Exercise Performance and Right Ventricular Remodeling After the Cone Repair in Patients With Different Severity of Ebstein Anomaly.

BACKGROUND: Although the cone repair has shown good outcome regarding valve competence, its effect on exercise performance and right ventricular remodeling are unclear. METHODS: Between January 2010 and June 2021, 60 patients underwent a cone repair at our center. We excluded patients with previous operation (n = 6), age <5 years (n = 3), in-hospital death (n = 2), replacement before discharge (n = 1), and missing follow-up (n = 2), resulting in a study population of 46 patients. Median age was 27 years (interquartile range, 5.6-68.2 years), and 28 (61%) were female. Patients were divided bas

Practical approach to using cardiac magnetic resonance imaging for pre-surgical planning in complex paediatric congenital heart disease.

Decision-making in complex congenital heart disease (CHD) is challenging and requires the integration of anatomic and physiological data. Recent advances in cross-sectional imaging, particularly cardiac magnetic resonance imaging (MRI), have refined this process. In addition to anatomic detail, MRI provides quantitative physiological data on cardiac function and flows through volumetry and phase contrast assessment. This review outlines the current scope for cardiac MRI and aims to provide a practical framework for using the data in four structural anomalies: borderline left ventricle, double

Cone reconstruction after carpentier repair in ebstein anomaly: yes we can!

The management of recurrent tricuspid regurgitation (TR) after initial tricuspid valve (TV) repair in patients with Ebstein's anomaly (EA) represents a complex challenge. Traditionally, tricuspid valve replacement has been the first-line option for these patients. The use of Cone Repair for recurrent TR remains under-reported. Herein, we report a successful cone repair for recurrent TR, twelve years after a Carpentier repair in a 39-year-old male patient.

Surgical trends and outcomes of neonatal Ebstein's anomaly: Multi-institutional study.

OBJECTIVE: Ebstein's anomaly is a rare condition that can be diagnosed at any age. Symptomatic neonatal Ebstein's anomaly represents a challenging clinical entity with high morbidity and mortality. This study aims to describe treatment modalities and outcomes in neonatal Ebstein's anomaly. METHODS: We performed a review of the Pediatric Health Information System from 2004 to 2023. Patients with an Ebstein's anomaly diagnosis who were admitted within the first 2 days of life and who underwent a surgical intervention during that admission were included. RESULTS: A total of 330 patients were id

Lithium-induced Ebstein's Anomaly.

Ebstein's anomaly (EA) is a rare congenital cardiac defect that occurs when the tricuspid valve leaflets fail to delaminate from the ventricular myocardium, they adhere to the underlying myocardium. Multiple etiologies such as genetic mutation in NKX2-5, family history of congenital cardiac anomalies, and exposure to teratogen such as lithium are associated with this rare cardiac anomaly. In our case, we present an undiagnosed adolescent male with EA who was exposed to lithium during organogenesis.

Unraveling septal perfusion abnormalities in Ebstein's anomaly: A case report contrasting PET MPI with myocardial reality.

In Ebstein's anomaly, septal perfusion abnormality poses a challenge in traditional interpretations of myocardial blood flow. We report a single case of a 43-year-old female with known Ebstein's anomaly who was referred for preoperative cardiac evaluation prior to bariatric surgery. Due to her multiple cardiovascular risk factors and vague chest discomfort, positron emission tomography (PET) myocardial perfusion imaging (MPI) and cardiac magnetic resonance imaging (MRI) were performed. PET MPI demonstrated a partially reversible septal perfusion defect involving 30% of the left ventricular myo

Biventricular remodeling after cone repair in Ebstein anomaly: magnetic resonance imaging data analysis.

BACKGROUND: Various surgical techniques have been reported for repairing Ebstein anomaly. Cone repair provides nearly anatomical tricuspid valve (TV) reconstruction with promising outcomes. We reviewed our experience with cone repair to evaluate biventricular remodeling and the outcomes of the annular support procedure. METHODS: Between January 2008 and December 2021, cone repair was performed in 33 consecutive patients with Ebstein anomaly. Mean age was 32.0±16.8 years (range, 1.1-66.8 years). Previous TV repair had been performed in two patients with the Hetzer and the Carpentier techniques

Atriofascicular pathway in Ebstein's anomaly: Revisiting the role of preoperative EP studies in pediatrics.

We present a case of a 6-year-old with Ebstein's anomaly (EA) who was found to have a right lateral decremental, antegrade only accessory pathway (AP), presumed atriofascicular AP, during a pre-operative electrophysiology study (EPS) prior to da Silva Cone repair (CR). Atriofascicular APs represent <3 % of APs but have an association with EA. To the best of our knowledge, this is the youngest reported case of EA with an atriofascicular AP. Providers should be aware of this possibility, as CRs are being done at younger ages, and they may be discovered in asymptomatic patients.

CARTO UNIVU-Guided Micra Implantation in Ebstein Anomaly.

BACKGROUND: Ebstein anomaly (EA) is a rare congenital heart defect characterized by tricuspid valve malformation and severe tricuspid regurgitation. This poses challenges for conventional pacemaker implantation. CASE SUMMARY: An 86-year-old man with EA, atrial fibrillation, complete atrioventricular block, and heart failure underwent Micra VR leadless pacemaker implantation. The right ventricular angiography was inadequate due to torrential tricuspid regurgitation, necessitating the use of advanced mapping technologies. CARTO electroanatomical mapping and CARTO UNIVU navigation systems facili

Successful management of critical neonatal Ebstein's anomaly with circular shunt.

The prognosis of neonatal Ebstein's anomaly requiring early surgery is poor, especially with a circular shunt. We report a critically ill case managed with emergency two-stage Starnes surgery immediately after birth. We hypothesised that the three components of the Starnes procedure, right ventricle exclusion, atrial septal defect creation, and systemic-to-pulmonary shunt, could be organised and reconstructed in a consecutive two-stage strategy to control postoperative circulatory fluctuations.

Mid-Term Recovery of Right Ventricular Function and Improvement of Left Ventricular Function After Da Silva Cone Procedure for Ebstein Anomaly.

BACKGROUND: The Da Silva Cone procedure for Ebstein anomaly has dramatically improved tricuspid valve competence and clinical outcomes. However, preoperative left ventricular (LV) dysfunction and immediate postoperative right ventricular (RV) systolic dysfunction are frequently observed. While excellent valve outcomes are well established, recovery of biventricular function following the Cone remains less defined. This study aimed to evaluate longitudinal changes in RV and LV function postoperatively and over a minimum of six months post-Cone operation. METHODS: A single center retrospective

Ebstein anomaly update: the evolution of care and the revolution of surgical strategy.

INTRODUCTION: Ebstein anomaly (EA) is a rare congenital heart defect with a broad spectrum of severity - both anatomically and clinically. Treatment options have evolved and hence the rationale for this review. AREAS COVERED: This manuscript reviews all of the current literature including the recent expert consensus document that describes the updated diagnostic and treatment strategies. A citation search in PubMed, Embase, Scopus, and Web of Science was performed using key words related to EA. The search was restricted to the English language and the years 1990 to present. EXPERT OPINION: T

Interventional Partial Cavopulmonary Connection: Initial Clinical Experience With Acute Procedural up to Midterm Results.

BACKGROUND: Creation of a partial cavopulmonary anasthomosis in patients with isolated right-sided heart failure or patients with cyanosis and intracardial right-to-left shunt and reduced blood flow in the pulmonary circulation may provide a significant improvement in exercise capacity and also facilitate the performance of activities of daily living in patients with a deteriorated clinical condition. However, surgical partial cavopulmonary connection creation may be a high-risk procedure in this patient group. An interventional partial cavopulmonary connection may provide a less invasive trea
