Predicting Suboptimal Outcomes After Initially Acceptable Aortic Valve Repair in Children.

BACKGROUND: Aortic valve repair is preferred for congenital aortic valve disease in children, but its durability is limited. Although acceptable immediate repair results improve outcomes, late failures still occur. This study aimed to identify risk factors for suboptimal outcomes after an initially acceptable repair. METHODS: We retrospectively reviewed 287 patients undergoing primary aortic valve repair (2014-2023). Acceptable repair was defined as less than moderate aortic stenosis (AS) and mild or less aortic insufficiency (AI) after bypass. Suboptimal outcomes were defined as more than mo

Silent but fatal: Sudden death from asymptomatic severe aortic stenosis.

Aortic stenosis (AS) is the most common valvular heart disease and typically progresses slowly, often becoming symptomatic in older people. Sudden death in previously asymptomatic individuals is rare, particularly in middle-aged adults. We report a case of a 48-year-old male carpenter who collapsed while engaging in his routine work. He was pronounced dead shortly after arrival at hospital. Medico-legal autopsy revealed a markedly enlarged heart weighing 682 g. The aortic valve was severely stenosed, heavily calcified and distorted, producing an orifice of approximately 1 cm. The valve appeare

Pack-Size-Dependent Effects of Bone Cement Use in Unicompartmental Knee Arthroplasty: A Prospective Randomized Trial.

UNLABELLED: The current therapeutic agents for the treatments of visceral leishmaniasis are ineffective, and cytotoxic. Therefore, there is the urgent need for new chemotypes for the treatment of the disease with novel mechanisms of action. In our previous investigation, we identified the triazolopyridazine, STOCK6S-84928 as a potential inhibitor of Leishmania donovani sterol methyltransferase (LdSMT) with IC50 value of 118.3 µM. To improve the biological activity of the initial hit compound, we hereby describe the results of structural-activity relationship studies on STOCK6S-84928 via chemic

Dysphagia Lusoria Revealing Bicuspid Aortic Stenosis With an Aberrant Right Subclavian Artery and Contralateral Subclavian Aneurysm.

Aberrant right subclavian artery is the most common congenital anomaly of the aortic arch and may present with dysphagia lusoria due to esophageal compression. Although bicuspid aortic valve disease is commonly associated with aortopathy, its coexistence with an aberrant right subclavian artery and a contralateral subclavian artery aneurysm is exceedingly rare. We report the case of a 33-year-old male presenting with exertional dyspnea, chest pain, presyncope, and intermittent dysphagia to solids. Echocardiography demonstrated severe bicuspid aortic stenosis with a mean gradient of 95 mmHg and

Durability of Bioprosthetic and Mechanical Valves Implanted for Aortic Valve Replacement in Pediatric Patients.

BACKGROUND: There is limited published information on bioprosthetic aortic valve durability. The purpose of this study was to assess the durability of bioprosthetic valves used for aortic valve replacement (AVR) in pediatric patients. METHODS: We reviewed all surgical AVR procedures performed from May 2002 to July 2024 in patients ≤21 years of age using a mechanical valve or bioprosthesis. Long-term valve-related outcomes were assessed with propensity-score-adjusted analysis. RESULTS: During the study period, 180 AVR procedures were performed in pediatric patients (median age, 14.3 years) us

Development and Validation of an Extended Risk Model for Secondary Surgery Following Repair of Discrete Membranous Subaortic Stenosis in Pediatric Patients.

BACKGROUND: Long-term outcomes remain heterogeneous for patients with discrete membranous subaortic stenosis who underwent surgical repair. We aim to develop and validate an extended risk model for predicting secondary surgery following repair of discrete membranous subaortic stenosis in pediatric patients. METHODS: We retrospectively analyzed 235 children undergoing discrete membranous subaortic stenosis repair between 2008 and 2023. The primary end point was secondary surgery for significant aortic regurgitation or recurrent left ventricular outflow tract obstruction. Building on our previo

Common Clinical Scenarios in Adult Congenital Heart Disease: A Practical Guide for General Cardiologists.

Adults with congenital heart disease (ACHD) represent a growing population of adults in the United States, necessitating lifelong care by ACHD specialists. Often, patients may be lost to follow-up after pediatric care or elude detection until adulthood, presenting to the emergency room with common clinical complaints such as chest pain, dyspnea on exertion, or palpitations. This review demystifies common clinical presentations in pulmonary stenosis, branch pulmonary stenosis, coarctation of the aorta, and subaortic stenosis and their associated pathophysiology, salient clinical features, multi

Case Report: Dual molecular diagnosis in complex congenital heart disease in an Ecuadorian patient with supravalvar aortic stenosis and pulmonary valve stenosis carrying pathogenic variants in ELN and BRAF gene.

BACKGROUND: Congenital heart diseases (CHDs) are the most common inherited anomalies worldwide and remain a major cause of mortality in pediatric populations. Advances in genomic medicine have improved the ability to identify molecular mechanisms underlying complex CHD phenotypes, including cases not fully explained by a single-gene disorder. CASE PRESENTATION: This case presents an Ecuadorian girl with the coexistence of congenital supravalvar aortic stenosis and congenital valvar pulmonary stenosis. Molecular analysis identified pathogenic variants in ELN and BRAF, together with a NOTCH1 p.

Incidence or Coincidence: Subaortic Membrane, Bicuspid Aortic Valve, and Aortic Root Dilatation in a Patient Presenting With Stroke.

Bicuspid aortic valve (BAV) is the most common congenital cardiac malformation and is frequently associated with progressive aortopathy. Subaortic membrane, a discrete form of left ventricular outflow tract (LVOT) obstruction, is uncommon in adults and rarely coexists with BAV outside of complex congenital syndromes. The simultaneous presence of BAV, aortic root dilation, and a subaortic membrane represents an unusual anatomical constellation with important diagnostic and clinical implications. We present the case of a 49-year-old man with no known medical history who developed sudden, painles

Severe Aortic Regurgitation with New Congestive Heart Failure Following Fetal Aortic Valvuloplasty - A Case Report.

Fetal aortic valvuloplasty (FAV) is performed in fetuses with critical aortic stenosis (CAS) to prevent progression to hypoplastic left heart syndrome (HLHS). While successful FAV often results in new-onset aortic regurgitation (AR), this typically improves during gestation. We present a rare case of persistent severe AR following FAV, complicated by fetal congestive heart failure. A 28-year-old woman was referred at 23 + 0 weeks of gestation due to CAS of the fetus. Fetal echocardiography showed CAS with a peak velocity of 2.79 m/s, significant LV dysfunction and grade 2–3 endocardial fibroel

Left ventricular rehabilitation after Ross operation for critical aortic stenosis with endocardial fibroelastosis.

OBJECTIVES: Congenital critical aortic stenosis is frequently accompanied by endocardial fibroelastosis (EFE), impairing diastolic compliance and long-term ventricular performance. An early Ross procedure, with or without concomitant EFE resection may restore left ventricular (LV) function; however, long-term functional outcomes remain poorly characterized. We evaluated long-term clinical and functional outcomes after Ross procedure in neonates and infants with and without EFE. METHODS: Patients who underwent a Ross operation at <1 year of age between January 2005 and January 2025 were includ

Minimally invasive cardiac surgery for tetralogy of Fallot: early outcomes from a single-center experience.

BACKGROUND: While minimally invasive techniques for simple septal defects have demonstrated comparable safety and efficacy to median sternotomy, clinical outcomes following minimally invasive repair of Tetralogy of Fallot (TOF) remain insufficiently characterized. METHODS: This retrospective single-center cohort included patients who underwent complete TOF repair through a modified right vertical infra-axillary thoracotomy (RVIAT) between January 2022 and December 2024. RESULTS: All 144 patients (mean age, 8.7 ± 7.8 months; mean weight, 7.6 ± 2.4 kg) underwent RVIAT repair without conversion

Growing Heart Valves in Congenital Cardiac Surgery: Mechanisms and Considerations for Tissue-Engineered Heart Valves, Noncryopreserved Homografts, and Partial Heart Transplantation.

Heart valve abnormalities are present in 30% of congenital heart defects. Additionally, there are acquired conditions like rheumatic heart disease. They often require surgery, and in complex cases, valve replacement becomes unavoidable. The current valve replacement options, either mechanical, bioprosthetic, homografts, or autografts, lack key properties for pediatric patient needs: availability, durability, and the ability to grow. Our goal is to review 2 clinically relevant innovations: tissue-engineered heart valves (TEHVs) and partial heart transplantation (PHT). We conducted searches in e

A pathobiologic classification of aortic stenosis: Moving beyond the degenerative-bicuspid-rheumatic paradigm.

BackgroundAortic stenosis is traditionally categorised as degenerative calcific, congenital bicuspid, or rheumatic. Contemporary molecular biology, multimodality imaging, and transcatheter valve therapy demonstrate that aortic stenosis represents a spectrum of mechanistically distinct diseases converging upon a shared haemodynamic endpoint. A four-category pathobiologic classification is proposed linking disease mechanism to operative strategy and procedural risk stratification.MethodsA narrative mechanistic review was performed using contemporary international guidelines, foundational biologi
