Prenatal Genetic Diagnosis of Williams-Beuren Syndrome with Atypical and Complex Phenotypes.

BACKGROUND: Williams-Beuren syndrome (WBS) is a severe congenital disorder that presents challenges in prenatal diagnosis due to the atypical or incomplete phenotypes exhibited by affected fetuses. This study investigated the relationship between genotype and complex phenotype in WBS fetuses using ultrasound, SNP array, and whole exome sequencing. METHODS: Chromosomal microarray analysis (CMA) and whole genome sequencing (WES) were conducted on pregnant women undergoing prenatal diagnosis. We analyzed genome-wide copy number variants (CNVs), regions of homozygosity (ROH), single nucleotide va

Rare Incidental Finding of an Interventricular Membranous Septal Aneurysm in an Adult Presenting for Aortic Valve Replacement.

Interventricular membranous septal aneurysm (IVMSA) is a rare cardiac anomaly that typically coexists with other cardiac malformations. Recognition of IVMSAs is important for risk mitigation of potentially serious complications such as right ventricular outflow obstruction and proper characterization of the lesion is critical for planning of surgical repair. Literature reports of this condition are sparse and appear limited to case reports. Bicuspid aortic valve (BAV) is a common congenital cardiac malformation tending to present later in life with symptomatic stenosis and is frequently associ

Valve-in-Valve TAVR for Paravalvular Leak After J-Valve in Bicuspid Aortic Stenosis With Sinus Aneurysm.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) has become a standard therapy for severe aortic stenosis, yet bicuspid aortic valve (BAV) patients remain challenging, especially with complex aortic root. CASE SUMMARY: A 72-year-old man with type 0 BAV and sinus of Valsalva aneurysm underwent transapical J-Valve implantation for severe stenosis. Post-procedural incremental paravalvular leak (PVL) due to aneurysm-related malapposition was successfully treated with a self-expanding valve-in-valve (ViV) prosthesis (TaurusElite, 29 mm). Multimodal imaging, including echocardiography, com

Virtual Therapy Planning of Aortic Valve Replacement for Preventing Patient-Prosthesis Mismatch.

BACKGROUND: Recent studies suggest that any degree of patient-prosthesis mismatch (PPM) increases morbidity and mortality after surgical aortic valve replacement (SAVR). We used computational fluid dynamics simulations to test the influence of prosthesis size and physical activity after SAVR. METHODS: In 10 patients with aortic valve stenosis, virtual SAVR was performed. Left ventricular outflow tract stroke volume and flow direction information (4D Flow) were used, and an increase in stroke volume of 25% was chosen for simulating physical activity. Pressure gradients (DP max) across the aort

The Combined Risk of Marfan Syndrome and Bicuspid Aortic Valve in the Elderly.

Marfan syndrome is a genetic condition that causes connective tissue abnormalities that can ultimately lead to cardiovascular complications such as aneurysms. When a bicuspid aortic valve is also present, which is a congenital heart defect characterized by the aortic valve having two cusps instead of three, a combined effect of serious cardiovascular complications can result. There is an additive and increased risk of aortic complications due to the combined impact on aortic integrity that can lead to aneurysm formation and/or dissections. A severe case of aortic stenosis with a thoracic aorti

Cardiological Manifestations in Males and Females Affected by NAA10 -Related Disease.

We report a family with two affected brothers presenting hypertrophic cardiomyopathy, prolonged QT interval, and intellectual disability who, after a dozen years of inconclusive genetic testing, were found to share a previously undescribed variant c.549delA (p.Gly184Alafs*67) in the X-linked NAA10 gene. Their mother was heterozygous for the variant and had a long history of unexplained cardiac arrhythmia. NAA10 (N-alpha-acetyltransferase 10) is a component of the N-terminal acetyltransferase A complex (also called the NatA complex) necessary for N-alpha-acetylation, among the most common post-

Patent Ductus Arteriosus with Left to Right Shunting Exacerbating Aortic Valve Stenosis.

Aortic stenosis (AS) leads to a reduced effective orifice of the aortic valve, and severity is based on obstructions in flow and velocity. In some patients, coexisting structural cardiac abnormalities that increase left ventricular volume, such as patent ductus arteriosus (PDA), may complicate evaluation and management. We present the case of a patient with severe AS and unrepaired PDA and discuss the hemodynamic implications and important physiological changes resulting from the interactions between these lesions. It is important for clinicians to consider the impact of PDA closure in the evo

Non-invasive tissue characterization in children and young adults with aortic coarctation-an MRI-based prospective study.

BACKGROUND: Aortic coarctation (CoA) necessitates long-term monitoring to identify late complications, including re-stenosis, aneurysms, arrhythmias and heart failure. Nonetheless, there remain gaps in understanding the effects of adverse left-ventricular (LV) remodeling at the myocardial tissue level, which may contribute to incipient heart failure. The aim of this study is to evaluate myocardial tissue characteristics in patients with CoA using advanced cardiac magnetic resonance (CMR) imaging techniques to identify markers of adverse tissue remodeling and their association with disease seve

TAVI in Complex Dual-Level Obstruction: A Case of Severe Aortic Stenosis and Subaortic Membrane.

This case addresses the challenges of treating patients with both severe aortic stenosis and subaortic stenosis. In this combined condition, transcatheter aortic valve implantation (TAVI) remains an off-label application, particularly in the presence of a subaortic membrane. Our multimodal imaging approach that incorporates echocardiography, cardiac computed tomography, and fluoroscopic guidance demonstrates the successful application of TAVI in a high-risk clinical scenario. The results underscore the potential of TAVI as a viable alternative to traditional surgical aortic valve replacement f

A case of congenital heart defects and familial exudative vitreoretinopathy caused by activation of a cryptic splice donor in NOTCH1.

BACKGROUND: NOTCH1 is associated with two disorders of vascular development, Adams-Oliver Syndrome 5 (AOS5) and aortic valve disease 1 (AOVD1). Here we report a disease-causing variant in NOTCH1 that has a previously undemonstrated effect on splicing. Additionally, we found that the proband has the optic phenotype of familial exudative vitreoretinopathy (FEVR) which has been reported for probands with pathogenic variants in genes in the notch signaling pathway, but never for NOTCH1. CASE PRESENTATION: The proband presented with a ventricular septal defect, pulmonic stenosis, and ocular findin

Cardiac amyloidosis in severe aortic stenosis patients: an updated review unraveling their complex interplay.

Aortic stenosis (AS) and cardiac amyloidosis (CA) often coexist in elderly patients, complicating diagnosis and treatment. AS is characterized by the narrowing of the aortic valve, leading to left ventricular outflow obstruction, primarily due to age-related calcification and congenital defects. CA, the most common cause of restrictive cardiomyopathy, results from amyloid protein deposits in the myocardium, causing diastolic dysfunction. Accurate diagnosis of CA in AS patients is challenging due to overlapping symptoms and the need for advanced imaging and biomarkers. Aortic valve replacement

Shear Stress Conditioning Promotes a Pro-Inflammatory Response in Porcine Endocardial Endothelial Cells.

PURPOSE: Discrete subaortic stenosis (DSS) is a congenital heart disease characterized by a narrowing of the passage below the aortic valve in the left ventricular outflow tract (LVOT). While endocardial endothelial cells (EECs) are known to play a role in DSS, the response of these cells to shear stress is not known. In this study, we hypothesize that the response of EECs to shear stress in the LVOT is a mediator of DSS. METHODS: To test this hypothesis, we conditioned porcine EECs to controlled shear stress regimes using cone-and-plate bioreactors. Subsequently, we quantified the concentrat

Development of Cardiac Computed Tomography for Evaluation of Aortic Valve Stenosis.

Aortic valve stenosis (AS) is a valvular heart disease that imposes a high afterload on the left ventricle (LV) due to restricted opening of the aortic valve, resulting in LV hypertrophy. Severe AS can lead to syncope, angina pectoris, and heart failure. The number of patients with AS has been increasing due to aging populations, the growing prevalence of lifestyle-related diseases, and advances in diagnostic technologies. Therefore, accurate diagnosis and appropriate treatment of AS are essential. In recent years, transcatheter aortic valve implantation (TAVI) has become feasible, and the num

From Aortic Stenosis to Cardiomyopathy: The Hidden Impact of Undiagnosed Acommissural Unicuspid Aortic Valve.

Unicuspid aortic valve (UAV) is a rare congenital anomaly, diagnosed in individuals aged 30 to 50. It is associated with aortic stenosis or aortic regurgitation, potentially leading to heart failure. We report a case of a 28-year-old male with an acommissural UAV, aortopathy, critical aortic stenosis, and heart failure with reduced ejection fraction, presenting with shortness of breath. This case is notable for late presentation of an acommissural UAV, which typically manifests in childhood.

TOWARDS PATIENT-SPECIFIC SURGICAL PLANNING FOR BICUSPID AORTIC VALVE REPAIR: FULLY AUTOMATED SEGMENTATION OF THE AORTIC VALVE IN 4D CT.

The bicuspid aortic valve (BAV) is the most prevalent congenital heart defect and may require surgery for complications such as stenosis, regurgitation, and aortopathy. BAV repair surgery is effective but challenging due to the heterogeneity of BAV morphology. Multiple imaging modalities can be employed to assist the quantitative assessment of BAVs for surgical planning. Contrast-enhanced 4D computed tomography (CT) produces volumetric temporal sequences with excellent contrast and spatial resolution. Segmentation of the aortic cusps and root in these images is an essential step in creating pa

Identification of novel MYO19 variants in neonatal hypertrophic cardiomyopathy: a familial analysis revealing oligogenic contributors to disease severity.

BACKGROUND: Pediatric hypertrophic cardiomyopathy (HCM) is a rare condition, particularly in neonates, and is characterized by rapid and extensive myocardial hypertrophy, often leading to severe clinical outcomes. HCM can arise from variants in sarcomeric genes, which are essential for myocardial contractions, as well as non-sarcomeric gene variants. Although genetic modifiers and oligogenic inheritance have been implicated in congenital heart disease and cardiomyopathy, their complexity in HCM has not been fully elucidated, especially in familial cases with variable phenotypes. Hence, this st

Thirty years of left atrioventricular valve reoperations following atrioventricular septal defect repair: Impact of valvar stenosis.

BACKGROUND: The most frequent reason for reoperation after an index atrioventricular septal defect (AVSD) repair is left atrioventricular valve (LAVV) dysfunction. LAVV stenosis (LAVVS), although less common than regurgitation, may be a more problematic lesion to repair. We sought to identify baseline LAVV anatomic characteristics and outcomes associated with both LAVV reoperation overall and LAVVS in particular. METHODS: A retrospective review was conducted of patients who underwent a biventricular index AVSD repair between January 1995 and August 2024. All AVSD subtypes were included. Univa
