Utilization of Venovenous Extracorporeal Membrane Oxygenation as Bridge to Heart Transplantation in a Child With Congenital Heart Defect.

Children with end-stage heart failure, waiting for heart transplantation, have limited bridging options. Unconventional strategies are often utilized in children with congenital heart disease. We report the case of a 12-year-old boy with Ebstein anomaly whose clinical course was complicated by valve thrombosis, right ventricular dysfunction, and intracranial bleed. Venovenous extracorporeal mechanical oxygenation was successfully used as bridge to transplantation, where other strategies were too precarious. Judicious use of venovenous extracorporeal mechanical oxygenation may be used as a brid

Maternal Cardiac and Perinatal Outcomes Among Pregnant Individuals With Ebstein's Anomaly.

Ebstein's anomaly is a rare congenital cardiac condition with limited data regarding the impact of this condition in pregnancy. The objective of this study was to describe the cardiac and perinatal outcomes associated with Ebstein's anomaly in pregnancy. We performed a retrospective cohort of pregnancies from Epic Cosmos, an integrated dataset from over 1,500 hospitals across the United States. Charts were queried for the coexistence of pregnancy and Ebstein's anomaly diagnosis code between July 5, 2021, and March 5, 2025. They were then compared to patients with other congenital heart disease

Perioperative management of a woman with Ebstein's anomaly undergoing caesarean section.

Ebstein's anomaly is a rare congenital heart anomaly which poses significant perioperative challenges, particularly in pregnant patients undergoing caesarean section. The condition is marked by apical displacement of the tricuspid valve, causing right atrial enlargement, severe tricuspid regurgitation and an increased risk of arrhythmias and right-sided heart failure. We describe a case of a woman in her 20s with Ebstein's anomaly and severe tricuspid regurgitation who underwent an elective lower-segment caesarean section under epidural anaesthesia. Intraoperative management focused on maintain

Prenatally diagnosed chromosome 1p36 deletions: a retrospective case series, literature review, and genotype-phenotype correlations.

BACKGROUND: Chromosome 1p36 deletion syndrome is the most common terminal autosomal deletion disorder. While postnatal features are well-defined, prenatal characterization remains less comprehensive. This study aims to delineate the prenatal sonographic and genetic spectrum of this syndrome and explore genotype-phenotype correlations. METHODS: A retrospective analysis was conducted on 21 consecutive prenatally diagnosed cases of 1p36 deletion (2017–2025) from a single tertiary center. Data on maternal demographics, ultrasound findings, genetic results, inheritance patterns, and pregnancy outco

Surgically Treated Giant Right Atrium in Isolated Severe Tricuspid Regurgitation.

Severe tricuspid regurgitation (TR) can lead to significant enlargement of the right atrium (RA) and poses unique clinical challenges. We report this case of a 17-year-old boy previously misdiagnosed with Ebstein anomaly who presented with dyspnea and palpitations. Initial examination revealed irregular heart rhythm, distended neck veins, and a significant murmur. Echocardiogram and computed tomography showed severe TR due to noncoaptation of the tricuspid valve, causing massive RA enlargement (1916 mL). The patient underwent successful surgical intervention, including tricuspid valve replacem

Secondary hyperparathyroidism in adult congenital heart disease: the CHD-HYPER-study.

AIMS: In non-congenital heart disease, secondary hyperparathyroidism (sHPT) is associated with an elevated risk of new-onset heart failure (HF) and an increased incidence of HF-related hospitalizations. Yet, for adults with congenital heart disease (ACHD), the role of sHPT and the factors contributing to its development remain poorly understood. METHODS AND RESULTS: This cross-sectional, single-centre study assessed the prevalence of sHPT in 754 patients with ACHD. Independent predictors of sHPT were identified in both, within the entire cohort and specifically in ACHD with biventricular phys

Primary Tricuspid Regurgitation: From Neglect to Clinical Relevance.

Primary tricuspid regurgitation (TR) is an underrecognized valve disease characterized by structural abnormalities of the tricuspid valve (TV) apparatus, including leaflet prolapse, flail, rheumatic degeneration, carcinoid involvement and congenital malformations such as Ebstein's anomaly. Historically neglected and often misclassified as functional, primary TR has recently gained attention due to advances in multimodality imaging and increased awareness of its pathophysiological complexity and adverse outcomes. A major challenge that remains is the accurate diagnosis of primary TR, as well as

Hypoplastic right heart with heterotaxy has worse five-year transplant-free survival than the hypoplastic left heart syndrome: a thirteen-year single-centre experience.

BACKGROUND: Outcomes of single ventricle heart defects may be influenced by the morphological type of the hypoplastic ventricle. Recent multi-centre studies have targeted on outcomes of hypoplastic left heart syndrome with limited focus on outcomes of hypoplastic right heart syndrome. We aimed at studying the clinical outcomes of hypoplastic right heart syndrome in the recent era. METHOD: We performed a retrospective analysis of all hypoplastic right heart syndrome patients (n = 153) born between January 2010 and January 2023. Five-year transplant-free survival was compared with hypoplastic l

Anesthetic Management and Outcomes of Pregnant Women With Ebstein's Anomaly: Prospective Report.

Fetomaternal outcomes and anesthetic management of pregnancy have rarely been described in a prospective cohort of women with Ebstein's anomaly. Therefore, due to the scarcity of data, diagnostic approaches and obstetric and anesthetic management plans are still evolving. In this prospective observational study, we describe the anesthetic management and fetomaternal outcomes of seven women with different grades of unrepaired Ebstein's anomaly. All women were in NYHA (New York Heart Association) Class II, none had cyanosis or cardiac decompensation at the time of admission during pregnancy. Pre

Thyroid storm in a neonate and young infant: A rare but lethal cause of heart failure.

Neonatal heart failure has varied causes, namely left-sided obstructive lesions such as aortic stenosis, hypoplastic left heart syndrome, and coarctation of the aorta, as well as conditions such as Ebstein's anomaly, supraventricular tachycardia, atrioventricular regurgitation, and large posttricuspid shunts. However, neonatal thyrotoxicosis is a rare but potentially fatal cause of heart failure if left untreated. We present two cases of neonatal thyroid storm manifesting as heart failure and cardiogenic shock, emphasizing the diagnostic challenges, treatment strategies, and transient nature o

Study on the Correlation Between Ventricular Function Evaluated by Z-Score of Atrioventricular Annular Plane Systolic Excursion and Pulmonary Artery Abnormality and Retrograde Ductus Arteriosus Flow in Fetuses With Ebstein Anomaly.

PURPOSE: Ebstein anomaly (EA) is a congenital heart disease that can cause intrauterine fetal death. Pulmonary valve stenosis or atresia (PS/PA) and retrograde ductus arteriosus flow (RDAF) are important factors leading to a poor prognosis, but the causes of its formation are still unclear. This study aimed to assess the correlation between left and right ventricular function and PS/PA and RDAF in fetuses with EA using MAPSE and TAPSE Z-score values measured by free angle M-mode (FAM) echocardiography. METHODS: This retrospective cohort study of 34 EA and 68 normal fetuses was enrolled. Diffe

Catheter ablation of macro-reentrant atrial tachycardia in adults with Ebstein anomaly: A narrative review.

Improving survival rates of patients with Ebstein anomaly have increased the relevance of catheter ablation of macro-reentrant atrial tachycardia (MRAT). Our objectives were, first, to present a case report and, second, to provide a comprehensive review. A search of articles providing detailed information on adults with Ebstein anomaly who underwent catheter ablation of MRAT was performed. PubMed was queried using "Ebstein" and "ablation" as Medical Subject Headings terms, followed by a review of references and citations. Of the 56 patients included in the final review, which included the case

Ebstein Anomaly and the Range of Right-Sided Heart Defects.

Ebstein anomaly (EA) is a rare congenital heart defect characterized by an abnormal development of the tricuspid valve (TV) and ventricular myocardium. The TV is displaced downward into the right ventricle (RV), leading to an atrialized basal portion of the RV above the closure plane of the TV and an often hypoplastic functional RV. Common consequences include cyanosis, TV regurgitation, arrhythmias, and heart failure. Although primarily a valve defect, EA can be regarded as a cardiomyopathy as well. Coexistence of EA with other cardiomyopathies, including left ventricular non-compaction (LVNC

Presentation of Asymptomatic Ebstein's Anomaly: A Case Report.

This report describes the case of a 58-year-old man with Ebstein's anomaly (EA) who remained asymptomatic until 55 years of age when he began experiencing acute dyspnea, palpitations, dizziness, and fatigue during exercise. Patients with EA have an increased risk for arrhythmia, right-sided heart failure, and sudden cardiac arrest. This case report highlights the late-onset complications of EA in a previously asymptomatic adult. While this case aligns with the typical collection of symptoms seen in EA, it is unique in that the complications of EA did not develop until the patient was an older
