Ross," in a 3-year old patient with a congenital unicuspid aortic valve. Pulmonary autograft was used in the aortic position, and an aortic root allograft from the cardiectomy of an adult heart transplant recipient served as the RV-PA conduit. We hypothesize that this

Triatrial Appearance in a Patient With Atrial Septal Defect: A Case Report.

Atrial septal defect (ASD) is a common congenital heart anomaly, often diagnosed with relative ease. However, the presence of additional structural abnormalities, such as cor triatriatum dexter (CTD) or a prominent eustachian valve, can create a triatrial appearance, complicating diagnosis and management. Accurate differentiation of these anomalies is essential to guide appropriate clinical decisions. We report the case of a 25-year-old male with exertional dyspnea and fatigue. Cardiovascular examination revealed a wide, fixed splitting of S2 and a systolic murmur. Transthoracic echocardiograp

Investigating the Long-term Durability of Prosthetic Valves in Patients Who Have Undergone Tetralogy of Fallot Surgery with Transannular Patch.

OBJECTIVES: This study aimed to determine the longevity of bioprosthetic valves (BPVs) or valved conduits in the pulmonary position and the factors associated with prosthetic valve longevity in patients who underwent surgical repair of tetralogy of Fallot (TOF). METHODS: We performed a single-centre retrospective study in patients undergoing placement of a bioprosthesis or valved conduit in patients operated for TOF with a transannular patch. The primary end-point was freedom from pulmonary valve replacement (PVR) reintervention. The composite secondary end-point was freedom from PVR reinterv

Does Valve-Sparing Repair Improve Outcomes in Tetralogy of Fallot? A Systematic Review.

BACKGROUND: The systematic review determines whether valve-sparing surgeries improve patient outcomes compared with non valve-sparing surgeries for tetralogy of Fallot. Although previous studies have reported superior outcomes in valve-sparing surgery, no systematic review has compared these outcomes with those of nonsurgical surgical methods. METHODS: In the present research, we address this gap in the literature by comparing valve-sparing and non valve-sparing surgery outcomes. PubMed, PubMed Central, and Medline served as the databases for data collection; following the PRISMA guidelines,

Long-Term Outcomes in Infants After Tetralogy of Fallot Repair: Does Repair Strategy Matter?

BACKGROUND: Many studies of tetralogy of Fallot (TOF) with pulmonary stenosis repair focus on early outcomes. This study aimed to assess associations between TOF repair types and midterm or late outcomes. METHODS: This was a single-institution, retrospective cohort study of infant primary TOF repair from 1997 to 2020. Valve-sparing repair was compared with transannular patch repair. Outcomes were postdischarge unplanned reintervention on the pulmonary valve, main pulmonary artery, branch pulmonary artery, other reintervention and a composite of 1 or more of these reinterventions (composite).

Left Atrial Strain and History of Life-Threatening Arrhythmia in Adults with Repaired Tetralogy of Fallot: A Cardiovascular Magnetic Resonance Study.

Risk stratification for ventricular arrhythmia in adults with repaired tetralogy of Fallot (r-ToF) remains suboptimal. We investigated the relationships between left atrial reservoir strain (LAS) measured by cardiovascular magnetic resonance (CMR) and history of life-threatening ventricular arrhythmia (h-LTA) in r-ToF. We retrospectively analyzed 39 adults with r-ToF (mean age 39 ± 15 years; 69% male) undergoing routine CMR. LAS was quantified from four-chamber cine images. Patients with h-LTA, defined as a documented episode of any ventricular tachyarrhythmia, namely non-sustained ventricular

Criss cross heart: an outflow tract defect?

BACKGROUND: Criss-cross heart (CCH) is a rare congenital malformation in which the atrioventricular inflow vectors are approximately perpendicular. CCH is associated with other defects including malposition of the great arteries, supero-inferior ventricles, and ventricular septal defects (VSD). A recent study in a mouse model demonstrates that CCH and associated malformations can be the result of a growth arrest of the outflow tract. In order to confront this hypothesis, we studied 16 cases of criss-cross heart with detailed anatomical description and clinical outcomes. RESULTS: All patients w

Optimizing outcomes for patients undergoing superior cavopulmonary connection procedure: what we know.

The Superior cavopulmonary connection (SCPC) is typically the second stage of the single ventricle palliation. There are modifiable factors that have a complex interplay in determining outcomes such as mortality or hospital length of stay. There is no clear consensus on the timing of SCPC as this involves a critical balance between the timing of operation and the length of the high-risk interstage period. Younger age has not been associated with reduced transplant-free survival but has been associated with longer hospital length of stay. Another critical decision is for children with viral res

The Role of First-Trimester Ultrasound in Detecting Aortic and Pulmonary Valve Agenesis: A Rare Case of Trisomy 13.

Congenital Absent Pulmonary Valve Syndrome (APVS) is a rare heart defect often associated with other cardiac anomalies, with approximately 5.5% of cases related to trisomy 13. This case report describes a fetus with trisomy 13, presenting with the rare combination of absent pulmonary and aortic valves, along with an interrupted aortic arch. The patient was a 34-year-old pregnant woman with an otherwise normal pregnancy. At 12 weeks, ultrasound examination revealed thickened nuchal translucency (NT), ethmocephaly with proboscis, and a heart showing a single large vessel with alternating red and

Arterial Tortuosity Syndrome: A Longitudinal Assessment of Cardiovascular Features and Interventions From the Collaborative for Longitudinal Aortic Research in the Young (CLARITY).

Arterial tortuosity syndrome (ATS) is an ultra-rare arteriopathy with a cardiovascular phenotype of vascular tortuosity, elongation, dilation, and stenosis. In 14 patients with ATS, aortic and pulmonary artery longitudinal growth, phenotypes, and interventions were evaluated. Medical records and images of 14 patients with ATS (defined as biallelic SLC2A10 variants with consistent phenotypes) were reviewed. Vascular measurements were collected from imaging reports. Z-scores were calculated. Dilation and hypoplasia were defined as z-scores ≥ +2.0 and ≤ -2.0, respectively. Slopes were calculated

Don't Miss the Warning Signs: Syncope Masquerading as a Major Congenital Heart Defect in a 42-Year-Old Patient.

Congenital heart defects (CHDs) are typically diagnosed in infancy, yet some may go unidentified until adulthood and pose significant clinical challenges. We present the case of a 42-year-old female patient with multiple CHDs, including sinus venosus atrial septal defect (ASD), partial anomalous pulmonary venous return (PAPVR), and moderate mitral regurgitation discovered incidentally after presenting to the emergency department with atypical chest pain. Initial warning signs were overlooked, and asymptomatic mitral valve prolapse (MVP) diagnosed in childhood had progressed into moderate mitra

Quantification of Pulmonary Regurgitation After Surgical Repair of Tetralogy of Fallot Using Vector Flow Mapping.

BACKGROUND: Pulmonary valvular regurgitation in postoperative patients with repaired tetralogy of Fallot (rTOF) significantly impairs exercise capacity and causes right heart failure. Quantitative evaluation of the pulmonary valvular regurgitation fraction (PRF) by cardiac magnetic resonance (CMR) is commonly used to determine the indication for surgical or catheter interventions, but less commonly using echocardiography. METHODS AND RESULTS: We retrospectively investigated the feasibility and validation of vector flow mapping (VFM) for the quantification of PRF (VFM-PRF) in 34 pediatric pati

Antithrombotic approach in percutaneous pulmonary valve implantation (PPVI): What is our standard of care? A study endorsed by the Association for European Paediatric and Congenital Cardiology.

BACKGROUND: Despite the widespread adoption of percutaneous pulmonary valve implantation, there remains a lack of consensus on the optimal management of peri-interventional and long-term antithrombotic therapies because of a lack of evidence. AIM: To clarify current practices in peri/postprocedural antithrombotic strategies for percutaneous pulmonary valve implantation. METHODS: An online survey was submitted to the Interventional Working Group of the Association for European Paediatric and Congenital Cardiology, and was completed by 76 congenital interventional cardiologists in 2023-2024.
