Chronic Malaria Is Associated With Trauma-related Splenic Rupture Requiring Splenectomy.

Splenic rupture is a recognized complication of acute Plasmodium falciparum and Plasmodium vivax malaria, but the risk of splenic rupture in chronic asymptomatic infections is unknown. In Timika, Papua, Indonesia, we determined the proportion of PCR-detectable asymptomatic peripheral parasitemia in patients undergoing trauma-related splenectomy (2015-21) and found it was more than twice the proportion compared to a 2013 household survey of the general population (87.9% [29/33] vs 38.6% [697/1807]; P < .0001). Our findings suggest asymptomatic parasitemia with either P. falciparum or P. vivax i

Malaria-associated splenic haemorrhage requiring emergency splenectomy: a rare complication managed in a resource-constrained setting.

Falciparum malaria and Vivax malaria are notorious for the development of multiple complications, including acute kidney injury, acute liver injury, severe hemolysis, bone marrow suppression, severe acidosis, hypoglycemia and CNS complications. We hereby present two cases of rare complications associated with Malaria, which were managed in a resource-constrained setting in Eastern Africa. Two individuals working in the malaria endemic region of South Sudan were detected with Mixed Plasmodium falciparum-Plasmodium vivax when they reported to the hospital with acute febrile illness for two days.

Spontaneous splenic rupture as a rare complication of Plasmodium vivax: a case report.

Spontaneous splenic rupture is a rare but potentially fatal condition. We report the case of a thirty-year-old Moroccan soldier who presented with fever six months after returning from a deployment in a malaria-endemic country, despite appropriate mefloquine chemoprophylaxis. Malaria due to Plasmodium vivax was confirmed by blood film, and oral treatment (Artemether-Lumefantrine) was initiated. Within twenty-four hours of admission, the patient developed acute abdominal pain with hemodynamic instability and a decrease in haemoglobin level. Abdominal computed tomography revealed a subcapsular s

Spontaneous splenic rupture in Plasmodium vivax malaria.

A case of pathological splenic rupture associated with malaria complicated by Plasmodium vivax is presented in a 28-year-old male patient from El Retorno (Guaviare). The patient presented with fever, left side chest pain, vomiting, diarrhea, tachycardia, tachypnea, and signs of hemodynamic hypotension associated with hypovolemic shock. Upon admission, he was treated with intramuscular analgesics (dipyrone, 500 mg, and tramadol, 100 mg) and intravenous fluids. The hemogram revealed neutrophilia and thrombocytopenia, while the thick smear was positive for P. vivax with 24 000 parasites per μl. D

Malaria-Associated Spontaneous Splenic Rupture With Coagulopathy and Hemodynamic Compromise: A Case Report.

Spontaneous splenic rupture is a rare, life-threatening condition that occurs without trauma, often presenting diagnostic challenges due to nonspecific symptoms. The risk of spontaneous splenic rupture increases significantly with underlying pathological conditions, including Plasmodium vivax malaria, where rapid splenic enlargement and altered red blood cell surface characteristics likely contribute to the increased risk of rupture. Here, we describe the case of a 23-year-old male patient who presented with severe abdominal pain and hypotension. Examination revealed jaundice, left upper abdom

Spontaneous splenic rupture due to falciparum malaria successfully treated with a conservative approach.

Malaria is frequently associated with splenomegaly. However, spontaneous splenic rupture is a rare and life-threatening complication. It is mostly seen in acute infection in non-immune adults and Plasmodium vivax and Plasmodium falciparum have been associated with the majority of cases. We describe a case of splenic rupture in an adult with complicated malaria by Plasmodium falciparum in which a conservative approach was used.

A Rare Twist: Spleen Rupture in Plasmodium vivax Malaria.

Plasmodium vivax malaria is known for its recurring febrile episodes, typically considered less severe than its counterparts. This case study presents a distinctive case of a 22-year-old male from a malaria-endemic region, experiencing spleen rupture and infarction as rare complications of vivax malaria. We explore the uncommon aspects of this case, emphasizing the importance of early diagnosis and tailored management.

Clinical characteristics and outcome of severe malaria in Kapit, Sarawak, Malaysian Borneo.

BACKGROUND & OBJECTIVES: Severe malaria is a medical emergency and can lead to severe complications and death if not treated promptly and appropriately. Along with Plasmodium falciparum, P. knowlesi is increasingly recognised as a significant cause of fatal and severe malaria. METHODS: We performed a retrospective review on 54 cases of severe malaria in a district hospital in Kapit, Sarawak, from January 2018 to May 2019. The patients' demographics, clinical features, complications based on organ involvement, and treatment outcomes were examined. RESULTS: There were 54 cases of severe malari

Spontaneous Splenic Rupture in Malaria Patients: Two Case Reports.

Atraumatic splenic rupture is a rare and life-threatening condition, if not diagnosed. We present two cases with a history of travel to endemic areas, who came to the emergency department with abdominal pain and were diagnosed to have spontaneous splenic rupture as a complication of severe malaria. Both patients were treated surgically by splenectomy. A high level of clinical suspicion is critical in every malaria patient presenting with abdominal pain, even if it is mild. Clinical imaging modalities like ultrasonography and computed tomography (CT) are crucial diagnostic tools in managing suc

Case Report: Sudden Splenic Rupture in a Plasmodium falciparum-Infected Patient.

Spontaneous splenic rupture is a rare and life-threatening complication of severe malaria. It demands particular attention since delayed or missed diagnosis can be potentially fatal. The exact incidence is unknown largely due to underreporting. Acute malarial infection accounts for most of the spontaneous splenic rupture. Plasmodium vivax has been associated with the majority of them; however, on rare occasion, other Plasmodium infections have also resulted in splenic rupture. We report the case of a 74-year-old male who was diagnosed with severe malaria caused by Plasmodium falciparum (P. fal

Subcapsular Splenic Hemorrhage in Vivax Malaria.

In malaria, splenic rupture is a serious complication potentially leading to death. Subcapsular hemorrhage of spleen is thought to be an impending sign of splenic rupture; however, the characteristics of subcapsular hemorrhage are not well known. We report 3 cases of subcapsular hemorrhage of the spleen in vivax malaria, with varying degrees of severity. Case 1 showed subcapsular hemorrhage without splenic rupture, was treated by antimalarial drug without any procedure. The healing process of the patient's spleen was monitored through 6 computed tomography follow-up examinations, over 118

Multiple Splenic Infarcts Complicating Plasmodium vivax Malaria.

Splenic infarct is a rare complication in Plasmodium vivax malaria. We report a case of splenic infarction with acute kidney injury in a case of P. vivax malaria in a 13-year-old male child who presented to the emergency department with severe pain in the left hypochondrium. The patient was managed with intravenous artesunate and oral primaquine. Pain in left hypochondrium in children with P. vivax malaria due to splenic infarction needs to be evaluated for any surgical emergency like rupture or abscess. A review of literature of the unusual but serious complication is presented along with the

Sudden spleen rupture in a Plasmodium vivax-infected patient undergoing malaria treatment.

BACKGROUND: Splenomegaly is one of the most common features of malaria. However, spontaneous splenic rupture, although unusual, represents a severe complication often leading to death. It is mostly seen in acute infection and primary attack, and it is most commonly associated with Plasmodium vivax. Here, a case of spontaneous splenic rupture diagnosed with a portable ultrasound apparatus shortly after starting treatment and with recurrent parasitaemia after splenectomy, is reported. CASE DESCRIPTION: In November 2015, a 45-year-old Brazilian man presented to the hospital in Manaus with fever,

Nescient Aetiology of Splenic Laceration - An Enigma Unveiled.

Malaria is a common endemic disease prevalent in developing countries like India that presents with wide spectrum of clinical symptoms and complications. Splenic rupture is an uncommon but life-threatening complication which can be either spontaneous or as a result of trauma. We present a case of 50-year-old man with left upper quadrant pain following a polytrauma. Based on the radiological evidence of laceration and rupture of markedly enlarged spleen, emergency splenectomy was performed. Postoperative haematological evaluation established the co-infection of Plasmodium falciparum and vivax w

Spontaneous Hemoperitoneum in a case of Plasmodium vivax Malaria without Splenic Rupture.

Vivax malaria is usually a benign infection but now trends are changing. We, here present an unusual case of P. vivax malaria presenting with spontaneous hemoperitoneum with thrombocytopenia. Spontaneous hemoperitoneum in P. vivax malaria has been reported earlier but with splenic rupture. However our case though had hemoperitoneum but no splenic rupture. In our case, thrombocytopenia was found to be the cause of hemoperitoneum which has not been reported earlier in Indian literature.

Spontaneous splenic rupture in a vivax malaria case treated with transcatheter coil embolization of the splenic artery.

An enlarged spleen is considered one of the most common signs of malaria, and splenic rupture rarely occurs as an important life-threatening complication. Splenectomy has been recommended as the treatment of choice for hemodynamically unstable patients. However, a very limited number of splenic rupture patients have been treated with transcatheter coil embolization. Here we report a 38-year-old Korean vivax malaria patient with ruptured spleen who was treated successfully by embolization of the splenic artery. The present study showed that angiographic embolization of the splenic artery may be

Case report: spontaneous rupture of spleen in patient with Plasmodium ovale malaria.

Malaria may lead to spontaneous splenic rupture as a rare but potentially lethal complication. Most frequently, this has been reported in patients infected with Plasmodium falciparum and Plasmodium vivax, while other parasitic agents are less likely to be the cause. We report a 29-year-old British Caucasian, who after returning from a business trip in Democratic Republic Congo was diagnosed with tertian malaria caused by Plasmodium ovale.During his in-patient stay, the patient suffered a splenic rupture requiring immediate surgical intervention and splenectomy. Splenic infarction and malaria.

BACKGROUND: Splenic infarction is a well recognised complication of malaria that has been infrequently reported in the literature. METHODS: A review was performed to describe the spectrum of characteristics associated with this complication. RESULTS: Most patients presented with fever, left upper quadrant pain and/or splenomegaly, but no specific symptoms or signs appear to predict underlying infarction. The majority of cases reported were associated with autochthonous Plasmodium vivax infections, whereas cases reported in travellers were mostly due to P. falciparum acquired in Africa. CONC

Spontaneous splenic rupture in Plasmodium vivax malaria.

Malaria can present with various clinical symptoms and complications. While a tertian malaria form that is especially prevalent in Korea is characterized by mild clinical progression, occasional splenic complications are known to occur. A 26-year-old Korean male soldier without prior medical history visited The Armed Forces Capital Hospital with left upper quadrant abdominal pain one day ago. Hemostasis under laparoscopic approach was attempted. The operation was converted into laparotomy due to friable splenic tissue and consequently poor hemostasis. Splenectomy was performed.
