Pseudo-differential operators and generalized random fields over tori.

Starting from a synthetic sample with composition Al2(SO4)3·16.6H2O, the high-temperature- and moisture-dependent behavior of alunogen has been unraveled by TGA measurements, in situ powder X-ray diffraction as well as by gravimetric moisture sorption/desorption studies. Heating experiments using the different techniques show that alunogen undergoes a first dehydration process already starting at temperatures slightly above 40 °C. The crystalline product of the temperature-induced dehydration corresponds to the synthetic equivalent of meta-alunogen and has the following chemical composition: A

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

A predictive index for the need for neonatal surgical intervention in severe Ebstein's disease.

Severe Ebstein's disease often necessitates early neonatal surgical intervention, although, in some patients, surgery may be delayed until infancy or later, and medical management may be undertaken instead. Various indicators, such as tricuspid regurgitation flow velocity, have been studied to predict single or biventricular circulation; however, indicators predicting the need for neonatal surgery have not been addressed. We aimed to identify predictive indices for neonatal surgical necessity by analysing the clinical characteristics of those requiring and not requiring neonatal surgery. We en

Decision-Making Patterns for Clinically Node-Positive, Hormone Receptor-Positive, HER2-Negative Early Breast Cancer: Insights From a Survey of Breast Cancer Experts.

PURPOSE: In hormone receptor (HR)-positive, human epidermal growth factor receptor 2-negative (HER2-) breast cancer, the omission of chemotherapy has become a viable option for select patients. However, when lymph node (LN) metastasis is suspected at diagnosis, the optimal initial treatment strategy (upfront surgery, neoadjuvant chemotherapy, or genomic testing) remains controversial. This study aimed to assess the preferences of multinational breast cancer specialists and to identify the factors influencing clinical decisions in this setting. METHODS: A cross-sectional survey was conducted a

Right Ventricular Remodeling in Complex Congenital Heart Disease.

In congenital heart diseases (CHDs) of moderate to great complexity involving the right ventricle (RV), the morphologic RV can be exposed to significant stressors across the lifespan, either in a biventricular circulation in a subpulmonary or subaortic position or as part of a univentricular circulation. These include pressure and/or volume overload, hypoxia, ischemia, and periprocedural surgical stress, leading to remodeling, maladaptation, dilation, hypertrophy, and dysfunction. In this review we examine the macroscopic remodeling of the RV in various forms of CHD and explore remodeling traj

Case report: Slow conducting accessory pathway in an Ebstein's anomaly patient: Is this a Mahaim fiber?

We report a case of multiple accessory pathways (APs) in an Ebstein's anomaly patient with a history of prior ablation. Electrophysiological study demonstrated a slow conducting AP which pre-excited ventricle only during atrial tachycardia or rapid atrial pacing. The non-decremental property and a longstanding fragment potential at the ablation target site rule out Mahaim fiber. The slow conducting AP might be caused by inadequate prior ablation, and highlights that delta wave absent and prolonged AV interval in target point could not definitely indicate anterograde conduction block when we ab

Left Bundle Branch Pacing in a Pediatric Patient With Tricuspid Valve Replacement for a Complex Ebstein Anomaly.

This paper presents a unique case of successful implementation of left bundle branch pacing (LBBP) in a pediatric patient with third-degree atrioventricular block following tricuspid valve replacement for a complex Ebstein anomaly. The procedure was performed under real-time recording technique, and resulted in the resolution of the patient's symptoms. This case underscores the feasibility and potential benefits of LBBP in pediatric patients, particularly in those with complex congenital heart conditions and following valve replacement surgery. The successful implementation of LBBP in this cas

Endomyocardial Fibrosis Associated With Myocardial Infarction.

Endomyocardial fibrosis (EMF) is the most common cause of restrictive cardiomyopathy in Africa and South America. We present a case of a 55-year-old man who experienced an anterior myocardial infarction due to thrombotic occlusion of the left anterior descending artery. Multimodality imaging revealed left ventricular (LV) apical obliteration, apical thrombus, and subendocardial fibrosis, without significant systolic dysfunction, consistent with EMF. EMF should be suspected in patients presenting with heart failure symptoms because of restrictive physiology and/or thrombotic manifestations with

Favourable outcome of Fusarium prosthetic valve endocarditis in a patient with an Ebstein anomaly.

UNLABELLED: Fungal endocarditis is rare, challenging to diagnose and treat and associated with high mortality. We describe a case of Fusarium solani endocarditis in a patient with Ebstein's anomaly with a favourable outcome. CONTRIBUTION: We emphasise that surgical intervention with source control of the infective vegetation is still the mainstay of treatment and highlight the importance of submitting infected source material for histology, culture and molecular testing to identify the causative organism.

Hemodynamic considerations for prenatal treatment of severe Ebstein anomaly with circular shunt based on two cases and review of the literature.

Prenatal severe Ebstein anomaly might be complicated by a circular shunt. In these cases, persistently elevated right atrial and venous pressure (due to severe tricuspid regurgitation) is complicated by a systemic ineffective blood shunt via a DA, resulting in diminished end-organ perfusion and acidosis, due to overall low cardiac output. Affected fetuses are at a significantly higher risk of intrauterine fetal demise. Reduction of ductal flow by prenatal treatment with nonsteroidal anti-inflammatory drugs has recently been described as a potential treatment option. However, published data are

Pathophysiology and management of adults with complex congenital heart disease after biventricular repair.

Surgical management of congenital heart disease encompasses a spectrum of procedures, ranging from biventricular repair to univentricular palliation, each tailored to the specific anatomical and hemodynamic features of individual cases. Among these, biventricular repair, which preserves a functional ventricle to sustain pulmonary circulation, is prioritized whenever feasible. Advances in approaches have significantly improved outcomes, enabling many patients with congenital heart disease to reach adulthood, including the majority who have undergone biventricular repair. Despite these advanceme

Comparative efficacy and safety of alternatives to sodium valproate in the management of bipolar affective disorder in people of child-bearing age: a narrative review by the European Society of Clinical Pharmacy's mental health specialist interest group.

BACKGROUND: The European Medicines Agency has recommended a series of restrictions on the use of sodium valproate (valproate) following research linking its exposure in utero to adverse congenital and neurodevelopmental effects in offspring. Recent research has highlighted a potential increased risk of neurodevelopmental disorders in children born to males taking valproate prior to conception. Clinicians and patients require guidance regarding suitable alternatives. AIM: To provide an overview of suitable alternatives to valproate in the management of bipolar disorder. METHOD: A narrative re

Successful Kidney Transplantation for Bilateral Renal Hypoplasia With Ebstein Disease.

BACKGROUND: Patients with renal hypoplasia are often associated with congenital heart disease (CHD). During the perioperative period of kidney transplantation (KTx), sufficient circulation volume is required to maintain renal blood flow in the donor kidney. However, little is known about the indication and management of KTx in patients with CHD who require precise hemodynamic assessment during transplantation. We report a pediatric case of successful living-donor KTx for renal hypoplasia with unrepaired Ebstein disease. METHODS: To ensure adequate blood flow to the donor kidney during the per
