Targeting Interleukin-8-Mediated Cellular Crosstalk Reverses Hypertrophic Cardiomyopathy and Cardiac Fibrosis in Noonan Syndrome.

BACKGROUND: Genetic variants in components or regulators of the RAS-mitogen-activated protein kinase (RAS-MAPK) signaling pathway are causative for severe and early-onset hypertrophic cardiomyopathy (HCM) in patients with Noonan syndrome (NS). Despite paracrine communication being considered to play a pivotal role in the etiology of cardiomyopathies, there is a paucity of knowledge about the underlying pathomechanism that leads to the development of HCM and cardiac fibrosis in NS. METHODS: To dissect the impact of noncardiomyocytes in the development of NS, we employed two-dimensional and thr

Computed tomography angiography of congenital left-sided obstructive heart lesions: a pictorial essay.

Congenital left-sided obstructive heart lesions (LSOLs) are a group of rare but serious congenital heart diseases. They encompass a spectrum of structural abnormalities that can obstruct blood flow at any point from the left atrium to the aortic arch. They are broadly divided into two main categories. The first is left ventricular inflow obstruction, including cor triatriatum sinister and congenital mitral stenosis. The second is left ventricular outflow obstruction, including subvalvular aortic stenosis, bicuspid aortic valve, supravalvular aortic stenosis, coarctation of the aorta, aortic ar

Unicuspid Aortic Valve Presenting With Severe Aortic Regurgitation and Successfully Repaired by Bicuspidization.

BACKGROUND: Unicuspid aortic valve (UAV) is a rare congenital anomaly and is often misdiagnosed as a bicuspid aortic valve (BAV). It typically presents with aortic stenosis rather than regurgitation. CASE SUMMARY: A 21-year-old man presented with exertional dyspnea and was diagnosed with severe aortic regurgitation (AR) and aortic root dilation. He had been followed as having BAV since childhood. However, transesophageal echocardiography revealed a unicommissural UAV. The patient underwent valve-sparing aortic root replacement using the reimplantation technique combined with bicuspidization.

A rare conotruncal anomaly: absent pulmonary valve syndrome, interrupted aortic arch, and severe aortic stenosis.

The coexistence of absent pulmonary valve syndrome, Type B interrupted aortic arch, and severe aortic stenosis represents an exceptionally rare and complex congenital cardiac phenotype. The diagnosis was established in a newborn using multimodality imaging. Given the profound airway compression secondary to the massive pulmonary artery dilatation and the associated poor prognosis, palliative care was chosen.

Hypertrophic obstructive cardiomyopathy with associated pulmonary arteriopathy: A histopathologic perspective from sudden cardiac death.

Hypertrophic obstructive cardiomyopathy (HOCM) is one of the common causes of sudden cardiac death which can be associated with pulmonary vascular remodelling changes also known as Pulmonary Arteriopathy (PA). Pulmonary Arteriopathy (PA) is a condition affecting the pulmonary arteries, characterised by a range of proliferative and obstructive vascular lesions that ultimately result in lumen obstruction. It may manifest under diverse settings, encompassing both acquired and congenital disorders. We present a case of a 52-year male with no previous known history of illness or disease, who was br

Sex-specific outcomes after transcatheter or surgical treatment of aortic valve stenosis: the DEDICATE-DZHK6 trial.

BACKGROUND AND AIMS: Aortic stenosis may be managed differently in women and men, but evidence remains limited. Sex-specific characteristics and outcomes of low- to intermediate-risk patients assigned to transcatheter aortic valve implantation (TAVI) or surgical aortic valve replacement (SAVR) from the DEDICATE-DZHK6 trial are described. METHODS: The DEDICATE-DZHK6 trial demonstrated non-inferiority for the primary outcome of all-cause death or stroke at 1 year. Sex-specific effects were evaluated in this predefined descriptive subgroup analysis. RESULTS: A total of 1394 patients (43.3% wome

Sphingosine kinase 1 is integral for elastin deficiency-induced arterial hypermuscularization.

Deficiency of elastin (ELN), the major component of elastic fibers, leads to excess smooth muscle cells (SMCs), which characterizes arterial diseases (for example, supravalvular aortic stenosis (SVAS)) as well as physiological ductus arteriosus (DA) closure. Here we demonstrate that sphingosine kinase 1 (SPHK1) is a key node in these contexts. Sphk1 is the most upregulated transcript in Eln(-/-) aortic SMCs at embryonic day 15.5 when these cells are initially hyperproliferative. The aorta of humans with SVAS also upregulates SPHK1. Reduced ELN increases levels of transcription factor early gro

Robotic aortic valve replacement with a mechanical prosthesis: procedural steps.

Robotic aortic valve replacement is the latest advance in the field of aortic valve intervention and is increasingly being adopted by various centres with favourable early results. It allows surgeons to implant any commercially available aortic valve prosthesis (sutureless, biological, mechanical) in a surgically conventional manner, while offering minimal surgical trauma (sternum-free, rib-free, pectoralis muscle sparing). We present a step-by-step standardized procedure of robotic aortic valve replacement with a mechanical prosthesis for a young female patient with severe symptomatic aortic

Efficacy and safety of hybrid cardiac telerehabilitation in patients with hypertrophic cardiomyopathy without left ventricular outflow tract obstruction and preserved ejection fraction: A randomized clinical trial.

BACKGROUND: Exercise capacity is frequently reduced in patients with hypertrophic cardiomyopathy (HCM), but structured training has historically been limited due to concerns about arrhythmias and sudden cardiac death. Recent data suggest that supervised, individualized exercise may be safe in selected patients; however, evidence for hybrid cardiac telerehabilitation (HCT) in non-obstructive HCM remains lacking. AIMS: To evaluate the efficacy and safety of a 12-week HCT program in patients with non-obstructive HCM and preserved left ventricular ejection fraction. METHODS: In this single-cente

The journey from repair to replacement in pediatric aortic valve stenosis: a 10-year single-center experience.

OBJECTIVE: The optimal reoperation strategy and long-term outcomes of pediatric patients with congenital aortic stenosis (AS) have not been well elucidated. This study aimed to evaluate the reintervention outcomes and long-term prognosis in patients with isolated AS following their initial aortic valve (AoV) repair. METHODS: A retrospective analysis was conducted on the clinical data of all patients with isolated AS who underwent initial AoV repair between 2013 and 2024. The primary outcome was the rate of freedom from reoperation after the initial procedure and secondary surgeries. RESULTS:

Evolution of the Aorta After Valve Replacement in Bicuspid Aortic Valves: Results at Mid-Term Follow-Up.

AIM: Bicuspid aortic valve (BAV) is a common congenital heart defect often associated with ascending aorta dilation. Current guidelines suggest surgical intervention for diameters ≥45 mm in conjunction with surgical aortic valve replacement (SAVR). However, the optimal management of ascending aortas measuring 40-44 mm remains under debate. This study aims to evaluate the evolution of aortic diameter in patients with BAV and ascending aorta diameters between 40 and 44 mm undergoing aortic valve replacement, assessing whether a conservative approach is justified. METHOD: We conducted a retrospe

TAVI-in-TAVI in a patient with morquio syndrome: a case report.

BACKGROUND: Morquio syndrome (MPS IV) is a rare multi-systemic disorder with significant cardiovascular implications, including early-onset valvular disease. Due to the improved life expectancy, these patients could require complex interventional solutions such as TAVI-in-TAVI procedure. CASE SUMMARY: We present the first reported case of a 65-year-old woman with Morquio syndrome undergoing a TAVI-in-TAVI procedure for structural degeneration of a prior transcatheter bioprosthetic valve. The procedure was technically challenging due to complex thoracic anatomy, small aortic annulus, and inter

Management of Conduit Stenosis and Iatrogenic Aortopulmonary Window Post-Ross Procedure.

This case report describes a 23-year-old male with a history of bicuspid aortic valve endocarditis managed with an emergency Ross procedure in 2020, whereby a xenograft pulmonary conduit prosthesis was implanted in the pulmonary position, followed by pulmonary conduit stenosis treated initially with high-pressure balloon dilatation 3 years later. The consequent diagnosis of iatrogenic aortopulmonary (AP) window, combined with severe recurrent pulmonary conduit stenosis, necessitated reoperation with right ventricle-to-pulmonary artery homograft replacement and repair of the AP shunt.

Emerging Trends in Hypertrophic Cardiomyopathy Research.

Hypertrophic cardiomyopathy (HCM), recognized as the most prevalent inherited heart condition, is found in about 0.2% to 0.5% of the population globally. Recent years have witnessed a notable surge in HCM-related scholarly investigations. The aim of this study was to delineate the trajectory of HCM research over the preceding decade through a comprehensive bibliometric analysis. Utilizing the Web of Science Core Collection (WoSCC) database, we retrieved data on HCM-related studies conducted between 2013 and 2023. The acquired bibliographic data were methodically processed and examined using VO

Outcomes of aortic valvuloplasty with pericardium patch for congenital aortic stenosis and regurgitation in pediatric patients.

BACKGROUND: Surgical strategies for congenital aortic stenosis and regurgitation in children, particularly in infants (<1 year), remain controversial. Aortic valvuloplasty (AVP) with pericardial patch has gained increasing attention, but its durability and clinical benefits remain uncertain. METHODS: We retrospectively analyzed pediatric patients (≤12 years) undergoing AVP with pericardial patch in our center between July 2017 and July 2025. Infants (<1 year) were analyzed separately as subgroups. Primary outcome was the change in aortic valve hemodynamics, including median peak gradient, med

Cardiovascular CT in Bicuspid Aortic Valve Disease: A State-of-the-Art Narrative Review of Advances, Clinical Integration, and Future Directions.

Bicuspid Aortic Valve (BAV) disease is recognized as the most common congenital heart condition and is frequently associated with complex valvular and aortic disorders. Cardiovascular computed tomography (CT) has become essential for diagnosing BAV, planning procedures, and evaluating patients after treatment. This is largely due to CT's high spatial resolution and its ability to perform volume imaging effectively. This review provides an up-to-date overview of the increasing role of cardiovascular CT in the management of bicuspid aortic valve (BAV). It covers various aspects, including BAV mo

Bicuspid Aortic Valve in Infants without Severe Congenital Heart Defects: Early Echocardiographic Findings Guide Surveillance Strategies.

INTRODUCTION: Bicuspid aortic valve (BAV) is one of the most common congenital heart defects (CHDs). However, guidelines for imaging surveillance frequency are not well established among young children, particularly those diagnosed during infancy without hemodynamically significant associated congenital lesions. METHODS: The Mayo Clinic echocardiography database was retrospectively searched for infants (age <1 year) diagnosed with BAV from 2001 to 2019. We initially identified 307 patients, and of these, 204 (66%) patients with BAV and concomitant significant CHD were excluded. Therefore, 103

The haemodynamic response to pregnancy in patients with left ventricular outflow tract obstruction.

BACKGROUND: Pregnancy exacerbates left ventricular outflow tract (LVOT) gradients due to increased cardiac output (CO). This is amplified in patients with LVOT obstruction (LVOTO). Ascertaining whether gestational elevations in LVOT gradients are transient haemodynamic responses that regress postpartum, or predictors of disease progression or adverse outcomes, is critical. METHODS: A retrospective, observational study was conducted between 2009 and 2023 in two tertiary centres. Clinical and echocardiographic data were collected preconception, antenatally, and postpartum. Key measurements incl
