Beyond the label: Long-term outcomes after surgical management of Ebstein anomaly.

Ebstein anomaly is a rare congenital malformation of the tricuspid valve and right ventricle characterized by marked heterogeneity in anatomy, physiology, and clinical presentation. This broad phenotypic spectrum strongly influences the natural history of the disease, the timing and type of intervention, and ultimately long-term outcomes. Patients range from critically ill neonates with severe hemodynamic compromise and circular shunts to asymptomatic children and adults with progressive tricuspid regurgitation, right ventricular dysfunction, or arrhythmias, as well as survivors of previous su

Elevated lithium levels in drinking water are associated with higher prevalence of Wolff-Parkinson-White diagnoses.

BACKGROUND: Supraventricular tachycardia (SVT) and Wolff-Parkinson-White (WPW) are potentially highly symptomatic and, rarely, lethal arrhythmias associated with atrioventricular accessory pathways. While the etiology of accessory pathways remains largely unknown, they are frequently observed with Ebstein's Anomaly, a congenital heart disease associated with maternal medicinal lithium use. Therefore, we evaluated whether in utero exposure to lithium from environmental sources is associated with the prevalence of accessory pathways. METHODS: We conducted a county-level ecological comparison of

Coexistence of Ebstein Anomaly and Cerebral Cavernous Malformation: A Case Report.

In most patients who have Ebstein anomaly (EA), neurological symptoms are either associated with thromboembolic episodes or due to arrhythmic complications. Cerebral cavernous malformation (CCM) is a vascular anomaly made up of sinusoidally dilated blood vessels, often presenting with seizure activity or intracranial bleeding. There has never been any documented report regarding coexisting EA and CCM. Herein, we have reported a 32-year-old female with an established history of Ebstein anomaly diagnosed in 2013 who had a history of generalized tonic-clonic fits since 2020. Neuroimaging studies

Cyanotic Heart Disease.

Cyanotic congenital heart defects are malformations of the heart that affect its structure and function. The etiology of most of these defects is multifactorial, but some common causes include maternal conditions, genetic defects, and genetic mutations. Six of the most common cyanotic congenital heart defects include Tetralogy of Fallot, transposition of the great arteries, persistent truncus arteriosus, total anomalous pulmonary venous return, tricuspid atresia, and Ebstein's anomaly. These defects share several common features, however, each is distinguished by key cardiac abnormalities and

Treatment Strategies in Ebstein's Anomaly: An Observational Study Over Three Decades.

OBJECTIVES: This study compared long-term outcomes of surgical management for Ebstein's anomaly before and after the introduction of an individualized surgical approach at our centre, including Cone repair. METHODS: We conducted a retrospective, single-centre analysis of patients who underwent surgery for Ebstein's anomaly between 1988 and 2022. Since 2012, early intervention to preserve right ventricular (RV) function before severe deterioration was prioritized. Survival was characterized using Kaplan-Meier estimates, and restricted mean survival time (RMST) was computed up to a specified ti

Importance of clinical practice guidelines for specialized delivery room resuscitation of newborns with prenatally diagnosed critical congenital heart disease.

Perinatal transition is uniquely complex in newborns with critical congenital heart disease (CCHD), whose cardiopulmonary physiology often diverges from the standard neonatal resuscitation framework. We developed lesion-specific clinical practice guidelines (CPGs) for delivery room (DR) management of six high-risk CCHD diagnoses - hypoplastic left heart syndrome, dextro- transposition of the great arteries, Ebstein anomaly, congenital complete heart block, total anomalous pulmonary venous return, and tetralogy of Fallot with absent pulmonary valve. Developed collaboratively between neonatologi

Anatomical severity of Ebstein's anomaly: a quantitative analysis based on cardiovascular MRI.

BACKGROUND: Ebstein's anomaly (EA) exhibits significant anatomical and clinical heterogeneity, warranting a systematic approach to risk stratification. While the Carpentier classification (CC) is commonly employed for qualitative disease assessment, there is increasing interest in quantitative imaging parameters to personalise monitoring strategies and therapeutic interventions. We sought to evaluate the association between cardiovascular magnetic resonance (CMR) parameters, CC and symptoms of heart failure (HF). Also, we investigated whether CMR-derived markers may predict the need for bidire

Cardiovascular Profile Score and Perinatal Survival Among Fetuses With Ebstein's Anomaly or Tricuspid Valve Dysplasia: A Multi-Center Retrospective Cohort Study.

OBJECTIVE: We sought to perform multi-variable modeling to assess the independent value of the CVPS in fetuses with Ebstein anomaly or tricuspid valve dysplasia (EA/TVD). METHODS: CVPS was assessed at a core lab using the first and last echocardiograms during gestation. A receiver operating characteristic (ROC) curve analysis was conducted. Changes in the CVPS from the first to the last echo were assessed with Wilcoxon signed-rank tests. RESULTS: There were 164 fetuses with EA/TVD with complete CVPS at the first echo. Nearly half, 48.8% (n = 80), had intrauterine fetal demise (IUFD) or died

Ablation outcomes in children with Mahaim tachycardia using the TactiCath contact force catheter.

INTRODUCTION: Mahaim ablation poses a significant challenge among accessory pathway ablations. This study aims to present our findings on Mahaim ablation performed using a contact force catheter, emphasizing both the effectiveness and safety of this technique. MATERIALS AND METHODS: This is a single-centre retrospective cohort study. Twenty-two consecutive patients who underwent Mahaim pathway ablation procedures between January 2017 and January 2025 were included in the study. The EnSite Precision System (Abbott, St. Paul, MN, USA) was used to facilitate mapping and to reduce or eliminate th

Timely surgical intervention and risk stratification in patients with Ebstein anomaly: a 20-year retrospective cohort study.

BACKGROUND: The optimal timing and choice of surgery for Ebstein anomaly (EA), a complex congenital heart defect, remain challenging due to the heterogeneity and lack of robust long-term data on EA. In this study, we aimed to evaluate the long-term outcomes of surgical management in patients with EA, identify the prognostic risk factors, and develop a predictive model. MATERIALS AND METHODS: We conducted a retrospective analysis of data from 332 patients with EA who were treated at a tertiary center between January 2000 and December 2021. Among them, 269 underwent surgery: tricuspid valve rep

Use of right atrial wall to repair severely dysplastic tricuspid valve in an infant with Ebstein's anomaly.

The management of infants with Ebstein's anomaly is challenging and requires complex involvement of the multidisciplinary team. Surgical repair technique is dependent on the degree of tricuspid leaflet dysfunction and the decision to undergo univentricular or biventricular repair. Patients with a severely dysplastic tricuspid valve are less suited to a cone repair alone and require leaflet reconstruction. Currently used patch materials include autologous untreated pericardium and treated pericardium, with limitations such as unpredictable shrinkage and no growth potential, respectively. To ove

Changing Course-Starnes Takedown in High-Risk Children and Review of the Current Literature.

Neonates with Ebstein anomaly or severe tricuspid valve dysplasia represent a high-risk patient cohort. The modified Starnes procedure was historically performed for cases incompatible with biventricular (BV) repair, leading them towards single-ventricle palliation. However, in recent years, a paradigm shift has emerged in which the Starnes procedure in the acute setting can provide a means for right ventricular (RV) rehabilitation, by providing relief of RV volume overload and time for recovery of RV function, thus serving as a bridge to subsequent one and a half ventricle or BV repair. Herei
