Severe Supravalvular Aortic Stenosis Treated With Double Patch Aortoplasty: A Case Report.

Severe supravalvular aortic stenosis in a young patient is a rare but critical condition requiring timely diagnosis and intervention. We report a 15-year-old girl presenting with exertional dyspnea, chest pain, and palpitations. Echocardiography and multidetector computed tomography (MDCT) confirmed severe supravalvular aortic stenosis, myxomatous mitral valve with moderate to severe mitral regurgitation, and reduced ejection fraction. A double patch aortoplasty was performed with a successful outcome. This case underscores the importance of early diagnosis and tailored surgical intervention a

Case Report: Exploring KMT2D mutation in Shone syndrome.

INTRODUCTION: Shone syndrome is a rare congenital heart disease characterized by multilevel left-sided obstructive lesions. KMT2D variants cause Kabuki syndrome and are frequently associated with left-sided obstructive congenital heart defects, but their contribution to Shone syndrome remains uncertain. METHODS: We report a 4-year-old girl with a Shone syndrome phenotype. Trio whole-exome sequencing (WES) was performed in the proband and her parents, with Sanger sequencing for confirmation and segregation analysis. Echocardiography and CT angiography were used to delineate cardiac anatomy. R

Trends and Projections for Aortic Stenosis and Hypertension-Related Mortality Among Older Adults in the United States: A Retrospective Population-Based Analysis.

INTRODUCTION: Aortic stenosis (AS) and hypertension (HTN) frequently co-exist in older adults, worsening disease progression and increasing mortality risk compared to either condition alone. AIM: To examine mortality trends and demographic-geographic disparities from comorbid AS and HTN in the U.S. from 1999-2020. METHODS: Data were extracted from the CDC WONDER Multiple Cause of Death database. Crude and age-adjusted mortality rates (CMRs, AAMRs) per 100,000 were calculated. Joinpoint regression estimated annual (APC) and average annual percent change (AAPC). Trends were projected to 2030 u

Outcomes of Heart Transplantation in Single-Ventricle Physiology: A Retrospective Single-Center Experience with Emphasis on Surgical Complexity.

Background: Patients with single-ventricle physiology represent a high-risk group for heart transplantation. Due to complex anatomical and physiological challenges, including multiple prior sternotomies, pulmonary artery abnormalities, and systemic consequences of altered circulation, they represent both a surgical and a clinical challenge. We aimed to analyze perioperative challenges, as well as early and long-term complications, in this specific group of patients. Methods: We performed a retrospective data analysis of a high-volume heart transplant center, focusing on patients with single-ve

Is There a Unified Etiology of Hypoplastic Left Heart Syndrome? Evaluating Genetic, Structural, and Hemodynamic Models of Disease Initiation.

Background: Hypoplastic left heart syndrome (HLHS) is defined as "a spectrum of congenital cardiovascular malformations with normally aligned great arteries without a common atrioventricular junction, characterized by underdevelopment of the left heart with significant hypoplasia of the left ventricle including atresia, stenosis, or hypoplasia of the aortic or mitral valve, or both valves, and hypoplasia of the ascending aorta and aortic arch". Without treatment, HLHS is usually lethal in the neonate. Many hypotheses have been advanced to explain the etiology of HLHS; however, no single theory

Myocardial Deformation Analysis in Congenital Aortic Valve Stenosis: Comparison Across Clinical Phenotypes in Children.

Left ventricular (LV) longitudinal strain (LS) has prognostic value in adults with valvar aortic stenosis (AS), and impaired left atrial (LA) strain is associated with elevated filling pressures. The role of LA and LV strain in children with congenital AS is unclear. We hypothesized that LA and LV strain would be worse with higher gradients and/or greater ventricular mass. We performed a retrospective cross-sectional study (2015-2025) of children aged 1-18 years with isolated congenital AS and preserved systolic function, stratified by Doppler-derived mean gradient as mild (<20 mmHg), moderate

Great debate: surgical aortic valve replacement is first choice for aortic stenosis in patients with a life expectancy beyond 5 years.

Transcatheter aortic valve implantation (TAVI) is increasingly used for the treatment of aortic stenosis. Recently published guidelines favour now TAVI in all patients 70 years or older who have tricuspid valves and suitable anatomy. However, while a number of randomized controlled trials confirm that TAVI and surgical aortic valve replacement (SAVR) provide equivalent outcomes up to 5 years, data beyond 5 years are still scarce and real-world registry data report indeed conflicting results. Higher rates of complications after TAVI such as pacemaker requirement and paravalvular regurgitation,

Cryoextraction for Airway Bleeding After Valve-in-Valve Procedure.

BACKGROUND: Hemoptysis after percutaneous valve implantation is a rare but life-threatening complication. CASE SUMMARY: An 84-year-old woman with prior mechanical aortic and biological tricuspid valve replacements presented with tricuspid stenosis and regurgitation. A transcatheter valve-in-valve procedure using a 24.5-mm Myval was performed successfully. However, she developed severe hemoptysis, hypoxia, and ventilation difficulties shortly after the procedure. Flexible bronchoscopy with cryoprobe-assisted clot extraction was performed, removing large obstructive clots from the bronchi. A se

Norwood procedure with left coronary artery reimplantation for hypoplastic left heart syndrome combined with Anomalous left coronary artery from the right pulmonary artery: a case report.

BACKGROUND: Anomalous origin of the left coronary artery from the pulmonary artery is a rare congenital heart defect associated with high mortality during infancy if not surgically corrected. Its occurrence combined with hypoplastic left heart syndrome is highly uncommon, presenting considerable diagnostic and technical challenges. Identifying such an anomaly before surgery can be challenging, particularly in patients with complex cardiac anatomy. Intraoperative recognition requires immediate adjustments of the surgical approach, especially regarding myocardial protection and coronary artery r

Ross procedure: Evidence for pulmonary autograft and early outcomes.

BackgroundRenewed interest in the Ross procedure in young adults has been tempered by questions over safety and early outcomes. This paper examines our institution's 1-year outcomes of the Ross procedure.MethodsFrom November 2019 to February 2025, 136 consecutive adult patients underwent a Ross procedure at an urban tertiary center. The first 50 patients had follow-up beyond 1 year and constitute the study group of this case series.ResultsMedian age was 41 years (IQR: 30, 47). The majority were male (n = 34, 68%). The most common indication for surgery was aortic stenosis with or without aorti

The Ross Operation in Children-What Is the Long-Term Fate of the Autograft?

OBJECTIVES: The Ross operation achieves excellent outcomes in children and adolescents, but the influence of age at surgery and underlying pathology on long-term results remains uncertain. Evidence on autograft dilatation necessitating reintervention is also limited. This study aimed to identify patient groups best served from the Ross procedure and to evaluate long-term autograft performance. METHODS: All consecutive patients <18 years undergoing a Ross or Ross-Konno procedure between 1991 and 2024 at a single centre were analysed. Demographic, operative, and follow-up data were collected. P

Beyond the Typical Marfan Phenotype: Severe Aortic Stenosis Caused by a Unicommissural Unicuspid Aortic Valve in Adolescence.

Unicuspid aortic valve (UAV) is an exceptionally rare congenital aortic valve anomaly. UAV is more commonly associated with significant aortic stenosis at a younger age than bicuspid aortic valve. Two forms of UAV have been described. Acommissural type of UAV typically presents in early infancy, and unicommissural type of UAV usually presents in late childhood or early adolescence. Echocardiography is the main imaging tool used to assess aortic valve morphology, aortic dimensions, and the presence and severity of stenosis or regurgitation. Early surgical intervention is typically required for

Myocardial remodeling and fibrosis by cardiovascular magnetic resonance in adults with congenital aortic valve stenosis.

BACKGROUND: Left ventricular (LV) remodeling is becoming increasingly important in assessing the impact of aortic stenosis (AS). However, data on structural LV remodeling in congenital AS patients is scarce. Therefore, this study aimed to describe LV remodeling before and after aortic valve replacement (AVR) in adults with congenital AS and identify markers associated with myocardial fibrosis. METHODS AND RESULTS: In this prospective study, adult patients with congenital AS were included with 1) at least mild AS (Vmax ≥2.5 m/s) or 2) prior AVR due to congenital AS. Cardiovascular Magnetic Res
