From Valve Anatomy to Molecular Trajectories: Integrating Proteomics into Precision Care for Bicuspid Aortic Valve Disease.

Bicuspid aortic valve (BAV) disease is a lifelong disorder in which congenital anatomy, tissue susceptibility, abnormal flow, and acquired fibrocalcific remodeling produce heterogeneous valve and aortic outcomes. This narrative review examined peer-reviewed literature indexed in PubMed/MEDLINE through June 2026 to evaluate how circulating proteomics could complement established imaging-based risk assessment. Published studies of incident aortic stenosis consistently implicate integrated stress, inflammation, apoptosis, and extracellular-matrix remodeling, with recurrent signals including GDF15

Subaortic Stenosis in Four Free-Ranging Eurasian Lynx (Lynx lynx) from the Alpine and Jura Populations in Switzerland.

As part of wildlife conservation efforts, the reintroduced Eurasian lynx (Lynx lynx) populations in Switzerland are the subject of ongoing health surveillance. Cardiac pathologies, such as heart murmurs and histopathologic changes in the cardiac tissue, including intramural cardiac vessels, are frequently observed. Here we describe four cases of subaortic stenosis (SAS), a congenital heart disease rarely reported in felids, in free-ranging adult male lynx from Switzerland. The affected lynx presented macroscopically with grades 1 to 3 of SAS. Histopathology confirmed characteristic subvalvular

Evaluation of Right Ventricular Size Metrics Measured with Cardiac Magnetic Resonance Imaging in Patients with Pulmonary Regurgitation.

In patients with repaired congenital heart disease (CHD) and pulmonary regurgitation (PR), a common metric for guiding the timing of pulmonary valve replacement (PVR) is right ventricular (RV) end-diastolic volume (EDV). The standard practices of indexing RV EDV to body surface area and calculating RV EDV Z-scores are imperfect, and guidelines for PVR based on threshold values for indexed volumes may be insensitive to individual patient circumstances. Indexing RV EDV to left ventricular EDV is an alternative method of normalizing RV volume in patients with PR. We reviewed clinically obtained c

Two decades of pediatric aortic stenosis: Balloon versus surgical interventions: A single center study.

INTRODUCTION: Congenital valvular aortic stenosis (AS) represents 3%-6% of congenital heart diseases. This study aimed to compare the outcomes of balloon valvuloplasty (BV) and surgical valvotomy (SV) in patients with AS. METHODS: This retrospective study was conducted on 69 patients. A retrospective analysis was done on 69 patients (who underwent BV or SV) at a tertiary heart center in Mashhad, Iran (between 2000 and 2023). RESULTS: The mean age of BV patients (n = 24, 17 males) was 67.08 ± 55.92 months, while it was 97.44 ± 64.18 months in SV patients (n = 45, 33 males). Both treatments re

Risk factors for early reintervention in patients with cryopreserved homografts in the pulmonary position.

INTRODUCTION AND OBJECTIVES: Cryopreserved homograft implantation in the pulmonary position is a common surgical approach for pulmonary valve replacement, although it carries a risk of early failure requiring reintervention. This study aimed to identify clinical, anatomical, surgical, graft, and postoperative factors associated with early failure, defined as reintervention (percutaneous or surgical) within 3 years. METHODS: In this retrospective cohort study (2017-2024), we included 136 patients (median weight 45.0 [IQR: 25.25-60.00] kg; age 14.11 [8.31-18.09] years) who received a homograft

Common Arterial Trunk with Intact Ventricular Septum: Morphologic and Developmental Considerations.

BACKGROUND: It is rare in clinical practice to encounter a common arterial trunk when the ventricular septum is intact. In this setting, other clinical diagnoses, such as hypoplastic left heart syndrome with aortic atresia, may be mistaken for a common arterial trunk. Data for this combination is largely limited to case reports and small case series. We have conducted a systematic review of reported cases, performing cluster analyses to provide an objective grouping of the cases. METHODS: A systematic review of the literature was performed to identify cases of a common arterial trunk with an

Single Ventricle: Radiology Primer for Preprocedural Evaluation.

Single ventricle is an uncommon congenital abnormality characterized by a single anatomic or functional ventricle that cannot sustain two separate serial circulations. Imaging plays an important role in classifying a single ventricle, quantitating ventricular and valvular function, detecting associated cardiovascular anomalies, and identifying critical factors for management. Echocardiography is the first-line imaging modality. MRI and CT are useful when echocardiography is indeterminate or suboptimal, especially for evaluation of vascular anatomy. Morphologically, two atria can connect to a s

Meta-Analysis of TAVR Versus SAVR in Bicuspid Valve: Mortality, Complications, and Long-Term Outcomes.

INTRODUCTION: Bicuspid Aortic Valve (BAV) is the most common congenital heart disease and often causes aortic stenosis in younger adults. There are few prospective randomized trials comparing Transcatheter Aortic Valve Replacement (TAVR) and Surgical Aortic Valve Replacement (SAVR) in BAV patients, since these patients are usually excluded from major studies because of their complex anatomy. A meta-analysis to compare clinical outcomes between TAVR and SAVR in people with BAV was conducted. METHODS: PubMed, Google Scholar, and SCOPUS were searched for studies published up to December 2023 tha

The Fontan Outcomes Network: Findings After 2 Years and 1121 Participants.

BACKGROUND: Survival after Fontan palliation for single ventricle heart disease has improved substantially, yet the long-term trajectory remains poorly defined. The Fontan Outcomes Network, a learning health network of 38 congenital heart centers in the United States and Canada, was established to address this gap. We report baseline characteristics and early findings from the first 1121 participants enrolled in this prospective clinical registry. METHODS: We performed a cross-sectional analysis of individuals who had undergone Fontan palliation enrolled in the Fontan Outcomes Network from Au

Impact of gestational age of onset of fetal atrial restriction on postnatal outcomes in hypoplastic left heart syndrome.

BACKGROUND: Patients with hypoplastic left heart syndrome who develop a restrictive atrial septum prenatally have higher rates of postnatal mortality compared to those without atrial restriction. It is not understood whether the gestational age at which restriction becomes apparent echocardiographically impacts clinical outcomes. METHODS: A single-centre, retrospective cohort study was performed, including 196 patients with hypoplastic left heart syndrome from 2008 to 2022. A restrictive atrial septum was diagnosed based on fetal echocardiographic findings of an intact atrial septum, antegrad

Long-term outcomes of pediatric heart transplantation in single-ventricle heart disease: A single-center analysis.

OBJECTIVE AND AIMS: Pediatric heart transplantation (HTx) for children with single-ventricle (SV) physiology remains a complex and high-risk procedure. This study aimed to retrospectively evaluate long-term outcomes in patients with SV undergoing primary HTx versus HTx after failed staged palliation. METHODS: Between 1991 and 2025, 22 pediatric patients (aged ≤ 18 years) with SV physiology underwent orthotopic HTx. Thirteen received primary HTx (Group 1), and nine had HTx after failed staged palliation (Group 2), including two post-Norwood, three post-Glenn, and four post-Fontan procedures.

Risk factors for complications of necrotizing enterocolitis in term neonates with congenital heart disease at a referral hospital in Colombia: A retrospective cohort study.

INTRODUCTION: Necrotizing enterocolitis (NEC) is one of the most severe complications in neonates and infants. However, term infants with congenital heart disease (CHD) represent a scenario of multiple complications due to pathophysiological mechanisms that differ from the classic pathogenesis. The objective of the study was to determine factors associated with complications of NEC in term infants hospitalized for CHD in a pediatric intensive care unit of a referral hospital in Colombia between 2010 and 2023. MATERIALS AND METHODS: A retrospective cohort study was conducted, including 148 ter

Hypokalemia-Induced Rhabdomyolysis Following Prolonged Diarrhea in a Child With Congenital Heart Disease Receiving Loop Diuretics: A Case Report and Literature Review.

Hypokalemia is a rare but important cause of rhabdomyolysis in children. Unlike typical rhabdomyolysis, hyperkalemia may be absent because the triggering factor is hypokalemia, potentially delaying diagnosis. We report a five-year-old boy with hypoplastic left heart syndrome after a Fontan procedure who developed severe hypokalemia and rhabdomyolysis following prolonged diarrhea while receiving loop diuretics. He presented with acute lower extremity weakness and inability to stand. Laboratory evaluation revealed potassium 1.9 mmol/L and creatine kinase 9,013 U/L, peaking at 32,908 U/L. Urinaly

Risk Factors for Aortopathy in Bicuspid Aortic Valve: Insights from a Large Echocardiographic Cohort.

BACKGROUND: Bicuspid aortic valve (BAV) is the most common congenital heart defect and is frequently associated with thoracic aortopathy. Risk stratification remains challenging, as predictors of aortopathy are incompletely understood and prior studies have been limited by modest sample sizes or incomplete echocardiographic characterization. METHODS: We retrospectively analyzed 1,602 patients diagnosed with BAV. Aortopathy was defined as dilation of the sinus of Valsalva or ascending aorta using guideline-recommended thresholds. Clinical and echocardiographic predictors included age, sex, bod

Computational Modeling of Oxygen Delivery in Norwood Physiology: Differential Effects of Systemic and Pulmonary Vasodilator Conditions.

BACKGROUND: After the Norwood operation, systemic and pulmonary circulations are supplied in parallel by a single right ventricle. Pulmonary blood flow depends on both native pulmonary vascular resistance and shunt/conduit resistance; therefore, balanced Qp:Qs can occur despite low native pulmonary resistance. Changes in systemic, pulmonary, or shunt/conduit resistance may alter flow distribution, systemic venous saturation, and oxygen delivery. OBJECTIVE: To evaluate the predicted hemodynamic and oxygen delivery effects of systemic and pulmonary vasodilator conditions in a computational Norw

Artificial I ntelligence-Assisted Fetal Echocardiography: Improving Early Detection and Neonatal Outcomes of Congenital Heart Disease.

Congenital Heart Disease (CHD) encompasses a range of structural abnormalities of the heart and great vessels present at birth and is associated with significant lifelong morbidity, including heart failure and arrhythmias. Early diagnosis is critical for optimizing perinatal management and improving outcomes. This review focuses on early fetal echocardiography, a specialized ultrasound technique that enables detection of cardiac abnormalities in utero, as early as 11-14 weeks of gestation, compared to traditional imaging performed at approximately 20 weeks. Advancements in early imaging have i

Percutaneous Deployment of the Sinus-SuperFlex-DS Stent for Hybrid Stage I Palliation in Neonates Weighing ≤ 2.5 kg: A Multicenter Study.

BACKGROUND: Hybrid stage I palliation (HS1P) has developed as an alternative to the Norwood stage I palliation for neonates with hypoplastic left heart and related left-sided obstructive lesions. HS1P is currently used in various clinical settings, such as single ventricle palliation, bridge to decision, bridge to biventricular repair, or destination therapy. The procedure comprises bilateral pulmonary artery banding and duct stenting, thus avoiding the need for cardiopulmonary bypass surgery, which may be particularly beneficial in high-risk patients. AIMS: The aim of this multicenter retros
