Totally 3-dimensional endoscopic ventricular septal defect closure and aortic valve replacement via a periareolar incision.

There are no prior reports of totally endoscopic minimally invasive cardiac surgery for combined ventricular septal defect closure and aortic valve replacement. We utilized a periareolar incision as the main access, inserting a cardioplegia line, vent tube and the aortic cross-clamp entirely through this incision. Only three ports (main incision, camera port and left-hand port) were used. A three-dimensional endoscope facilitated precise techniques. The type I ventricular septal defect was approached and closed from the aortic side using a Gore-Tex patch, with mattress sutures for the lower ed

Improved quantification of aortic regurgitation with direct regurgitant jet measurement by four-dimensional flow cardiovascular magnetic resonance in complex congenital heart disease.

BACKGROUND: Due to the presence of complex flow states and significant jet eccentricity in patients with congenital heart disease (CHD), accurate quantification of aortic regurgitation (AR) using standard echocardiographic or conventional cardiovascular magnetic resonance (CMR) imaging measures remains challenging. Four-dimensional flow (4DF) CMR permits transvalvular flow quantification under non-laminar flow states, although it has not been well validated for AR quantification in CHD. METHODS: In 186 patients with moderate or complex CHD, we evaluated the agreement between different methods

Fate of Residual Patch Leak After Surgical Closure of Congenital Perimembranous Ventricular Septal Defects.

IntroductionVentricular septal defects (VSDs) account for 50% of congenital heart defects in children. Surgical closure of moderate to large VSDs using patch material is considered the standard treatment in children to prevent future complications.ObjectivesWe evaluated the fate of VSD patch leak and its impact on aortic and tricuspid valve regurgitation at one year after surgery.MethodsThis retrospective study consisted of patients with a perimembranous VSD who underwent surgery between January 2016 and July 2021. Demographic, echocardiographic, clinical, and operative data were retrospective

Ruptured aneurysm of the non-coronary sinus of Valsalva with detachment of the tricuspid valve annulus: an unusual complication of a rare cardiac anomaly.

Sinus of Valsalva aneurysm is a rare cardiac anomaly, often associated with ventricular septal defects, aortic valve dysfunction, and frequently asymptomatic until rupture. Acute rupture typically occurs into the right ventricle and requires prompt surgical intervention. In this article, we describe the case of a 36-year-old female patient in good overall health who presented with ruptured sinus of Valsalva aneurysm complicated by detachment of the commissure between the septal and anterior cusps of the tricuspid valve, resulting in severe regurgitation. The patient underwent emergent cardiac

Right Atrial Appendage for Aortic Valve Reconstruction.

BACKGROUND: Congenital aortic valve disease, often bicuspid, is a common problem. Current repair techniques, including valve replacement or use of pericardium for repair, are imperfect and limited by lack of ideal tissue. Based on our experience using live right atrial appendage (RAA) tissue for pulmonary valve reconstruction, we describe preliminary results using this tissue for aortic valve reconstruction. METHODS: Six patients with unicuspid or bicuspid aortic valves and predominant aortic insufficiency were referred for intervention. Intraoperative assessment of the native valve was follo

The utility of neocuspidization in the surgical management of congenital aortic valve pathology: mid-term results of single-center experience with AVNeo procedure in children.

BACKGROUND: Aortic valve (AV) pathology in children presents a significant surgical challenge, with mid- and long-term outcomes of current techniques remaining controversial. This study evaluates our experience with aortic valve neocuspidization (AVNeo) in the pediatric population, analyzing immediate and mid-term results. METHODS: Ten children underwent AVNeo between June 2017 and August 2019. The clinical data were prospectively collected and retrospectively analyzed. The primary outcomes included failure to perform AVNeo, intraoperative conversion to the alternative technique, in-hospital

Early inspiris resilia valve failure in a patient with idiopathic pulmonary valve regurgitation.

BACKGROUND: Pulmonary valve failure requiring replacement (PVR) is more commonly seen in children and young adults with congenital heart disease (CHD). Adults with CHD and pulmonary regurgitation have traditionally undergone PVR with bioprosthetic valves. The inspiris resilia bovine pericardial valve is an FDA-approved bioprosthesis for the aortic position with encouraging data on 7-year outcomes. Previous reports on PVR using the Inspiris valve in young patients with CHD have demonstrated early failure of the valve. We report the early failure of this device in an elder patient with idiopathi

Successful Transcatheter Pulmonary Valve Implantation in a Patient with Mechanical Valve in Aortic Position.

The Harmony™ transcatheter pulmonary valve (TPV) is specifically designed to treat severe pulmonary regurgitation in a native or surgically repaired right ventricular outflow tract. To date, no reports have described TPV implantation (TPVI) in patients with prior aortic valve replacement (AVR) using a mechanical valve. Therefore, the risks of technical challenges or complications, particularly coronary compression or device fracture associated with prior AVR, remain unknown. This is the first report of successful TPVI using a Harmony™ TPV in a 52-year-old Asian man diagnosed with tetralogy of

Early Outcomes of Self-Expandable Versus Balloon-Expandable Valves for Managing Dysfunctional Right Ventricular Outflow Tracts.

BACKGROUND: Self-expandable valves are emerging complements to balloon-expandable valves for transcatheter pulmonary valve replacement in dysfunctional right ventricular outflow tracts, though their safety and efficacy remain underexplored. We aim to compare patient characteristics and outcomes of self-expandable valves and balloon-expandable valves in transcatheter pulmonary valve replacement. METHODS: Baseline and early follow-up data were prospectively analyzed for 145 patients who underwent transcatheter pulmonary valve replacement with Edwards SAPIEN 3 (ES3) balloon-expandable valves or

Patient with Behçet Syndrome with Subaortic Pseudoaneurysm Who Underwent the Bentall Operation Showed Improvement after Medical Treatment.

Aortic regurgitation (AR) in children is usually caused by congenital valve anomalies, and Behçet syndrome (BS) can be suspected in cases of isolated AR. Patients with BS undergoing aortic valve surgery due to aortic valve invasion have a high risk of complications, such as leakage around the valve and dehiscence. Cardiovascular involvement occurs in 7%-46% of adult patients with BS and is the main cause of mortality; however, its prevalence is unclear and rare in children. A 12-year-old boy was diagnosed with severe AR associated with BS. A progressive subaortic pseudoaneurysm was observed af

Echocardiographic findings of a valvular aortic cyst in a dog.

A 12-year-old, 19.8-kg, female neutered Nova Scotia Duck Tolling retriever was referred for investigation of a heart murmur. The dog was asymptomatic at the time of examination. On cardiac auscultation, a soft (2/6) diastolic heart murmur, louder at the left cardiac base, was present. Two-dimensional transthoracic echocardiography revealed a cyst-like structure attached to the aortic valve leaflets. Doppler examination showed that the cyst was fluid-filled and connected to the aortic and ventricular lumen. A moderate aortic valve insufficiency was also present. The left heart was mildly dilate

Aortic and mitral valve repair in a child with rheumatic heart disease.

The patient had rheumatic heart disease, which resulted in severe aortic and mitral valve regurgitation. Repair of both valves was performed at 9 years of age. During surgery, the retracted aortic valve cusps required extension with bovine pericardial patches and suture reduction annuloplasty, and the mitral valve was repaired using a Cosgrove-Edwards (Edwards Lifesciences LLC, Irvine, CA) annuloplasty band. Post-operative echocardiography showed no regurgitation in either valve. The patient is doing well at 2 years of follow-up.

Transthoracic echocardiography-guided subaortic ventricular septal defect closure in infants: a case report.

INTRODUCTION: Subaortic VSD, while similar to perimembranous defects, pose a higher risk for aortic valve insufficiency and AV block. This case aims to assess the safety and efficacy of percutaneous subaortic VSD closure in infants under 10 kg using transthoracic echocardiography-only guidance. CASE PRESENTATION: A one-year-old infant, 8.9 kg, was scheduled for subaortic VSD closure due to concerns of failure to thrive. Percutaneous closure was performed using a retrograde transarterial approach with a 7/5 mm Konar-MF VSD Occluder (Lifetech) under TTE guidance. Apical 5-chamber view showed sm

Robotic patch repair of a ventricular septal aneurysm during an aortic valve replacement surgery.

Interventricular septal aneurysm represents a rare cardiac anomaly traditionally associated with ischaemic heart disease or healed congenital ventricular septal defects. In this report, we present the first documented case of robot-assisted surgical repair of an interventricular septal aneurysm. Our 47-year-old male patient presented with symptomatic heart failure secondary to moderate aortic stenosis and severe aortic regurgitation. During pre-operative evaluation, imaging revealed an incidental membranous interventricular septal aneurysm without an associated shunt. The patient underwent suc

Surgical Insights into the Functional Anatomy of the Neo-aortic Root.

Although the normal aortic and pulmonary roots have some features in common, they also exhibit important anatomical and functional differences. These differences increase when compared with surgically created neo-aortic roots in individuals with congenitally malformed hearts, with the substrates initially having been either a pulmonary root originating from the right or left ventricle, or a truncal root with variable ventricular origin. With the increasing application and success of these surgeries, our attention has now turned toward understanding late outcomes. Subsequent dilation, usually p
