Unplanned Readmissions due to Pleural Effusion Following the Fontan Operation.

BackgroundThe Fontan operation is the final stage of palliation for single-ventricle patients. While operative survival has improved, pleural effusions remain a common postoperative complication, increasing morbidity and hospitalization. Our aim was to investigate the incidence and risk factors for unplanned readmissions due to post-Fontan pleural effusion.MethodsA retrospective review of all single ventricle patients who underwent the Fontan procedure from 2012-2023 was performed. This case-control study compared post-Fontan patients readmitted to the hospital due to pleural effusion with tho

Sex-specific outcomes after transcatheter or surgical treatment of aortic valve stenosis: the DEDICATE-DZHK6 trial.

BACKGROUND AND AIMS: Aortic stenosis may be managed differently in women and men, but evidence remains limited. Sex-specific characteristics and outcomes of low- to intermediate-risk patients assigned to transcatheter aortic valve implantation (TAVI) or surgical aortic valve replacement (SAVR) from the DEDICATE-DZHK6 trial are described. METHODS: The DEDICATE-DZHK6 trial demonstrated non-inferiority for the primary outcome of all-cause death or stroke at 1 year. Sex-specific effects were evaluated in this predefined descriptive subgroup analysis. RESULTS: A total of 1394 patients (43.3% wome

Noninvasive optical monitoring of cerebral hemodynamics immediately after birth in neonates with congenital heart disease.

OBJECTIVE: Critical congenital heart disease is associated with altered cerebral hemodynamics in the neonatal period, but the effect on cerebral physiology earlier in life during the fetal to neonatal transition period is yet to be elucidated. This period of neonatal resuscitation could represent an opportunity for intervention, so we aimed to characterize cerebral hemodynamics immediately after birth in four neonates with critical congenital heart disease using noninvasive optical monitoring. METHODS: This case series analysis included term neonates with hypoplastic left heart syndrome or wi

Quadricuspid Aortic Valve Presenting with Aortic Regurgitation in Adulthood: A Case Report.

Quadricuspid aortic valve (QAV) is a rare congenital cardiac anomaly characterized by four aortic cusps instead of the normal three. It is most commonly associated with progressive aortic regurgitation and is often diagnosed incidentally on echocardiography. A woman in her fifties presented with palpitations and exertional dyspnea (New York Heart Association Class II). Clinical examination revealed an early diastolic murmur at the left sternal border. Transthoracic echocardiography demonstrated a QAV with Hurwitz and Roberts Type F morphology and moderate aortic regurgitation with preserved le

Importance of clinical practice guidelines for specialized delivery room resuscitation of newborns with prenatally diagnosed critical congenital heart disease.

Perinatal transition is uniquely complex in newborns with critical congenital heart disease (CCHD), whose cardiopulmonary physiology often diverges from the standard neonatal resuscitation framework. We developed lesion-specific clinical practice guidelines (CPGs) for delivery room (DR) management of six high-risk CCHD diagnoses - hypoplastic left heart syndrome, dextro- transposition of the great arteries, Ebstein anomaly, congenital complete heart block, total anomalous pulmonary venous return, and tetralogy of Fallot with absent pulmonary valve. Developed collaboratively between neonatologi

Perioperative Care of Children with Congenital and Acquired Heart Disease Undergoing Non-Cardiac Surgical Procedures.

Children with congenital and acquired heart disease are at elevated risk for perioperative morbidity and mortality. These patients have high rates of coexisting conditions that necessitate noncardiac surgical procedures, some of which may occur outside of centers with specific expertise in pediatric cardiac care. The heterogeneity of anatomic and physiologic states in congenital heart disease presents a challenge to clinicians caring for these patients. This article aims to educate the general pediatric practitioner on the cardiac physiology associated with high-risk categories of congenital a

Effect of Valved Versus Non-valved Conduits on Ventricular Size and Function in Stage I Palliation for Hypoplastic Left Heart Syndrome.

Interstage mortality remains high after introduction of right ventricle (RV) to pulmonary artery conduits to stage I palliation for hypoplastic left heart syndrome (HLHS). Prior studies have not shown long-term survival benefit with conduits, likely due to multiple factors including regurgitation contributing to decreased RV function. Valved conduits (VC) were introduced to address these limitations. The study aimed to assess differences in RV size and function using echocardiography during the interstage period between patients undergoing stage I palliation with VC versus non-valved conduits

Association between route of admission and preoperative mortality in patients with duct-dependent congenital heart disease.

BACKGROUND: Duct-dependent congenital heart diseases (CHDs) require prompt postnatal intervention to prevent life-threatening deterioration. While prenatal diagnosis may improve outcomes, the impact of route of admission on preoperative mortality remains unclear in Japan. METHODS: This cross-sectional study examines the impact of the route of admission on preoperative mortality in 2704 admissions of patients with duct-dependent CHD between 2014 and 2019, at hospitals employing the Diagnostic Procedure Combination payment system. The route of admission was categorized as either born in hospita

Hypoplastic Left Heart Syndrome Cardiomyocytes Exhibit Intrinsic Stress Vulnerabilities and Augmented Stress Responses in vitro.

BACKGROUND: Hypoplastic left heart syndrome (HLHS) is a severe congenital heart defect characterised by underdevelopment of left-sided cardiac structures. While genetic predisposition contributes to HLHS, the relevance of environmental stressors is increasingly recognised, yet the cellular mechanisms linking genetic susceptibility to environmental vulnerability remain unclear. We aimed to identify molecular and functional differences between cardiomyocytes derived from HLHS patients and healthy controls to uncover potential susceptibilities contributing to the HLHS phenotype. METHODS: Human i

High rates of congenital airway anomalies in neonates with single ventricle congenital heart disease.

Neonates with single ventricle congenital heart disease (SV CHD) may have noncardiac comorbidities, including congenital airway anomalies (CAA). Using a national administrative database, we analyzed neonatal admissions with ICD-10 codes for SV CHD with and without CAA from October 2020 to December 2024, excluding admissions with genetic syndromes to reduce confounding by the potential contribution of syndromes to CAA. Among 5144 neonatal admissions with SV CHD, 535 (10.4%) had CAA, compared to 23,837 (0.4%) of 5,942,621 neonatal admissions without CHD (odds ratio [OR] [95% confidence interval]

Fetal Intervention Using an Everolimus-Eluting Stent in HLHS With Intact Atrial Septum.

BACKGROUND: In utero stenting of the atrial septum has been introduced for fetuses with hypoplastic left heart syndrome (HLHS) and intact atrial septum (IAS) to prevent secondary pulmonary vascular damage caused by left atrial hypertension. Previous reports revealed a high risk of significant or complete in-stent obstruction after successful intervention due to endothelial proliferation. CASE SUMMARY: We report a fetus with HLHS and IAS complicated by nutmeg lung who underwent successful fetal atrial septal stenting using an everolimus-eluting stent (Xience Sierra; Abbott). A One-Dimensional (1D) Computational Fluid Dynamics Study of Fontan-Associated Liver Disease (FALD).

Fontan-associated liver disease (FALD) is a disorder arising from hemodynamic changes and venous congestion in the liver. This disease is prominent in patients with hypoplastic left heart syndrome (HLHS). Although HLHS patients typically survive into adulthood, they have reduced cardiac output due to their univentricular physiology (i.e., a Fontan circuit). As a result, they have insufficient blood delivery to the liver. In comparison, patients with double outlet right ventricle (DORV), also having a univentricular circuit, have a lower incidence of FALD. In this study, we use a patient-specif
