Targeting Interleukin-8-Mediated Cellular Crosstalk Reverses Hypertrophic Cardiomyopathy and Cardiac Fibrosis in Noonan Syndrome.

BACKGROUND: Genetic variants in components or regulators of the RAS-mitogen-activated protein kinase (RAS-MAPK) signaling pathway are causative for severe and early-onset hypertrophic cardiomyopathy (HCM) in patients with Noonan syndrome (NS). Despite paracrine communication being considered to play a pivotal role in the etiology of cardiomyopathies, there is a paucity of knowledge about the underlying pathomechanism that leads to the development of HCM and cardiac fibrosis in NS. METHODS: To dissect the impact of noncardiomyocytes in the development of NS, we employed two-dimensional and thr

Novel Lung Perfusion Technique Quantifying Preferential Pulmonary Blood Flow in Fontan Patient With Scimitar Syndrome.

BACKGROUND: Patients with Fontan physiology have increased pulmonary perfusion heterogeneity compared with patients with normal hearts. CASE SUMMARY: A 9-year-old girl with hypoplastic left heart syndrome, Scimitar syndrome, and unilateral right pulmonary vein stenosis underwent stent placement to relieve compression of the reimplanted right pulmonary vein after Fontan completion. Serial upper extremity lung perfusion scans demonstrated decreased right lung pulmonary blood flow despite cardiac catheterization demonstrating no in-stent stenosis and normal Fontan pressures. Pulmonary blood flow

Computed tomography angiography of congenital left-sided obstructive heart lesions: a pictorial essay.

Congenital left-sided obstructive heart lesions (LSOLs) are a group of rare but serious congenital heart diseases. They encompass a spectrum of structural abnormalities that can obstruct blood flow at any point from the left atrium to the aortic arch. They are broadly divided into two main categories. The first is left ventricular inflow obstruction, including cor triatriatum sinister and congenital mitral stenosis. The second is left ventricular outflow obstruction, including subvalvular aortic stenosis, bicuspid aortic valve, supravalvular aortic stenosis, coarctation of the aorta, aortic ar

Pulmonary venous Doppler as a marker of restrictive atrial physiology in fetal critical aortic stenosis after fetal valvuloplasty.

Fetal balloon aortic valvuloplasty (fBAV) is performed in carefully selected fetuses with critical aortic stenosis to prevent progression to hypoplastic left heart syndrome. We report a fetus with critical aortic stenosis who underwent successful fBAV at 26 weeks' gestation with restoration of antegrade aortic flow. Serial fetal echocardiography subsequently demonstrated progressive restriction and eventual functional closure of the foramen ovale despite technically successful valvuloplasty. Pulmonary venous Doppler revealed a prominent atrial reversal wave with prolonged reversal duration, wh

Outcomes in infants with restrictive atrial septum and pre-stage 1 palliation septal intervention.

BACKGROUND: Infants diagnosed with hypoplastic left heart syndrome (HLHS) who have a restrictive or intact atrial septum (RAS) have worse outcomes in single center analyses. Multi-center studies are lacking, and optimal identification and management of atrial restriction remains unclear. METHODS: Multicenter retrospective cohort study of infants undergoing stage 1 palliation (S1P) from 2016 to 2024 and included in the National Pediatric Cardiology Quality Improvement Collaborative (NPC-QIC). Infants were classified based on atrial septal anatomy and intervention as follows: RAS with pre-S1P i

Unicuspid Aortic Valve Presenting With Severe Aortic Regurgitation and Successfully Repaired by Bicuspidization.

BACKGROUND: Unicuspid aortic valve (UAV) is a rare congenital anomaly and is often misdiagnosed as a bicuspid aortic valve (BAV). It typically presents with aortic stenosis rather than regurgitation. CASE SUMMARY: A 21-year-old man presented with exertional dyspnea and was diagnosed with severe aortic regurgitation (AR) and aortic root dilation. He had been followed as having BAV since childhood. However, transesophageal echocardiography revealed a unicommissural UAV. The patient underwent valve-sparing aortic root replacement using the reimplantation technique combined with bicuspidization.

Decoding Long Noncoding RNAs in Cardiac Development and Congenital Heart Disease: Recent Advances and Clinical Perspectives.

Long noncoding RNAs (lncRNAs) have emerged as crucial regulators in cardiac development and congenital heart disease. With advances in high-throughput sequencing, single-cell omics, and epigenetic profiling, accumulating evidence suggests that lncRNAs participate in transcriptional, posttranscriptional, and structural remodeling processes underlying cardiac morphogenesis. This in-depth review summarizes recent progress in elucidating the roles of lncRNAs in heart development and congenital heart disease pathogenesis. Specifically, we discuss how lncRNAs modulate key developmental signaling pat

Evidence-based treatment strategies for hypoplastic left heart syndrome: The role of EFNA1/EphA.

Cardiovascular health among the US general population remains largely suboptimal. Major contributing factors include diet, physical activity, and sleep, and these influences begin early in life, despite broad access to health information regarding healthy choices and access to medical care. Advances in perinatal care allow earlier detection of both maternal and congenital conditions, enabling interventions and planning that can improve outcomes. Research exploring the foundations of hypoplastic left heart syndrome (HLHS) has significantly expanded in the last 25 years. HLHS represents a protot

A rare conotruncal anomaly: absent pulmonary valve syndrome, interrupted aortic arch, and severe aortic stenosis.

The coexistence of absent pulmonary valve syndrome, Type B interrupted aortic arch, and severe aortic stenosis represents an exceptionally rare and complex congenital cardiac phenotype. The diagnosis was established in a newborn using multimodality imaging. Given the profound airway compression secondary to the massive pulmonary artery dilatation and the associated poor prognosis, palliative care was chosen.

Congenital heart disease in England: a national cohort study from fetal diagnosis to end of infancy.

BACKGROUND: Population studies of congenital heart disease (CHD) often include only children receiving cardiac interventions, underestimating the burden of cases without intervention. We evaluated outcomes for all detected structural CHD cases in England from fetal life to the age of 1 year. METHOD: We linked the National Congenital Anomaly and Rare Disease Registration Service, the National Congenital Heart Disease Audit, and Office for National Statistics mortality records to construct an incident cohort with estimated delivery/birth dates 2018-2020. Outcomes were: termination of pregnancy,

Hypertrophic obstructive cardiomyopathy with associated pulmonary arteriopathy: A histopathologic perspective from sudden cardiac death.

Hypertrophic obstructive cardiomyopathy (HOCM) is one of the common causes of sudden cardiac death which can be associated with pulmonary vascular remodelling changes also known as Pulmonary Arteriopathy (PA). Pulmonary Arteriopathy (PA) is a condition affecting the pulmonary arteries, characterised by a range of proliferative and obstructive vascular lesions that ultimately result in lumen obstruction. It may manifest under diverse settings, encompassing both acquired and congenital disorders. We present a case of a 52-year male with no previous known history of illness or disease, who was br

Review of Management of the Borderline Left Ventricle: A Focus on Current Transcatheter and Hybrid Approaches.

PURPOSE OF REVIEW: A borderline small left ventricle (LV), in which the LV is underdeveloped but not diminutive, represents an ongoing area of research and a challenge in clinical management. In this paper, the authors aimed to review the definition of borderline LV and recruitment strategies with a focus on hybrid, and wholly transcatheter approaches, to achieve the goal of two ventricle circulation. RECENT FINDINGS: Hampered by a lack of consensus definition; historically, patients with borderline LV were managed with single-ventricle palliation. However, due to the suboptimal long-term out

The first reported coexistence of hepatopulmonary fusion and surgically palliated hypoplastic left heart syndrome.

Hepatopulmonary fusion is a rare defect describing the physical connection of liver and lung. A neonate with hypoplastic left heart syndrome arrived at the hospital with concern for congenital diaphragmatic hernia, later found to have hepatopulmonary fusion. In the coming months, he underwent a series of operations, including ligation of the hepatopulmonary fusion and the first two stages of surgical palliation of hypoplastic left heart syndrome. He ultimately died of septic shock at 15 months of age.

Influence of atrial septal defect on mitral valve growth after repair of coarctation of the aorta or an interrupted aortic arch in infants.

BACKGROUND: Patients with coarctation of the aorta (CoA) and an interrupted aortic arch (IAA) may present with small mitral valves (MVs) and a reduced left ventricular (LV) volume. Biventricular repair (BVR) in these patients is dependent on adequate size of the left cardiac structures. PURPOSE: This study evaluated the impact of the hemodynamic characteristics of atrial septal defects (ASDs) on MV growth following surgical repair. METHODS: We retrospectively reviewed the data of patients diagnosed with CoA or IAA between 2007 and 2024. The z score for MV size measured 6 months postoperative

Determination of cardiac output, shunt-fraction, and active circulatory volume in children with hypoplastic left heart syndrome after the Norwood procedure with RV to PA-shunt.

Management of neonates with hypoplastic left heart syndrome following the Norwood procedure seeks to optimize systemic oxygen delivery while maintaining an appropriate distribution between systemic and pulmonary blood flow. The objective of this study was to quantify actively circulating blood volume, total cardiac output, the pulmonary-to-systemic blood flow ratio (Qp/Qs), and vascular resistance indices at fractions of inspired oxygen (FiO2) of 0.21, 0.5, and 0.9 during the early postoperative period. Measurements were obtained in 16 neonates using an ultrasound dilution technique that deter

Augmenting Venous Return in Fontan Circulation: The Role of the Skeletal Muscle Pump and Implications for Mechanical Compression Devices-A Review of the Literature.

Post-Fontan patients often face significant hemodynamic challenges, including systolic dysfunction and diminished cardiac output due to reduced preload and venous return. Long-term complications such as hepatic dysfunction and protein-losing enteropathy arise from poor venous return, increasing morbidity and mortality. Enhancing venous return could potentially improve long-term outcomes for these patients. This literature review examines the role of the skeletal muscle pump in Fontan circulation and evaluates non-pharmacological strategies to augment venous return, with discussion of mechanica
