The haemodynamic response to pregnancy in patients with left ventricular outflow tract obstruction.

BACKGROUND: Pregnancy exacerbates left ventricular outflow tract (LVOT) gradients due to increased cardiac output (CO). This is amplified in patients with LVOT obstruction (LVOTO). Ascertaining whether gestational elevations in LVOT gradients are transient haemodynamic responses that regress postpartum, or predictors of disease progression or adverse outcomes, is critical. METHODS: A retrospective, observational study was conducted between 2009 and 2023 in two tertiary centres. Clinical and echocardiographic data were collected preconception, antenatally, and postpartum. Key measurements incl

Ebstein's anomaly with life-threatening arrhythmia: an unusual cardiovascular manifestation of Williams syndrome - a case report.

BACKGROUND: Williams syndrome (WS) is a rare genetic multisystem disorder that often affects the cardiovascular system. However, an association with Ebstein’s anomaly (EA) is extremely uncommon. Previously, only two such cases have been reported. Here, we present a third patient with concurrent WS and EA, manifesting with life-threatening ventricular arrhythmias. CASE PRESENTATION: Our case involves an 11-year-old boy, diagnosed prenatally with EA and 3-months postnatally with WS on the basis of a 7q11.23 deletion. Over the course of follow-up since birth, life-threatening ventricular arrhythm

Papillary Fibroelastoma Originating From the Coumadin Ridge: An Unusual Location of a Primary Cardiac Tumor.

BACKGROUND: Papillary fibroelastomas are rare, benign cardiac tumors with potential for systemic embolization. Although typically valvular, they may arise in unusual locations and are often detected incidentally during cardiac imaging. CASE SUMMARY: A 76-year-old woman with chronic kidney disease and severe aortic stenosis presented with progressive fatigue and dyspnea. Preoperative imaging for aortic valve replacement revealed a left atrial mass. Transesophageal echocardiography demonstrated a mobile, gelatinous lesion arising from the Coumadin ridge. The mass was surgically excised concurre

Analytical validation and clinical evaluation of a high-sensitivity cardiac troponin I immunoassay for use in cats.

Cardiac troponin I (cTnI) is a cardiac specific biomarker of myocardial damage in humans, dogs, and cats. The ADVIA Centaur XP High-Sensitivity Troponin I assay (AC-cTnI-HS) has been validated for use in humans and dogs, but not for use in cats. The study objective was to analytically validate the AC-cTnI-HS assay for use in cats and to evaluate cTnI measurements in healthy cats compared to those with cardiac disease to assess the clinical utility of this assay. Surplus serum samples from cats were used for analytical validation. Intra- and inter-assay variability, dilutional parallelism, and

Unusual hypertrophic cardiomyopathy with severe right ventricular outflow tract obstruction and two septal aneurysms: a case report.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is characterized by left ventricular wall thickening, while right ventricular involvement in HCM is also common and has been identified to relate to worse prognosis. Right ventricular outflow tract (RVOT) obstruction is a rare clinical disorder with diverse etiologies, among which HCM is one of the most prevalent specific causes. Currently, severe RVOT obstruction caused by HCM remains rarely reported. Its phenotypic and genetic characteristics remains uncharacterized, warranting more attention. CASE PRESENTATION: This case presented a 33-year-old

Infantile Manifestation of a Rare Congenital Cardiac Triad Comprising Left Ventricular Noncompaction, Large Patent Ductus Arteriosus, and Bicuspid Aortic Valve.

Left ventricular noncompaction (LVNC) is a rare cardiomyopathy marked by excessive myocardial trabeculations and profound intertrabecular recesses, frequently identified incidentally, yet occasionally linked to significant cardiac dysfunction and adverse outcomes. It is uncommon for LVNC to coexist with other congenital cardiac anomalies, and the concurrent occurrence of LVNC, patent ductus arteriosus (PDA), and bicuspid aortic valve has not been previously reported. We report a 7-month-old female infant who exhibited exertional cyanosis and an episode of syncope in early infancy. Transthoraci

Over 40 Years of Experience With Aortic Valve Surgery in the Paediatric Population-What Is the Current Best Strategy?

OBJECTIVES: Congenital aortic valve (AoV) disease has limited treatment options in growing children. Different initial strategies, AoV repair/surgical valvuloplasty (SAV), AoV replacement (AVR), and Ross procedure were compared to elucidate the current best strategy. METHODS: All paediatric patients undergoing different initial AoV surgeries from 1976 to 2024 were included. Factors including prior balloon dilation (balloon valvuloplasty [BAV]), valve morphology, and initial disease (stenosis/regurgitation/mixed) were analysed. Survival and incidence of AoV reoperation/≥moderate AoV insufficie

UNICORN TAVR in Native Complex Congenital Aortic Stenosis at High Risk of Coronary Obstruction.

BACKGROUND: Coronary artery obstruction is a rare but serious complication of transcatheter aortic valve replacement (TAVR). Undermining iatrogenic coronary obstruction with a radiofrequency needle (UNICORN) is a novel technique of intraleaflet TAVR valve deployment for mitigating this risk. There is limited experience in native aortic valve anatomy, particularly in congenital aortic stenosis. CASE SUMMARY: A 26-year-old man in cardiogenic shock secondary to severe congenital aortic stenosis was successfully treated with emergency TAVR, with leaflet modification using the UNICORN technique du

"⅓ Yacoub" for correction of supravalvular aortic stenosis: valve-sparing root enlargement and ascending aorta replacement in an adult patient.

Congenital supravalvular aortic stenosis is a rare form of left ventricular outflow-tract obstruction, which is frequently associated with genetic disorders such as Williams-Beuren syndrome, despite spontaneous occurrence still being possible. Over the years, different surgical approaches to relieve the stenosis have been proposed; among them the McGoon, the Doty and the Brom repair are the best-known. Nevertheless, all these techniques especially address a paediatric population, and treatment options in adults are rarely described. Older patients, indeed, do not present the concern of preserv

Simultaneous aortic valve replacement and pulmonary artery graft replacement in a patient with idiopathic pulmonary artery dilatation and severe aortic stenosis: a case report.

BACKGROUND: Idiopathic pulmonary artery dilatation (IPAD) is a rare condition characterized by isolated dilatation of the pulmonary artery in the absence of congenital heart defects or other secondary causes. Here, we report a rare case of IPAD complicated by severe aortic stenosis (AS), managed with simultaneous aortic valve replacement (AVR) and pulmonary artery graft replacement. CASE SUMMARY: A 74-year-old woman with type 1 diabetes was followed for IPAD. Initial evaluations ruled out shunt diseases and secondary causes of pulmonary artery dilatation. Over 5 years of follow-up, mild AS pr

Dual-Level Left Ventricular Outflow Tract Obstruction in an Adult.

BACKGROUND: Discrete subaortic membranes are an uncommon cause of left ventricular outflow tract (LVOT) obstruction in adults and may coexist with aortic valve disease. CASE SUMMARY: A 50-year-old man presented with acute decompensated heart failure. Transthoracic echocardiography showed reduced ejection fraction (30%-35%) and demonstrated severe calcific aortic stenosis with regurgitation. Transesophageal echocardiography revealed a discrete subaortic membrane causing additional subvalvular obstruction, consistent with dual-level LVOT obstruction. Invasive hemodynamics confirmed advanced aft

From Valve Anatomy to Molecular Trajectories: Integrating Proteomics into Precision Care for Bicuspid Aortic Valve Disease.

Bicuspid aortic valve (BAV) disease is a lifelong disorder in which congenital anatomy, tissue susceptibility, abnormal flow, and acquired fibrocalcific remodeling produce heterogeneous valve and aortic outcomes. This narrative review examined peer-reviewed literature indexed in PubMed/MEDLINE through June 2026 to evaluate how circulating proteomics could complement established imaging-based risk assessment. Published studies of incident aortic stenosis consistently implicate integrated stress, inflammation, apoptosis, and extracellular-matrix remodeling, with recurrent signals including GDF15

Subaortic Stenosis in Four Free-Ranging Eurasian Lynx (Lynx lynx) from the Alpine and Jura Populations in Switzerland.

As part of wildlife conservation efforts, the reintroduced Eurasian lynx (Lynx lynx) populations in Switzerland are the subject of ongoing health surveillance. Cardiac pathologies, such as heart murmurs and histopathologic changes in the cardiac tissue, including intramural cardiac vessels, are frequently observed. Here we describe four cases of subaortic stenosis (SAS), a congenital heart disease rarely reported in felids, in free-ranging adult male lynx from Switzerland. The affected lynx presented macroscopically with grades 1 to 3 of SAS. Histopathology confirmed characteristic subvalvular

Evaluation of Right Ventricular Size Metrics Measured with Cardiac Magnetic Resonance Imaging in Patients with Pulmonary Regurgitation.

In patients with repaired congenital heart disease (CHD) and pulmonary regurgitation (PR), a common metric for guiding the timing of pulmonary valve replacement (PVR) is right ventricular (RV) end-diastolic volume (EDV). The standard practices of indexing RV EDV to body surface area and calculating RV EDV Z-scores are imperfect, and guidelines for PVR based on threshold values for indexed volumes may be insensitive to individual patient circumstances. Indexing RV EDV to left ventricular EDV is an alternative method of normalizing RV volume in patients with PR. We reviewed clinically obtained c

Two decades of pediatric aortic stenosis: Balloon versus surgical interventions: A single center study.

INTRODUCTION: Congenital valvular aortic stenosis (AS) represents 3%-6% of congenital heart diseases. This study aimed to compare the outcomes of balloon valvuloplasty (BV) and surgical valvotomy (SV) in patients with AS. METHODS: This retrospective study was conducted on 69 patients. A retrospective analysis was done on 69 patients (who underwent BV or SV) at a tertiary heart center in Mashhad, Iran (between 2000 and 2023). RESULTS: The mean age of BV patients (n = 24, 17 males) was 67.08 ± 55.92 months, while it was 97.44 ± 64.18 months in SV patients (n = 45, 33 males). Both treatments re

Risk factors for early reintervention in patients with cryopreserved homografts in the pulmonary position.

INTRODUCTION AND OBJECTIVES: Cryopreserved homograft implantation in the pulmonary position is a common surgical approach for pulmonary valve replacement, although it carries a risk of early failure requiring reintervention. This study aimed to identify clinical, anatomical, surgical, graft, and postoperative factors associated with early failure, defined as reintervention (percutaneous or surgical) within 3 years. METHODS: In this retrospective cohort study (2017-2024), we included 136 patients (median weight 45.0 [IQR: 25.25-60.00] kg; age 14.11 [8.31-18.09] years) who received a homograft

Meta-Analysis of TAVR Versus SAVR in Bicuspid Valve: Mortality, Complications, and Long-Term Outcomes.

INTRODUCTION: Bicuspid Aortic Valve (BAV) is the most common congenital heart disease and often causes aortic stenosis in younger adults. There are few prospective randomized trials comparing Transcatheter Aortic Valve Replacement (TAVR) and Surgical Aortic Valve Replacement (SAVR) in BAV patients, since these patients are usually excluded from major studies because of their complex anatomy. A meta-analysis to compare clinical outcomes between TAVR and SAVR in people with BAV was conducted. METHODS: PubMed, Google Scholar, and SCOPUS were searched for studies published up to December 2023 tha

Risk Factors for Aortopathy in Bicuspid Aortic Valve: Insights from a Large Echocardiographic Cohort.

BACKGROUND: Bicuspid aortic valve (BAV) is the most common congenital heart defect and is frequently associated with thoracic aortopathy. Risk stratification remains challenging, as predictors of aortopathy are incompletely understood and prior studies have been limited by modest sample sizes or incomplete echocardiographic characterization. METHODS: We retrospectively analyzed 1,602 patients diagnosed with BAV. Aortopathy was defined as dilation of the sinus of Valsalva or ascending aorta using guideline-recommended thresholds. Clinical and echocardiographic predictors included age, sex, bod

Ross(-Konno) Procedure in Neonates and Infants: A Multicentre Experience.

OBJECTIVES: Neonates and infants who receive a Ross or Ross-Konno operation form a different population from older children and adults who undergo this procedure. We aimed to provide a contemporary reassessment of the procedure in patients younger than 1 year of age. METHODS: We conducted a retrospective, observational multicentre study of patients younger than 1 year who underwent a Ross procedure. Clinical data were obtained from hospital records. Survival was analysed with Kaplan-Meier analysis and Cox regression, and reinterventions with competing-risk models. RESULTS: Eighty-one childre
