A four-month-old infant with severe congenital aortic stenosis underwent successful percutaneous transcatheter balloon valvuloplasty. While initially stable, ventricular bigeminy was detected at six-week follow-up, likely due to post-procedural myocardial inflammation. The arrhythmia resolved spontaneously by 12 weeks. This case highlights the need for vigilant post-percutaneous transcatheter balloon valvuloplasty monitoring to detect and manage late-onset arrhythmic complications.

The present study sought to evaluate the intracardiac morphology and associated cardiovascular anomalies in patients with double inlet left ventricle (DILV) on multidetector CT angiography. A retrospective search of the electronic departmental database was conducted from January 2014 to January 2023 to identify patients with a diagnosis of DILV on cardiac CT angiography. Patients with a common atrioventricular valve were excluded. The scans were systematically reviewed in a sequential segmental fashion for evaluating the intracardiac morphology and associated cardiovascular abnormalities. DILV

CLINICAL CONDITION: We present a case of transcatheter pulmonary valve endocarditis in a 38-year-old woman with a history of atrial septal defect, pulmonary stenosis, and bicuspid aortic valve, all previously repaired with mechanical and bioprosthetic valve replacements. Despite negative initial studies, 18F-fluorodeoxyglucose positron emission tomography-computed tomography revealed septic pulmonary emboli and abnormal uptake at the pulmonary valve. Metagenomic sequencing identified Haemophilus parainfluenzae as the pathogen.
KEY QUESTIONS: What is the differential diagnosis of a cardiac mas

BACKGROUND: Bicuspid aortic valve (BAV) is the most common congenital heart anomaly, often leading to significant aortic stenosis (AS) or aortic regurgitation (AR), which may require surgical intervention. Echocardiography is typically used for the diagnosis of BAV, and the integration of artificial intelligence (AI) can enhance diagnostic accuracy and guide surgical decisions.
CASE SUMMARY: We present two patients with BAV: a 17-year-old male football player with isolated AR due to prolapse undergoing aortic valve repair and a 68-year-old male with combined AS and AR, candidate for aortic va

Porcelain aorta, though rare, is often considered a contraindication for open surgery due to the risk of associated neurological complications due to calcium embolization. We present the case of a 70-year-old female with a prior 19mm bioprosthetic aortic valve replacement who, during routine follow-up, was found to have significant aortic valve stenosis, severely reduced leaflet mobility and a mean gradient of 40mmHg. Pre-operative computed tomography angiography revealed a calcified 'porcelain aorta'. Transcatheter aortic valve-in-valve replacement was not possible due to the small 19mm valve

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is a common genetic cardiovascular disease characterized by significant genetic heterogeneity. While the T-box transcription factor 1 (TBX1) gene is known to cause congenital cardiovascular defects, it has not been previously associated with HCM.
METHODS: Whole-exome sequencing (WES) was performed to identify causative gene mutations in a recently diagnosed Chinese family with HCM. The mutation was confirmed using Sanger sequencing, which was also employed to investigate familial co-segregation. A literature review was conducted to analyze previou

BACKGROUND: Bicuspid aortic valve (BAV), the most common congenital heart defect, is strongly predisposed to early calcification, yet the molecular drivers remain poorly defined. This study aims to identify the functional role of transient receptor potential vanilloid 2 (TRPV2) in valve interstitial cell (VIC) calcification and to explore its upstream and downstream regulatory network.
METHODS: Transcriptomic data from calcified BAV tissues (GSE83453, n = 18) were analyzed using gene set variation analysis, weighted gene co-expression network analysis, and multiple machine learning algorithms

Bicuspid aortic valve (BAV) is the most common congenital valvular anomaly, affecting roughly 1-2% of the population and predisposing to premature aortic stenosis and thoracic aortopathy. Surgical aortic valve replacement (SAVR) remains the standard therapy, while transcatheter aortic valve implantation (TAVI) is increasingly adopted across a broader range of risk profiles due to accumulating evidence and advancements in device technology. Observational registries and early trial data indicate that TAVI is technically feasible in selected BAV anatomies, with device-success rates exceeding 90%.

BACKGROUND: Double aortic arch (DAA) is a rare congenital vascular anomaly that is typically diagnosed in infancy. In adult patients undergoing transcatheter aortic valve replacement (TAVR), an undetected DAA can present unique anatomical and procedural challenges.
CASE SUMMARY: A 71-year-old woman with end-stage renal disease and severe aortic stenosis was incidentally found to have a DAA during preprocedural TAVR evaluation. The device was inserted via the right femoral artery, as it provided the largest vessel diameter and most favorable anatomy for device passage. The pronounced curvature

INTRODUCTION: Coarctation of the aorta is a congenital narrowing of the thoracic aorta associated with hypertension and significant pressure gradients across the coarctation site. Coarctoplasty by percutaneous approach is the preferred method of treatment. However, complications like stent dislodgement may result and must be immediately managed to prevent adverse outcomes.
CASE PRESENTATION: A 29-year-old male, who had previously undergone a mitral valve replacement, presents with exertional dyspnea and systemic hypertension. Blood pressure measurements revealed a 50 mmHg gradient between the

Critical aortic stenosis (AS) in infants is a severe congenital heart defect requiring early intervention to prevent cardiogenic shock and death. Critical AS results from dysplastic valve morphology obstructing left ventricular outflow, presenting with ductal-dependent systemic circulation until intervention can be performed. Balloon aortic valvuloplasty (BAV) and surgical valvotomy are established treatments, with BAV increasingly favored for its less invasive profile. Despite high-reintervention rates, individualized treatment based on anatomy and institutional expertise remains a key to opt

BACKGROUND Renal ectopia (RE) is a congenital malformation in which one or both kidneys fail to reach their normal anatomical position. Most cases are asymptomatic, but they can occasionally lead to complications. This case report presents the incidental finding of a pelvic ectopic kidney during a pre-transcatheter aortic valve replacement (TAVR) evaluation, showing the importance of recognizing anatomical variations in surgical planning for vascular access. CASE REPORT A 68-year-old man undergoing evaluation for severe symptomatic aortic stenosis was incidentally found to have a left pelvic k

Unicuspid aortic valve (UAV) is a rare congenital defect. Compared with valve replacement, aortic valve repair is a better potential strategy to achieve a low rate of valve-related events and an enhanced quality of life. We herein report two cases of aortic valve repair for UAV accompanied by aneurysm of the ascending aorta. By retaining the free margin tissue and forming a neocommissure to the side of the left lateral commissure that is 180 degrees opposite and the same height, we were able to achieve bicuspidization of the UAV. A glutaraldehyde-treated autologous pericardium patch was placed

Williams syndrome (WS) is a genetic disorder marked by intellectual disability, distinctive craniofacial features, dental abnormalities, and congenital heart defects, particularly supravalvular aortic stenosis-a narrowing above the aortic valve. Patients with WS are at increased risk of perioperative sudden cardiac death, necessitating thorough cardiovascular assessment. Additionally, craniofacial anomalies such as micrognathia and retrognathia may lead to difficult airway management and often require awake intubation. However, awake intubation and hospitalization can be distressing for patien

BACKGROUND: Williams syndrome (WS) is a rare neurodevelopmental disorder caused by a microdeletion on chromosome 7q. Previous research indicates that adults with WS are prone to having overweight and obesity and also have decreased fat-free mass (FFM) on body composition analysis. To date, no explorations of factors associated with measured resting energy expenditure (mREE) or with the accuracy of predictive resting energy expenditure (pREE) equations have been performed. This study aimed to (1) obtain mREE in adults with WS and examine contributing factors; (2) compare mREE between adults wit

The Melody valve (Medtronic) is commonly used for pulmonary valve replacement in patients with dysfunctional right ventricle to pulmonary artery conduits or bioprosthetic pulmonary valves. Rare reports exist of its use in the aortic position, including only 1 prior description of its use in a patient with hypoplastic left heart syndrome. We describe a 31-year-old patient with hypoplastic left heart syndrome who had severe neoaortic valve stenosis and moderate regurgitation that was successfully treated with percutaneous Melody valve placement in the neoaortic position.

Background: Bicuspid aortic valve (BAV) is the most common congenital heart defect, and its complications (namely, dilatation of the thoracic ascending aorta) raise concerns regarding the proper timing of aortic surgery. The study aim is to unravel the genetic basis of BAV and its complications through a high-throughput sequencing (HTS) approach and segregation analysis if family members were available. Methods: Fifty-two Italian BAV patients were analyzed by HTS using the Illumina MiSeq platform. Targeted sequencing of 97 genes known to be or plausibly associated with connective tissue disord
