[Presystolic increasing maximal flow velocity in the intracranial vessels in 3 children with heart failure].

The doppler-sonographic examination of flow velocities in intracerebral arteries is of central importance in the diagnostic evaluation of potential neurologic complications in intensive care patients. Furthermore pulsed Doppler-sonography can assess the hemodynamic consequences of several congenital heart defects (e.g. the ductus arteriosus Botalli). Characteristics of the normal flow profile in the intracranial arteries are the systolic and diastolic forward flow caused by the "Windkessel" function of the aorta. Well known pathologic flow profiles (systolically and diastolically increased flo

Development and progression of aortic valve stenosis: atherosclerosis risk factors--a causal relationship? A clinical morphologic study.

Hospital records of 120 patients undergoing aortic valve replacement were retrospectively reviewed for risk factors associated with early aortic valve replacement. Patients were separated into four groups (rheumatic, congenital bicuspid, degenerative, and miscellaneous) based upon the morphologic etiology of aortic stenosis. Multiple regression analysis was performed using age at surgery as the dependent variable. Independent variables for the study were race, gender, systemic hypertension, total triglyceride level, total serum cholesterol level, tobacco smoking history, diabetes mellitus, and

Umbilical vein draining into the inferior vena cava via the internal iliac vein, bypassing the liver.

This is the third report of an anomalous umbilical vein draining ectopically in a left pelvic vein, probably the left internal iliac, and through the inferior vena cava into the right atrium. The anomaly was encountered in a newborn infant with nonimmune hydrops fetalis, hypertrophic cardiomyopathy, multiorgan failure and possibly Noonan Syndrome.

Hypoplastic left heart syndrome: more potential transplant recipients than suitable donors.

Neonatal cardiac transplantation offers the prospect of survival for babies with hypoplastic left heart syndrome--but only if suitable donors are available. In a retrospective survey in the Northern health region of England and Wales, the likely need for neonatal cardiac transplantation far outweighed the potential availability of donors. Over 8 years (1983-90) hypoplastic left heart syndrome was identified in 38 newborn babies and in 9 fetuses in utero. Of 41 live births (including 3 diagnosed prenatally) 31 would have been candidates for a cardiac transplant, but only 4 suitable donors could

Congenital aortic regurgitation caused by absence of the right coronary cusp.

We describe a patient with congenital aortic regurgitation, well documented from the age of 1 month. Surgery was performed at the age of 7 years, when echocardiography showed progressive dilatation of the left ventricle with decreasing shortening fraction. Inspection of the valve during the operation showed a basically tricuspid valve, but with a rudimentary right coronary cusp. At histological examination, myxoid dysplasia was found in all the cusps. An aortic homograft was inserted and the left ventricular internal dimensions returned to normal within 3 months after the operation.

Color Doppler flow mapping of fetal heart.

Color Doppler flow mapping of fetal heart was performed in 582 fetuses between 16 and 38 weeks of gestation. Congenital heart diseases were excluded in 522 fetuses correctly. In 59 fetuses structural and/or functional cardiac abnormalities were diagnosed. In one fetus small multiple ventricular septal defects were missed. The most important additional information obtained by color Doppler flow mapping was: (1) Diagnosis of insufficiencies of atrioventricular valves; (2) Demonstration of turbulent high velocity jet in stenosis of semilunar valve; (3) Reverse flow in ascending aorta in atresia o

Surgical treatment of aortico-left ventricular tunnel.

Thirteen children (age range 6 h-11 years, median 4 months) operated upon for aortico-left ventricular tunnel since 1971 are reported. All patients had systolic and diastolic murmurs, 4 of 5 neonates were in severe congestive heart failure. Twelve children had left ventricular hypertrophy on ECG with an ischaemic pattern in the neonates. Chest X-ray revealed cardiomegaly with a broad upper mediastinal shadow from the enlarged aorta. Echocardiography (2DE) revealed a tubular communication between the aorta and the left ventricle bypassing the aortic valve and massive regurgitation through this

Quadricuspid aortic valve. Case report and review of the literature.

Quadricuspid aortic valve is a rare congenital anomaly that may be found unexpectedly at surgery or diagnosed preoperatively by means of echocardiography or aortography. The most common hemodynamic abnormality associated with this anomaly is aortic insufficiency. We describe a representative case involving a 62-year-old man with severe aortic insufficiency and mild aortic stenosis. At operation, the aortic valve was found to have 4 cusps. The valve was replaced with a mechanical prosthesis. The patient was released 7 days after operation and was clinically well at 6-month follow-up.

Aortic-ventricular tunnel in a neonate: diagnosis and management based on cross sectional and colour Doppler ultrasonography.

A five day old symptom free neonate was referred for assessment of a to and fro murmur associated with large volume pulses. Cross sectional echocardiography and colour flow mapping confirmed the diagnosis of an aortic-ventricular tunnel with forward flow into the aorta and regurgitant flow into the ventricle through both the tunnel and the dilated aortic valve ring. Surgical correction by patch closure of the aortic end of the tunnel was successfully undertaken two weeks later without any additional investigations. Postoperative echocardiography and colour flow imaging showed no aortic regurgi

Biplane transesophageal echocardiography: clinical applications of the long-axis plane.

Tomography of the heart and great vessels is limited during conventional transesophageal echocardiography by the inability to rotate the transducer at the end of the transesophageal echocardiographic endoscope. A biplane transesophageal echocardiographic probe has been developed recently with a second transducer capable of long-axis imaging that yields long-axis views in addition to the familiar short-axis views. We describe our experience with 37 patients with a variety of cardiovascular diseases. In 17 patients new diagnostic information was obtained, although management was not changed. Lon

Neonatal Marfan syndrome with congenital arachnodactyly, flexion contractures, and severe cardiac valve insufficiency.

We describe a male neonate with severe arachnodactyly, hypermobility of the fingers, flexion contractures of elbows, wrists, hips, and knees, micrognathia, crumpled ears, rockerbottom feet, loose redundant skin, and ocular abnormalities. Severe cardiac valve insufficiency and aortic dilatation resulted in cardiac failure and death 20 hours after birth. This case represents the severe end of the clinical spectrum of Marfan syndrome. As similar patients have been reported, they may represent a separate mutation.

Transesophageal echocardiography in evaluation and management after a Fontan procedure.

Transesophageal echocardiography was used in 18 patients (aged 1.6 to 34 years, mean age 12.6) to assess the immediate (5 patients) or intermediate (13 patients) results after a Fontan-type procedure. The findings were correlated with precordial echocardiographic (all patients) and cardiac catheterization (11 patients) data. Atrial shunting was documented by transesophageal studies in three patients (precordial in one patient). In two patients it was confirmed by cardiac catheterization; the third underwent reoperation based on the transesophageal study alone. Pulmonary artery obstruction was

Aneurysm of the left sinus of Valsalva causing severe mitral regurgitation.

A 24-year-old male presented with exertional dyspnoea of one year's duration. Echocardiography revealed an unruptured aneurysm of the left sinus of Valsalva distorting the mitral valvar apparatus and producing severe regurgitation in the absence of myocardial ischemia or infarction. We propose an alternate mechanism for the mitral regurgitation.

[Multiple thromboembolism associated with anti-thrombin III deficiency].

Congenital deficiency of antithrombin III is considered as one serious primary hypercoagulable state. The authors present a case of deficiency of antithrombin III in a 28 years old young male, with aortic valve disease and several thromboembolic events. When discussing their diagnostical and therapeutical aspects they call the attention to the need for investigation of the situations in which thrombophilia presents with recurrent venous and arterial thrombosis.

Aortic valve reconstruction in Rubinstein-Taybi-syndrome: the valuable aid of transesophageal echocardiography.

A fourteen year old boy with Rubinstein-Taybi-syndrome presented with a severe congenital subvalvular fibrous aortic stenosis with associated aortic regurgitation. Transthoracic echocardiographic imaging and left heart angiography showed a transvalvular peak systolic pressure gradient of 90 mmHg and a regurgitant fraction of 30%. The surgical treatment consisted of resection of the subvalvular fibrous tissue and subsequent aortic valvuloplasty. Intraoperative transesophageal echocardiography revealed a wide systolic opening of the aortic valve and good coaptation of the aortic valve leaflets i
